8. Autorisation médicale

enfant a un accident ou est malade durant son séjour, nous autorisons la famille hôtesse ou les
responsables locaux de l’organisme EDI à faire procéder à tous actes médicaux et chirurgicaux jugés
nécessaires. La famille hôtesse et les correspondants de l’organisme EDI ont tous pouvoirs en cas
d’urgence.
As parents of the participant, we do hereby authorise EDI and its foreign partners to consent to any medical surgical diagnosis, treatment or hospital care which is
advisable by and is rendered under the general supervision of a licensed physician or surgeon. We also agree that if the applicant becomes ill or incapacitated,
EDI may take such actions as it deems necessary including securing medical treatment . We release EDI from any liability related to such actions.
Autorizamos a nuestro/a hijo/a a participar a una estancia linguistica. Si nuestro/a hijo/a sufre un accidente o enfermedad durante su estancia en el extranjero,
permitimos a la familia anfitriona o a los responsables locales de la organización EDI a proceder en cualquier tipo de acción medical o intervención cirúrgica que
se juzguen necesarias. La familia anfitriona y los corresponsales de la organización EDI asumen toda potestad en caso de emergencia.
Als Eltern des Teilnehmers, erlauben wir die Gastfamilie und den lokalen Vertreter von EDI sämtliche notwendige medizinische Massnahmen (Diagnose,
Behandlung, Chirurgische Eingriffe) ergreifen zu lassen. Die Gastfamilie und der Vertreter von EDI verfügen über alle Rechte im Notfall).
9. Les cigarettes et l’alcool
Smoking or drinking alcohol
Fumer ou consommer de l’alcool aux Etats-Unis est illégal et peut entraîner des poursuites
judiciaires. Tout participant à un séjour dans ce pays doit prendre l’engagement, avant le départ, de ne
pas fumer et de ne pas boire d’alcool. Tout jeune qui transgressera cette règle sera immédiatement
renvoyé en France aux frais de ses parents.
Smoking or drinking alcohol in the USA is against the law and will lead to legal pursuit. Participants agree not to smoke or drink alcohol.Participants who do not
follow this rule will be immediately sent back to their home country at their parents’ expense.
10. La vie de la famille et les amis
Family life and friends
Sur une page séparée (document word), insérez et commentez une photo de votre choix dans la langue
du pays d’accueil.
Pour tout séjour aux USA : inclure au minimum 3 photos commentées comme suit :
- Une photo de vous et votre famille
- Une photo de vous et vos amis
- Une photo de vous pratiquant votre sport ou hobby préféré.
Australie : les lettre et photo ne sont pas nécessaires car nous vous enverrons un autre dossier.
11. Lettre à la famille d’accueil

Sur une page séparée (document word), écrire une lettre dans la langue du pays d’accueil à votre
famille hôtesse (300 mots minimum pour les USA). Donnez toutes les informations qui pourraient
l’aider à mieux vous connaître de la façon la plus amicale possible. N’oubliez pas de préciser vos nom
et prénom.
Commencez votre lettre par : "Dear host family"/"Querida familia"/"Sehr geehrte Familie"
12. Acceptation des consignes
En cochant la case ci-contre, parents et participant certifient l’exactitude des informations données
ci-dessus et acceptent les règles et consignes énoncées dans ce dossier d’inscription.
DEAR HOST FAMILY,
HELLO, my name is Robin Dufils, I am fifteen and I am a school boy. I live in
France, in Normandy , it's the region of camembert and cider . I live in country.
I love travelling , it's my activity preferred , I never go to U.S.A but I dreamt
since a longtime but, I went to England, Germany, Switzerland, Poland, Italy
and Croatia ,I like really much .
I go sometime at Paris with my mother to see exposition. I take the train during
1 h 30 to go to Paris.
I have got a lot's friends in my school. I enjoy the pictures and taking photos
and I like the bowling and cinema.
I love also the sport's cars. I read magazines of cars. I like also different music
(hip hop, rap, rock, guitar...) I've got a piano at home, I play sometime.
I love riding my bike, I like food American very much, I like almost all food.
My mother is Michèle, she was a teacher, now, she is a retreat, she is fifty-six.
My father is a farmer but I live with my mother.
I have got three brothers , Nicolas , Sébastien ,Benjamin. The older is thirty,
after, twenty-seven, after, twenty-five. I am the youngest. I am also uncle
because Benjamin has a daughter of one year, her name is Lola, she’s very nice.
I have also my grandfather who is in a retirement home near my home.
I love the pet's : I have got two cats and four baby cats and also a dog , it' s
Unick and six sheep . I speak English a little , I hope to make myself
understood , I get on very well with teen girls or boys .I hope to kept the
contact with you . Thank you for accepting me in your home.
