Les anti-sérotoninergiques augmenteraient le risque d'hémorragie digestive en cas de prise isolée (risque relatif : 2,4) mais surtout concomitante d'AINS (RR : 6,3). 7. Dans les ectasies vasculaires antrales hémorragiques les résultats obtenus par coagulation au plasma argon sont aussi bons qu’il y ait ou non une cirrhose mais la difficulté est majorée s’il y a la présence d’une cirrhose. 10. Larssen L, Moger T, Bjornbeth BA et al. Transcatheter arterial embolization in the management of bleeding duodenal ulcers: a 5.5-year retrospective study of treatment and outcome. Scand. J. Dans cette étude rétrospective sur les ulcères duodénaux hémorragiques chez 36 patients en échec primaire (5 % de la population initiale) ou secondaire (20 %) l’embolisation permettait une hémostase définitive et un succès clinique dans respectivement 92 % et 72 % des cas. Dans les télangiectasies duodénales sporadiques d'autres localisations, jéjunales, iléales et coliques, sont présentes avec un RR de lésions du grêle de 10. 13. Un traitement anticoagulant et un volumineux polype accentuent le risque de saignement retardé après polypectomie dans cette série ou l'incidence était de 1 % et le délai moyen de survenue de 6 jours. 14. Une hémorragie digestive se produit chez 2,7 % des coronariens aigus traités par aspirine, clopidogrel et enoxaparine. Les facteurs prédictifs sont un antécédent d'ulcère et un choc cardiogénique. Les inhibiteurs de la pompe à protons réduisent le risque d'hémorragie. La mortalité est de 4,7 %. 16. Spaander MC, Murad SD, van Burren HR et al. Dans les hypertensions portales par bloc infra-hépatique le traitement endoscopique est aussi efficace qu’en cas de cirrhose. Une nouvelle étude comparant l’administration de lansoprazole par voie orale vs. veineuse et ne montrant pas de bénéfice de cette dernière sauf pour la rapidité d'obtention d'un pH élevé. 22. Chez les cirrhotiques présentant une rupture de varice œsophagienne un score de Meld supérieur ou égal à 18, des besoins transfusionnels dans les 24 premières h supérieurs à 4 unités ou une hémorragie active à l'endoscopie initiale sont des facteurs de décès à 6 semaines. 24. Au cours des syndromes coronariens aigus le risque d'hémorragie digestive est minime (0,7 %), y compris celui d’une forme sévère (0,2 %), et ne semble pas diminué par la prise d'IPP. Les antécédents d'ulcère, une insuffisance rénale et les prises d'AAP ou d'AINS accroissent ce risque. 31. Marmo R, Koch M, Cipolletta L et al. Ce travail italien sur 1.020 hémorragies digestives hautes montre un taux de récidive faible (3,2 %), une mortalité de 4,5 %, 1 fois sur 3 dans les 24 h et sur 10 du fait d'une co-morbidité. 32. Bosch J, Thabut D, Albillos A et al. Recombinant factor VIIa for variceal bleeding in patients with advanced cirrhosis: a randomized controlled trial. Hepatol. 2008; 47: 1604-14. Dans cet essai multicentrique européen le facteur VIIa n'a pas d'effet bénéfique au cours de l'hémorragie digestive du cirrhotique grave. 33. Fernandez-Esparrach G, Bordas JM, Pellisé M et al. A propos de 6 cas d'une complication rare après by-pass chirurgical gastrique. 34. Laine L, Long GL, Bakos GJ et al. Optimizing bipolar electrocoagulation for endoscopic hemostasis: assessment of factors influencing energy delivery and coagulation. Gastrointest. Endosc. 2008; 67: 502-8. Cette étude sur modèle animal laisse supposer qu'une technique optimale d'électrocoagulation bipolaire à visée hémostatique associe une basse puissance (15 Watts), une durée de contact longue (10 secondes) et une tamponnade du site hémorragique. 35. Yeomans N, Lanas A, Labenz J et al. Efficacy of esomeprazole (20 mg once daily) for reducing the risk of gastroduodenal ulcers associated with continuous use of low-dose aspirin. Am. J. Dans cet essai randomisé l'ésoméprazole à 20 milligrammes par jour réduit le risque de symptômes et de lésions digestives hautes chez les plus de 60 ans sans lésion initiale à l'endoscopie et recevant de l'aspirine à visée antiagrégante à des doses de 75 à 325 mg/j. 36. Takizawa K, Oda I, Gotoada T et al. Routine coagulation of visible vessels may prevent delayed bleeding after endoscopic submucosal dissection: an analysis of risk factors. Endosc. 2008; 40: 179-183. Ce travail rétrospectif montre qu'après dissection sous-muqueuse la coagulation préventive de vaisseaux visibles dans la zone de résection pourrait faire baisser le risque hémorragique dans les suites du geste. 37. Vassiliou I, Papadakis E, Arkadopoulos N et al. Gastrointestinal emergencies in cardiac surgery: a retrospective analysis of 3.724 consecutive patients from a single center. Cardiol. 2008; 31: 94-101. Après chirurgie cardiaque l'ischémie intestinale (n = 11) et les cholécystites (n = 8) sont plus fréquentes que les hémorragies digestives (n = 6). Une intervention en urgence et une fraction d'éjection basse sont des facteurs de risques de complication digestive. 38. Saini SD, Schoenfeld P, Fendrick et al. Cost-effectiveness of proton-pump inhibitor cotherapy in patients taking long-term, low-dose aspirin for secondary cardiovascular prevention. Arch. Intern. Med. 2008; 165: 1684-90. L'association IPP et aspirine à faible dose à visée préventive n'est d'un bon rapport coût/efficacité que s’il y a un risque élevé d'hémorragie digestive. 39. Rerknimitr R, Chanyaswad J, Kongkam P et al. Risk of bacteremia in bleeding and non-bleeding gastric varices after endoscopic injection of cyanoacrylate. Endosc. 2008; 40: 644-9. Après obturation des varices gastriques par colle le risque de bactériémie n'est significatif qu'en cas d'hémorragie active. Une antibioprophylaxie n'est donc indiquée que dans cette situation. 40. Seewald S, Ang TL, Imazu H et al. A standardized injection technique and regimen ensures success and safety of N-butyl-2-cyanoacrylate injection for the treatment of gastric fundal varices. Endosc. 2008; 40: 644-9. Une technique germano-égyptienne de traitement des varices gastriques du fundus 100% efficace et sans morbidité utilisant 0,5 millilitres de cyanoacrylate et 0,8 ml de lipiodol sans dépasser 1 ml par injection. On en redemande ! 41. Wells C, Harrison ME, Gurudu SR et al. Treatment of gastric antral vascular ectasia (watermelon stomach) with endoscopic band ligation. Gastrointest. Endosc. 2008; 68: 231-6. Cette étude préliminaire sur 9 sujets invite à penser que la ligature élastique serait un traitement efficace de l'estomac pastèque. 42. Herrera S, Bordas JM, Llach J et al. The beneficial effects of argon plasma coagulation in the management of different types of gastric vascular ectasia lesions in patients admitted for GI hemorrhage. Gastrointest. Endosc. 2008; 68: 440-6. Un nouveau travail confirmant l'efficacité de la coagulation au plasma argon dans le traitement des ectasies vasculaires gastriques hémorragiques. 43. Lo GH, Chen WC, Lin CK et al. Improved survival in patients receiving medical therapy as compared with banding ligation for the prevention of oesophageal variceal bleeding. Hepatol. 2008; 48: 580-7. Cet essai de prévention primaire de la rupture de VO montre, en terme d'incidence d'hémorragie, la supériorité de la ligature élastique sur un traitement médical associant nadolol et isosorbide. 44. Longacre AV, Imaeda A, Garcia-Tsao G et al. A pilot project examining the predicted preferences of patients and physicians in the primary prophylaxis of variceal hemorrhage. Hepatol. 2008; 47: 169-76. En prévention primaire de la rupture de VO les médecins, comme les malades, préfèrent la ligature élastique aux bêta-bloquants, mais pour rester conformes aux recommandations ils prescrivent ceux-ci… Trouvez l'erreur ! 45. Elmunzer BJ, Young SD, Inadomi JM et al. Systematic review of the predictors of recurrent hemorrhage after endoscopic hemostatic therapy for bleeding peptic ulcers. Am. J. Dans les ulcères peptiques le taux de récidive hémorragique est de 16,4 %, les facteurs de risque étant un saignement actif à l'endoscopie initiale, un gros ulcère et des localisations à la face postérieure du bulbe et à la petite courbure gastrique. 46. Poultsides GA, Kim CJ, Orlando III R et al. Angiographic embolization for gastroduodenal hemorrhage. Arch. Surg. 2008; 143: 457-61. Dans les hémorragies digestives hautes le taux de succès de l’embolisation s’il est de 94 % au niveau technique chute sur le plan clinique à 51 %, notamment en post-opératoire ou/et après transfusions de plus de 6 culots. Cette méthode est uniquement efficace si elle n'est pas initiée trop tardivement. 47. Andriulli A, Loperfido S, Focareta R et al. High- versus low-dose proton-pump inhibitors after endoscopic hemostasis in patients with peptic ulcer bleeding: a multicentre, randomized study. Am. J. Gastroenterol. 2008; 103: 3011-8. Cette étude multicentrique italienne portant sur 474 malades avec ulcère peptique hémorragique montre qu'après hémostase endoscopique efficace un bolus de 40 mg/j d'oméprazole pendant 3 j fait aussi bien qu'un bolus de 80 mg suivi d'une perfusion de 8 mg/h durant 3 j également. 48. Toy E, Rojany M, Sheikh R et al. Capsule endoscopy's impact on clinical management and outcomes: a single-center experience with 145 patients. Am. J. Gastroenterol. 2008; 103: 3022-8. Dans cette série homogène monocentrique la rentabilité de la capsule vidéo-endoscopique est de 70 %, mettant en lumière dans 25 % des cas des angiodysplasies, dans 13 % des ulcérations intestinales et des anomalies gastriques ou duodénales et dans 4 % des lésions tumorales. 49. Avec une incidence de 1,5 % la survenue au décours d'un accident neurologique ischémique d'une hémorragie digestive est peu fréquent mais augmente par 3 le risque de décès ou de dépendance ultérieure et de moitié le risque de décès à 6 mois. 57.
