 If you are unable to have page 2 of this form completed by a priest, minister, pastoral animator, Faith First
coordinator or catechist, you must still fill in page 1 of the form.
 IMPORTANT NOTES (this information applies mainly to Catholic families):
o If you do not attend a parish at least once a month, please do not ask the parish priest to complete the form.
Simply fill in page 1 of the form and return it to the school.
o If your priest does not wish to fill in the form, your son’s Faith First Coordinator or catechist may do so.
o If your priest decides not to fill in the form and does not wish those involved in the catechetical program to
do so either, please have your son bring his Faith First Workbook to his interview at Loyola.
