1.
I, the undersigned hereby give my consent and permission for the Rossy Cancer Network (RCN)
to film, videotape, record and/or photograph me and use my name and/or the photos and video
footage taken of me (the “Images”) for any use at any time in any publicity and/or promotion for
the RCN.
2.
RCN is hereby authorized to use or cause to be used said Images for advertising, publicity, or
other promotional purposes for the RCN, including the right to broadcast and/or transmit the
Images on television and other media.
3.
I hereby release the RCN, any of its associated or affiliated companies, their directors, officers,
agents and employees (the “RCN and Representatives”) from all claims of any kind on account
of such use.
4.
I understand that there will be no monetary compensation for the use and/or publication of the
Images.
5.
I agree to protect, defend, indemnify and hold harmless the RCN and Representatives from and
against all losses, damages, costs or expenses, or other claims made by third parties or liabilities
that may be claimed against the RCN and Representatives by reason of any actual or alleged
negligent or wrongful act or omission in connection with the use of the Images.
6.
For the purpose of all legal proceedings, this Consent shall be governed, construed and interpreted
according to the laws of the Province of Québec and federal laws of Canada applicable therein.
Each of the parties hereby attorns to the jurisdiction of the courts of the Province of Québec, and
the parties agree to and consent to the exclusive jurisdiction and venue of the courts in Montreal,
Québec.
7.
The undersigned has expressly requested that this Consent be drafted in English. Le soussigné a
expressément demandé que ce consentement soit rédigé en anglais.
8.
This Consent cannot be amended or modified except by a writing executed by an authorized
representative of RCN.
IN WITNESS WHEREOF, this Consent and Release has been executed and is effective this

1. Je, soussigné(e) donne mon consentement et la permission au Réseau de cancérologie Rossy
(RCR) de me filmer, m’enregistrer et/ou me photographier et d’utiliser ces photos, enregistrements et
séquences vidéo, pour lequel il est également permis de dévoiler mon nom (collectivement, les
« Images »).
2. Par les présentes, le RCR peut utiliser ou se servir des Images à des fins publicitaires et
promotionnelles, y compris le droit de diffuser les Images à la télévision ou de les transmettre par
quelconque moyen ou support.
3. Par les présentes, je donne quittance complète au RCR et ses compagnies associées ou affiliées,
leurs administrateurs, dirigeants, agents et employés (« RCR et Représentants ») et je renonce à
toute réclamation de toute nature liée à l’usage des Images.
4. Je comprends qu’aucune compensation pécuniaire ne sera donnée pour l’utilisation et la diffusion des
Images.
5. J’accepte de protéger, défendre et tenir indemne le RCR et Représentants de toute perte, dommage,
coût et dépense, de toute autre réclamation faite par toute tierce partie et de toute responsabilité
encourue par ou réclamée contre le RCR et Représentants en raison d’actes ou d’allégations de
négligence, de manquement ou omission liée à l’usage des Images.
6. Le présent consentement, son application et son interprétation sont régis par les lois de la province
de Québec et les lois du Canada afférentes. Les parties conviennent, pour toute poursuite judiciaire,
d’élire la juridiction du district judiciaire de la ville de Montréal, province de Québec, à l’exclusion de
tout autre district judiciaire.
7. Le présent consentement ne peut être modifié sans le consentement écrit d’un représentant dûment
autorisé du RCR.
