 Necessary documents to sign up for French health insurance To apply for and benefit from French health insurance coverage, you must visit a branch of the Caisse Primaire d'Assurance Maladie (CPAM) and present the following documents: ‐

Proof of identity. Your valid residence permit. Proof of residence (preferably dated within the past 3 months). Your first payslip, as well as your 3 most recent payslips. Your birth certificate, including the names of your father and mother, your birth date and location, and your gender. A birth certificate is also required for each family member applying for coverage. IMPORTANT: each birth certificate must be translated in France by a sworn translator

Translations from outside France, including translations by a sworn translator certified by a French embassy or consulate, are not accepted. However, countries signatory to Convention no. 16 of the International Commission on Civil Status can provide multilingual birth certificates, thereby avoiding the need for translation. Following is a list of this convention’s signatory countries: Bosnia and Herzegovina, Croatia, France, Germany, Greece, Italy, Lithuania, Luxembourg, Austria, Belgium, Moldavia, Montenegro, Poland, Portugal, Serbia, Slovenia, Spain, Switzerland, the former Yugoslav Republic of Macedonia, the Netherlands and Turkey. Birth certificate authenticity verification: In accordance with international law and unless otherwise agreed, foreign civil status documents and their official copies must be authenticated or, if need be, certified via “apostille” in order to be considered valid in France.
