➤ L’objectif épidémiologique de ce projet repose sur le développement et l’utilisation d’une méthodologie de surveillance poussée et sur la conduite d’études cas-témoin et d’investigations d’épidémies. Les systèmes de données ont été conçus pour permettre un transfert de données entre les sites participants et pour faciliter le développement et la gestion globale du projet. Le recueil des données et les analyses de laboratoires au sein des différents volets vont être standardisés de manière à ce qu’elles puissent être compilées et comparées. Une base de données centrale, accessible sur internet, permettra de soutenir, coordonner et suivre le recueil de données de chaque site, de créer des séries de données pour analyse, et facilitera le transfert des informations relatives au projet aux partenaires nationaux et internationaux tels que Enternet et PulseNet. Les compétences en épidémiologie nécessaires pour réaliser ce projet seront assurées en offrant une formation et une expérience pratique via le Programme allemand de Formation à l’Epidémiologie de Terrain (German Field Epidemiology Training Programme - FETP) et le Programme Européen de Formation à l’Epidémiologie de Terrain (European Programme for Intervention Epidemiology Training – EPIET), et en collaborant avec les membres du service de santé public dans les divers volets du projet. ■ References ➤ Data collection and laboratory testing among the project components will be standardised so that data from each component can be combined and compared. Dix d’entre eux présentaient une diarrhée sanglante et six étaient porteurs asymptomatiques. A l’issue des interviews réalisées auprès de 11 des 16 cas, l’épidémie a été associée à une ferme alpine et à la consommation d’un fromage au lait de vache produit localement. Les analyses microbiologiques ont révélé la présence de S. Oranienburg dans le fromage et le poulailler. This paper presents the results of the epidemiological investigation of this outbreak. B Investigation From 10 August to 29 September 1999, 17 initial isolates of S. Oranienburg were cultured from human stool specimens; two of the initial isolates, found during official source screening, were from the same person (a mistake in spelling the surname). La première patiente était de Zams, et ses trois vaches avaient passé l’été dans les pâturages d’« Alpe Lorain » à Galtuer. Les dix autres patients avaient consommé du fromage au lait de vache provenant d’Alpe Lorain. Dans la ferme laitière alpine, le fromage était produit chaque jour avec le lait provenant des vaches ayant passé l’été à Alpe Lorain. Le lait du matin était utilisé tout de suite après la traite, et celui du soir était conservé au froid toute la nuit pour être traité le lendemain avec le lait frais du matin. Quatre personnes travaillaient dans cette fromagerie. Les cultures starter étaient ajoutées au lait non raffiné pendant 40 minutes à environ 32°C, puis le mélange était porté à 46°- 48°C. Le fromage était ensuite laissé dans une cave fraîche (environ 8°C) pendant cinq à six semaines pour maturation. Because of the concentration of cases in Galtuer, people were asked primarily about contacts with that village. The first patient was from Zams; her three cows spent the summer on the alpine pasture ‘Alpe Lorain’ in Galtuer. The other ten respondents had eaten cow‘s milk cheese from the Alpe Lorain. In the alpine dairy farm cow’s milk cheese was made daily using milk from cows that spend the summer months at the Alpe Lorain. The morning milk was used immediately and the evening milk was cooled overnight to be processed the next morning with the fresh morning milk. Four people worked in cheese production. The cultures were added to unrefined milk for 40 minutes at about 32°C, and then heated to 46°- 48°C. The cheese was put in a cold cellar for five to six weeks to ripen at about 8°C. Oranienburg, à savoir un fromage alpin de production locale. La transmission de salmonelles par le lait ou par le bétail est peu probable. En effet, aucune preuve d’infection du bétail n’a pu être mise en évidence. Il semble que l’infection des porcs soit due au fait qu’ils aient été nourris au petit lait et autres déchets contaminés issus de la production du fromage. Une relation avec une infection humaine paraît être exclue. Le fromage alpin est rarement responsable d’infections à Salmonella. Dans le cas présent, l’investigation épidémiologique de cette épidémie a été déclenchée par l’apparition d’un sérotype atypique, qui jusqu’alors n’avait pas été identifié chez l’homme au Tyrol. Les infections à S. Oranienburg sont rares chez l’homme. En 1992, une épidémie avec 11 cas était survenue dans le Minnesota (États-Unis) et avait été attribuée à un fromage mozzarella de fabrication industrielle (1). En 1999, une épidémie importante était survenue au Japon. Il semble qu’elle était due à de la poulpe séchée de fabrication industrielle (2). En mai 2000, le producteur de lait de Alpe Lorain a été inculpé pour infraction à la législation relative à l’alimentation. Il a été condamné à payer avec sursis une amende de 84 000 schillings autrichiens (6104,5 euros). Les conséquences possibles des procédures criminelles peuvent gêner les enquêtes épidémiologiques. Les investigations de la chaîne d’infection sont essentielles pour mettre en place des mesures préventives spécifiques et sont, de ce fait, indispensables d’un point de vue de santé publique. Discussion Thus, without performing a case control study and without using molecular methods to compare patients and food isolates, we identified locally produced alpine cheese epidemiologically and through culture as the vehicle of this outbreak of S. Oranienburg infection. Transmission of salmonella through milk or infected cattle is unlikely because we found no evidence that the cattle was infected. It appeared that the infection of the pigs was a result of their being fed with whey and other contaminated waste products from the cheese making process. They seemed to have had no relation with human infections. Alpine cheese is seldom found to cause salmonella infections, and in this case, the epidemiological outbreak investigation was triggered by the appearance of an atypical serotype, not previously found in humans in Tyrol. An outbreak of S. Oranienburg that affected 11 cases in Minnesota (United States) in 1992 was attributed to industrially produced mozzarella cheese (1). S. Oranienburg infections are rare in humans; a large outbreak in Japan in 1999 appeared to be due to industrially produced dried octopus (2). In May 2000, the alpine dairyman of the Alpe Lorain was charged with violating food legislation and given a suspended sentence of an 84 000 Austrian Schillings (6104.5 Euro) fine. The possible consequences of criminal proceedings may hinder epidemiological outbreak investigations. Investigating the chain of infection is essential to implement specific preventive measures and therefore indispensable in view of public health, but may carry unforeseen legal consequences for those involved. People with S. Oranienburg infection who refused to cooperate actively justified their refusal by saying the investigation might have negative consequences for the alpine dairy business. ■ 1.
