This document has to be filled in in case of an international warranty case. Original document: must always stay with the invoice. Copy 1: to be sent back by
the AD Garage to his national central office. Copy 2: copy for the motorist. Copy 3: copy for the AD repair shop who repaired under warranty.
Gelieve dit document in te vullen in geval van een internationale garantie. Origineel document: dient altijd de factuur te vergezellen.
Kopie 1: door de AD Garage / het AD Autobedrijf terug te sturen naar zijn landelijke AD organisatie. Kopie 2: exemplaar voor de automobilist.
Kopie 3: exemplaar van de AD Garage / het AD Autobedrijf die/dat de reparatie onder garantie verricht.
Merci de remplir ce document en cas d’une garantie internationale. Document original: devra accompagner toujours la facture de la réparation sous garantie.
