Job opportunity for a Clinician
We offer a contract (2.5 years) with the University Hospital of Heidelberg starting in October
2008 or later for a clinician knowledgeable in French with special training in paediatrics,
general medicine, surgery or internal medicine. The person will be posted in the District
Hospital of Nouna / Bukina Faso and will share with her / his Burkinabe clinical counterpart
the responsibility (1) of improving the quality of care and (b) of developing clinical research in
the hospital. These activities are part of the long standing research cooperation between the
University of Heidelberg and the Nouna Health Research Centre (CRSN) and in particular of
the special research focus (SFB) ‘Control of Tropical Infectious Disease’. The post offers the
opportunity (a) to practice clinical medicine in a district hospital in the tropics, (b) to develop
clinical management skills in a resource poor setting and (c) to start or further develop a
research career in clinical tropical medicine. With respect to research there is the opportunity
to participate in ongoing research activities which are already fully funded (see list of
research projects below). The rotation into the clinical team in Heidelberg after completing
the assignment in Nouna is possible. The salary is in accordance with the standards of the
University Hospital Heidelberg.
Context of the clinical post in Nouna / Burkina Faso
The clinical research activities within the research collaboration between Heidelberg and
Nouna have increased in recent years, including a longitudinal study of meningococcal
carriage and disease, a HIV-mother-child cohort study, as well as several malaria drug trials.
The post offered is a central part of the clinical core project of the special research focus
(SFB) “Control of Tropical Infectious Diseases” to support clinical research at CRSN and
Nouna district hospital .
The district hospital in Nouna has 88 beds and is the referral hospital for 27 dispensaries in a
district of about 300.000 inhabitants. It has benefited from sustained support by the CRSN.
Recently, a new emergency unit was built, an ultrasound unit set up and the research
laboratory integrated into the hospital services.
The post offered is a continuation of a successful “counterpart model” which has been
established in 2004 with a senior clinician from Heidelberg working together with a clinician
from Nouna. The first period (2004/2005) concentrated on the improvement of basic
processes and practices in the hospital, the second period (2006) on building up laboratory
capacity and the third period (2007/2008) on the Paediatric Department including
improvement of medical documentation and management of severe malnutrition. The
prospective observational study “Investigating cerebral malaria versus bacterial meningitis in
children with impaired consciousness” with the aim of generating spin-off into day-to-day
patient care was successfully launched in 2007.
The aim of next period is to improve patient care and data management further, maintain and
extend the achievements moving towards an internationally recognized high-quality clinical
trial site in Nouna.

Nous offrons un contrat à durée déterminée (2.5 ans) avec l’Université de Heidelberg dès
octobre 2008 ou à convenir pour un médecin francophone spécialisé en pédiatrie, médecine
générale, chirurgie ou médecine interne. Ce collaborateur sera positionné à l’Hôpital de
District de Nouna / Burkina Faso et partagera avec son/sa collègue burkinabé/e la
responsabilité de l´amélioration de la qualité des soins médicaux, du développement de la
recherche clinique au sein de l´hôpital, de l´élaboration de nouveaux protocoles de
recherche ainsi que de l´écriture d´articles scientifiques en collaboration avec le Centre de
Recherche de Santé de Nouna (CRSN) / Ali Sié. Ces activités font partie d’une coopération à
long terme entre le Programme de Recherche sur le Contrôle des Maladies Tropicales (SFB
Control of Tropical Infectious Disease) de l’Université de Heidelberg et le CRSN.
Ce poste offre la possibilité de a) pratiquer une médecine clinique dans un hôpital de district
en région tropicale, b) développer des capacités de gestion dans un contexte pauvre en
ressources et c) commencer ou poursuivre une carrière de recherche en médecine clinique
tropicale dans le cadre de projets en cours et déjà entièrement financés (voir la liste des
projets de recherche ci-dessous). La poursuite d´une activité clinique au sein de l´équipe
médicale à Heidelberg à l´issue des deux années passées à Nouna est possible. Le salaire
est calculé selon les barèmes de l´Hôpital Universitaire de Heidelberg.
Présentation du poste médical à Nouna / Burkina Faso
Les activités de recherche en collaboration entre Heidelberg et Nouna ont augmenté au
cours de ces dernières années. Parmi celles-ci nous relèverons une étude longitudinale sur
l’association entre le portage du méningocoque et la survenue de la maladie, une étude de
cohorte sur la transmission verticale de mère à enfant du VIH, de même que plusieurs
études pharmacologiques de traitements contre le paludisme. Au sein du Programme de
Recherche sur le Contrôle des Maladies Tropicales une division appuie spécifiquement tout
projet à composante clinique au CRSN ou à l’Hôpital de District de Nouna.
L’hôpital du district de Nouna comprend 88 lits. C´est l´hôpital de référence pour 29
dispensaires couvrant une région d’environ 300.000 habitants. Une aide soutenue du CRSN
a permis la construction d´un Service des Urgences, le développement d´une unité
d´échographie et l’intégration d’un laboratoire de recherche au sein de l´établissement.
Dès 2004 une collaboration étroite entre un médecin spécialiste en médecine tropicale à
Heidelberg et son collègue expatrié à Nouna a été mise en place. La première période
(2004/2005) s´est concentrée sur l’amélioration des procédures et pratiques élémentaires à
l’hôpital, la deuxième période (2006) sur le développement des capacités de laboratoire et la
troisième (2007-2008) sur l´amélioration du Service de Pédiatrie y compris la prise en charge
et le suivi des cas sévères de malnutrition. L’étude d’observation sur la «Discrimination du
paludisme cérébral de la méningite bactérienne chez des enfants présentant une diminution
de l´état de conscience » dont l´objectif est de faciliter la prise en charge initiale des patients
a été lancée avec succès en 2007.
L’objectif de la prochaine période est d´améliorer les soins médicaux, de renforcer la
collection des données, ainsi que de poursuivre les objectifs visant à une reconnaissance
internationale du centre de Nouna dans le domaine de la recherche clinique.
