I realize that, at times throughout my son/daughter’s school year, it
may become more expedient for him/her to be transported to and/or
from an extra curricular activity by private vehicle other than by taxi
or bus.
I give permission for my child to be transported throughout his/her
school year for said purpose by a student volunteer driver at times it is
deemed appropriate by the staff advisor.
I understand that each volunteer driver is required to complete the
necessary school form acknowledging, among other facts, third party
liability insurance coverage.
