The Mississauga Halton CCAC is the only organization in the region to coordinate the full range of health and social services that residents need. • In fiscal 2013/14 (April 1, 2013 to March 31, 2014), Mississauga Halton CCAC care coordinators designed personalized care plans and organized services for 43,582 patients , an increase of 5.9% from the previous fiscal year, in response to the higher health care needs of our older patients. • In fiscal 2013/14, the Mississauga Halton CCAC supported patients and families in the region, while keeping administration costs to 4.7% of total budget, and plant operations and IT at 4.4%. • The Mississauga Halton CCAC provides quality care to patients, in fiscal year 2013/14, in an independent provincial survey, 96.69% of patients and caregivers “would recommend our services to family and friends.” To support the shift from hospital-based community care to the community, which is what Ontarians want and is safer and more cost-effective, we are designing new programs to help meet the current and future needs of residents. In fiscal 2013/14, Mississauga Halton CCAC care coordinators designed personalized care plans and coordinated services for patients so they could leave hospital sooner, achieving an alternate level of care (ALC) rate of 9.1 per cent. That means 91 out of 100 hospital spaces were available for patients needing care. In fiscal 2013/14, the Mississauga Halton CCAC cared for 10.8% more patients at their end-of-life so they could fulfill their wish to die at home with dignity. • This past year the Mississauga Halton CCAC created a Share Care Council – a 15-member committee consisting of patients, substitute decision-makers and family caregivers – volunteers committed to improving home and community are. It’s a partnership that ensures a direct voice for patients and families to inform the development of new programs and services at the Mississauga Halton CCAC. Most recently, they initiated the creation of a Patient & Caregiver Bill of Rights.
