copies;
Please mail/courier back the above forms along with the followings:
 A photocopy of your Articles of Incorporation;
 A cheque payable to MINISTER OF FINANCE in the amount of $150.00 for the government
disbursement;
 A certified cheque or money order payable to Best Ontario Inc. in the amount of $155.00 plus HST
for service fee. Your payment to us also can be in cash, credit card, debit card, money order, certified
cheque, or Email Money Transfer. (If you would like to make the payments by credit card, please fill
out the bottom of the price list.)
 If you wish to change the address of the corporation or to add/remove/update corporation directors’
information, please check the information under Electronic Filing for Notice of Change Form.
 If you wish to obtain Restated Articles of Incorporation which consolidates all amendments and the
original Articles into one set of Articles with updated directors’ information, please check the
