Nuisance Wildlife Control Operator Report Form General
Instructions
Attached are report forms that MUST be submitted to the Fish and Wildlife Branch at the end of
each calendar year. Please make a report of any animal or groups of animals handled under the
authority of a Nuisance Wildlife Control Operator’s Licence. Species exempted from this reporting
requirement are: all rats, mice, moles and voles; as well as brown-headed cowbird, common
grackle, european starling, red-winged blackbird, house sparrow and rock dove (pigeon).
Provide the information as follows:
Species:
indicate the species of animal handled (common name).
Date:
indicate the month that the animal was handled.
WMZ:
indicate the Wildlife Management Zone where the action was taken.
A WMZ map is attached.
Address:
indicate the address where the action was taken.
Complaint
give a brief description of the nature of the complaint that required

action to be taken.
Name of
Complainant:
record the name of the person who requested your services.
Control Method:
give a brief description of the control method(s) used, i.e., box traps, killing traps,
leg-hold traps, snares, snare pole, netting, etc.
No. Destroyed:
record the number of animals of the species indicated that were destroyed.
No. Relocated:
record the number of animals of the species indicated that were relocated and
released.
Relocated WMZ:
indicate the WMZ in which the relocated animals were released.
Nuisance Wildlife Control Operators are not permitted to relocate/release any of the six rabies vector
species: raccoon, skunk, fox, coyote, groundhog or mink. These animals must be humanely
euthanized according to the humane techniques approved by the Canadian Council on Animal Care
and described in the NWCO Course. The removal, exclusion or killing of bats requires a
Nuisance Wild Animal Control Permit which can be obtained from your local ERD office. All
euthanized bats shall be submitted to ERD.
At the end of each calendar year, but no later than January 14th, please send any
completed report forms to:
