Approximately, 25% to 50% of infertile women have endometriosis and 30% to 50% of women with endometriosis are infertile. Although the treatment of women with endometriosis associated with infertility is well described, it is still a controversy of how to trat old women with endometriosis associated with infertility, IVF before surgery or laparoscopic surgery first? The objective of the study was to investgate whether IVF before the surgery inol women with endometriosis associated with infertility could obtain a better pregnancy outcome. Materials /Patients and methods: A retrospective observational study was carried out on 45 infertile patients with range of the age from 35 to 42. All patients first dignosed endometriosis-associated cyst with the diameters of 5-8cm by ultrasound or Magnetic Resonance Imaging. All patients had no positive reaults in uterus, ovulation and tubal patency. Among of them, 20 patients underwent laparoscopic surgery for endometriosis cyst and GnRHa treatment of 3 months at follow-up in vitro fertilization and embryo transfer (IVF-ET). 25 patients underwent endometriosis cyst by puncture at follow-up IVF-ET. Results: our preliminary results showed that the numbers of oocyte in the surgery approach was more decreased significantly than the puncture approach (P<0.05). The fertilization rate of IVF in the surgery approach was more reduced significantly than the puncture approach (P<0.05). The pregnancy rate in the surgery approach was significantly lower (4/20, 20%) than the puncture approach(11/25, 44%) (P<0.05). The patients of surgery approach had poorer ovarian reserve and lower embryo quality than the puncture approach. Conclusions: A reasonable evidence demonstrates an association between endometriosis and infertility has not been clearly established. However, endometriosis can result in adhesions or distorted pelvic anatomy that precludes fertility. This is debatable that whether the surgery can improve the pregnancy outcome of the infertile women of old age with endometriosis.
