exemplaires, et des résultats des plus efficaces dans toutes les sphères de
la profession.
Notre mission
Rendre le savoir accessible aux soins infirmiers et de santé : Augmenter la
qualité de la pratique clinique et améliorer la santé des personnes et du
système de soins.
L’Unité de recherche sur les pratiques exemplaires en soins infirmiers
(URPESI) est une collaboration entre des chercheurs et des professeurs de
l’Université d’Ottawa et l’Association des infirmières et infirmiers autorisés
de l’Ontario (AIIAO) une association professionnelle active. L’Unité génère
des connaissances de pointe en sciences infirmières basées sur des
résultats probants et favorise la collaboration et l’échange auprès de
décideurs et groupes de citoyens au Canada et à travers le monde. L’Unité
vise l’amélioration de la pratique et des effets positifs sur la santé des
personnes, sur les soignants, les organismes et le système tout entier. Elle
favorise chez les professionnels, les décideurs et les étudiants la production
et l’utilisation des savoirs basés sur des résultats probants quels que soient
leurs rôles et les secteurs dans lesquels ils se trouvent..

Introduction
Jenny Ploeg, Jennifer Skelly, Nancy Edwards, Barbara Davies,
Doris Grinspun, Irmajean Bajnok et Angela Downey ont remporté
le prix 2011 Best of WORLDviews on Evidence Based
Nursing.
Elles ont gagné ce prix pour leur article intitulé : The Role of
Nursing Best Practice Champions in Diffusing Practice
Guidelines: A Mixed Methods Study.

International tenue en novembre dernier au Texas.

Chers collègues,
L'Unité de recherche sur les pratiques exemplaires en soins infirmiers (URPESI) a l'immense plaisir de vous présenter son cinquième
rapport annuel, pour l'année 2011-2012. Ce rapport reflète un partenariat unique entre le monde universitaire et une association
professionnelle. Il met en valeur une kyrielle de projets et de publications de recherche démontrant l'expertise de nos membres qui
s'assurent de la production et de la diffusion de pratiques exemplaires afin d'améliorer les soins infirmiers et d'optimiser le rendement
auprès des patients et des organisations de santé.
L’URPESI s'assure que les soins infirmiers et de santé reposent sur les meilleures connaissances disponibles, grâce à des partenariats
riches. Ce rapport illustre notre impact collectif sur la pratique basée sur des preuves et sur la recherche portant sur les résultats
d’interventions. Il souligne aussi l'importance de bénéficier de collaborations étroites, pour faire progresser la recherche en sciences
infirmières et pour garantir l'excellence dans la prestation des soins.
L’URPESI est heureuse de rapporter que, grâce à une subvention des Instituts de recherche en santé du Canada (IRSC), elle a pu
organiser un symposium de recherche, au cours duquel des chercheurs et des décideurs ont façonné la vision future et le plan des études
à mener à l'URPESI. À la suite de ce symposium, on compte élaborer de nouvelles orientations pour l'infrastructure de l'URPESI, de
nouvelles approches pour la diffusion de la recherche auprès de la population infirmière et des travailleurs en santé ainsi que de nouveaux
programmes de recherche qui orienteront les activités de l'URPESI au cours des cinq prochaines années.
Ce sont les réussites et l'engagement constant de nos membres (personnes et organismes) qui ont rendu possibles les succès
déterminants obtenus cette année. Vous avez tous contribué à paver la voie vers l'excellence dans le domaine de la recherche en
sciences infirmières en plus d’assurer l'implantation d’une culture basée sur les résultats probants dans le domaine des soins de santé.
Ensemble, nous faisons en sorte que le grand public bénéficie de la recherche en sciences infirmières.

Notre programme de recherche met l’accent sur les « pratiques exemplaires »
permettant le rapprochement entre la collectivité des soins infirmiers et le savoir
fondé sur des données probantes. La connaissance fondée sur ces données peut
prendre plusieurs formes, dont celles de lignes directrices des pratiques
exemplaires décrites dans les objectifs stratégiques et opérationnels.
Mot des co-directrices
Il nous fait plaisir de vous présenter ce cinquième rapport annuel de l'URPESI qui souligne la capacité accrue en recherche
dans notre secteur, ainsi qu’un accent de plus en plus marqué sur des objectifs en soins infirmiers et en recherche
interprofessionnelle, permettant une amélioration de la prestation des soins de santé. À titre de co-directrices de l’URPESI,
nous sommes très fières de constater la croissance du rayonnement de l'Unité au cours des cinq dernières années, et très
heureuses que les résultats et le transfert des connaissances en soins infirmiers soient devenus un secteur stratégique sur
lequel se concentrent les chercheurs, les éducateurs, les étudiants diplômés, les associations professionnelles, les
organismes de santé et les décideurs. Le nombre de nos membres augmente, parallèlement à l'accroissement de la
recherche liée à la pratique basée sur des faits et à son impact.
Les 170 projets et dossiers de recherche et les 234 publications produits cette année attestent de la quantité, de la
nature et du type de recherches effectuées par nos membres. Le rapport souligne l'importance accrue attribuée à la
recherche sur les résultats pour les clients, liée à l'application de lignes directrices d'utilisation de pratiques exemplaires et à
l'implantation de cultures de pratique en soins infirmiers fondés sur des savoirs probants. Plus de 69 membres individuels,
locaux, nationaux et internationaux, soutiennent avec brio notre Unité. De plus, le nombre des organismes membres
atteint maintenant 30 Organismes vedettes en pratiques exemplaires® (OVPE®s), ainsi que d'autres environnements de
soins de santé dans lesquels les chercheurs collaborent avec le personnel infirmier, les administrateurs, et d'autres
professionnels de la santé pour améliorer et évaluer la prestation de services fondés sur des savoirs probants.
L'accès accru aux lignes directrices de l'Association des infirmières et infirmiers autorisés de l'Ontario (AIIAO) pour les
pratiques exemplaires cliniques (38) et en milieu de travail (9), et leur disponibilité en français, espagnol, italien, mandarin,
japonais et anglais, contribuent à intensifier leur utilisation et leur évaluation. Des membres de l'URPESI ont dirigé cette
année, en Australie, au Chili, en Colombie, en Afrique de Sud, en Espagne et aux États-Unis, des ateliers internationaux
portant sur des sujets comme : la prestation de services de soins infirmiers fondés sur des savoirs probants; la prise de
décisions interprofessionnelles communes; les aides aux décisions relatives aux patients; ou la diversité et la durabilité des
soins infirmiers. Nos membres ont placé un accent exceptionnel sur le développement et le transfert des connaissances.
Nous sommes toutes les deux très impliquées dans ce partenariat unique, continu et dynamique, entre l’Université d'Ottawa
et l'AIIAO, qui vise à intégrer aux soins de santé axés sur les clients et les patients, des lignes directrices cliniques et de
milieu de travail sain, basées sur la recherche, émanant de l'AIIAO et d'autres sources. Une solide collaboration entre les
décideurs et les chercheurs est un des piliers de notre travail commun. Nous vous prions donc de prendre le temps de lire ce
rapport, et de le consulter souvent dans le but d'améliorer l'excellence dans les domaines clinique, politique, académique et
de la recherche. Faites en sorte que vos travaux soient mis en valeur dans le prochain rapport annuel de l'URPESI.

Les organismes membres de l'URPESI ont envoyé les soumissions ci-après, qui reflètent leur
engagement d'appliquer, d'évaluer et d'améliorer ces lignes directrices afin de favoriser l'implantation de
cultures de pratique basées sur les faits.
CanCare Health Services Inc.
L'objectif de CanCare est d'être la référence de qualité en ce qui concerne la fourniture de soins de
santé en Ontario, en mettant partout en oeuvre des pratiques de tout premier plan en matière clinique et
d'évaluation des risques. Notre personnel se consacre à coordonner les diverses initiatives de
recherche auxquelles nous participons, ce qui nous a permis, au cours de notre cheminement OVPE®,
de collaborer avec plusieurs agences et partenaires externes, ainsi qu'avec des établissements
d'enseignement. CanCare a participé à l'étude intitulée Guideline Implementation for Improved
Client/Patient Outcomes (GICOM), visant à améliorer les processus de surveillance, ainsi que
l'évaluation et l'interprétation des principaux résultats liés à l'utilisation des LDPE. Cela nous a donné la
chance de travailler avec des chercheurs de divers centres académiques et d'autres organismes
OVPE®. Dans le cadre de l'étude GICOM, nous avons élaboré une carte de vérification, un outil qui
servira à mesurer de manière cohérente les résultats pour les clients de deux des lignes directrices de
pratiques exemplaires que nous appliquons, à savoir : l'Évaluation et traitement des plaies du pied des
diabétiques et l’Évaluation du risque et prévention des lésions de pression.
CarePartners
CarePartners est un organisme en cours de restructuration, car nous fusionnons avec un autre
pourvoyeur de soins de santé. Cette nouvelle entité, qui n'a pas encore été baptisée, s'est engagée à
faire de la recherche liée aux soins à domicile. Les priorités en matière de recherche sont dictées par
une équipe de pratique clinique axée sur l'utilisation de lignes directrices de pratiques exemplaires en
milieu communautaire. Actuellement, CarePartners fait de la recherche aussi bien sur le plan interne
qu'avec des partenaires externes. Nous avons mis fin l'an dernier à notre participation à l'étude GICOM,
et nous continuons à travailler avec l'université McMaster à un projet lié à la dissémination des LDPE en
milieu communautaire. Cette année, CarePartners a terminé une étude interne sur le Nurse-led
debridement of diabetic foot ulcers. Cinq infirmières ont participé à ce projet, et étant donné que l'étude
a montré que les résultats cliniques étaient positifs, nous prévoyons explorer les variables spécifiques
qui influent sur la guérison des plaies liées au débridement.
Alliance de santé de Chatham-Kent
Soutenir une culture de pratique basée sur les faits fait partie de la mission de l'Alliance de santé de
Chatham-Kent qui consiste à fournir des soins de santé empreints de compassion et de qualité, en
constante amélioration, et de notre vision consistant à être un hôpital communautaire de tout premier
plan, qui soit une référence pour tous et dépasse sans cesse les attentes. D'une perspective liée aux
patients, nous nous efforçons d'intégrer les soins dans tout le continuum afin de mieux servir nos
patients et la collectivité. Grâce à l'engagement de notre équipe d'infirmières praticiennes, nous
continuons à développer notre capacité et à améliorer notre infrastructure de recherche. La participation
des infirmières à des recherches constitue un volet important de la pratique professionnelle, si bien que
plusieurs études sont en cours. Par exemple, nous étudions l'impact de la pratique d'infirmières
praticiennes basées à l'hôpital au sein d'une équipe interprofessionnelle, examinant les effets des
instructions de sortie sur les taux de réadmission des patients souffrant d'insuffisance de santé globale.

Centre d’accès aux soins communautaires de Champlain
Le plan stratégique du Centre d’accès aux soins communautaires (CASC) de Champlain vise à assurer
le partage des évaluations basées sur les résultats et des recherches en milieu communautaire, afin
d'améliorer la qualité des soins. Même si le CASC de Champlain n'a reçu aucun financement externe
spécifiquement destiné à soutenir la recherche en soins infirmiers, il participe activement à des
recherches, en consacrant des ressources à la collaboration et à l'identification de domaines d'étude qui
sont prioritaires pour le CASC.
Le CASC de Champlain s'efforce d'élargir son mandat de recherche dans les domaines de
l'amélioration de la qualité et de la sécurité à domicile, de la mesure des résultats, des soins palliatifs et
de la pratique interdisciplinaire, en plus de la pratique de la gestion des cas.
Centre hospitalier pour enfants de l’est de l’Ontario (CHEO)
Le plan stratégique de recherche en soins infirmiers du CHEO reflète son engagement à participer à
des études pour faire progresser les soins infirmiers pédiatriques en développant de nouvelles
manières d'utiliser les connaissances et la recherche, qui lui permettront de fournir des soins
exemplaires axés sur la famille. En 2012, le personnel infirmier du CHEO a reçu cinq subventions
externes de recherche (p. ex. une équipe de cliniciens et de chercheurs a reçu des fonds des IRCS
pour organiser deux «Café Scientifiques» pour présenter leurs recherches sur la douleur lors de forums
publics). La recherche en soins infirmiers est facilitée par la directrice, Nursing Research & Knowledge
Transfer Consultant, la Endowed Chair in Nursing, Nursing Research Council, et les services de
l'Institut de recherche du CHEO. Des membres du personnel infirmier dirigent ou codirigent 14 projets
de recherche (p. ex. sur les interventions en matière de douleur et de transfert des connaissances), 35
sont co-chercheurs, et beaucoup participent à des clubs de publications, des examens systématiques
(p. ex. sucrose) et à l'élaboration de lignes directrices basées sur des faits (p. ex. l'emploi
d'antiémétiques en oncologie).
Fairview Mennonite Home
Le Fairview Mennonite Home est un foyer de soins à long terme de 84 lits qui a eu l'opportunité de
participer à deux projets de recherche pendant la période de 2009 à 2012, durant laquelle elle était
candidate à devenir OVPE®. La première étude, financée par le Collège des hygiénistes dentaires de
l'Ontario, visait à répondre à la question : «Quel serait l'impact des services d'hygiène dentaire sur la
santé buccale dans les établissements de soins de longue durée?» Le personnel de première ligne a
appliqué matin et soir la même approche des soins buccaux, et les résultats de l'étude ont permis une
amélioration de la sensibilisation à l'importance de la santé buccale, débouchant sur une modification
du protocole de santé buccale utilisé à Fairview. Ces résultats se sont avérés très positifs pour les
résidents et leurs familles, soulignant l'impact de l'application des lignes directrices de pratiques
exemplaires de l'AIIAO intitulées Oral Health: Nursing Assessment and Intervention. Notre organisme a
également participé à l'étude multi-sites GICOM, avec un accent sur la prévention des chutes. Grâce à
notre participation à cette étude, un nouveau protocole d'évaluation des risques de chute a été élaboré,
en collaboration avec notre médecin, notre pharmacien et du personnel du programme. Ce protocole
nous permet d'identifier les patients présentant un risque élevé de chute dans l'environnement de notre
établissement de SLD. La participation à cette étude s'est avérée très bénéfique, et elle nous a aidés à
identifier et à mettre en oeuvre de nombreuses nouvelles interventions pour prévenir les blessures dues
à des chutes.

Hôpital de Grand River
L'hôpital de Grand River a apprécié l'opportunité de participer à l'initiative OVPE® de l'AIIAO. Nos
efforts pour obtenir notre classification OVPE®, nous ont donné pendant trois ans l'élan nécessaire pour
utiliser 14 LDPE au sein de notre établissement, dans le but d'améliorer les résultats pour les patients,
ainsi que l'environnement de travail. Notre participation au programme OVPE® nous a donné l'occasion
d'implanter une culture basée sur les savoirs probants. La production des faits probants par notre
personnel, grâce à sa participation au processus de recherche, constitue la prochaine étape logique de
ce processus. L'hôpital communautaire de Grand River, avec ses 600 lits et son éventail de
programmes et de services régionaux, est heureux d'être partenaire avec divers établissements
d'enseignement dans le cadre de recherches visant à accumuler davantage de faits probants à l'appui
de l'utilisation de lignes directrices de pratiques exemplaires, et à promouvoir une profession infirmière
axée sur la connaissance. Plusieurs de nos leaders infirmiers sont individuellement membres de
l’URPESI, et ont hâte de codiriger et (ou) de faciliter le processus de recherche à l'hôpital de Grand
River.
Hôpital général de Guelph
Aidés par notre communauté et guidés par nos valeurs, nous fournissons des soins de qualité axés sur
les patients. À l'hôpital général de Guelph (HGG), la participation à des recherches en soins infirmiers
contribue à la qualité des soins de santé et à l'amélioration des résultats pour les patients. L'an dernier,
l'HGG a eu la chance de collaborer avec divers organismes vedettes en pratiques exemplaires® et avec
la faculté de l’Université d'Ottawa dans le cadre de l'étude multi-sites GICOM. Grâce à ce travail,
financé par l'AIIAO par l'entremise de l'initiative OVPE®, nous avons identifié la gestion de la douleur
comme un indice de rendement clé de notre établissement. Étant un hôpital communautaire, nos
ressources internes pour la recherche sont limitées. Notre participation à l'étude GICOM nous a permis
d'augmenter la capacité de recherche de notre personnel. Notre personnel infirmier participe
actuellement à tout un éventail d'études, y compris l'exploration de l'expérience de la douleur chez les
patients, les pratiques d'accès vasculaire, et une étude multi-sites à grande portée sur la chirurgie
vasculaire.
Hamilton Health Sciences (HHS)
HHS offre toute une gamme d'opportunités éducatives et financières dans le domaine de la recherche
en soins infirmiers, qui favorisent les lignes directrices de pratiques exemplaires à tous les niveaux de
l'organisme. Ces travaux sont financés par des partenaires provinciaux et nationaux. Par exemple, au
cours des dernières années, Hamilton Health Sciences a participé à des études financées par l'AIIAO
visant à évaluer l'application des lignes directrices de pratiques exemplaires sur la Prévention des
chutes et des blessures associées chez la personne âgée. Certains résultats de cette étude ont
récemment été publiés dans le journal Worldviews on Evidence-Based Nursing dans un article intitulé
The Real World Journey of Implementing Fall Prevention Best Practices in Three Acute Care Hospitals:
A Case Study. Du personnel infirmier jouant toutes sortes de rôles dans notre centre de sciences de la
santé multi-sites participe à des recherches. À HHS, plus de cent membres de notre personnel infirmier
continuent à contribuer à des projets sur le leadership ou le changement, visant à améliorer la qualité

Centre d’accès aux soins communautaires (CASC) Haldimand Brant de Hamilton Niagara
(HNHB)
Le CASC Haldimand Brant de Hamilton Niagara, en collaboration avec plusieurs partenaires
communautaires, donne des orientations en matière de recherche axée sur l'application de lignes
directrices. La mise en oeuvre de lignes directrices et leur durabilité sont des sujets qui figurent
constamment dans le plan stratégique annuel de l'organisme, et elles continuent d'être un volet
important de notre travail en milieu communautaire. Nous collaborons actuellement à une recherche
étudiant comment les lignes directrices de pratiques exemplaires liées aux aînés sont appliquées et
diffusées, ce qui approfondi nos connaissances sur l'adoption de lignes directrices, surtout en milieu
communautaire. Le CASC HNHB a aussi contribué au recueil de données pour une autre étude axée
sur la prévention des chutes dans les hôpitaux communautaires, réalisée par l'entremise du réseau
local d'intégration des services de santé. Cette année, un article portant sur l'expérience des
gestionnaires de cas en matière d'application des lignes directrices de pratiques exemplaires en milieu
communautaire a été publié, et un autre article étudiant la collaboration intersectorielle sur l'application
des lignes directrices et leur adoption dans la pratique est en cours d'élaboration.
Réseau local d'intégration des services de santé (RLISS) Haldimand Brant de Hamilton Niagara
(HNHB)
Le RLISS HNHB est le premier RLISS à participer à l'initiative OVPE®. Dans le cadre de notre travail
pour le OVPE®, nous nous sommes beaucoup concentrés sur la recherche, et nous y avons fait
participer des partenaires pourvoyeurs de services de tout le réseau afin d'améliorer notre capacité de
recherche. Notre projet de recherche s'intitulait Implementation of the Falls Prevention Guideline across
the HNHB LHIN: A Longitudinal Study of Outcomes. Trois hôpitaux communautaires du RLISS HNHB
ont participé à cette étude dont la méthode mixte se composait de groupes de consultation et
d'entrevues avec du personnel délocalisé, des leaders et d'autres participants, ainsi que du mentorat de
la part de l'équipe de recherche, sur une période de huit mois dans les trois hôpitaux communautaires
participants. Dans le cadre de cette étude, on a organisé 14 groupes de consultation, auxquels ont
participé 60 infirmiers et infirmières, 82 membres du personnel ont subi des entrevues qualitatives, dont
23 chefs cliniciens, 44 membres du personnel délocalisé et huit chefs de projet. Un article résumant les
résultats de ces travaux est en cours d'élaboration.
Hôpital de réadaptation pour enfants de Holland Bloorview
Le plan stratégique de l'hôpital de réadaptation pour enfants Holland Bloorview met l'accent sur la
recherche, car nous croyons qu'elle est la pierre angulaire des progrès des soins cliniques, du
développement de technologies d'appoint novatrices, et de l'amélioration de la participation sociale des
enfants ayant un handicap et de leurs familles. Nous avons mis en place une Evidence to Care Unit, qui
regroupe des cliniciens et des chercheurs travaillant à élaborer des approches de pratique clinique
basées sur les meilleures recherches disponibles. Plusieurs membres de notre personnel infirmier
participent actuellement à des recherches, notamment au sein d'une équipe scientifique et clinique
étudiant l'obésité chez les enfants et les jeunes atteints de spina bifida. Nous participons aussi à l'étude
multi-sites GICOM, avec un accent spécial sur la surveillance des résultats cliniques liés à la douleur
chez les enfants, dans le cadre de notre application des lignes directrices de pratiques exemplaires
intitulées Évaluation et prise en charge de la douleur.
Hôpital Montfort
En collaboration avec l’URPESI, l’hôpital Montfort continue à participer activement à l'initiative GICOM.
La participation de Montfort à cette étude vise principalement à promouvoir et à maintenir les

améliorations des résultats pour les clients/patients découlant de l'application des Lignes directrices sur
les pratiques en allaitement maternel à l'intention des infirmières de l'AIIAO. Cette l'année, neuf
membres du personnel infirmier du Centre familial de naissance (CFN) ont mis en place diverses
stratégies pour faciliter le transfert des connaissances relatives à l'allaitement maternel, et implanter les
conditions idéales pour la promotion et le soutien de l'allaitement maternel au sein du CFN. La
participation des deux chercheurs principaux et de leaders sur place nous a permis de surveiller
efficacement les résultats pour les clients, liés à la durabilité. De plus, l'étude nous a aidés à obtenir la
désignation d'Organisme vedette en pratiques exemplaires® de l'AIIAO.
Hospital for Sick Children
SickKids nursing research agenda aligns with our strategic plan by using knowledge translation (KT) strategies to
facilitate rapid uptake of new knowledge. RNs are supported in conducting clinical research and KT activities;
funding is available to support nursing research, internships and the Clinical Research Nurse Coordinator
orientation program. These opportunities provide multiple avenues for nursing research dissemination: Lunch and
Learn, Nursing Research Day, and Nursing Week. SickKid’s major areas of nursing research include KT in
pediatric pain and evaluation of organizational projects. External funding from CIHR and other agencies has been
received to support a program of research which includes KT research on pediatric pain, involving seven other
Canadian pediatric hospitals. Nurses involved in this research include a Clinician Scientist, a Research
Associate, a Research Coordinator, three Clinical Research Nurses, approximately 55 CRNCs and five Research
Trainees.
Mackenzie Health
Alors que nous nous efforçons de maintenir notre culture de pratique basée sur des données
probantes, la recherche en soins infirmiers est intégrée à notre plan stratégique, et elle s'y reflète par
l'entremise de processus d'évaluation. Pour évaluer l'impact des 17 lignes directrices de pratiques
exemplaires appliquées à Mackenzie Health au cours des trois dernières années, on a effectué auprès
de multiples unités de notre organisme une recherche étudiant l'efficacité des équipes de transfert des
connaissances dans la diffusion de l'évolution des pratiques. Cette étude, menée en partenariat avec la
faculté de l'université York, a impliqué de nombreux infirmiers et infirmières membres d'équipes de
transfert des connaissances, qui ont facilité l'implantation des recommandations des lignes directrices,
par l'entremise de modules d'apprentissage électronique, d'ateliers, de réunions éclair et de
présentation par affiches.
Soins de santé Saint Elizabeth
Notre équipe de plus de 6 500 infirmiers et infirmières, thérapeutes en réadaptation, préposés aux
services de soutien à la personne, et travailleurs des services d'intervention d'urgence, fait près de cinq
millions de visites de santé par an. Ceci est possible grâce aux cinq domaines stratégiques prioritaires
de l'organisation, le secteur de la recherche en soins infirmiers se reflétant dans plusieurs domaines, et
notamment celui de l'expérience et de la sagesse des clients. Pour soutenir ce travail, on effectue de la
recherche en soins infirmiers à tous les niveaux de l'organisation, du bureau de direction au personnel
clinique. Par exemple, un associé de recherche fait partie de l'équipe corporative de recherche, et est
employé à mi-temps par l’Université d'Ottawa et à mi-temps par Soins de santé Saint Elizabeth. La
recherche est également facilitée par les membres du programme de pratique avancée en soins
infirmiers, l’équipe de direction et les infirmières cliniques des centres locaux de prestation de services,
par l'entremise de partenariats avec des établissements d'enseignement comme des universités. Les
employés de première ligne sont impliqués en participant à des recherches par l'entremise de groupes
de consultation et d'autres mécanismes de recueil de données, ainsi qu'en mettant les données

probantes en pratique. Cinq projets de recherche en soins infirmiers sont en cours, et deux études
axées sur l'expérience et la sagesse des clients sont en développement.
St. Joseph’s Healthcare de Hamilton
Le nouveau plan stratégique (2012-2017) du St. Joseph’s Healthcare de Hamilton identifie la recherche
et l'éducation comme un des cinq secteurs prioritaires, et «un des piliers fondamentaux de notre
organisation». Ceci constitue une opportunité pour les soins infirmiers, et nous évaluerons au cours de
l'année à venir comment un agenda de recherche en soins infirmiers peut refléter cette orientation. Pour
l'instant, aucune ressource interne n'est officiellement consacrée à la recherche en soins infirmiers. L'an
dernier, nous avons participé à deux études subventionnées, à savoir : Implementation of a falls
prevention guideline across the Hamilton Niagara Haldimand Brant Local Health Integration Network: A
longitudinal study of outcomes (2011 – 2012), financée par l’Association des infirmières et infirmiers
autorisés de l’Ontario; et Healthy workplaces through a smoke free/tobacco initiative and client-centred
care in a mental health setting: A case study (2009–2011), financée par le programme de partenariat
pour la recherche sur les soins infirmiers, de la Fondation des infirmières et infirmiers du Canada. Parmi
les sujets étudiés, notons la lutte contre le tabagisme, les soins sensibles au traumatisme, et la mise en
oeuvre des lignes directrices de pratiques exemplaires.
St. Joseph’s Heath Care de London
Le plan stratégique du St. Joseph’s Heath Care de London va être élargi et inclure l'utilisation de lignes
directrices dans toute l'organisation et les promouvoir à titre de ressource pour aider à l'élaboration
éclairée de politiques. Nous avons engagé plus de 80 champions des pratiques exemplaires qui se
consacrent à leur amélioration et à leur implantation. Ils continuent à explorer des moyens d'améliorer
les outils d'évaluation et les ressources pédagogiques, et à les intégrer à nos systèmes électroniques
afin de faciliter leur durabilité. De plus, une de nos infirmières est bénéficiaire d'une bourse de doctorat
de l'AIIAO, pour des recherches portant sur les résultats cliniques liés au diabète et à la fourniture
d'insuline. De plus, dans le cadre des initiatives LEAD, plus de 40 infirmiers et infirmières ont participé à
divers projets de recherche sur l'amélioration de la qualité. Parmi les secteurs de recherche en constant
développement, citons notre équipe de prévention des chutes, les stratégies avancées de gestion de la
douleur, et l'amélioration des processus et de la documentation visant à garantir l'utilisation des
pratiques exemplaires.
Hôpital St. Michael
Dans le plan stratégique de l'hôpital St. Michael, la recherche et l'innovation liées à la pratique clinique
sont identifiées comme des catalyseurs clés de la qualité des soins aux patients. Pour illustrer notre
engagement envers la recherche en soins infirmiers, nous avons subventionné à l'interne la diffusion,
par l'entremise de conférences, des recherches en soins infirmiers approuvées par le Comité d'éthique
pour la recherche (CER), et la Chaire en recherche en soins infirmiers de l'association bénévole de
l'hôpital St. Michael. Parmi les partenaires financiers externes qui soutiennent la recherche en soins
infirmiers, citons les Instituts de recherche en santé du Canada, la Fondation des infirmières et
infirmiers du Canada, le Canadian Critical Care Trials Group, l'Association des infirmières et infirmiers
autorisés de l'Ontario, le ministère de la Santé et des Soins de longue durée, et Sigma Theta Tau
International. Notre programme Cœur et vaisseaux sanguins dirige présentement une initiative visant à
collecter des données probantes de style TEACH-BACK avec des patients souffrant de déficience
cardiaque dans le but d’optimiser nos stratégies de gestion thérapeutique. Une infirmière soignante fait
partie d'une équipe de recherche évaluant l'application dans notre clinique de fracture des lignes

directrices de pratiques exemplaires de l'AIIAO intitulées La violence faite aux femmes : dépistage,
identification et intervention initiale, en fonction du travail qu'elle a dirigé dans notre département des
urgences.
L'Hôpital d'Ottawa
L'Hôpital d'Ottawa est un grand hôpital de soins actifs offrant onze programmes cliniques. La plupart
des recherches continues sont directement reliées aux secteurs cliniques spécifiques, y compris ceux
qui sont à l'échelle de toute l'organisation, et notamment le Modèle de pratique infirmière
professionnelle, le Modèle interprofessionnel de soins aux patients (MIPSP), la durabilité des lignes
directrices de pratiques exemplaires, et la sécurité des patients. La recherche en soins infirmiers est
une priorité à l'Hôpital d'Ottawa, où elle est appuyée par une solide équipe de dirigeants incluant
l'infirmière en chef, le chef de la recherche en soins infirmiers, le groupe de travail sur la recherche en
soins infirmiers, et 30 infirmières en pratique avancée. Le rapport annuel de soins infirmiers de l'Hôpital
d'Ottawa mentionne les recherches subventionnées dirigées par des infirmières et infirmiers, et
notamment : la durabilité des lignes directrices de pratiques exemplaires intitulées Évaluation et prise
en charge de la douleur de l'AIIAO; la surveillance des événements indésirables; l'impact de
l'implantation du Modèle de pratique infirmière professionnelle (MoNCP©) à l'Hôpital d'Ottawa; la
mobilité après blessure chez les aînés; et l'application de lignes directrices pour l'amélioration des
résultats pour les clients et patients. Cent-onze membres du personnel infirmier ont participé à ces
recherches en soins infirmiers.
Unité de santé du district de Thunder Bay
Les infirmières et infirmiers de l'Unité de santé du district de Thunder Bay participent à des activités de
recherche qui cadrent avec le plan stratégique de l'Unité visant à favoriser l'implantation d'une culture
basée sur des données probantes à l'appui de la prestation de services de santé publique. Cette
approche cadre avec la stratégie de qualité de l'organisation, qui est conforme à la norme fondamentale
stipulée dans les Normes de santé publique de l'Ontario. L'an dernier, les activités de recherche se sont
concentrées dans deux secteurs liés à l'application de lignes directrices. Afin de favoriser l'allaitement
maternel chez les femmes autochtones, on a effectué une étude, en partenariat avec l'université
Lakehead, visant à valider une échelle d'auto-efficacité de l'allaitement maternel, et mis en oeuvre un
projet pilote de soutien de pairs tenant compte de facteurs culturels. Quatre membres du personnel
infirmier de l'Unité de santé du district de Thunder Bay faisaient partie de cette équipe de recherche. De
plus, du financement de la Fondation communautaire de Thunder Bay, complété par un soutien de
l'Unité de santé du district de Thunder Bay et du comité de sensibilisation aux drogues de Thunder Bay,
ont soutenu un programme visant à mettre des étudiants en relation les uns avec les autres et à
supprimer les obstacles et les stéréotypes relatifs aux étudiants autochtones. Quatre infirmières en
santé publique ont collaboré à cette étude, avec des partenaires communautaires.
Réseau universitaire de santé (RUS)
Au sein du Réseau universitaire de santé, l'esprit de recherche est bien présent dans les nombreuses
avenues qui valident la pratique fondée sur des données probantes. Le Nurses for Tomorrow Research
Fellowship Program est une de ces avenues visant à améliorer les résultats des soins aux patients.
L'approbation tous les ans de dix propositions dirigées par du personnel infirmier, visant à financer des
recherches sur des sujets reliés aux pratiques exemplaires et à la sécurité des patients est une preuve
de cet engagement envers les pratiques basées sur des données probantes. Les bourses obtenues l'an
dernier traitent de divers sujets liés aux lignes directrices de l'AIIAO, dont le délire post-opératoire, la
continence urinaire, les soins buccaux, l'éducation des patients et les environnements de travail sains.

La surveillance formelle de l'intégration des lignes directrices de pratiques exemplaires de l'AIIAO se
poursuit grâce au partage public, par l'entremise de notre carte de pointage équilibrée du RUS,
d'indices de résultats tenant compte des soins infirmiers. Les idées cliniques et de recherche sont
examinées dans la «carte routière», par l'entremise d'une série de tendances futures dans des thèmes
clés comme : la technologie médicale; l'informatique et le recueil et l'évaluation des renseignements sur
les patients; la recherche en matière de services de santé; et la thérapeutique expérimentale. On
complète chaque tendance par des informations relatives à la capacité actuelle du RUS dans ce
secteur, à la compétence souhaitée, et à la manière de l'obtenir.
Département de sciences infirmières de l'Université de Hartford
Tous les étudiants inscrits aux programmes de sciences infirmières de l'Université de Harftord sont des
infirmières et infirmiers praticiens autorisés. L'application de lignes directrices dans tout le programme,
et la facilitation de leur adoption par les infirmières dans tout l'État soutient la mission de transfert des
connaissances du Collège où se trouve le département de sciences infirmières, ainsi que l'engagement
de l'université d'avoir un «objectif public». Un donateur généreux a soutenu le travail effectué par le
département de sciences infirmières dans le domaine des lignes directrices. Ce financement nous a
permis d'organiser des ateliers «des champions» à l'intention de nos étudiants, des infirmières et
infirmiers de la région, et des éducateurs en sciences infirmières de divers établissements d'éducation
et d'État en soins de santé et en santé publique. Les professeurs de la faculté consacrent du temps et
de l'expertise à la recherche que nous menons pour évaluer la connaissance et la compréhension
qu'ont les étudiants de lignes directrices choisies. On présente à tous les professeurs à plein temps et
aux 250 infirmières et infirmiers inscrits à nos programmes les lignes directrices qui ont été intégrées à
notre programme. Des recherches constantes visent à évaluer la connaissance et la compréhension
qu'ont les étudiants de lignes directrices choisies, ainsi que les résultats d'ateliers récemment mis sur
pied pour guider les enseignants à intégrer ces lignes directrices dans le contenu du programme de
sciences infirmières.
Infirmières de l'Ordre de Victoria du Canada (VON)
VON s'est engagée à offrir des services novateurs, influencer les politiques, et fournir des soins de
haute qualité en toute sécurité. La recherche est axée sur la sécurité au foyer, la sécurité de la
médication dans les soins à domicile, et les soins primaires basés sur des données probantes,
administrés aux personnes en deuil. En soutien de cette tâche, une infirmière chercheuse de VON
engage du personnel qui soutient la recherche en participant directement aux activités de recherche,
supervise le comité d'éthique en recherche de VON, et aide au transfert et à la diffusion des
connaissances. Les infirmières et infirmiers de VON participent activement à des équipes de recherche,
y compris celle de l'étude sur la gestion des médicaments, et six ont participé à des groupes de
consultation, cinq ont fait des entrevues, et six ont pris part à un forum du projet de soins aux
personnes en deuil. Cinq infirmières sont co-chercheuses dans diverses études continues. VON a
réussi à se procurer du financement externe pour l'étude du Ministère de la Santé et des Services
Sociaux intitulée : Evidence Informed Primary Bereavement Care: A Knowledge to Action Approach; et
pour l'étude NSHRF Safety in Homecare: A focus on medication management.
Centre de soins de santé de West Park
Au Centre de soins de santé de West Park, la recherche est une importante priorité stratégique qui vise
à améliorer l'enseignement, l'apprentissage et l'application de nouvelles connaissances. Le gestionnaire
de la pratique professionnelle et expert clinique, dirige notre programme de transfert des

connaissances, en collaboration avec nos infirmières en pratique avancée. Mis à part le temps que les
infirmières en pratique avancée consacrent à la direction de recherches, 100 membres du personnel
infirmier environ participent directement ou indirectement à nos projets empiriques actuels. West Park
met beaucoup d'emphase sur le projet de recherche de 18 mois, subventionné par l'entremise du
groupe financier de la Banque TD, qui étudie le développement de pratiques comme stratégie de
soutien de l'excellence des soins cliniques. Du personnel de l'organisation participe à une autre étude
d'un an, financée à l'externe par l'entremise d'une bourse de recherche SIGMA, et intitulée The Use of
Photovoice, qui vise à améliorer le milieu de travail.
Centre de santé communautaire de Windsor Essex
Le Centre de santé communautaire de Windsor Essex (CSCWE), qui est le premier centre de santé
communautaire à atteindre cet objectif, a obtenu sa désignation OVPE® en mettant en oeuvre quinze
lignes directrices de pratiques exemplaires au cours des trois dernières années, par l'entremise de son
équipe interprofessionnelle. Nos pourvoyeurs de soins de santé et notre personnel des diverses
disciplines (travailleur social, médecin, infirmière praticienne, podologue, diététicien, psychologue,
massothérapeute, pharmacien, et travailleur social communautaire) aident le personnel infirmier à
appliquer ces lignes directrices. L'an dernier, nous avons participé à l'étude GICOM, en nous
concentrant sur l'impact sur les résultats pour les clients de l'application des lignes directrices de
pratiques exemplaires intitulées La cessation du tabagisme dans la pratique quotidienne des soins
infirmiers. Grâce à ce partenariat, le CSCWE a été invité à participer à une étude de thèse de maîtrise
en sciences infirmières intitulée Interprofessional Collaboration and the Introduction of Guidelines at
Best Practice Spotlight Organizations®. Nous sommes enchantés à la perspective de futures
collaborations et participations à des recherches qui nous permettront d'améliorer nos connaissances et
notre expertise dans ce domaine.
