------------------------------------Laparoscopic placement of cervical sutures has been shown to have favourable outcomes and is becoming a popular
approach for cervico-isthmic cerclage. The indications for placement of transabdominal cerclage include previous failed
transvaginal cerclage, severe anatomic defect of cervix and history of extensive cervical surgery. A step-wise technique
for placement of laparoscopic cervico-isthmic cerclage has previously been described in the literature by Whittle WL et
al. (AJOG, 2009) and is reviewed in this video. Two challenging situations for laparoscopic cerclage placement are
presented in this video: pregnancy and the fibroid uterus. In pregnancy, the procedure is challenging due to the size of
the gravid utrerus, increased vascularity and inability to use a uterine manipulator. The challenges in the setting of
fibroids include the shape and size of uterus and distortion caused by fibroids making suture placement difficult, even
possibly impairing suture placement.
