Summary and Explanation of the Test Thyroid stimulating hormone (Thyrotropin, TSH) is a glycoprotein produced by the anterior pituitary gland which stimulates the production and secretion of thyroxine (T4) and triiodothyronine (T3) by the thyroid gland. Circulating free T 4 and free T3, in turn, regulate TSH secretion by a direct inhibitory action on the thyrotrophic cells of the pituitary. When thyroid secretion is too low, TSH secretion is stimulated; when thyroid secretion is too high, TSH secretion is inhibited. As a result, the circulating concentrations of T4 and T3 are normally maintained within narrow limits. This occurs not only in patients with obvious clinical disease but in patients with mild hypothyroidism as well. Elevated TSH may be found in patients with various diseases, although thyroid hormone concentrations are normal, indicating that elevation in TSH is the first measurable change that occurs when thyroid secretion declines1-4. Low or undetectable levels of TSH may be found in patients with hyperthyroidism. The major clinical value of TSH levels is in the diagnosis of primary hypothyroidism due to dysfunction within the thyroid. However, hypothyroidism may arise from a dysfunction in the pituitary or hypothalamus and TSH may be low or normal. In addition to basal serum TSH determinations, assessment of serum TSH after the intravenous administration of TRH (Thyrotropin Releasing Hormone) may provide useful clinical information5-8. It has been suggested that serum TSH is a more reliable tool than serum T4 and FTI (free thyroxine index) for the detection of thyroid disease in severely ill patients9. Principle of the Test In an immunoradiometric assay, or IRMA, labeled antibody is reacted with analyte in the presence of antibody bound to a solid support. IRMA is often referred to as "twosite IRMA" or "sandwich assay". The quantitation of the separated, radioactive antibody-analyte-solid phase antibody complex shows a direct relationship between the level of radioactivity and the concentration of analyte in the calibrator or patient sample. Reagents For In Vitro Diagnostic Use. 1. TSH [125I] Tracer, 125 I TRACER Catalog No. 06B264903, ready to use preparation of less than 10 µCi (370 kBq) [125I] TSH MAb (mouse) in buffer with bovine albumin, goat serum, mouse serum, dye, stabilizers, 0.1% sodium azide** and other preservatives. 25 mL. Number an appropriate number of antibody coated tubes for the standard curve and clinical samples. Add 200 µL TSH Calibrators and clinical samples to tubes according to the pipetting scheme shown in the outline which follows. Add 500 µL TSH Tracer Solution to all tubes. Vortex briefly. Cover tubes. Incubate at 37 1°C in a water bath for 3.0 hours. Remove all tubes from the water bath and uncover them. Aspirate or decant. Add 2.0 mL distilled water. Repeat step 7. Aspirate or decant a final time. Appropriate quality control material. ± Specimen Collection No special patient preparation is required. The laboratory should be advised of possible radioactivity in the sample, from previous diagnostic in vivo studies. 1. 2. 3. Preparation of Specimen for Analysis: Collect the blood in a 5 or 10 mL evacuated glass tube. If plasma is desired for analysis, EDTA may be used as the anticoagulant. Allow blood to clot at room temperature. Centrifuge and collect the serum or plasma. Serum should be separated within one hour after collection. Lipemic, hemolyzed or icteric samples should not be used. Storage: Serum may be stored at 2-8°C after collection for up to 7 days or stored frozen for up to 1 month. Repeated freezing and thawing should be avoided. After thawing, serum should be kept cold (+2 to +8°C) and assayed within 7 days. The standard curve may be plotted using the corrected cpm for each calibrator level on the y-axis versus the calibrator concentration on the x-axis using log-log graph paper. 4. TSH Calibrators have been calibrated for TSH against the Medical Research Council, World Health Organization, Standard WHO/MRC 80/558. Test calibrators in duplicate with each run. Calibration curves may be generated using manual or automated methods as described in the Calculation of Results section. When changing to new lot numbers of tubes or tracer, new calibration curves must be run. Do not intermix reagents from different lots within a run. Quality Control Assay at least two levels of quality control material (normal and abnormal) in duplicate in each run. If the results do not meet the laboratory's established criteria for acceptance, the data should be reviewed and the run repeated as necessary according to the laboratory's procedure manual. The standard curve covers the approximate range of 0.0 to 80 µU TSH per mL. Refer to the labels for exact concentrations of TSH. Calibrator A acts as a nonspecific binding correction for the TSH IRMA and may be used to correct for background counts and radioactive tracer trapped by or bound non-specifically to the tube. For patient samples which have TSH values greater than the Calibrator G value, dilute the sample with Calibrator A as follows: to 50 µL sample, add 400 µL of Calibrator A. Mix gently and use 200 µL of this diluted sample per assay tube. Read the results from the standard curve and multiply by the dilution factor nine (9) to obtain the correct value. A high dose hook effect has not been observed with TSH levels up to 500 µU/mL. Do not intermix reagents from different lots. If two vials of the same reagent are to be used in one assay run, the contents of the vials should be pooled and mixed before starting the procedure. The presence of radioactivity in a patient sample may interfere with the assay. In early pregnancy, the ranges of TSH normal results will overlap the hyperthyroid ranges; caution should be exercised in interpreting these results. This assay has not been validated for use in testing or screening neonates. As the radioactive tracer decays with age, increased counting times may be required, but this will probably be unnecessary with efficient equipment. Total pipetting time when sample and tracer are pipetted separately must be limited to two hours when pipetting manually and when using liquid handling systems in a sequential delivery mode. When using a liquid handling system which pipets sample and tracer simultaneously, the total pipetting time must be limited to one hour. 5 Sera from 132 apparently euthyroid individuals (65 male and 67 female) were analyzed for TSH with MP Biomedicals TSH SPCS reagents. The range represents a central 95% analysis. Normal Range Mean 0.54 - 3.70 µU/mL 1.53 µU/mL Each laboratory should, however, define its own normal range in a group of normal subjects. Sera from clinically diagnosed hyperthyroid (n = 66) and hypothyroid (n = 61) patients were evaluated in the MP Biomedicals TSH SPCS. Hyperthyroid Range: ≤ 0.06-0.42 µU/mL Hypothyroid Range: > 9.0 µU/mL Whenever laboratory data conflict with clinical findings or impressions, clinical judgement should be exercised and additional evaluation undertaken. Specific Performance Characteristics Clinical Performance: Correlation Study Three hundred and fifty five (355) human serum samples were assayed with the TSH SPCS assay and the TSH MAb SPCS assay. Zusammenfassung und Erläuterung des Tests TSH ist ein Glycoprotein, das von der Hypophyse produziert wird. Es stimuliert die Schilddrüse zur Produktion und Sekretion von Thyroxin (T4) und Trijodthyronin (T3). Zirkulierendes freies T3 (FT 3) und freies T4 (FT 4) wiederum reguliert über eine negative Rückkopplung die TSH-Sekretion, indem es direkt die thyreotropen Zellen der Hypophyse hemmt. Sinkt die Sekretion der Schilddrüse, wird die TSH-Sekretion stimuliert. Umgekehrt gilt, daß eine hohe Sekretionsrate der Schilddrüse die TSHFreisetzung hemmt. Dieser Regelkreis bewirkt, daß die zirkulierende Konzentration an T4 und T3 unter Normalbedingungen innerhalb von engen Grenzen konstant gehalten wird. Patienten mit einer primären Hypothyreose weisen erhöhte TSH-Konzentrationen auf. Dies trifft nicht nur für Patienten mit einem eindeutigen klinischen Bild zu, sondern auch bei einer leichten Hypothyreose. Erhöhte TSH-Konzentrationen lassen sich bei Patienten mit Erkrankungen verschiedener Genese nachweisen, obwohl die Schilddrüsenhormone im Normalbereich liegen. Die TSH-Erhöhung ist als erstes Indiz für eine herabgesetzte Schilddrüsenfunktion zu sehen.1-4 Niedrige oder nicht meßbare TSH-Werte können bei Patienten mit Hyperthyreose auftreten. Der wichtigste klinische Nutzen der TSH-Bestimmung besteht in der Diagnose einer primären Hypothyreose, der eine Fehlfunktion der Schilddrüse zugrunde liegt. Eine Hypothyreose kann jedoch auf eine Fehlfunktion der Hypophyse oder des Hypothalamus zurückzuführen und mit normalen oder niedrigen TSH-Werten verbunden sein. Zusätzlich zum basalen TSH-Wert kann deshalb eine Bestimmung des TSHSpiegels nach i.v. Gabe von TRH (Thyrotropin Releasing Hormon) von klinischer Bedeutung sein.5-8 Der TSH-Test wird zum Nachweis von Schilddrüsenerkrankungen bei schwerkranken Patienten als sicherere Methode angesehen, als die Bestimmung von T4 und FTI9 (Free Thyroxin Index). Testprinzip In einem immunoradiometrischen Assay (IRMA) läßt man den markierten Antikörper und das Antigen mit einem zweiten, fest an einen Träger gebundenen Antikörper reagieren. Oft wird der IRMA daher als "2-Lagen IRMA" oder als "Sandwich-Test" bezeichnet. Die Messung des radioaktiven "Antikörper-Antigen-2. Allerdings sollten sie dieser Temperatur nicht länger als unbedingt notwendig ausgesetzt werden. Für die Waschvorgänge wird destilliertes Wasser empfohlen. Tabelle 1 zeigt typische Werte. Seren von 132 gesundenen euthyreoten Individuen (65 Männer und 67 Frauen wurden mit dem MP Biomedicals TSH SPCS untersucht. Die angegebenen Werte repräsentieren den 95% -Bereich. Standards sollten in jedem Lauf in Doppelbestimmung gemessen werden. Zur Berechnung der Standard Kurven Können manuelle oder automatisierte Verfahren herangezogen werden, wie in Abschnitt "Berechnung der Ergebnisse" beschrieben. Bei Ver wendung einer neuen Röhrchen - oder Tracer - Charge muß eine neue Standard Kurve erstellt werden. Reagenziem aus verschiedenen Chargen dürfen nicht innerhalb eines Laufs gemischt werden. Werden Probe und Tracer separat pipettiert, darf die gesamte Pipettierzeit bei manueller Durchführung sowie bei sequentieller Pipettierung mit einer Pipettierstation zwei Stunden nicht überschreiten. Bei simultaner Pipettierung von Probe und Tracer mit einem Automaten darf eine Stunde nicht überschritten werden. Die Standard-Kurve deckt den Meßbereich von 0,0 bis 80 µU TSH pro ml ab. Beachten Sie die Etiketten für die genaue TSH Konzentration. Standard A wird für die unspezifische Bindung in dem TSH-IRMA eingesetzt. Im Falle von Patientenproben, die höhere TSH-Werte als der Standard G aufweisen, kann die Probe mit Standard A wie folgt verdünnt werden: auf 50 µl Probe 400 µl Standard A hinzugeben, leicht mischen und 200 µl dieser verdünnten Probe pro Teströhrchen einsetzen. Die Ergebnisse von der Standard-Kurve ablesen und mit dem Verdünnungsfaktor 9 multiplizieren, um den richtigen Wert zu erhalten. Bis 500 µU/mL TSH wurde Kein high dose hook - Effekt beobachtet. Explication sommaire du test La thyréostimuline (TSH) est une glycoprotéine produite par l'adénohypophyse, et qui stimule la production et la sécrétion de la thyroxine (T4) et de la triiodothyronine (T3) par la glande thyroïde. A leur tour, la T4 libre (T4L) et la T3 libre (T3L) circulantes modulent la sécrétion de TSH par une action inhibitrice directe sur les cellules thyréotropes de l'hypophyse. Lorsque la sécrétion thyroïdienne est trop faible, la sécrétion de TSH est stimulée; lorsque la sécrétion thyroïdienne est trop élevée, la sécrétion de TSH est inhibée. Il en résulte que les concentrations de T4 et de T3 circulantes sont en général maintenues dans des limites étroites. On trouve des taux de TSH élevés chez des malades à hypothyroïdie primaire. Ceci se rencontre non seulement chez des sujets présentant des signes cliniques évidents, mais également chez des malades souffrant d'hypothyroïdie bénigne. On peut constater des taux élevés de TSH chez des personnes souffrant de diverses maladies, bien que les concentrations en hormones thyroïdiennes soient normales, ce qui indique qu'une augmentation du taux de TSH est la première modification mesurable qui se produit lorsque la sécrétion thryoïdienne diminue1-4. On peut relever des taux faibles, voire indétectables de TSH chez des malades hyperthyroïdiens. L'intérêt clinique principal du dosage de la TSH réside dans le diagnostic des hypothyroïdies primaires dues à un dysfonctionnement au niveau de la thyroïde. Toutefois, l'hypothyroïdie peut provenir d'un disfonctionnement de l'hypophyse ou de l'hypothalamus, et le taux de TSH peut être faible ou normal. En complément des dosages de TSH sérique de base, une mesure du taux de TSH sérique après injection intraveineuse de TRH (Thyrotropin Releasing Hormone) peut fournir des renseigne-ments cliniques fort utiles. 5-8 Certains auteurs ont suggéré que la TSH sérique est un outil plus fiable que la T4 sérique et que l'Index de Thyroxine Libre (ITL) pour la détection d'une maladie thyroïdienne chez des sujets gravement malades. 9 Principe du test Dans un dosage immuno-radiométrique (IRMA), l'anticorps marqué réagit avec l'analyte en présence d'un anticorps fixé sur un support solide. On appelle couramment le dosage IRMA un dosage "IRMA à deux sites" ou un dosage "sandwich". Le laboratoire doit être prévenu si l'échantillon contient une certaine radioactivité provenant d'études diagnostiques in vivo. 1. 2. 3. Préparation de l'échantillon pour analyse: Prélever le sang dans un tube en verre sous vide de 5 ou 10 ml. Si on a besoin de plasma, on peut utiliser de l'EDTA comme anticoagulant. Laisser le sang former un caillot à température ambiante. Centrifuger et prélever le sérum ou le plasma. Le sérum doit être séparé dans l'heure qui suit le prélèvement et congelé jusqu'au moment du dosage. Ne pas utiliser d'échantillons lipémiques, hémolysés ou icterique. Stockage: On peut conserver les sérums à 2-8°C pendant 7 jours apìes le prélèvement; congeler l'échantillon si le dosage doit être effectué dans le mois suìvant le prélèvement. Se reporter à chaque flacon d' étalon pour connaître les concentrations de TSH. Calculs automatisés: Ous les réactifs et tous les échantillons doivent être amenés à température ambiante avant utilisation, mais ne pas les laisser à cette température plus longtemps que nécessaire. Il est conseillé d'utiliser de l'eau distillée pour le lavage des puits. On peut utiliser des techniques de réduction des données pour calculer les taux de TSH. Ajouter 200 µl de chaque étalon de TSH et 200 µl d'échantillon clinique dans les tubes comme indiqué dans le tableau ci-dessous. Ajouter 500 µl de solution de marquage de la TSH dans chaque tube. Agiter brièvement les tubes au Vortex. Recouvrir les tubes. ± Incuber au bain-marie à 37 1°C pendant 3,0 heures. Retirer tous les tubes du bain-marie et les découvrir. Aspirer ou décanter tous les tubes. Ajouter 2,0 ml d'eau distillée. Répéter l'étape 7. Aspirer ou décanter une derniére fois. Le sérum de 132 individus (65 hommes et 67 femmes) apparemment euthyroïdiens a été soumis à un dosage de la TSH avec le système MP Biomedicals TSH SPCS. La zone normale a été définie à partir de 95% des résultats centraux de cette population. On doit analyser les étalons en double lors de chaque série. Les courbes étalons peuvent étre obtenues par méthode manuelle ou automatique comme décrit dans le paragraphe Calcul des Résultats. Lors d' un changement de lot de tubes ou de traceur, une nouvelle courbe étalon doit être réalisée. Ne pas mélanger les réactifs de lots différents au cours d'ume même série. Tester au moins 2 miveaux de contrôle (normal et pathologique) en double par série. Si les résultats ne répondent pas aux critères établis par le laboratoire, les résultats peuvent être reuus et, si nécessaite, la série recommencée selon la procédure interne au laboratoire. Valeurs extrêmes Etant donné que le marqueur radioactif se désintègre en vieillissant, on peut être amené à augmenter le temps de comptage, mais ceci ne sera probablement pas nécessaire avec du matériel performant. Le temps total de pipettage lorsque les échantillons et le traceur sont pipettés séparément ne doit pas dépasser 2 heures en pipettage manuel ou avec un système de distribution automatique. Pour un système de distribution automatique simultanée des échantillons et du traceur, le temps total de pipettage doit ètre limité à 1 heur. La courbe-étalon englobe les taux compris entre environ 0,0 et 80 µU de TSH par ml. Se reporter aux étiquettes pour connaître les concentrations exactes de TSH. On peut utiliser l'étalon A pour corriger une fixation non spécifique pour le test IRMA TSH, et pour corriger les comptages dus spécifiquement au tube. Pour les échantillons sériques cliniques qui ont des taux de TSH supérieurs à celui de l'étalon, diluer le sérum avec l'étalon A comme suit: pour un échantillon de sérum de 50 µl, ajouter 400 µl l'étalon A. Mélanger doucement et utiliser 200 µl de cet échantillon dilué par tube. Lire les résultats sur la courbe étalon et multiplier par le facteur de dilution de 9 pour obtenir le taux réel. On n'observe pas d'effet crochet à haute dose jusqú à 500 µU/mL. Ne pas panacher de réactifs provenant de différents lots. Si deux flacons de réactif du même lot sont utilisés pour une série d'analyses, il convient de regrouper et de mélanger le contenu des deux flacons avant de commencer la procédure. La présence de radioactivité dans un échantillon clinique risque d'avoir une influence sur les résultats du dosage. Les valeurs normales indiquées risquent de ne pas être applicables pendant la grossesse. Calcolo manuale Procedimento di Dosaggio Portare i reagenti e i campioni a temperatura ambiente prima dell'uso. Al fine comunque di ridurre il deterioramento, evitare di mantenerli non refrigerati se non per il tempo necessario al loro uso. L'allestimento delle provette dei campioni e dei calibratori, così come dei controlli, deve essere effettuato simultaneamente. Per il lavaggio, si raccomanda l'uso di acqua distillata. 31 1. 2. 3. La presenza di radioattività nel campione può interferire sul dosaggio. Gli intervalli di normalità determinati possono non essere applicabili in gravidanza. Questo dosaggio non é stato convalidato per il dosaggio o lo screening neonatale. Valori Attesi Sono stati dosati sieri di 132 individui apparentemente eutiroidei (65 uomini e 67 donne) cora il kit TSH della MP Biomedicals. Poichè il tracciante radioattivo decade, possono rendersi necessari tempi superiori di conteggio, ma ciò non è probabilmente necessario se l'attrezzatura è efficiente. Il tempo totale di pipettamento quando campione e tracciante sono aggiunti separatamente deve essere limitato a 2 ore in caso di pipettamento manuale e in caso di pipettamento automatico a dispensazione sequenziale. Se si utilizza una stazione di pipettamento che dispensa campione e tracciante simultaneamente, il tempo di pipettamento deve essere limitato ad 1 ora. La curva standard copre il range approssimativo: 0 - 80 µU TSH/ml. Vedere le etichette dei flaconi per le concentrazioni esatte di TSH. Lo standard A rappresentà il legame non specifico del sistema TSH-IRMA e può essere usato per correggere i conteggi per la quota di radioattività di fondo e per quella quantità di tracciante assorbito o co munque legato non specifica-mente alle pareti della provetta. Per quei campioni che risultino a concentrazione superiore allo calibratori G, si consiglia di diluire con calibratori A come segue: aggiungere 400 µl di calibratori A a 50 µl di campione. Mescolare e dosare 200 µl del diluito. Leggere il valore sulla curva standard e moltiplicarlo per il fattore di diluizione 9, ottenendo così il valore corretto. 33 Trecentocinquantacinque (355) campioni di siero umano sono stati dosati con il kit TSH MAb. Para uso diagnóstico "in vitro" Resumen y explicación de la prueba 2. La hormona estimulante del tiroides (Tirotropina, TSH) es una glicoproteína producida por la glándula pituitaria anterior, que estimula la producción y secreción de tiroxina (T4) y Triyodotironina (T3) por la glándula tiroides. A su vez, la T4 libre circulante (FT 4) y T3 libre (FT 3) regulan la secreción de TSH por una acción inhibitoria sobre las células tirotióficas de la pituitaria. Cuando la secreción tiroidea es demasiado baja, se estimula la secreción de TSH; cuando es demasiado elevada, se inhibe. El resultado es que las concentraciones circulantes de T3 y T4 se mantienen dentro de límites normal-mente estrechos. 3. En pacientes con hipotiroidismo primario se encuentran concentraciones altas de TSH. Esto sucede no sólo en pacientes con enfermedad clínica obvia, sino también en casos de hipotiroidismo leve. Se pueden encontrar niveles elevados de TSH en pacientes con distintas enfermedades, aún con concentraciones normales de hormona tiroidea, lo que indica que el primer cambio apreciable cuando desciende la secreción tiroidea, es el aumento de TSH. 1-4 En pacientes con hipertiroidismo se pueden encontrar niveles bajos o no detectables de TSH. El valor clínico principal de los niveles de TSH es el diagnóstico del hipotiroidismo primario, debido a una disfunción interna del tiroides. Sin embargo puede aparecer hipotiroidismo como consecuencia de una disfunción en la pituitaria o el hipotálamo, siendo la TSH baja o normal. Si se prefiere analizar el plasma, puede utilizarse EDTA como anticoagulante. Dejar que la sangre se coagule a temperatura ambiente. Centrifugar y recoger el suero o plasma. El suero debe separarse en la hora siguiente a su obtención y se mantendrá congelado hasta el momento del análisis. No deben utilizarse muestras lipémicas, hemolizadas o ictéricas. Conservación: El suero debe se conservado a 2-8°C tras ser tomado durante 7 dias o congelado durante 1 mes. Evitar la congelación y descongelación repetida. El suero descongelado debe mantenerse en frío (2° a 8°C) y analizarse antes de 7 días. Envío de muestras: El suero, cuidadosamente envasado, se enviará y recibirá congelado. Procedimiento del ensayo Los sueros controles deben analizarse paralelamente a las muestras de pacientes. La curva patrón y las determinaciones clínicas deben realizarse simultáneamente. Todos los reactivos y muestras deben ponerse a temperatura ambiente antes de utilizarlos, pero no deben mantenerse a esa temperatura más tiempo del necesario. Agitar (Vortex) brevemente. Tapar los tubos. Incubar a 37 1°C en baño de agua durante 3,0 horas. Sacar todos los tubos del baño de agua y destaparlos. Aspirar o decantar. Añadir 2,0 ml de agua destilada. Repetir el paso 7. Aspirar o decantar una última vez. Contar la radiactividad en estos tubos y en los de recuento total (si fuera necesario) en un contador gamma, durante 1 minuto. ± Agua destilada. Cuando se cambia a números de lote nuevos de tubos o de trazador, se deben realizer nuevas curvas de calibración. No mezclar reactivos de lotes diferentes dentro de in ensayo. Control de Calidad Analizer al menos dos nireles de material de control de calidad (normal y anormal) en duplicado en cada ensayo. Si los resultados no encajan con el cuterio de aceptación establecido por el laboratorio, se deberian revisor los datos y repetir el ensayo si fuera necesario sequin el manual de procedimento del laboratorio. Cálculos manuales: 1. 2. 3. 4. Calcular cpm para los tubos 1-2. Calibrador A. Sustraer la cpm promedio para los tubos 1-2 de cada cpm en los tubos sucesivos para obtener la cpm corregida. Puede trazarse la curva patrón utilizando la cpm corregida para cada punto del calibrador sobre el eje-y frente a la concentración del calibrador en el eje-x, utilizando papel de gráficas log-log. La Figura 1 muestra una gráfica de ejemplo los datos se ofrecen únicamente a modo de ejemplo representativo y no deben utilizarse en lugar de la curva patrón obtenida con cada ensayo. Como el trazador radiactivo decae con el tiempo, podrán ser necesarios tiempos de recuento más prolongados, aunque un buen equipo resolverá seguramente ese problema. El tiempo total de pipeteado, cuando la muestra y el trazador sean pipeteados separadamente, debe de ser limitado a dos horas curando se pipetee manualmente y curando se utilizen sistemas de manipulatción liquidos en un modo secuencial. Cuando se use un sistema de manipulación liquids que pipetee la muestra y el trazador simultáneamente, el tiempo total de pipteado se deberá limitar a una hora. La curva patrón cubre aproximadamente desde 0,0 a 80 µU de TSH por ml. Consultar en las etiquetas las concentraciones exactas de TSH. El Estándar A actúa como corrección de fijación inespecífica para el IRMA TSH y se puede utilizar para corregir las cuentas de fondo y el trazador radiactivo fijado inespecíficamente al tubo. Para las muestras de suero de paciente con valores de TSH por encima del valor Calibrador G, se prediluye el suero con Calibrador A de la forma siguiente: Añadir 400 µl de Calibrador A a una muestra de suero de 50 µl. Mezclar cuidadosamente y usar 200 µl de esta muestra diluida por tubo del ensayo. Leer los resultados a partir de la curva patrón y multiplicar por el factor de dilución nueve 9 para obtener el valor correcto. No se la observado el efecto "high dose hook" con niveles de TSH de hasta 500 µU/mL. No mezclar reactivos de distintos lotes. Si se van a utilizar dos viales del mismo lote de reactivo en un ensayo, deben reunirse sus contenidos y mezclarlos antes de empezar. La presencia de radiactividad en una muestra de un paciente puede interferir con el ensayo. Los valores normales en uso pueden no ser aplicables durante el embarazo. Este ensayo no ha sido validado para utilizarse en análisis o screening de neonatos. Los calibradores de TSH lan sido calibrados par la TSH frente al estándar WHO/MRC 80/558 (consejo de Investigación Médica, Organización Mundial de al Salud. 41 42 Comparación de Regresión Linear de la ticnica TSH SPCS con la TSH MAb SPCS: Muestras < 6 µU/mL. Valores esperados Se analizaron los sueros de 132 individuos aparentemente eutiroideos (65 varones y 67 hambras) para las TSH con los reactivos TSH SPCS de MP Biomedicals.
