Le concept du “diamant” The “DIAMOND” concept L. Obert, T. Lascar Poser l’indication du meilleur traitement en cas de fracture de l’humérus proximal n’est pas chose aisée. Arriver à le faire c’est connaître les différents paramètres et leur poids respectifs dans le résultat fonctionnel. Aujourd’hui certains paramètres semblent jouer un rôle certain que les opérateurs expérimentés connaissent. En fait poser la “meilleure indication” c’est arriver à définir un guide de questions à poser afin de construire un chemin de réponses qui nous mèneront à proposer le bon traitement pour le bon patient. Deciding on the indication for the best treatment in cases of fracture of the proximal humerus is not an easy task. In order to get there, the various parameters and their respective importance in achieving a functional result must be explored. Today, certain parameters seem to play a set role that experienced surgeons know. In fact deciding on the “best indication” is a way of outlining a guide of questions that should be asked in order to build a chain of responses which will lead us to offer the best treatment for the right patient. Nous avions travaillé sur ce concept “du guide” avec Peter Giannoudis et David Marsch qui ont plus tard publié avec Gerard Schmidmaier, le “Diamond Concept” pour les pseudarthroses [1]. Il s’agissait, devant une pseudarthrose de déterminer lequel ou lesquels des acteurs il avait manqué pour obtenir une consolidation : de cellules, de facteurs de croissance, de support, ou de stabilité …le schéma d’un losange (un diamant) représentait ces 4 outils. We had worked on this concept of a “guide” with Peter Giannoudis and David Marsch who later went onto publish with Gerard Schmidmaier, the “Diamond Concept” for pseudarthrosis [1]. With a pseudarthrosis, it involved determining which factors were missing in the attempt to achieve consolidation: Cells, factors concerning growth, mount or stability… and the lozenge shaped diagram (a diamond) represented these 4 tools. Et là encore on peut imaginer un schéma simple permettant de guider le jeune opérateur à ne rien négliger (fig. 1). Faced with a fracture of the proximal humerus we can also try to ask the essential questions which will lead to answers and therefore choices. And in this instance we are trying to design a simple diagram which helps guide the novice surgeon to not overlook anything (fig. 1). Ainsi cette fragilité doit rendre prudent dans l’ambition de la fixation. Car même si les systèmes de vis verrouillée permettent aux vis de tenir dans la plaque ou dans le clou, rien aujourd’hui ne nous permet d’obtenir une meilleure fixation dans l’os porotique. Plus l’épaisseur des 2 corticales du col huméral est inférieure à 4 mm, plus les patients sont âgés (> 70, 80 ans…), moins l’ostéosynthèse a de chance de tenir et d’éviter le déplacement secondaire ou la faillite du matériel. Si à cette “fragilité osseuse” on rajoute le type de fracture (tête fracturée, fracture à 4 fragments) on comprend que l’ostéosynthèse est plus risquée (fig. 2). L’analyse de cette fracture se fera idéalement par un scanner dès que cette fracture n’est plus une fracture sous tubérositaire (à 2 fragments). Qui : Le patient et le chirurgien La mortalité d’une fracture de l’humérus proximal se rapproche de celle d’une fracture du fémur proximal [1]. Ce paramètre est peu connu mais il doit être pris en compte dans le projet thérapeutique et ne peut plus être ignoré. L’opérateur n’aura droit qu’à un coup comme bien souvent en chirurgie fonctionnelle. Et comme dans les fractures du fémur proximal une évaluation de l’autonomie et des facteurs de comorbidités permettent de choisir la meilleure option. Mais à côté de l’humain blessé, il y a l’humain qui opère. La chirurgie des fractures de l’humérus proximal est une chirurgie complexe et le choix d’une ostéosynthèse ou d’une prothèse ne peut se faire qu’avec de l’expérience et ne doit jamais se faire au bloc opératoire. If we add to this “bone fragility” the type of fracture (fractured head, fracture into 4 fragments) we see that osteosynthesis holds more risks (fig. 2). Analysis of this fracture would ideally be carried out with a scanner since this fracture is no longer a sub-tuberosity fracture. (with 2 fragments). Who : The patient and the surgeon The rate of mortality from a fracture of the proximal humerus is close to that from a fracture of the proximal femur [1]. This parameter is little know but should be taken into account in therapeutic planning, and can no longer be ignored. The surgeon will only have one chance as is often the case in functional surgery. And as with fractures of the proximal femur, an evaluation of independence and comorbidity factors help the better option to be chosen. However, besides the injured person, there is the person operating. Surgery on fractures of the proximal humerus is a complex operation and the choice between an osteosynthesis and a prosthesis can only be made with experience, and should never be made in the operating theatre. Quand : Le bon moment When: The best time Qu’une ostésoynthèse ou qu’une prothèse soit choisie l’intervention peut être réalisée dans les 2 premières semaines [3]. Par contre, c’est avec les bons outils et les bons humains qu’il est cohérent d’envisager cette chirurgie : après 60 ans et en cas de 4 fragments l’hémi-arthroplastie doit être en salle si une ostéosynthèse veut être posée coûte que coûte ; même chose après 70 ans en cas de fracture à 4 fragments une prothèse inversée devra être discutée et accessible. Conversely, it is with good tools and good personnel that it is consistent to suggest this surgery: Over the age of 60, and in a case of 4 fragment fracture, the hemi arthroplasty should be considered if an osteosynthesis is to be used at all costs; equally over the age of 70, and with a 4 fragment fracture an inverse prosthesis should be discussed and available. The 4 questions in this part help to avoid mistakes in prosthesis cases. Ces quatre questions sont : quelle est la bonne hauteur (la bonne fixation de l’implant) ? quel type de métaphyse ? quel type de greffe ? quel type d’ostéosuture ? These questions are: What is the correct height (correct implant fixation)? What type of metaphysis? What kind of graft? What type of osteosuture? Tous ces points sont traités dans les chapitres suivants.
