Holz Holz Handpiece for RPE-Sheet-Transplantation Funktion dieses Handstück ist die sichere und atraumatische intraokulare Translokation eines peripher exzidierten RPE-Patchs in den submakulären Raum nach Exzision einer subfovealen CNV. This handpiece is designed for safe and atraumatic intraocular translocation of peripherally excised RPE-patch into the submacular space following excision of a subfoveal CNV with or without submacular hemorrhage. Dabei stellen das vorübergehende Halten des exzidierten autologen RPE-Patches und die nachfolgende Lösung des translozierten Patches im subretinalen Raum zentrale Schritte dar. Das Halten des Patch wird erzielt durch Ansaugen über eine kleine Öffnung an der Oberseite der Instrumentenspitze. Die Lösung des Gewebeteilchens erfolgt durch zwei Mechanismen: zum einen durch die Injektion von BSS über dieselbe Öffnung. Die klinische Erfahrung hat gezeigt, dass durch diesen Schritt alleine oftmals eine Lösung des Patches nicht möglich ist, da sich Fasern des choroidalen Gewebsanteils in der kleinen Öffnung nach dem vorherigen Ansaugen verhaken. Diese Fasern können bei diesem Instrument nun durch manuelles Vorschieben eines vorne geschärften Schlitten (ähnlich einem Zigarrenabschneider) durchtrennt und damit der Patch vollständig im submakulären Raum gelöst werden. Zusammenfassend kombiniert das Design des Transplantationshandgriffs greifende und lösende Funktionen in einem einzigen Instrument, was eine kontrollierte Translokation eines peripher exzidierten RPE-Patches in den subretinalen Raum ermöglicht. Es unterstützt die Operation und minimiert das mechanische Trauma, welches dem Transplantat zugefügt wird. Dies erweist sich als wichtig für das Erhalten der RPE-Zellen, und demzufolge auch für die Photorezeptoren des Auges nach dem Entfernen einer subfovealen CNV. Hereby transient apposition of the autologous RPE-patch to the tip of the instrument during translocation intraocularly and subsequent release of the patch in the submacular space represent essential steps. Transient attachment to the tip of the instrument and movement of the RPE/choroid patches in the eye is achieved by manually controlled fluid suction via a small opening. Release of the patch is achieved both by (i) flushing BSS through the same opening and (ii) a flat slide that can be moved anteriorly on the surface of the spatula over the opening to completely remove remaining choroidal tissue fibres of the basal patch side from the suction hole (similar to a cigar-cutter). Clinical expierience has shown that flushing alone is often insufficient as choroidal fibres become trapped in the suction and hole and, thus, prevent complete release of the patch. In summary, the design of the transplantation handpiece combines holding and release functions into a single instrument allowing for controlled translocation of peripherally excised autologous RPE patches into the subretinal space. It facilitates the surgical procedure, and minimizes mechanical trauma to the graft which appears essential for sustained RPE cell and, thus, photoreceptor function in eyes following removal of subfoveal CNV. 37200 Prof. Dr. med.
