Older age (p = 0.032) and injuries with finger fracture (p < 0.0001) are associated with lower TAM values. Conclusion This study shows a favorable effect of CAM protocol on early TAM recovery. Further studies are required to strengthen such thesis and to investigate, whether faster recovery of TAM is associated with shorter inability to work. Although CAM patients had slightly lower complication rates there was no statistical evidence for a difference between the protocols for adhesion or rupture. This study was undertaken to determine results of primary flexor tendon surgery in zone 1 and 2 from 2006-2011 comparing passive mobilisation (modified Kleinert protocol) with controlled active motion (CAM) which was introduced in our unit in 2011. Exclusion criteria were replantation, revascularization, age <16 years and missing data about postoperative rehabilitation. As primary endpoints we defined complications (adhesion, rupture, infection) and total active motion (TAM) at 4 and 12 weeks after surgery. Secondary endpoint was the comparison of modified Kleinert protocol with CAM facing TAM values at 4 and 12 weeks, respectively. A naive analysis of TAM measurements under the rehabilitation protocols was conducted using a t-test with unequal variances. Die Dicke der A1 Ringbänder kann mit hochauflösenden Ultraschallsonden ausgemessen werden. Bisher ist noch keine Korrelation der sonografisch gemessenen Ringbanddicke mit vitalem Gewebe bekannt. In der Literatur gibt es Vergleiche von sonographischen und Kadavermessungen sowie mit bildgebender Diagnostik wie zum Beispiel MRI. Ziel dieser Studie ist es, die sonografischen Messungen mit der intraoperativen Ringband dicke makro- und mikroskopisch zu vergleichen. Material und Methoden: Results 228 patients were operated from 2006-2011. 191 patients with 231 injured digits were included in the study. Overall complication rates were 16.4% (adhesion), 7.4% (rupture) and 0.4% (infection). For the comparison of rehabilitation protocols there were 122 digits from 100 patients available for analysis after removing missing values. There is clear evidence that the CAM protocol increases a patients TAM earlier (at 4 weeks) in the rehabilitation process than the Kleinert protocol (p = 0.0002). Bei diesen Patienten wurde während der operativen Ringbandspaltung ein 2 x 10 mm breiter Streifen des A1 Ringbandes exzidiert, mittels Messschieber (TESA ® Technology) sowie unter dem Mikroskop ausgemessen und histologisch 4 4 aufgearbeitet. Return to work was also recorded. Ergebnisse: Der durchschnittliche sonografische Normwert der Ringbanddicke der Probenden liegt bei 0.40mm im Vergleich zu 0.75mm bei den Patienten. Die noch laufenden statistischen Auswertungen zeigen mit 0.789 (Pearson-Koeffizient) eine grosse Korrelation zwischen den präoperativen sonografischen Messungen und den mittels Messchieber gemessenen Ringbanddicken. Eine mittlere Korrelation (PearsonKoeffizient: 0.413) kann zwischen den prä- und intraoperativen Ringbandlängenmessungen nachgewiesen werden. Die histologischen Auswertungen sind zum heutigen Zeitpunkt noch in Arbeit. Schlussfolgerungen: Erste statistische Auswertungen der Studie zeigen, dass die sonografische Messung zuverlässig mit der intraoperativen Messung des A1 Ringbandes korreliert. Somit können diese sonografischen Messwerte von Probanden und Patienten als Richtwerte dienen. Voraussetzung dazu ist eine hochauflösende Ultraschallsonde und Erfahrung in der Sonografie der Weichteile am Bewegungsapparat. Bei unklaren klinischen Pathologien können diese Messwerte hilfreich sein. Allfällige Interventionen können präzise geplant werden. Conclusion: we confirm previous reports that in long lasting CECS a wide and complete opening of the deep fascia with a supplemental dermatomy of the volar forearm is mandatory, leading to a faster recovery and preventing a partial disappearance of clinical symptoms. Valentina Corni , Cesare Fusetti , Stefano Lucchina (1Locarno) 1 1 FM4 1 Health economic evaluation of collagenase injection for dupuytren contracture in Switzerland Purpose: There is a general consensus that miniinvasive and endoscopic-assisted fascial decompression of the flexor-pronator mass in the forearm seems to be as efficient as the open procedure in chronic exertional compartment syndrome (CECS). In the scientific literature there are no reports that compared directly the postoperative follow-up of the endoscopic and open technique. Florian S. Gutzwiller1, Patricia R. Szucs1 (1Basel; 2Zürich) Background The treatment options in Dupuytren’s Disease (DD) include percutaneous needle fasciotomy (PNF) and limited fasciectomy (LF). Xiapex®, a collagenase injection (CI), is a recent non-surgical treatment option. In Switzerland, the treatment with CI is reimbursed since January 2012. Methods: A 26 yo male woodcutter with a marked hypertrophy of the forearm flexor muscles presented with a 3-year history of chronic bilateral volar pain exacerbated by repeated gripping. The pain was associated with visible swelling in the volar forearm and paresthesias in the long fingers. The patient noted his arm “hard as a rock” with a bilateral 5 5 FM5 The objective of the present study is to determine the health economic impact of current treatment options in DD patients from a Swiss third party payer perspective. Human progenitor tenocytes and extracellular matrix scaffold to treat hand flexor tendon injuries Methods Data on recurrence rates, complications and utility values were derived from published literature and expert interviews. Hospitalisation rates were abstracted from health insurance billing data. Effectiveness was measured in quality-adjusted life years (QALYs), avoided hospitalizations, complications or recurrences. Total costs of compared treatments were estimated by using health insurance billing data. On this basis, incremental cost effectiveness ratios (ICER) were determined over a time horizon of four years. Healing process is long and loss of mobility or re-rupture is frequent, resulting in an important impact on work. In the worst cases, some people are even forced to change career. In extended injuries, autologous tendon grafts can be needed to restore acceptable function. However, since autologous grafting is associated with donor site morbidity, a big effort is made to develop a tendon-like tissueengineered construct. Such a device should ideally combine a scaffold that presents the same mechanical properties as tendon tissue, and cells to stimulate tendon regeneration. Results Health insurance billing data from 741 patients (aged≥18 years) were analysed. The population differed in size of treatment groups and characteristics like sex and comorbidities. In total, 28 patients (3.8%) were in the CI group, 682 patients (92%) in the LF group and 31 patients (4.2%) in the PNF group. In the LF group, 433 patients (63%) were treated as inpatients. Mean costs for procedures were CHF 2’380 for CI, CHF 3173 (outpatients; CHF 2’836 inpatients) for LF and CHF 1’213 for PNF. Over the study period of four years, the average cost per patient was CHF 2’881 for PNF, CHF 3’841 for CI and CHF 4’146 for LF. Total QALYs accrued ranged between 3,83 (no treatment) and 3,86 (CI). Versus the reference strategy of “no treatment”, the ICER for PNF was CHF 146’609/QALY. The next best strategy was CI with an ICER of CHF 186’511/QALY versus PNF. CI dominated LF with lower costs and higher QALYs. Compared to PNF, CI saved about 25% (LF: 30%) of recurrences, resulting in CHF 3’840 per avoided recurrence. Compared to LF, CI and PNF save CHF 485 and CHF 2’009 per avoided hospitalisation, respectively. CI resulted in 4.7% fewer tendon ruptures compared to PNF (CHF 2’0427/avoided complication). Extracellular matrix (ECM) scaffolds allow cells to adhere, migrate and proliferate due to their natural tridimensional structure. We used horse tendon as a source, because of its structural similarities with human tendon, its low immunogenicity and its complete traceability. With a conservative decellularization technique, it was possible to achieve a cell-free tissue up to a depth of 5 to 10 mm, with a minimal impact on the structural architecture. Based on characteristics of starting material, it would be possible to adapt the size of scaffolds up to a length of 15 cm. The cells need to be adaptable to clinical practice by respecting Good Manufacturing Processing (GMP). In this study, we evaluated human progenitor tenocytes as a potential cell line, because of good results already shown clinically by these types of cells in tissue regeneration. With a Cell Titer assay, we observed a rapid and steady growth until passage 8, with a mean time of 4.63 days per population doubling. FACS realised on several passages showed a homogenous population and revealed absence of immunogenic HLA class II antigens, which assures potential for transplantation. The cells had a strong ability to produce matrix components, notably type I collagen and glycosaminoglycans. A slight stimulating effect on the activity of adult tenocytes was evident with a method based on conditioned media. Finally, the ability of cells to survive in a horse collagen environment was demonstrated by a live/dead assay. Conclusion Our results show that recurrences, hospitalisations or complications differ depending on the treatment of choice. Over the study period, CI accrued most QALYs. Our results are a valuable resource towards a more informed decision-making concerning choice of treatment alternative. The seeding of human progenitor tenocytes on the obtained scaffold could lead to a very promising 6 6 biomaterial for hand flexor tendon repair. Biomechanical and further histological tests are required to assure the most optimal tendon-like result of our construct. of our series are not as good after 6 months, and a longer follow-up would be needed to determine if this procedure provides long-lasting benefit. FM6 Nonanimal Stabilized Hyaluronic Acid for Tissue Augmentation of the Dorsal Hands a Prospective Study on 38 Patients Franck Marie Leclère1, Mario Trelles1, Esther Vögelin1 (1Bern) Figure 1.png BACKGROUND: Often ignored, hands are one of the most telltale signs of aging. This prospective study was initiated to evaluate the effect of subcutaneous hyaluronic acid (HA) injections in aging hands, with special attention to complications and long-term outcomes. Figure 2.png METHODS: FM7 Between January 2010 and December 2010, a total of 38 patients with skin phototypes II-IV and between 58 and 76 years old were treated with HA injection for aging hands. The quantity of injection never exceeded 1.0-1.5 ml HA per hand. A clinical follow-up was performed at 2 weeks, 4 weeks, 3 months, and 6 months after injection. Complications were reviewed for the whole series. At the first follow-up, 2 weeks after the procedure, ultrasound was carried out to determine if additional filling material was required. At each follow-up, patients were asked to fill out a satisfaction questionnaire. Qualitätskontrolle im ambulanten OPZentrum – Erfassung der Patientenzufriedenheit mittels Tablet-PC Miriam Marks1, Stephan Schindele1 (1Zürich) Hintergrund: Die Qualitätskontrolle nimmt in medizinischen Einrichtungen einen immer grösser werdenden Stellenwert ein. Die Patientenzufriedenheit als Teil einer Qualitätskontrolle kann heute mit elektronischen Tools einfach erfasst und dokumentiert werden. Neben soziodemographischen und behandlungsspezifischen Parametern wird die Patientenzufriedenheit auch von sogenannten Umgebungsfaktoren, wie z.B. die Freundlichkeit des Personals, beeinflusst1,2. Ziel dieser Studie war es, eine Umfrage auf einem Tablet-PC zu implementieren und damit die Zufriedenheit unserer Patienten mit den Umgebungsfaktoren zu evaluieren. RESULTS: Nine patients developed slight ecchymosis that disappeared after 1 week. No other complications were seen in the series. Pain during the injection and discomfort after the procedure were minimal. At the 2week follow-up, after ultrasound control, nine patients received a complementary injection. At each follow-up, overall patient satisfaction was high and was validated by clearance of rhytids, veins, bony prominences, and dermal and subcutaneous atrophy. Methodik: Mithilfe des Programmes Adobe LifeCycle Designer wurde ein Fragebogen erstellt, der per Email auf ein Tablet geschickt wurde. Der Fragebogen umfasste zehn Fragen zur Freundlichkeit/Kompetenz des Personals, zum Informationsfluss sowie zur allgemeinen Atmosphäre. Die Fragen wurden auf einer 4-Punkt-Likert Skala beantwortet, wobei 1 „sehr gut“ und 4 „sehr schlecht“ bedeutete. In einer offenen Frage konnten Patienten weitere Anmerkungen geben. Die Patienten unseres ambulanten OP -Zentrums füllten diesen Fragebogen auf einem Tablet im Anschluss an CONCLUSION: Skin revitalization with injectable HA can improve the clinical appearance of the back of the hands. However, this therapy requires knowledge of the possible complications and their remediation as well as knowledge and respect of injected doses. METHODS A retrospective review was performed in 12 patients with comminuted distal radius type C3 treated with temporary distraction plate. All patients had pre and post-operative x-ray and CT scan. We obtained radiological (radial length, pre and post-operative ulnar variance, radial inclination, time of consolidation), surgical (additive procedures on the radius or ulna), and clinical assessments (age, professional activity, post-operative clinical examination). Ergebnisse: Insgesamt füllten 323 Patienten den Fragebogen aus und waren durchschnittlich sehr zufrieden mit einer Gesamtnote von 1.1 (±0.4). Am besten schnitt der Bereich „Freundlichkeit des Personals“ ab (1.0 ± 0.2). Verbesserungsbedarf hingegen gab es bei den Punkten „Abbau von Ängsten und Unsicherheiten“ (1.2 ± 0.4), „Zeit zum Fragen stellen“ (1.2 ± 0.4) sowie „Aufklärung zur bevorstehenden Operation“ (1.2 ± 0.4). Die offenen Fragen ergaben grösstenteils positive Rückmeldung aber auch einige Verbesserungsvorschläge, z.B. bezüglich der Lagerung. RESULTS We treated 12 patients, mean age of 57 y. We had 2 DRUJ instability associated with ulnar styloide fracture and 3 radio-triquetral lesions. All patients had a supplemental fixation with pins in the radial styloid in 10 patients, anterior VA plate in 3 patients, screws in 5 patients. Three patients needed an additive treatment of the ulna: 1 Darrach, 1 plate and 1 DRUJ stabilization with TFCC reinsertion. The mean pre-operative ulnar variance was + 1.42 mm. Post-operative ulnar variance was + 0.76 mm, radial inclination 19° and radial length 10.4 mm. All patients had CT-scan before removal of material. Three months after surgery 7 patients had removal of their material. Mean post-operative range of motion was F/E: 38/0/68 and P/S: 43/0/58. After removal of the plate the mean DASH score was 22, the EVA at rest was 0/10. Schlussfolgerung: Mit relativ geringem Aufwand konnten wir eine Zufriedenheitsumfrage an unserem OP-Zentrum implementieren. Somit lässt sich dieser Aspekt der Qualitätssicherung, beispielsweise im Rahmen einer ISO-Zertifizierung, einfach umsetzen und dokumentieren. Die positiven Antworten bestärken uns in unserer Arbeit, wohingegen wir aufgrund der kritischeren Antworten wissen, in welchen Bereichen noch Steigerungspotential vorhanden ist. 1. Marks, M. et al. J Hand Ther 2011;24:303-12 CONCLUSION Temporary bridge plate is indicated in highly comminuted distal radius fractures in elderly patients with poor bone quality and young polytrauma patient. The plate is acting like an external fixator but is internal closed to the bone maintaining radial length. This technique allowed patients without protection to perform activities of daily life thanks to intrinsic stability. This is an advantage compared to the external fixator. Freie Mitteilungen/Communications libres II A: Bone FM8 Treatment of comminuted distal radius fractures with temporary distraction plate: case reviews Cindy Bouvet1, Alexander De Smet1, Philippe Vostrel1, Marie Loret1, Jean-Yves Beaulieu1 (1Genève) INTRODUCTION Support of highly comminuted distal radius fractures in elderly patients with poor bone quality or in young polytrauma patients need a surgical technique in order to rebuild an anatomical wrist alignment. The aim of the study was to show post-operative results in 12 patients with temporary bridge plate. Before and after surgery 8 8 supination were respectively 96,2° and 134°, and strenght with 77% of contralateral, before hardware removal. 17 patients had plate removal in mean of 7,6 months with an improvement in score assessement. We found an EPL tendon rupture, an breaking plate, an early osteoarthritis, 8 tenosynovitis and 2 CRPS. CONCLUSION Surgical management of intra-articular shear beyond the watershed line of the radius fracture, using specific intra-articular plate, seems to be an advantage with good reduction results. However, a higher rate of tendon irritation needs to limit indications to some specific cases, when usual plates are not effective. Ergys Gjika1, Jean-Yves Beaulieu1, Jan Van Aaken1, Alexander De Smet1, Marie Loret1, Philippe Vostrel1 (1Genève) INTRODUCTION Many studies have reported the successful use of volar locking plates in various distal radius fractures, with good results on functional outcome and radiological reduction criterion. However, there are some situations in which most of volar plates seem limited, especially in the intra-articular shear beyond the watershed line. The main purpose of this study was to evaluate the usefulness of open reduction and internal fixation using specific implant placed beyond the watershed line. MATERIALS AND METHODS Between 2011 and 2013, 28 patients, with mean age of 55.8 years, were treated for complex radius fracture by using intra-articular plating system. The mean followup period was 9.7 months. There were 2 B3, 16 C2 and 10 C3. Clinical evaluation was performed in range of motion and grip strengh. Radiographic measurements were made preoperatively and after post operative reduction then statistical analysis was performed using the Student test, to compare X-Ray value. Objective functional assessment was obtained through quickDASH score, Mayo Wrist score and PWRE. The delay and the reason of hardware removal was also collected, same as complications related to the implant. RESULTS All fractures consolidated at the last appointment. 3 patients wasn't able to return to previous employment. Radiographs in preoperative and immediate postoperative period showed respectively, a radial sagittal tilt of -3° to 5,8°, radial frontal tilt of 18° to 22,6°, radial height of 9,6 mm to 12 mm. We found statistically significative differences of radiological values before and after surgical management (p<0,005). L’effet console et les vis verrouillées offrent la stabilité et permet la mobilisation rapide. La prise en charge des fractures du radius distal intraarticulaires avec comminution dorsale et enfoncement articulaire, résultant souvent d'un trauma à haute énergie, est difficile. Cette étude rétrospective non comparative veut montrer que l’abord dorsal avec 2 plaques dorsales 2.4mm à vis verrouillées est à envisager lors de ces fractures complexes du poignet lorsque spécifiquement il n’y a pas d’atteinte métaphysaire palmaire du radius. Patients and Methods: In 2012 three female patients with a mean age of 29 years (range: 18-43 years) presented with chronic dorsal wrist pain at the SL interval after a fall on the outstretched hand. Symptoms were in accordance to a SL ligament lesion with pain over the dorsal SL interval, positive resisted extension test but negative Watson test. Conventional radiographs showed normal carpal alignments and dynamic SL instability was excluded by kinematography. MRI failed to show any ligamentous lesions. Conservative treatment for a minimum of 3 months failed to improve the condition. Suspecting a predynamic SL instability arthroscopies were performed, revealing a Geissler 1 lesion in one patient and normal SLIL in the other two cases. A detachment of the dorsal capsule to the SLIL was found in each patient. On conversion to an open approach, a detachment of the DIC ligament at the SL level became evident. Fixation of the DIC ligament with an anchor at the dorsal aspect of the lunate was performed. Postoperative immobilization of the wrist was 10 weeks, followed by hand therapy. Les radiographies montraient une luxation isolée antérieure du scaphoïde et une fracture de la styloïde radiale. Le traitement a permis la réduction sanglante avec embrochage, suture du plan ligamentaire antérieur et neurolyse du médian. We retrospectively analyzed 29 consecutive cases with unspecific chronic wrist pain who received SPECT/CT between January 2011 and September 2012 following non-diagnostic or non-conclusive adequate standard investigations. All diagnostic procedures were performed by board certified specialists (hand surgeon, radiologist, SPECT/CT: double certified radiologist/nuclear medicine specialist). All patients received at least two different imaging techniques before SPECT/CT was employed. The new diagnostic information provided by SPECT/CT was grouped according to its effects on patient management (change/confirmation of diagnosis, change of management). Overall 29 patients were included (16 m/13 f, mean age: 38 yrs, range 21-63 yrs). Im Rahmen der Diskussionen um die endoskopische Dekompression des Nervus ulnaris am Ellbogen hat sich in diversen Studien bestätigt, dass die alleinige Dekompression gleichwertige Resultate wie die Vorverlagerung erzielt. Angesichts der erheblichen möglichen Komplikationen bei einer Vorverlagerung verzichten wir bei Primäreingriffen darauf. RESULTS: There were no intraoperative complications in the series. Endoscopy allowed both identifying and removing all the compressive structures. In one case, the proximal radial neuropathy developed for 10 years without therapy and a massive hour-glass nerve constriction was observed intraoperatively which led us to perform a concurrent complementary tendon transfer to improve fingers and thumb extension. Excellent results were achieved according to the modified Roles and Maudsley classification in five out of six cases. All but one patient considered the results excellent. The poorest responder developed a CRPS II and refused post-operative physiotherapy. CONCLUSION: In einer retrospektiven Studie haben wir 45 Patienten nach offener Dekompression mit kurzer Inzision (2010 – 2012) nachkontrolliert. Die Operationstechnik und Resultate werden vorgestellt. Die Ergebnisse wurden mittels modifiziertem Bishop-Score erfasst. Die Resultate sind sehr gut. Postoperative Komplikationen traten nicht auf. Endoscopically assisted decompression in rare compression syndrome of the upper extremity is highly appreciated by patients and provides excellent functional results. However, in children, neither normal compartment pressures nor the maximum tolerable pressures after a forearm fracture are known. The aim of this study was to assess normal pressures on the healthy side and maximal tolerable pressures on the fractured side in all three forearm compartments. In most cases there is complete recovery without surgery. In selected patients recurrence of symptoms and/or persistent motorsensory deficits leads to surgical decompression. Methods: The prospective study included healthy children with forearm fractures that needed acute reduction and osteosynthesis. Between 2009 and 2012, 39 children at an average age of 9.15 years (4.0 to 15.4) were included. Pressures were measured in the superficial and deep volar as well as in the dorsal compartments on both forearms during surgery. All patients were admitted until pain could be managed with oral analgesics and were followed in the outpatient clinic. We report our experience in surgical decompression of 7 ulnar and 6 median nerves in 5 patients with persistent sensory-motor deficits. Careful clinical examination and appropriate tests, including electromyography and genetic testing, confirmed the diagnosis of HNPP. All patients underwent decompressive surgery in general anaesthesia and completely recovered. Despite a well-documented genetic pathophysiologic mechanism, implications for anaesthesia in patients with HNPP are only speculative and the use of regional anaesthesia is debatable. The application of an Esmarch garrot was not influent in the recovery. Results: One patient had a clinical suspicion of a compartment syndrome with an intraoperative pressure of 50 mmHg in the deep volar compartment. He underwent an immediate fasciotomy and was excluded from further evaluation. Patients and Methods We conducted a retrospective review of outpatients with non-traumatic nerve lesions of the upper extremity referred for evaluation of nerve surgery between 1997 and 2011. All Patients were examined both by a hand surgeon and a neurologist and were investigated by electrophysiological evaluation. Some patients were investigated either by MRI or ultrasonography. Most of the patients (12/13) were examined by a neurologist on follow up examination. Results 13 Patients with anterior interosseus nerve (AIN) palsies were included in the study, 6 women, 7 men, 21-64 at age of onset. In 11/13 cases the onset was preceded by intensive pain of the affected extremity. Although there were comorbidities in some patients we could not determine a statistically significant factor. All the patients were treated conservatively. Outcome was classified as good, fair and poor. Of 13 cases of AIN palsies, 11 had a good outcome, two a fair outcome with some limitations in daily activities. Time to improvement was in the shortest 5- and in the longest case 18 months. There was no correlation of initial pain onset and poorer outcome. There was a correlation between the age and the appearance of signs of recovery. Conclusion Neuralgic amyotrophy or Parsonage-Turner Syndrome of the upper extremity represents a frequently encountered entity among patients for surgical evaluation. The most frequent clinical presentation in our series is an anterior interosseous nerve syndrome. The reason that we did not encounter more cases with more extended brachial plexus affections is probably explained by prior neurological investigation of these particular patients. The outcome of most of these conservatively treated patients was good. We conclude that in acute non-traumatic nerve paresis, a thorough history, clinical evaluation and electrophysiological work up is eligible for patients Introduction: The epineural coaptation of digital nerves following the finger injury is a common standardized surgical treatment. However, its indication is recently being questioned due to poor clinical outcome, observed especially in elderly patients. Materials and Methods: In this non-randomized study, the clinical outcome after epineural coaptation of injured digital nerve has been assessed by neurosensory testing at least 12 months after the operation. Results: Ninety-three digital nerves in 83 patients (56 males and 27 females) have been epineurally coaptated at our institution between February 2006 and November 2011. The mean follow-up period was 3.5 years. The average patient`s age at the time of operation was 43 years. Fifty patients (60.2%) were smokers or ex-smokers with an average of 16 packyears. Most common injuries were to the palmar Zone I (50.5%), followed by Zone II (41.9%). The most common trauma mechanism was a sharp injury (77%) with involvement of accompanying palmar artery in 52% of cases. Two patients developed neuroma (2.1%) and required subsequent neurolysis. One patient developed a CRPS. Sharp-blunt discrimination was preserved in 68% and cold-warm discrimination in 79% of patients. In 41% of patients a cold intolerance was reported to some degree. The mean standard 2point discrimination was 10.6 mm (vs. 4.4 mm on contralateral side) and 20.8 g/mm2 (vs. 5.2 g/mm2 on contralateral side) as assessed by Pressure Specified Sensory Device (PSSD). Conclusion: The epineural coaptation remains a therapy of choice in patients with digital nerve injury, regardless of the age, inury mechanism, involvement of the artery and other so far commonly assumed unfavourable prognostic factors. Patients can expect a satisfactory neurological outcome, however the main advantage of a surgical therapy should be tributed to prophylaxis of neuroma. Collagen nerve guides can be used in clinical practice for peripheral nerve defects, but their use is generally limited to lesions up to 3 cm. Autologous cell transplantation in the neural conduits conduit may be an alternative strategy to create a favorable environment for nerve regeneration, supporting regeneration over longer gaps. We coupled commercially available collagen conduits to regenerative cells and investigated initial effects on nerve regeneration. Methods: 1 Mio. of each of the following cell types were introduced into biodegradable fibrin conduits: rat adipose-derived stem cells (rASCs), Schwann cell (SC)-like differentiated rASC (drASC), rat SCs (rSCs), human (h-)ASCs from the superficial and deep abdominal layer as well as human stromal vascular fraction (SVF). The sciatic nerve injury model was used creating a 10mm gap in the left nerve of female Sprague Dawley rats (7 groups of 7 animals, 8 weeks old) and was bridged through this conduit. As a control we re-sutured a 10mm cut nerve segment backwards as an autograft. Long-term evaluation was carried out after 12 weeks in a multimodal manner comprising walking track and morphometric, as well as MRI analysis. The Sciatic Function Index (SFI) was calculated with the help of a functional evaluation tool. Moreover, cross sections of the nerve proximal, distal and in between the two sutures, corresponding to the former gap, were analysed. Furthermore gastrocnemius muscle weights between groups were compared and in addition imaging analysis (MRI) was performed. Results: Materials and Methods Cell retaining of commercially available collagen conduits (NeuraGen nerve guides) was investigated in vitro: after 48 hours of incubation into the conduits, cell adhesion was verified by electronic microscopy analysis. In the in vivo experiment, three groups (each n=5) of NeuraGen guides were seeded with primary Schwann cells (SC), SC-like differentiated mesenchymal stem cells (dMSC), SC-like differentiated adipose-derived stem cells (dASC). A control group was formed by empty NeuraGen guides for a total n=20. Conduits were used to cross a 1 cm rat sciatic nerve gap in a brief experiment (2 weeks). Immunohistochemical analysis was performed to assess proximal axonal regeneration and proximal and distal infiltration of Schwann cells. MRI revealed muscle atrophy across all groups and proved biodegradation of the fibrin conduit.Correlating trends throughout the different evaluation techniques could be shown: Superficial human ASC supported regeneration better than deep, in line with published in vitro data. SC-like drASC had the best regeneration Results Primary SC showed significant improvement in distal stump sprouting. The commercially available nerve guide could be modified in future tissue-engineering applications (e.g. extracellular matrix peptides) to better exploit cell potential. Even if a significantly increased Schwann cell migration in the distal stump was observed in primary SC-loaded conduits, a long-term experiment is necessary to assess final differences in nerve regeneration and functional outcomes among the groups. Le score de Cooney moyen était de 80/100 : 6 excellents résultats (25%), 6 bons résultats (25%), 9 moyens résultats (37.5%), 3 mauvais résultats (12.5%). Le score DASH moyen était de 15 et le PRWE de 23/100. La durée moyenne de l’arrêt de travail était de 16 semaines. L’arc de flexion-extension moyen du côté lésé était de 62 % en comparaison au côté sain. La force moyenne du côté lésé était de 78 % (42 kg) par rapport au côté sain. La déviation radiale-ulnaire et la pro-supination moyenne étaient respectivement de 72 % et 100% par rapport au côté sain. Conclusion : Cette étude met en évidence la nécessité de la rapidité de la prise en charge des fractures luxations périlunaires. Les 3 mauvais résultats sont dû au type de traumatisme (very high energy trauma, VHET), la prise en charge ayant été la même que pour les autres résultats. Les résultats sont compatibles avec une reprise des activités antérieures à condition que les patients soient traités dans des centres spécialisés et que la blessure ne soit pas du type VHET. Introduction L’évolution des fractures de l’extrémité distale du radius, articulaires, est parfois défavorable avec une nécrose des fragments cartilagineux. Cette dégénérescence arthrosique est responsable d’un enraidissement du poignet et de douleurs invalidantes. Les arthrodèses radio-scapho-lunaires (RSL) ont donc des indications bien spécifiques car l’interligne médiocarpien doit être de bonne qualité. Matériel et Méthode : Cette série rétrospective, réalisée de janvier 2004 à mai 2013, inclus les patients opérés pour une fracture-luxation périlunaire dont 17 formes trans-scapho-périlunaires selon la classification radiologique de Herzberg. Sur les clichés de profil, 16 lésions (66%) étaient à déplacement postérieur (10 stades I : lunatum en place sous le radius et 6 stades II : lunatum luxé en avant du radius). 10 présentaient une luxation antérieure stade II. La série comportait que des hommes, 12 travailleurs manuels (50%), pour un âge moyen de 34 ans. L’acte chirurgical, à ciel ouvert, comprenait un abord dorsal (15 cas) ou dorsal et palmaire (9 cas) et associait toujours un vissage ou brochage scapholunaire ou scaphocapital. L’immobilisation, en moyenne de 10 semaines par But Dans la littérature, la résection du pôle distal du scaphoïde augmente le taux de consolidation de l’arthrodèse. Cette procédure permet également une augmentation de la flexion et de l’inclinaison radiale par rapport aux RSL sans résection scaphoïdienne. L’objectif de notre étude est de décrire les différentes étapes de notre technique et d’évaluer si le gain peropératoire des mobilités lors de la scaphoïdectomie est retrouvé à long terme. 16 16 Résultat Nous avons évalué rétrospectivement 9 patients traités par une arthrodèse RSL pour une arthrose posttraumatique symptomatique et 1 dans le cadre d’une ostéo-arthrite septique, entre 2008 et 2012. Tous les patients ont bénéficié d’une scaphoïdectomie et d’une technique de distalisation d’un capot cortical dorsal pris au dépend du radius. Le recul moyen est de 2.8 ans. La consolidation de l’arthrodèse est obtenue chez tous les patients. L’évolution vers une arthrose de l’articulation médio-carpienne est survenue dans 3 cas : 2 arthrodèses pan-carpiennes et 1 implant Amandys. Nous déplorons une rupture tendineuse et une fracture métaphyso-diaphysaire du radius. La mobilité est de 18° en moyenne pour la flexion, 30° pour l’extension, 10/19° pour l’inclinaison radiale/ulnaire, 70/70° pour la prono-supination. La force moyenne post-opératoire est 58 % du côté controlatéral. Les mobilités per-opératoires après scaphoïdectomie ont été recueillies chez 5 patients. Lors de la comparaison au dernier recul de la flexion/extension (44/41° per-op, 25/32° post-op) et de l’inclinaion radiale/ulnaire (17/23° per-op, 10/18° post-op), on observe que le gain de mobilité per-opératoire n’est pas conservé. Ergebnisse: Der Mittelwert der Ulnavarianz bei den scapholunären Läsionen lag bei -0,47 mm, die Ulna-minus-Varianz wurde bei 56% der Patienten festgestellt. In der Kontrollgruppe lag der Mittelwert der Ulnavarianz bei -0,40 mm, die Ulnaminus-Varianz wurde bei 44% der Patienten nachgewiesen. Unsere Resultate zeigen bezüglich Mittelwert der Ulnavarianz vergleichbare Werte bei einem leicht erhöhten Anteil an Ulna-minus-Varianzen in der Gruppe der Patienten mit einer scapholunären Läsion, jedoch jeweils ohne statistisch signifikante Differenzen. Schlussfolgerung: In unserem Patientenkollektiv scheint eine negative Ulnavarianz beim Handgelenkstrauma kein Risikofaktor für eine scapholunäre Läsion zu sein. Conclusion Lors de l’arthrose radio-scapho-lunaire avec un interligne médio-carpien conservé, l’arthrodèse RSL reste une bonne option chirurgicale. La résection du pôle distal du scaphoïde permet l’augmentation des mobilités en per-opératoire, mais elle n’est pas conservée au long terme. Cette résection est indispensable pour autant car elle diminue le risque de pseudarthrodèse et d’arthrose scapho-trapèzotrapèzoïdienne. The computer tomography showed a sufficient gap between the scaphoid and the distal part of the trapezoid in 90.5% of the cases. Results We included 10 patients (3 female, 7 male) with a mean age of 67.5 years (±7.1). The active range of motion of affected PIP joint increased slightly from 41.5° (±16.3) preoperatively to 51.1° (±28.4) after one year (p=0.312). Patients reported a significant pain relief from preoperatively 7.9 (±0.4) to 1.1 (±1.5) at 12 months follow-up (p=0.018). Compared to preoperatively there was also a significant improvement in the quickDASH from 43.2 (±12.2) points preoperatively to 14.8 (±17.0) points after one year (p=0.007). Similar results were found for the PEM (p=0.038). All implants remained intact over the postoperative time and no migration, no osteolysis and no implant fractures occurred. Conclusion: After surgical treatment of trapeziometacarpal joint arthritis, patients with coexistent STT arthritis can have persistent pain. Our results show that resectionsuspension arthroplasty (CMC I) with simultaneous resection of the proximal part of the trapezoid is a reliable, effective and simple surgical procedure for concomitant trapeziometacarpal and STT arthritis. The outcome is good, with a low pain level and a high satisfaction among patients. Further studies with larger series are needed to evaluate the long-term results of this surgical technique. Conclusion FM26 One year after the first ten surgeries using the CapFlex PIP© implant, we yielded promising results. All implants showed completely osteointegration and no evidence of radiological migration. The positive subjective and clinical outcomes confirm the radiological results. The aim of this retrospective study was to compare the respective treatment outcome. Early Posttraumatic Arthritis of the Small Joints: A Descriptive Analysis Rahel Meier1, Frederik Hahn2, Parham Sendi1, Esther Vögelin1 (1Bern; 2Zürich) Methods In group A, 26 patients with 27 hyperextension injuries were treated with custom made extension block splinting in 10° flexion (Laporte, J. et al. 1992). In group B, 25 patients with an equal number of injuries were treated with buddy taping and dorsal night splinting in 10° flexion. Introduction: Posttraumatic arthritis of the small joints is a known but rarely described entity. The diagnostic difficulty includes a trauma limited to softtissue or within small joints, especially in the early posttraumatic period. Involvement of joints may be due to direct inoculation of pathogens or to migration of leucocytes following inflammation of the adjacent softtissue. Most experts recommend a preemptive antibiotic therapy with amoxicillin/clavulanate after the trauma. In this study, history, location, aetiology of early posttraumatic small-joint arthritis (EPTSJA) of the hand was analysed, including the efficacy of preemptive antimicrobial treatment. Results In group A, 14 of 26 patients at a mean of 8.2 weeks showed excellent, 8 good, 1 patient a poor result with of on average 92° total active motion (TAM) in the PIP and a mean extension deficit of 4.6°. In group B, 13 of 25 patients at a mean of 12 weeks had an excellent, 7 a good, none a poor result with a mean TAM of 94° and a mean extension deficit of 4.5°. No patient developed palmar joint instability. There were 3 patients in group A, 5 in group B, who got lost to follow up. Methods: We reviewed the patient charts for arthritis of phalangeal and wrist joints between 2005 – 2009. The definition of arthritis included clinical signs of inflammation, the need for arthrotomy and the microscopic presence of leucocytes. The interval from trauma to occurrence of arthritis was ≤ 4 weeks for the definition of ‘early’. The trauma was categorized in ‘stab/cut’, ‘bruise’, or ‘animal bites’. The involved fingers and the number of affected joints were registered. The wrist was accounted as one single joint. Microbiology samples only from the joints and not from the adjacent soft-tissue were included. Conclusion There were no statistically significant differences between the treatment protocols regarding TAM, extension deficit, duration of treatment or number of treatment sessions but buddy taping seems to be faster and easier to adjust, less expensive and provides more comfort to the patient. Results: 64 episodes of small joint arthritis were identified, of which 20 (31.25%) fulfilled the above criteria of EPTSJA; 13 (65%) were male, and the median age was 38.6 (range 18.3 – 82.3) years. The dominant traumas were cuts (11; 55%), followed by bruises (5, 25%) and animal (i.e. cat) bites (4; 20%). The median interval from trauma to arthritis was 3.5 (range 1 – 28) days; 12 (60%) episodes presented ≤ 7 days. The wrist was involved twice (10%), joints of 1 finger in 16 (80%) and joints of 2 fingers in 2 (10%) of the cases. Middle (7, 35%) and index (6, 30%) fingers were most commonly affected, as were the PIP and MCP joints (each 7, 35%). In 8 cases no pathogen grew from culture; in 4 grew S.aureus, in 3 each Streptococcus and Pasteurella spp. and in two Enterbacter cloacae. Including the culture negative cases, preemptive amoxicillin/clavulante was effective in 90% of the cases. Extension block splint Conclusion: In this series, cut traumas on one finger, either the index or middle finger, involving either PIP or MCP joints were the most common aetiologies for Buddy tapes 19 19 EPTSJA. Preemptive amoxicillin/clavulante was effective in the vast majority of the cases. In fractures of the metacarpals and phalanges a general or a brachial plexus block is usually performed. Nevertheless, important indicators suggest that using local anesthesia could be advantageous with regards to the number of complications, pain-level during application, anesthesia onset time and duration, hospitalization time and expenses. There were no perioperative complications concerning anesthesia, only 19 reoperations (19%) occurred, all of them for OSME. Which anesthesia technique was chosen had no influence on the osteosynthesis technique. The mostly used technique in MC I fractures was the combined median and radian nerve block (6 of 11), in MC V fractures the singular ulnar nerve block (19 of 38), in MC II to IV a complete hand block (5 of 9). Standard conservative treatment included extension splinting for 2 weeks, taping to the adjacent digit and Coban TM bandages. Results In 20/50 patients, x-rays of the palmar plate avulsion did not extactly correlate to ultrasound imaging. Sonographically, the initial palmar bony avulsion could be localized precisely. The amount of soft tissue oedema and the mobility of the palmar plate parallelled the clinical outcome. There were 2 patients with hyperextension of the PIP joint of whom 1 required palmar plate refixation. One patient with chronic luxation of PIP joint required arthrodesis. Using ultrasonography, soft tissue injuries of the palmar plate, the collateral ligaments and the extent of bony avulsion fractures can be diagnosed as well as information on stability obtained. One treatment option is a dynamic external fixator. However, common shortcomings of current fixators include obscuring the fracture side on true lateral radiographs, time consuming assembly, and lack of adjustability. For these reasons we designed a new dynamic external fixator (Adynex). This study reflects its use in a cadaver model. Material and Methods 50 patients with palmar plate injuries of the PIP joints (24 distorsions, 26 luxations) were clinically, radiologically and sonographically examined (Philips iU 22, 17.5 MHZ transducer) at their first consultation and at 6 weeks and 4 months post injury. The extent of Materials and Methods We created an unstable PIP joint fracture-dislocation model in cadaveric long and 22 22 FM35 little fingers (fig. 1). We applied the Adynex, which consists of a pair of rods and nuts (radiolucent spacers) made of Polyetheretherketone (PEEK) and a pair of stainless steel springs. The Adynex was fixed to two parallel Kirschner wires which were drilled through the rotational center of the head of the proximal and middle phalanges. The distraction force was generated by turning the springs (fig. 2). Fracture reduction and congruency of the PIP joint was documented throughout full PIP joint motion under dynamic lateral fluoroscopy. Plate Fixation of Fractures of Proximal Phalanx – Are The Problems Solved with New Implants? The fracture site was not obscured on a lateral view (fig. 2). The distraction force could be easily changed by turning the springs on either side during the distraction period. Limited range of finger motion is a frequent complication after plate fixation of phalangeal fractures. For a few years, new low-profile implants for internal fixation of phalangeal fractures of the hand are available. The purpose of this study is to evaluate the results of plate fixation of extraarticular fractures of the proximal phalanx using 1.2mm and 1.5mm minifragment-systems. Can these new implants reduce tendon adhesion, and thus improve postoperative range of motion? What complications are to be expected? Conclusion The Adynex fulfilled its objectives in a cadaver model and will next be applied in the clinical Methods From 2006 to 2012, 32 patients with 36 extraarticular fractures of the proximal phalanx of the fingers were treated at our department with open reduction and plate fixation using 1.2 and 1.5mm mini-fragment-systems. Patients presenting with 2nd, 3rd and 4th degree open fractures or relevant laceration of adjacent structures were excluded from the study. We retrospectively evaluated the rate of failed initial conservative treatment, the rate of malunion or non union after plate fixation, the need for revision surgery, for plate removal, and for tenolysis. Data were analysed for further complications with regard to infections or complex regional pain syndrome (CRPS). setting. Fig.1 fracture-dislocation model Results In 6 patients, open reduction and plate fixation was preceded by attempts of initial conservative treatment using static or functional casts. A total of 36 fractures were operated. No postoperative infections were noted. Five patients developed complex regional pain syndrome. Postoperative TAM after a minimum of 6 weeks postoperatively averaged 164°, and all 32 patients required hand therapy. At the end of treatment, the mean postoperative TAM was 220°. Extension lags of PIP joints were found in 67%, ranging from 5 to 45°. Secondary surgery was necessary in 44% (2 corrective osteotomies, 12 plate removals including 7 explicitly because of reduced mobility). fig. 2 applying fixator and results 23 23 Conclusions to be excluded due to infection or excessive screw length. Despite of the new implant designs (improved surface properties, low plate profile, use of locking screws), significant problems persist. Adhesions of extensor tendons leading to limited range of finger motion are still the most frequent complications after open reduction and plate fixation. Hence, functionalconservative casts are recommended for treatment of well-reduced extraarticular fractures of the proximal phalanx of the fingers. Conclusion: The results after six weeks are interesting, as they indicate clearly better results for the group with adhesion barrier. It remains to be discussed why the results of both groups were practically identical after six months. The risk of infection where cellulosis matrix is used will have to be critically monitored but only one infection has to date been detected in a patient with adhesion barrier. Im Allgemeinen besteht für Frakturen mit Gelenksbeteiligung von über 30%, Frakturen mit Luxations- oder Subluxationsstellung, aber auch Frakturen mit intraartikulärer Stufenbildung eine Operationsindikation. In der Literatur werden jedoch über unbefriedigende funktionelle Resultate berichtet mit bis zu 63% Komplikationsrate. So haben wir unsere operierten Patientin der letzten Jahre nachuntersucht, um die funktionellen Ergebnisse zu überprüfen. Introduction: Postoperative extensor tendon adhesions after osteosynthesis of finger P1 fractures often lead to results with dissatisfactory functionality. The idea of an adhesion barrier has often been mentioned. Due to unconvincing results, however, they have not reached a level of acceptance among hand surgeons. This means that the problem of postoperative adhesions remains unresolved. It is the aim of a prospective randomised trial to evaluate the value of a product consisting of denaturised cellulose. Methods: 38 of 42 projected patients have been included between February 2010 and February 2013, of whom 37 were conclusively evaluated. The trial included all isolated and closed P1 fractures which had an indication of plate osteosynthesis. The trial excluded patients with multiple injuries and those with preexisting functional deficits. This is a one center study and all patients where seen and treated by the department of Handsurgery, Kantonal hospital St Gallen. Pre-operative randomisation into two groups (with or without application of the adhesion barrier). Measurement after 6 weeks and 6 months after surgery of range of motion as an expression of functionality of the affected finger in comparison with the unaffected opposite side and measurement of DASH score. Recording of need for intraoperative tenolysis as part of the metal removal six months after osteosynthesis. Methode In der laufenden Studie wurden bislang 55 von 88 Patienten, welche von 1999 bis 2009 wegen eines ossären Strecksehnenausrisses operativ versorgt wurden, klinisch und radiologisch nach durchschnittlich 8.3 (2.7-13.6) Jahren nachkontrolliert. Es wurde das Bewegungsausmass, Kraft und Röntgen der Verletzten, wie auch der Gegenseite untersucht. Ausserdem wurden die Patienten zu ihrer Zufriedenheit mit dem Resultat befragt. Drei verschiedene Operationstechniken wurden angewandt; 26 Schraubenosteosynthesen, 17 Kirschner-Draht-Osteosynthesen, 11 Mal eine Naht mittels Lengemann-Ausziehdraht. Ergebnisse Results: After six weeks, the group which had been treated with an adhesion barrier showed a significantly better ROM and DASH score. After six months, both groups showed practically identical measurements. Ein Patient entwickelte die Maximalvariante eines CRPS der Hand und des gesamten Unterarmes, was zu einer Exartikulation im Ellbogengelenk führte. Proben während der folgenden Handgelenksarthroskopie und Osteosynthesematerialentfernung konnte mikrobiologisch die Diagnose eines Handgelenksinfektes nicht sicher bestätigt werden. Wir präsentieren den radiologisch und klinisch gut dokumentierten Verlauf dieser seltenen Komplikation bei einem Routineeingriff, sowie einen Überblick über die aktuelle Literatur zum Thema Infektionen nach Osteosynthesen am Karpus, Arthroskopie bei Handgelenksinfektionen und Therapieoptionen bei (Low-grade) Schlussfolgerung Abgesehen von dem einen ausgeprägten CRPS, was eine Unterarmamputation zur Folge hatte, waren nur 6 von 55 Fälle unbefriedigend, wobei von diesen 6 wiederum nur ein Patient den Finger im Alltag als störend empfand. Was das gute Resultat eventuell etwas trübt, ist die lange Periode der Nachkontrolle, wobei einige Patienten angaben, sich mit den Jahren an den einst störenden und schmerzhaften Finger gewöhnt zu haben. Scaphoidinfekt1.jpg Low-grade Infekt nach perkutaner Scaphoidverschraubung – Eine Fallvorstellung Björn Walter1, Abdul Rahman Jandali1 (1Winterthur) Der Fall eines 23-jährigen Patienten, der mit einer undislozierten Scaphoidfraktur in unserer Klinik vorstellig wurde, überraschte uns mit einer seltenen Komplikation. Nach perkutaner Osteosynthese konnte zunächst ein unkomplizierter Verlauf beobachtet werden. Sechs Wochen nach Versorgung wurde in einer Computertomographie die Konsolidation bestätigt. Zu diesem Zeitpunkt bemerkte der Patient eine Schwellung und leichte Schmerzhaftigkeit des Handgelenkes. Unter dem Verdacht einer Infektion erfolgte sonographisch gesteuert eine Punktion ohne Keimnachweis im Punktat und eine empirische Antibiotikagabe. Im weiteren Verlauf kam es zunächst zur deutlichen Besserung und vollen Arbeitsfähigkeit, dann aber wieder zu Schmerzen und Schwellung und nun auch radiologisch deutlichen Zeichen einer beginnenden Lyse um die HCS-Schraube sowie einer beginnenden Gelenksflächendestruktion und SL-BandLockerung. Although the number of patients in the present study is small, our results suggest that dynamic repair of SLIL instability is an effective treatment option leading to near total pain relief and high satisfaction among patients. FM40 Scapholunate interosseous ligament (SLIL) injuries are the most common wrist ligament injury, leading to instability of the wrist. Treatment of SLIL injuries remains challenging. Several options for surgical treatment using dynamic correction have been described, yet a study describing follow-up results does not exist. This study presents the results of a novel operative technique using a partial extensor carpi radialis longus (ECRL) transfer and an interponat of free palmaris longus (PL) tendon to the distal pole of the scaphoid for the treatment of symptomatic scapholunate instability. Optimized food packaging makes life easier for patients with hand disorders Stefanie Hensler1, Daniel Herren1, Miriam Marks1 (1Zürich) Introduction Opening packaged food is a complex daily activity carried out worldwide. This process is often associated with difficulties or even an inability to open the packaging. About 40% of people between 62 and 82 years, and up to 78% of patients with hand disorders, have difficulties opening peelable packaging or are unable to do so1,2. The aim of this cross-sectional study is to investigate the possibility of producing easy-toopen meat packaging that provides high consumer satisfaction for patients with hand disorders. Furthermore we evaluated whether patients were significantly more satisfied with the optimized peelable packaging than with the old-style packs. Material and methods. Eight consecutive patients treated for symptomatic scapholunate instability were retrospectively reviewed. Partial ECRL transfer and an interponat of free PL tendon to the distal pole of the scaphoid was performed to correct the palmar tilt of the scaphoid. Clinical and radiographic examinations in addition outcome measures including Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaire score, Green & O’Brien score and Visual Analogue Scale were concluded to determine outcome. Methods Results. One hundred patients with osteoarthritis or rheumatoid arthritis of the hands opened the old, currently available in supermarkets, and a new meat packaging, which was optimized to improve those parameters causing problems in opening1. A questionnaire about customer satisfaction was completed by patients. Subjective difficulties perceived during opening process were also recorded on a six-point Likert scale with higher values indicating a better rating. The average age at time of surgery was 46 years. The mean follow-up period was 19 months. Surgery was completed an average of 26 months from injury or diagnosis. In five cases the dominant hand was operated on. All patients achieved total or near total pain relief. Pain averaged at time of follow-up on the VAS was 1.5 out of 10. DASH questionnaire score averaged 7.1 and Green & O’Brien score 90.6, indicating no wrist impairment. Postoperative range of motion in flexion and extension compared to the uninjured side was equal, 119° versus 121°. Scapholunate angle measurements obtained for all patients were preoperatively 72 degrees and postoperatively 58 degrees. Postoperative grip strength was slightly limited, 91.1% on average on the injured side. A gliding of the ECRL on ultrasonography was seen in all cases. Seven patients returned to their Results The results showed that patients were significantly more satisfied with the new, optimized meat packages than with the old-style packs (p=0.000). Moreover, 96% of the optimized packages were opened by patients. Regarding tear tab size, tear tab palpability, and subjective required force, the old packaging achieved low mean scores of 2.8, 2.8 and 3.2, 26 26 respectively. In 2 patients callus distraction was carried out on an average of 6 months (range 1-11 months) after wound closure following amputation and in the remaining two patients lengthening was started on an average of 14 months (range: 12-16 months) after unsuccessfull replantation. Callus distraction was performed with a rate of 0,75-1 mm lengthening/day using the external ring fixator system. Conclusion Our study provides evidence that patients are significantly more satisfied with the optimized meat packaging than with the old-style packs. More than 95% of the new packages can be opened by patients, which value is considerably higher, than the recommended values from the packaging industry3. If patients with hand disorders are satisfied with the optimized packaging, we can assume that healthy people will also be happy with it. We have shown that it is possible for manufacturers today to produce easyto-open food packages that afford greater consumer satisfaction and would, in turn, enhance brand loyalty. Results: The mean lengthening was 26,5mm (range: 23-30mm). The mean healing index (number of months per centimetre of lengthening) was 1,2 month/cm (range 1,0-1,4 months). Complications were noted in 2 patients. One patient had a superficial pin-track infection, treated conservatively with antibiotics. The other patient had a failure of the distraction apparatus at its weakest point due to an accident. We did not observe any loss of the axis. This study was sponsored by Coop (Switzerland) and the meat packaging was provided by Bell (Switzerland). Conclusion: Continuous distraction lengthening with an open ring fixation system at the level of the proximal phalanx of the thumb is an effective reconstruction method to gain length of a short thumb. The axis and the length of the distraction can be satisfactory controlled and adapted to the patient’s needs. This technique is an alternative when a toe to thumb transfer is no option. 1. The best treatment in these cases depends on the location of the amputation, the patient’s age, sex, occupation and functional demands. We evaluated the results of proximal phalanx lengthening using an external ring fixator device. This prototype was developed at our clinic based on the principles of the Ilizarov external fixator. Es existieren verschiedene chirurgische Ansatzpunkte; von der trapezio-metacarpalen Arthrodese über die Trapezektomie mit verschiedenen Aufhängeplastiken bis hin zum künstlichen Gelenkersatz. Diese Studie präsentiert die Langzeitresultate nach 3-5 Jahren nach Distraktions-Arthroplastie des trapezio-metacarpalen Gelenks, welche von Bufalini 2007 erstmals beschrieben wurde. Methods: Between November 2010 and May 2013, we performed four distraction lengthening procedures in 4 patients. All patients were male with a mean age of 37 years (range: 24-53 years). Aufgezeichnet werden Langzeitresultate (zwischen 3-5 Jahre postoperativ) der funktionellen Parameter (max. radiale Abduktion, Opposition, Kraftentwicklung im Spitz- und Schlüsselgriff und Faustschluss), subjektive Einschätzung des Patienten (DASH-Score, Schmerzempfinden mittels VAS) und die radiologische Evaluation des Abstandes zwischen Metacarpale I und Trapezium. das phalangeale Grundglied osteotomiert werden. Längsschnittstudien haben gezeigt, dass Reoperationen in 40 bis 56 % der Fälle notwendig sind (1). Am Kinderspital Zürich wurden seit Oktober 2003 40 Doppeldaumen primär operiert. In unserer Fallserie dominierte das männliche Geschlecht (75% Knaben) und die reche Hand (60% Daumen rechts). Bei über der Hälfte der Fälle handelte es sich um Doppeldaumen Wassel IV. Am zweithäufigsten wurden operative Korrekturen von Doppeldaumen Wassel II durchgeführt (20%) und bei 12.5% handelte es sich um Doppeldaumen Wassel VII. Die Kinder wurden primär im Alter von 7 bis 62 Monaten (median 12 Monate) operiert. Die Korrekturen erfolgten entweder nach Bilhaut-Cloquet, oder es wurden der hypoplastische Doppelpartner reseziert und die Weichteile augmentiert. Alle Patienten wurden klinisch und radiologisch nachkontrolliert und fotodokumentiert. Von diesen 40 Patienten ist es bei zwei zu einem Wundinfekt und bei einem zu einem Narbenneurinom gekommen. Bei zwei Patienten mussten die Indikationen zu Sekundäreingriffen gestellt werden, einmal aufgrund eines kosmetisch unbefriedigenden Nagels nach Bilhaut-Cloquet Operation, einmal wegen sekundärer Achsendeviation. Im gleichen Zeitraum wurden bei fünf primär auswärts korrigierten Patienten Reoperationen im Alter von 4 6/12 bis 15 9/12 Jahren (median 8 1/12 Jahre) wegen Instabilität im Interphalangealgelenk durchgeführt. Ergebnisse In 8 Fällen waren die Patienten 2 Monate postoperativ beschwerdefrei, zufrieden und wieder voll arbeitsfähig. Eine Patientin zeigte einen protrahierten Verlauf mit Schmerzen und Bewegungseinschränkung sowie eine Tendovaginitis de Quervain, welche nach Steroidinfiltration und intensiver Ergotherapie im Verlauf von 10 Monaten besserte. Eine Patientin wurde aus der Studie ausgeschlossen, da es zu einer Insuffizienz der Arthroplastie kam, worauf eine Trapezektomie und zusätzlich Aufhängeplastik mittels Abduktor policis longus-Sehne (Operation nach Lundborg) durchgeführt wurde. Die vollständige Auswertung der Langzeitresultate (3-5 Jahre postoperativ) wird bis zum SGH Kongress 2013 definitiv vorliegen. Die primäre Korrektur von Doppeldaumen um den ersten Geburtstag ergibt meist funktionell und kosmetisch befriedigende Resultate, falls sowohl die Korrektur der skelettären und ligamentären Elemente sowie die Weichteilbalancierung primär durchgeführt werden. Aufgrund der inhärenten Daumenhypoplasie muss aber trotzdem im Adoleszentenalter mit Überlastungen vor allem der Interphalangealgelenke gerechnet werden. Schlussfolgerung Die Resultate 2 Monate nach DistraktionsArthroplastik nach Bufalini waren zufriedenstellend. Es wurde in den meisten Fällen eine Beschwerdefreiheit mit gutem funktionellen Ergebnis erreicht und die für eine weitere Arthroseprogredienz ungünstige Subluxationsstellung korrigiert. Die Operationstechnik erlaubt das Belassen des Trapeziums. Bei Insuffizienz der Arthroplastie und Beschwerden kann sekundär eine Trapezektomie immer noch durchgeführt werden. (1) Townsend DJ et al. L’IRM est d’une grande aide diagnostique. Le traitement chirurgical doit être extensif pour améliorer la mobilité et limiter les récidives. Cas clinique : Un homme de 43 ans droitier , informaticien, présente depuis 1 an, sans antécédent traumatique significatif, une augmentation progressive du volume de l’IPP de l’index gauche, avec limitation indolore de la mobilité. L’examen clinique objective une masse palpable, ferme, du bord radial du col de P1 au bord ulnaire de la face palmaire de l’IPP. La radiographie montre des calcifications multiples centrées sur l’IPP avec une érosion corticale palmaire au col de P1. Klinisch zeigt sich eine Kraftminderung links im Seitenvergleich um 12 kg, kein Hofmann-Tinel über Karpalkanal, Loge de Guyon und Sulcus ulnaris, leichte Atrophie der intrinsischen Handmuskulatur, 2PD 5 mm an allen Fingerkuppen. 29 29 Neurologisch besteht eine „fortgeschrittene axonale Schädigung von N. medianus und N. ulnaris links im Unterarm- und Handgelenksbereich. Keine eindeutige Lokalisation möglich – eine chirurgische Dekompression wird empfohlen.“ entweder direkter Anastomose oder die Rekonstruktion mittels Veneninterponat zur Verfügung. Wir präsentieren eine retrospektive Studie mit 17 Patienten, welche wir 1995-2013 behandelt haben. Klinische und angiologische Nachkontrollen erfolgten zwischen 1 und 18 Jahren nach Therapieabschluss. Ergebnisse: Nach Abwägung der Klinik, möglicher Differentialdiagnosen und Literaturrechereche wurde bei dem Patienten ein Post-Polio-Syndrom diagnostiziert und keine Operation durchgeführt. Methode: Die 17 Patienten mit einem Aneurysma der A.ulnaris wiesen ein Durchschnittsalter von 49 Jahren (31-67 Jahre)auf. 9 Patienten wurden chirurgisch versorgt, 5 erhielten nach der Resektion des Aneurysmas eine Direktnaht, 4 Patienten ein Veneninterponat. Bei 8 Patienten wurde eine konservative Therapie mit Ilomedininfusionen zwei Mal wöchentlich über 2 Monate, durchgeführt. Die Nachkontrolle der Patienten erfolgte im Durchschnitt nach 5,5 Jahren (9 Monate -18 Jahre). Arbeitsunfähigkeit, Berufswechsel, Schmerzsymptomatik und die Durchgängigkeit der A. ulnaris mittels Sonographie wurden evaluiert. Es handelt sich hierbei um eine Postpoliomyelitische progressive spinale Muskelatrophie als Zweiterkrankung mindestens 15, meist 35-50 Jahre nach Erstinfektion. Die Inzident wird auf bis zu 70% (!) aller Erstinfektionen geschätzt. Einschlusskriterien des von Dalakas erst 1995 ausführlich beschriebenen Krankheitsbildes sind PolioAnamnese, partielle oder komplette Erholung der Paresen (stabile Periode minimal 15 Jahre, meist zwischen 35 und 50 Jahre), residuelle asymmetrische Muskelatrophie / - schwächen, Ermüdbarkeit und Schmerz, ggf. Areflexie, aber normale Sensibilität und Sphinkerfunktion. Ergebnis: Bei den 9 chirurgisch versorgten Patienten war in 3 Fällen die A. ulnaris verschlossen, diese waren mit einem Veneninterponat versorgt worden. Bei den mit einer Direktnaht der A. ulnaris versorgten war die A.ulnaris durchgängig. Die 8 Patienten nach IlomedinTherapie wiesen in 7 Fällen einen Verschluss der A. ulnaris auf. Bei den chirurgisch versorgten Patienten lag eine durchschnittliche Arbeitsunfähigkeit von 14 Tagen (13-15 Tage) bei den Ilomedin-Patienten von 16 Tagen (0-16) vor. Keiner der Patienten hat nach Therapieende den Beruf gewechselt. Therapeutisch ist keine kausale Therapie oder spezifische Medikation bekannt, Operationen sind kontraindiziert. Empfohlen wird eine Schonung geschwächter Muskeln, zeitweise Orthesen / Rollstuhl oder orthopädische Hilfsmittel zu benutzen und nicht ermüdende Muskelübungen, evt. Psychotherapie mit Info-Angebot sowie Strategien zur Vermeidung von Überlastung. Schlussfolgerung: Das Post-Polio-Syndrom ist eine wichtige, möglicherweise nicht selten übersehene Differentialdiagnose zu häufigen Pathologien in der Handchirurgie, wie z. B. Nervenkompressionssyndromen. Die Relevanz dieser Ausschlussdiagnose wird bei langer Latenz in Zukunft wahrscheinlich zunehmen. Operative Eingriffe sind nicht indiziert. Zusammenfassung: Im Vergleich zu den Patienten mit Ilomedin-Therapie zeigten die chirurgisch versorgten Patienten mit einer Direktnaht ein besseres Resultat bezüglich der Durchgängigkeit der A.ulnaris, was als klarer Vorteil für die chirurgische Therapie gesehen werden kann, wobei die Resultate nicht mit einer in der Literatur auch beschriebenen Ligatur der A.ulnaris verglichen worden sind. Those patients underwent all a full-thickness skin transplantation on the hand with a graft taken from the groin (15 pts.) or retroauricular region (11 pts.) between 2001 and 2011 at the University Children’s Hospital Zurich. All patients were examined in the outpatient clinic. Pigmentation and vascularization of both, the skin graft and the surrounding skin of the hand were evaluated with the Pantone Skin Tone Guide. Patient and parent satisfaction as well as the Patient and Observer Scar Assessment Scale (POSAS) were assessed with a questionnaire. Results Changing the donor site from the groin to the retroauricular region significantly improved the pigmentation match between grafts and surrounding skin of the hand, additionally there was a clear advantage regarding the observer’s evaluation of pigmentation, thickness, relief and opinion on the recipient site. Parents’ opinion on the donor site as well as their satisfaction profited significantly from the change of technique. Furthermore, significant correlations between parents’ satisfaction and the findings for the recipient site as well as for the donor site were found. Patients’ satisfaction improved significantly with the time of postoperative follow-up in both patient groups. The cross-sectional study design and limited patient numbers are limitations of the study. Conclusion Our results suggest that the use of retroauricular instead of inguinal skin grafts improves the surgical outcome and parents’ satisfaction significantly. Wird dann von ärtzlicher Seite eine midkarpale Instabilität diagnostiziert, wird den KlientInnen vor dem operativen Eingriff häufig eine konservative Therapie angeboten. « L’attelle baïonnette » est une possibilité de traiter plus durablement l’instabilité médio-carpienne de façon conservatrice. Die "Bajonettschiene" stellt nun eine Möglichkeit dar, die midkarpale Instabilität konservativ nachhaltiger zu behandeln. Zudem kann die Schiene bei Ruheschmerzen auch schmerzlindernd wirken. In der Literatur wird sie als eine Drei-Punkt-Schiene mit palmarem Druck auf das Os Pisiforme beschrieben. Sowohl karpale Instabilitäten, als auch Instabilitäten des distalen Radioulnar-Gelenks können die Lebensqualität der Betroffenen und deren Handfunktion in Beruf und Freizeit signifikant beeinträchtigen. Il y a suffisamment d’évidence pour l’efficacité d’un entraînement proprioceptif comme celui-ci, effectué après une blessure capsulo-ligamentaire de l’articulation de la cheville, par exemple. Par contre, bien que ces articulations se ressemblent anatomiquement, les connaissances de la proprioception du poignet sont relativement limitées. La recherche à ce sujet s’est développée ces dernières années, et les résultats actuels démontrent que, pour le poignet aussi, les réflexes proprioceptifs sont d’une importance capitale pour la stabilité de l’articulation. Lors d’une recherche dans la littérature et les études, j’ai rassemblé les résultats actuels. Il ressort de l’interprétation de ces nouvelles connaissances des propositions de traitement concrètes et de nouvelles approches thérapeutiques qui s’intègrent facilement dans la pratique actuelle. Die Therapie der Wahl allgemein bei Gelenksinstabilitäten ist ein gezieltes Kraft- und Propriozeptionstraining. In der Physiotherapie versucht man durch den Aufbau der gelenksstabilisierenden Muskulatur und durch sensomotorische Stimulation der entsprechenden Rezeptoren im Kapsel-Band-Apparat die neuromuskuläre Kontrolle des betroffenen Gelenks wiederherzustellen und somit das Gelenk zu stabilisieren. Für die Effektivität eines solchen Propriozeptionstrainings, zum Beispiel in Folge von Kapsel-Band-Verletzungen des Sprunggelenks, gibt es ausreichend Evidenz. Im Gegensatz dazu, auch wenn sich die beiden Gelenke anatomisch gesehen sehr ähnlich sind, ist das Wissen über sie Propriozeption im Handgelenk relativ beschränkt. Die Forschung diesbezüglich hat in den letzten Jahren deutlich zugenommen und aktuelle Studienergebnisse deuten darauf hin, dass auch im Handgelenk propriozeptive Reflexe von entscheidender Bedeutung für die Gelenksstabilität sind. In einer Literatur – und Studienrecherche habe ich die bisherigen Forschungsergebnisse zusammengetragen. Die flexible und dreidimensionale Anpassungsfähigkeit der proximalen Carpusreihe zwischen distaler Carpusreihe und Radius mit Ulna stellt eine notwendige Voraussetzung für das Aufspannen des Gewölbes der Mittelhand dar. It is associated with functions such as automated movements, measure, feedback and correction. Robotized devices may provide repetitive, precise and totally instrumented mobilization of the limbs. They may offer force feedback and hence various haptic viscoelastic interaction types. Robotics is now increasingly used in rehabilitation therapies. Motorized mobilization provides controlled movements and forces along with the possibility to implement various sensory motor feedback strategies. Furthermore, the intelligence of such devices also allows measurement, recording and assessment of the progress or of a particular treatment. Mit dem Tablet können sowohl Fotos von Händen und auch gleich Beschriftungen und ergänzende Zeichnungen beim Patienten vorgenommen werden, sowie weitere Informationen eines üblichen Befundes dokumentiert werden. Motivée par la recherche d’efficacité, d’adéquation et d’économicité (LAMal art. 32, conditions) et les possibilités électroniques s’améliorant constamment, la question se posa si l’on ne pourrait pas passer d’un recueil de données sur papier à un recueil de données sur ardoise tactile. Ceci permet un recueil de données directement auprès du patient et plus tard un remaniement à l’ordinateur. Avec l’ardoise tactile il est possible de prendre des photos des mains et de tout de suite ajouter des étiquetages et dessins complémentaires, ainsi que de noter toutes les données d’un recueil de données habituelle. Als Nachteil erachten wir, wenn mehrere Finger betroffen sind, dann ist je nach Konstellation das ICAM nicht anwendbar. Das Norwich Regime bietet eine gute Alternative als früh aktives Nachbehandlungs-Protokoll. Ein Vorteil des Norwich Regime ist, dass es gut bei Begleitverletzungen, wie z.B. Frakturen angewendet werden kann. Discussion : Dû au fait que ce formulaire n’est pas un logiciel d’application, les possibilités de présentations sont limitées. Bei wissenschaftlichen Beobachtungen ist eine Systematik notwendig, die einen Beobachtungsplan beinhaltet bezüglich was, von wem, wann und wo beobachtet wird. Zusätzlich braucht es eine klare Dokumentation der Beobachtung die nach Kriterien definierte Interpretation der Beobachtung zulässt. Alltagsbeobachtungen helfen während des gesamten Therapieprozesses, wahrzunehmen wie es dem Klienten und uns ergeht. Sie sind wichtig als Frühindikatoren um Hinweise zu geben, wo mehr Aufmerksamkeit benötigt wird. Wir brauchen in der Therapie beide Beobachtungsformen, das systematische, wissenschaftliche Beobachten um unsere Erfassung zu vervollständigen und auch unser Outcome zu dokumentieren, sowie Alltagsbeobachtungen, um zu sehen ob wir das Ziel verfolgen. En thérapie nous avons besoin des deux formes d’observation, l’observation systématique scientifique pour compléter notre recueil de données et pour documenter notre résultat, et l’observation quotidienne pour voir si nous poursuivons bien notre but. Cet exposé a pour but de donner un aperçu sur les différentes formes d’observation et les instruments d’évaluation pour la réinsertion professionnelle. Methode: Die APA ist ein Screening-Instrument zur Erkennung von Risikofaktoren am Arbeitsplatz. Das Instrument analysiert den Arbeitsplatz, die zu hantierenden Lasten, Arbeitshaltungen, Bewegungen und lokale Belastungen, die Exposition von Licht, Temperatur, Lärm und verschiedene psychosozialen Belastungsfaktoren. Im Fallbeispiel wird auf die veränderten Arbeitshaltungen, Bewegungen und lokalen Belastungen eingegangen sowie die Möglichkeiten und Grenzen aufgezeigt die Arbeitsmittel den neuen motorischen und sensorischen Gegebenheiten anzupassen. Resultate: Aus der Analyse am Arbeitsplatz wurden nötige Verhaltensänderungen beim Einsatz der rechten Hand formuliert und technische und organisatorische Massnahmen genannt und alles in einem Bericht mit Bildern dem Zuweiser und dem Klienten zugestellt. Aufgrund eines durch die SUVA gewünschten Follow ups – wurden nach der Analyse die Umsetzungen der Massnahmen überprüft. La réinsertion dans son emploi en tant qu’ouvrier communal s’est avérée très difficile même un an après l’accident. Après avoir exploité toutes les possibilités de la thérapie de la main, l’équipe d’ergonomie a proposé une clarification ergonomique (APA) [1]. Hintergrund und Zweck: Für die OutcomeMessungen bei RA werden in der Ergotherapie Messungen der Körperfunktionen, standardisierte Assessment der Handfunktion oder Fragebogen zur Selbsteinschätzung der Einschränkungen im Alltag verwendet. Der Zusammenhang zwischen diesen Assessments und der effektiven Performanz bei Aktivitäten des täglichen Lebens (ADL) ist jedoch unbekannt. Ziel dieser Studie war es diese Zusammenhänge zu eruieren. Méthode : l’APA est un instrument de dépistage pour reconnaître les facteurs de risque au lieu de travail. L’instrument analyse le lieu de travail, les poids à manier, les positions de travail, les mouvements et les charges locales, l’exposition à la lumière, à la température, au bruit et les différents facteurs psychosociaux. À l’aide de l’exemple du cas, les changements des positions et mouvements et les charges locales au travail sont démontrés ainsi que les possibilités et limites d’adaptation des outils à la nouvelle réalité motrice et sensorielle. Folgende Variablen der Körperfunktionen wurden erfasst: Grobkraft: Jamar; Beweglichkeit der Daumen: Kapandji-Index; Beweglichkeit der Langfinger: MCP-Ulnardeviation, Extensionsdefizit im MCP und Sperrdistanz; Beweglichkeit des Handgelenks: Flexion und Extension. Dexterität: SODA (Sequential Occupational Dexterity Assessment"-Test der bimanuellen Handfunktion bei ADL). Die Selbsteinschätzung der Einschränkungen im Alltag wurde mit dem QuickDASH-Fragebogen (Kurzversion des Fragebogens "Disability of Arm, Shoulder, Hand"), die Performanz bei ADL wurde mit einem Beobachtungsinstrument, dem AMPS („Assessment of Motor and Process Skills"), erhoben. Alle Datenerhebungen fanden am Wohnort der TeilnehmerInnen statt. Die Korrelationen zwischen den Assessments wurden über Spearmans nonparametrischen Korrelationskoeffizienten bestimmt. Résultats: Après l’analyse du lieu de travail les changements de comportement nécessaires par rapport à l’usage de la main droite furent formulés et les mesures techniques et organisationnelles furent nommées. Le tout fut envoyé dans un rapport avec images au référent et au client. L’application des mesures fut contrôlée après l’analyse suite à une demande de suivi de la SUVA. Grâce aux adaptations et changements de comportement, le patient fut capable de reprendre sa tâche à 100% dans presque tous les domaines du travail communal. Résumé: Après une longue période d’inaptitude au travail une clarification détaillée du lieu de travail sous la forme d’un APA peut objectiver le rapport d’une limitation fonctionnelle au niveau de l’activité et de la participation et des mesures utiles peuvent être mises en place à des niveaux différents pour ainsi soutenir le retour au travail habituel. Diese Studie zeigte zudem den Unterschied zwischen Leistungsfähigkeit (SODA) und Leistung (AMPS) gemäss ICF auf. Zudem scheinen subjektive und beobachtungsbasierte Messung der ADLBetätigungsperformanz verschiedene Aspekte der ADL-Performanz zu zeigen. Die Ergebnisse dieser Studie können ErgotherapeutInnen dabei helfen den Fokus einer Behandlung und die entsprechenden Assessments auszuwählen. This session will present and analyze pathology of some overuse problems affecting the hand. Today we see a number of “new” and sometimes odd diagnoses like Colonoscopist’s thumb, Ticketcollector’s thumb, Baby wrist, Mouse hand, Mouse thumb, BlackBerry thumb, Teen texting tendonitis, but also other well known, diagnoses that still can be tricky to handle and cure like Trigger thumb/fingers, Gamekeeper’s thumb, wrist ganglion, and general or specific hypermobility or hyperextensibility causing pain. Some of these overuse syndromes can be caused by a trauma which happened many years ago which can be hard to track but important to analyze to handle the syndrome correct. These syndromes can cause long lasting pain and weakness not in proportion to the primary injury. Hypermobility in general and especially among teens has increased over the last twenty years. There’s an increased incidence of musclutoskeletal injury in this 56 56 group and it’s known that hypermobility can cause diffuse muscleskeletal pain and injuries with no specific inciting event. This hypermobility seam to be a problem we as handtherapists will have to deal with more often in the future not only among adults but among kids and teens too. To make sure that there is as good dynamic stability and to work in normal movement patterns with respect for the present biomechanical condition is essential for optimal balance in the hand. It is important that the loading and the stress to which we expose the thumb, fingers and wrist are in phase with the reparative healing processes needed to withstand the stress that’s put on the tissues. For a correct analyze of each patients specific problems we need to know and recognize the biomechanical conditions we are dealing with. To counteract overuse problems we have to know not only the biomechanics of the hand but of the entire arm and the circumstances around the person we are dealing with.
