Evaluating responsiveness to change of the French version and comparison
with EQ-5D

------------------------------------Purpose
The Endometriosis Health Profile 5 (EHP-5) is a short version of an endometriosis-specific quality of life assessment
questionnaire. The objective of the study was to confirm construct validity and to evaluate its sensitivity to change,
comparing with a general quality of life questionnaire, the EQ-5D.
Materials
Prospective observational study including 250 symptomatic women with proven endometriosis undergoing surgical or
medical treatments, in two French referral centers. The EHP-5 and the EQ-5D were administered before beginning of
treatment (time 1) and within 12 months after (time 2). Construct validity was evaluated by testing respectively the
relationships of EPH-5 and EQ-5D with endometriosis? or patient?s characteristics. To evaluate and compare
responsiveness to change, mean effect size of each scale was calculated overall and in each treatment group
separately.
Results
232 patients were analyzed at time 1, 188 in the surgical group and 44 in the medical one. 18 patients didn?t filled in one
of the two questionnaires at this time. The EHP-5 was sensitive to posterior location with ES= 0,48 (95%CI =0,06-0,89),
severity of the disease especially intestinal attempt with ES= 0,65 (95%IC= 0,08-1,21) and pain as the main complaint
ES =1,32 (95%CI =0,83-1,79). The EQ5D was also sensitive to the presence of intestinal attempt ES= 0,55 (95% IC=
0,05-1,05) and pain ES= 1 (0.59-1.39), but in a lesser extent. 139 patients returned the questionnaire at time 1 and 2.
Overall mean effect size for responsiveness to change of EHP-5 scale was 0,83 (95%IC = 0,54- 1, 11) compared to 0,81
(95%IC =0,56-1, 06) for EQ-5D. For the patients who underwent surgical treatment, the EHP-5 also proved good
sensitivity with ES= 0,88 (95% IC 0,58-1,18) versus ES= 0,85 (95%IC 0,57-1,17) for EQ5-D.
Conclusions
Results suggest that EHP-5 and EQ-5D have equivalent responsiveness to change but EHP-5 is more sensitive to the
endometriosis? characteristics. It should be a valuable specific tool for French-speaking patients with endometriosis,
both for daily practice and studies evaluating the impact of surgical interventions
