Hubert (Angers). Introduction Les fractures du cuboïde sont très rares. Leur traitement est donc mal codifié comme en témoigne le manque de littérature à ce sujet. Les progrès de l’imagerie dont la TDM 3D ainsi que l’existence de plaques dédiées ont permis une prise en charge spécifique, planifiée. L’objectif de cette étude est d’évaluer les résultats cliniques des ostéosynthèses de fractures du cuboïde par plaque verrouillée. Matériels et Méthodes Il s’agit d’une série rétrospective monocentrique de 9 patients (7 hommes, 2 femmes) opérés pour des fractures du cuboïde entre Janvier 2009 et Décembre 2015. Une TDM pré-opératoire avec reconstruction 3D était réalisée. L’analyse par soustraction des os du tarse postérieur permettait d’analyser les surfaces articulaires. Une voie d’abord dorso-latérale centré sur le 4e rayon était pratiquée. La réduction articulaire était faite en s’aidant d’un mini-distracteur, la fixation était réalisée par plaque verrouillée AO (SynthesTM). Les lésions associées au niveau du pied étaient traitées dans le même temps. Une TDM post-opératoire a été réalisée. Les patients étaient revus par un observateur indépendant qui réalisait un examen clinique, un score AOFAS et un Maryland foot score. Résultats Le recul moyen était de 24,4 mois. Seul un patient présentait une fracture isolée du cuboïde. Tous les patients ont consolidé, aucun n’a eu d’arthrodèse au niveau de la colonne latérale du pied. Le Maryland Foot Score à révision était de 93,4. Le score AOFAS moyen à révision était de 89,4. Aucun patient n'est décédé. 1 patient a été perdu de vue. Conclusion Les fractures du cuboïde peuvent être traitées efficacement avec une imagerie adaptée et particulièrement des reconstructions TDM 3D. La plaque verrouillée spécifique semble être une solution satisfaisante pour le traitement de ces fractures. Une étude prospective sur le long terme serait intéressante pour objectiver l’apport réel de la prise en charge de ces fractures. The objective of this study is to evaluate the clinical results of internal fixation of fractures of the cuboid by locked plate. Materials and methods This is a single-center retrospective study of 9 patients (7 men, 2 women) underwent surgery for fractures of the cuboid between January 2009 and December 2015. Preoperative CT with 3D reconstruction was performed. The subtraction analysis of posterior tarsal bones allowed to analyze the articular surfaces. A dorsolateral surgical approach centered on the 4th metatarsal was practiced. The articular reduction was made with the help of a mini- distractor, the binding was performed by AO locked plate (SynthesTM). The lesions of the foot were treated at the same time. Postoperative CT was performed. Patients were reviewed by an independent observer who was making a clinical examination, AOFAS score and a Maryland foot score. Results The average follow-up was 24.4 months. Only one patient had an isolated fracture of the cuboid .All patients were consolidated, none had arthrodesis in the lateral column of the foot. Maryland Foot Score was revised to 93.4. The mean AOFAS score was 89.4 review. No patients died. 1 patient was lost to sight. Conclusion The cuboid fractures can be effectively treated with a suitable imaging and particularly 3D CT reconstruction .Specific locked plate seems to be a satisfactory solution for the treatment of these fractures .
