A retrospective cohort study was conducted after 2 confirmed and 8 probable cases of VTEC 0157 occurred in a schoolgroup from Somerset following a trip to France. The strain of the 2 confirmed cases was verocytotoxin type 2, phage type 34. Despite its low statistical power due to small numbers, this study suggested the most likely vehicle of infection was cucumber salad (crude RR=4.3, 95%CI [0.6-29], adjusted OR=107, 95%CI indeterminate, p=0.08). The cucumbers were purchased from Belgium but it was not possible to trace them back to source. The international nature of this outbreak emphasises the importance of close collaboration between organisations in the management of outbreaks, of ensuring international standards in food safety, and of agreeing a common standard in VTEC typing across Europe. Une infection à VTEC O157 peut se manifester cliniquement par une diarrhée modérée, une colite hémorragique, un syndrome hémolytique urémique, voire un purpura thrombocytopénique thrombotique qui peut être fatal en particulier chez les personnes âgées (2). Des épidémies ont été rapportées dans des pays aussi éloignés les uns des autres que le Royaume-Uni, l’Afrique du Sud, le Japon, et les Etats-Unis (2,3). En Angleterre et au Pays de Galles, les données de surveillance montrent que la transmission par les aliments compte pour 63% des cas d’infection contractés localement (4). Les autres principaux modes de transmission incluent le contact de personne à personne, le contact avec le bétail ou la proximité d’un élevage de bovins, ovins ou caprins (5,6). Il a été estimé que 12% des cas ont été acquis à l’étranger (4). Près de 90% des cas au Royaume-Uni sont sporadiques (5). Les épidémies touchant des voyageurs internationaux y sont rarement identifiées et leurs investigations relèvent généralement d’autres administrations que les agences du RoyaumeUni. Cependant, en 1997, une équipe internationale incluant la Finlande, l’Espagne, la Suède, le Danemark et le Royaume-Uni a enquêté sur une épidémie d’infections chez des touristes ayant séjourné dans les Iles Canaries. L’infection a été associée à la consommation d’eau issue d’un système d’approvisionnement privé (7). Cet article décrit l’investigation d’une flambée d’infection à VTEC O157 dans un groupe scolaire anglais du Somerset, Angleterre, lors d’un voyage en France en mai 2002. L’enquête a débuté lorsqu’un cas a été identifié chez un élève de 15 ans, peu de temps après son retour d’un voyage scolaire avec 29 autres lycéens et trois enseignants. Tous avaient été dans un centre d’études de la langue française à Merlimont, du 3 au 7 mai 2002. Depuis son retour, cet élève s'était senti malade et avait présenté diarrhée, fièvre, céphalées et crampes abdominales le 10 mai. L’analyse d’un échantillon de selles a révélé la présence d’une souche VTEC O157 le 16 mai. Matériels et méthodes ➤ The clinical spectrum of VTEC O157 infection includes mild diarrhoea, haemorrhagic colitis, haemolytic uraemic syndrome and thrombotic thrombocytopaenic purpura, a condition that can be fatal especially in the elderly (2). Outbreaks have been documented from countries as widely dispersed as the United Kingdom (UK), South Africa, Japan, and the United States (2,3). In England and Wales surveillance data indicate that foodborne transmission accounts for 63% of indigenously acquired VTEC O157 cases (4). Other important modes of transmission include person-to-person spread, contact with livestock and environments where cattle, sheep and goats are farmed (5,6). It has been estimated that 12% of cases acquire infection abroad (4). Around 90% of infection in the UK is sporadic (5). Outbreaks affecting international travellers are rarely identified and investigations usually fall outside the remit of UK agencies. However, in 1997 an international team from Denmark, Finland, Spain, Sweden, and the UK investigated an outbreak of infection among tourists visiting the Canary Islands. The infection was found to be associated with the consumption of water from a private supply (7). This paper describes the investigation of an outbreak of VTEC O157 infection associated with a trip to France in May 2002 of pupils from Somerset, England. The investigations started when a 15 year old school boy was diagnosed with VTEC O157 infection shortly after returning from a school trip with 29 other pupils and three staff. All went to a French language study centre in Merlimont between 3 and 7 May 2002. He had felt generally unwell since returning home from France and had developed diarrhoea, fever, headaches and stomach cramps on 10 May. A stool sample analysis revealed a VTEC O157 infection on 16 May. Methods and materials Une étude rétrospective de cohorte a été conduite. A retrospective cohort study was undertaken. The cohort was defined as staff or pupils who participated in the school trip to France. Cases were defined as confirmed if they had any symptoms and had VTEC O157 isolated from their faeces, as probable if they had diarrhoea but no laboratory confirmation of VTEC O157, and as possible if they had other symptoms without laboratory confirmation. Members of this cohort were asked to complete a standard VTEC O157 ‘trawling’ questionnaire. This questionnaire covered a range of potential exposures over the seven days prior to the onset of symptoms in the index case, and included questions on foods eaten and places recently visited such as farms or food venues. Faecal samples were also requested from all staff and pupils on the school trip, as well as family members of the confirmed cases. The questionnaire also covered basic demographic details as well as information on gastrointestinal illness within the last month. This questionnaire was distributed to all participants to the school trip. In France, personnel from the centre and all the other groups that had visited the centre around the time of the visit of the children from Somerset were contacted by the Institut de Veille Sanitaire (National public health surveillance centre) in Paris to identify any other cases of gastro enteritis. Ce second cas était un élève de 15 ans qui avait commencé à se sentir malade durant le voyage à cause de céphalées et de nausées, mais qui n’avait pas présenté de diarrhée par la suite. L’analyse univariée considérant comme malades tous les cas confirmés et probables (n = 10) a montré que les rapports des risques étaient supérieurs à 3.0 pour la salade de concombre, les fruits et les carottes (tableau 2). De plus, les odds ratios pour l’escalope de dinde, les préparations avec des hot-dog et les pommes de terre n’ont pu être directement calculés car aucun de ceux qui en ont mangé ne sont tombés malades. Bien que les odds ratios ne soient pas élevés, 9 des 10 cas ont mangé de la lasagne et des rouleaux de dinde. Ces huit aliments ont tous été inclus dans le modèle de régression logistique multivarié et les variables ont été exclues selon les critères définis. L’élimination séquentielle des aliments étudiés un par un a finalement conduit à un modèle final incluant le concombre, les fruits, les carottes et les préparations avec des saucisses (tableau 3). Les odds ratios les plus élevés ont été observés pour le concombre et les entrées avec les saucisses. This indicated that the strain was Verocytoxin type 2, phage type 34. Stool samples collected from the other pupils and staff who attended the trip to France identified a second confirmed case of VTEC O157, which was subsequently typed and found to be the same strain as the index case. This second case was a 15-year old schoolboy who had started to feel unwell with headache and nausea whilst on the trip, but had not subsequently developed any diarrhoea. Single variable analysis defining confirmed and probable cases as ill (n=10) showed that the risk ratios were greater than 3.0 for cucumber salad, fruit and carrots (table 2). In addition, the risk ratios could not be directly calculated for the turkey escalope, hot-dog pastry and potatoes, since none were ill in those known not to have eaten the food. Although the risk ratios were not high, nine out of ten cases ate lasagna and turkey roll. These eight food items were all included in a multi-variable logistic regression model and the variables were excluded according to the defined criteria. Consecutively choosing to exclude one food item at a time on this basis led to a final model which included cucumber, fruit, carrots, and hot-dog pastry (table 3). The highest odds ratios were found for cucumber and hot-dog pastry. This model included seven ill children, all seven of whom ate cucumber and hot-dog pastry. The cucumber and hot dog pastry were again found to have high odds ratios, although neither of the p values for these two food items were significant at the 5% level. Etant donné la période d’incubation de 2 à 8 jours, l’étude de la courbe épidémique suggérait une source d’exposition commune au début du voyage. Les analyses statistiques des questionnaires spécifiques sur les aliments ont mis en évidence deux produits différents très fortement associés à une infection confirmée ou probable par une souche d’E. coli O157 vérocytotoxique : la salade de concombres et les entrées au hot-dog. La puissance statistique de cette étude étant faible, et la preuve microbiologique de la contamination n’ayant pu être confirmée, il n’a pas été possible d’identifier le véhicule de l’infection. Samples from this batch were tested at the production centre and found to be negative for microbiological contamination. Discussion An outbreak of VTEC O157 infection, comprising two confirmed and eight probable cases occurred among pupils and staff on a school trip to France in May 2002. Given a 2-8 day incubation period, the epidemic curve for this outbreak was consistent with a point source exposure early in the trip. Statistical analyses of food specific questionnaires highlighted two different types of food as having the strongest associations with confirmed or probable VTEC O157 infection: cucumber salad and hot-dog pastry. As statistical power of this study was low and microbiological evidence of contamination was not confirmed, it was not possible to identify the vehicle of infection. However cucumber is a plausible vehicle, the time of eating cucumber salad was consistent with the expected time of exposure to the source, and association between illness and cucumber salad had the lowest p value in multivariable analysis. This is the first reported investigation of an outbreak of VTEC O157 infection affecting residents in England and Wales in which foods consumed abroad have been implicated. Most foodborne outbreaks of VTEC O157 infection reported in the literature have been associated with food of bovine origin, i.e. beef and dairy products. Chicken, the main constituent of the hot dog pastry in this outbreak, is an uncommon source of VTEC infection. Several outbreaks associated with the consumption of raw fruit and vegetable products have been reported in recent years (8-13). While some are likely to be due to crosscontamination from meat products during food preparation, others are probably due to direct contamination with animal faeces in the field (14). VTEC O157 phage type 34 is an unusual phage type in England and Wales, accounting for only 4% of all isolates in 2001 (15), but it was the second most common phage type in a survey of Escherichia coli among Belgian cattle (16). The application of phage typing for VTEC across Europe is important for the investigation of outbreaks that cross international boundaries. One of the objectives of Enter-net (17), the European surveillance for enteric pathogens, is to promote the use and reporting of the results of VTEC O157 phage-typing on an international basis. However, routine phage typing of VTEC is not currently undertaken in most European countries and we were not able to identify other cases who might have been infected from the same source. This outbreak highlights several important areas for consideration. Firstly, prompt and close collaboration between organisations is especially important in the management of any outbreak that spans international boundaries, to ensure timely and effective investigations and control measures are undertaken. Secondly, the increase in international travel and the range of food substances implicated as sources of potentially serious illness emphasize the importance of ensuring high international standards in food safety. These standards should enable to trace any food product back to the original source, thus ensuring potential sources of infection can be investigated and appropriate control measures instigated. Thirdly, agreement on one standard VTEC typing system across Europe is essential for the investigation of outbreaks that cross country boundaries. An unexplained outbreak of skin disorder with itching papules or pustules occurred among drugs body packers in a detention clinic in the Netherlands in October–November 2002. The descriptive cohort study suggests this outbreak could be associated with an expired batch of disposable linens, with an overall attack rate of 30% in the study population (n=118). No additionnal cases have been noted since this expired batch was replaced. Les informations suivantes ont été obtenues : données démographiques (par ex., date de naissance, sexe, pays de naissance), informations sur l'arrivée et le départ des ➤ Background n 24 October 2002, the National Institute of Public Health Oand the Environment (RIVM) in the Netherlands received a call from the Ministry of Justice regarding an unusual amount of skin problems (papules/pustules syndrome) in a detention centre in the north of the Netherlands. This detention centre (clinic A) was founded in 2002 as a special clinic/detention centre for body packers. Body packers are individuals who have swallowed or inserted into the vagina or rectum cylindrical shaped containers containing hardened cocaine or heroin (1,2). The majority of body packers arrive in the Netherlands by plane. To slow down intestinal movements during the flight, most body packers consume high doses (10-16 mg in 24 hours) of Loperamide (an anti-diarrheal agent) just before embarking on the plane. Upon arrival at the international airport, the Dutch border police detain nearly all of them. Within two hours of arrest, all body packers are transferred to clinic A. Typically, most body packers stay in clinic A for at least 1-2 weeks until all containers are expelled and the contents recovered. Thereafter, male inmates are transferred to prison A, which is adjacent to clinic A. Female inmates are transferred to other detention centres in the Netherlands. On 28 October 2002, the Ministry of Justice, in collaboration with EPIET (European Programme for Intervention Epidemiology) and the Regional Health Services, decided to send a team to the site to carry out an epidemiological investigation with the following objectives: 1) to describe the extent of the outbreak; 2) to identify a possible cause for the outbreak; 3) to identify the population at risk; 4) to exclude a possible infectious source; and 5) to provide recommendations. The following report summarises the results of the outbreak investigation. Methods Study design A descriptive cohort study was initiated to assess the extent of the outbreak. The main source of information to identify cases was a questionnaire survey of all inmates present in ➤ EUROSURVEILLANCE VOL. 8 - N° 04 AVRIL / APRIL 2003 87 ➤ divers centres pénitentiaires, informations sur le tableau clinique (début des dermatoses) et utilisation de médicaments. No analytical study was conducted as part of this assessment. Case definition A probable case was defined as an inmate with a specified rash (papules or pustules) on the face, neck, abdomen, or upper back or another rash with itching, occurring after arrival in clinic A and present at clinic A or prison A on 4 or 5 November 2002. The most plausible hypothesis suggested is that the use of disposable bed linens may have caused an irritable contact dermatitis. Therefore, it is suggested that sweating may have caused an extensive direct exposure to the salt concentration and that this may have caused the occurrence of the skin problems on the face/neck (ie contact points for the pillowcase) and upper back/abdomen region (ie contact points with bed sheet). In addition, since week 43, the inmates were not provided with body lotion and some inmates with more sensitive skin may have had some cutaneous irritation. This, and the exposure to the disposable linens, may have caused the skin problems observed. Since no complaints have been observed since the replacement of the expired batch, this would indicate a possible association between the itching papules/pustules syndrome and the disposable bed linens. However, this hypothesis should be tested with an analytical epidemiological study. Other risk factors, such as the use of Loperamide, and laundry and cleaning detergents were considered. Experts judged it implausible, however, that these factors could have caused the outbreak, and the factors were not considered further. Aussi, l'établissement d'une base de données en ligne pour les dossiers médicaux a été recommandée dans tous les centres pénitentiaires pour pouvoir suivre et mettre à jour les données sur tous les détenus de leur arrivée jusqu'à leur libération. Un tel système devrait aussi pouvoir être utilisé pour la surveillance épidémique et les alertes précoces, et ainsi faciliter les investigations épidémiques futures. ■ In addition to curbing the outbreak, the study did reveal several other important issues for the logistics of detention centres for body packers in general. First, that the knowledge of dermatological problems in dark skinned people was limited among the medical staff, resulting in unwarranted anxiety. Therefore, additional training in dermatological problems in dark skinned people was recommended for medical staff employed in detention centres in the Netherlands. Second, in order to provide the inmates with adequate medical care, it is imperative to have an updated complete (electronic) medical record that is accessible at each detention centre where the inmate is housed. Therefore, a relational online database for medical records at all detention centres was recommended to assure complete and follow up data on all inmates from time of arrival until release. Les formulaires de notification du paludisme sont renseignés par le médecin traitant hospitalier, puis envoyés au responsable régional de Santé publique qui les fait suivre au FMSG. Toutes les données manquantes sont collectées par le département d’épidémiologie du ministère auprès de l’hôpital concerné et saisies dans une base de données EPI INFO. Les analyses de tendance ont été effectuées avec SPSS en utilisant un modèle de logarithme linéaire et des modèles non paramétriques (9). In Austria, the case definition for confirmed cases as described in the EU decision (8) is used: “A confirmed case of malaria is a case with an episode of laboratory –confirmed malaria parasitemia in any person symptomatic or asymptomatic”. Reporting forms on malaria are filled in by the treating hospital doctor and then delivered to the district public health officer who sends the forms to the FMSG. Any missing data are collected from the treating hospital by the epidemiological department of the ministry, and entered into an EPI INFO database. Trend testing was done in SPSS by using a log linear model and non–parametric models (9). The proportion of P. falciparum infections is increasing and could lead to increasing numbers of severe malaria cases in the future. The appearance of six malaria cases due to P. falciparum within a five week period in Tyrol in 1999 resulting in two deaths despite intensive care at an early stage of the disease, might indicate this development (4). Discussion Les données de notification du paludisme associé aux voyages entre 1990 et 2000 ont été analysées. En moyenne, 84 cas ont été déclarés par année. La proportion d’infections à P. falciparum est en augmentation et pourrait entraîner une élévation du nombre de cas de formes graves du paludisme dans l’avenir. Les six cas de paludisme à P. falciparum survenus en cinq semaines au Tyrol en 1999 avec une issue fatale pour deux personnes malgré les soins intensifs à un stade précoce de la maladie pourraient confirmer cette tendance (4). De façon prévisible, la plupart des cas de paludisme ont été contractés dans les zones hautement endémiques (région C). Les plus grands nombres de cas ont été rapportés dans les Etats fédéraux les plus peuplés, probablement en raison du plus grand nombre de touristes. As expected, most malaria cases appeared in high endemic areas (Region C). Tourists aged between 31 and 45 years were most affected suggesting these age groups are most active in travelling to tropical regions. Again, due to lack of information on age distribution in the total number of tourists, an age dependent risk cannot be verified statistically. The annual number of infections in children up to five years was between one and six without trend for increase or decrease. There was also no change in the number of lethal cases (0–3/year). As expected, highest numbers of cases were reported from the Federal States with highest population. Results of preventive measures were of special interest. Data on type of chemoprophylaxis were available in about 80% of cases. Mefloquine and Chloroquine were most commonly used. Although data on compliance were available for about half of the cases only, the analysis showed that only about two thirds of the cases completed chemoprophylaxis. Adequate chemoprophylaxis was identified as major problem in an earlier study on Austrian tourists travelling to tropical countries (11). A study on German tourists stated that only 31% of malaria patients completed chemoprophylaxis (12). Malheureusement, en 2000, l’utilisation du questionnaire détaillé de TropNetEurop a entraîné une détérioration significative de la qualité des données épidémiologiques et probablement aussi un taux de déclaration moins élevé. Il a fallu recourir à de nombreux appels téléphoniques pour obtenir les données manquantes. Un formulaire amélioré de notification du FMSG a été élaboré et mis en circulation en 2002. Ce nouveau formulaire insiste spécialement sur les questions concernant la prophylaxie (13). Les limites Le principal problème dans l’évaluation des cas importés de paludisme réside dans la détermination du dénominateur : le nombre total de voyageurs reste inconnu, cette donnée n’étant accessible qu’aux agences de voyage (12). Les données sur le tourisme recueillies par des organisations internationales comme l'Organisation mondiale du commerce ne sont diffusées qu’avec un retard considérable et ne contiennent pas suffisamment de détails pour permettre d'obtenir un dénominateur. Seule une évaluation descriptive était donc possible, et non une analyse des risques. De plus, les données sur la chimioprophylaxie ne sont disponibles que depuis 1992, celles sur l’âge depuis 1995 et sur le sexe depuis 2000. Bien que la qualité des données de notification doive être améliorée, nous estimons qu’il existe une réelle sous-notification des cas de paludisme et que seuls 60 à 70% de la totalité sont déclarés. L'exploitation des bases de données de notifications reste cependant acceptable, à condition d’en énoncer clairement les limites et d’interpréter les résultats avec prudence. Les mesures de santé publique recommandées Le paludisme associé au voyage représente une menace sérieuse pour les touristes allant dans des pays tropicaux. Les campagnes de santé publique doivent être concentrées sur deux axes essentiels : improved FMSG notification form was developed in 2002 with particular emphasis on questions relating to prophylaxis (13). Limitations The main problem in assessing malaria imported cases was to determine the denominator: the total number of travellers cannot be determined because this data is only accessible to travel companies (12). Data on tourism collected by international organisations such as the World Trade Organization are only available with substantial delays and are not detailed enough to define a denominator. Thus it was only possible to perform a descriptive evaluation and not a risk analysis. Furthermore, data on chemoprophylaxis have only been available since 1992, data on age since 1995, and data on gender since 2000. Although the quality of reporting data has to be improved, we estimate that malaria is severely under reported and that only approximately 60–70% of the total cases are reported. It is still valid, however, to explore reporting data sets, as long as limitations are clearly stated and results are interpreted with caution. Recommended Public Health measures Travel associated malaria is a serious threat for tourists travelling to tropical countries. Public health actions have to be focussed on two major interventions: 1. Better information on malaria should be released towards tourists and promotion of compliance to chemoprophylaxis (11,15). Additionally, the use of repellent substances and proper clothes, and bed nets impregnated with Permethrine should be encouraged (16). 2. Early diagnosis: early stages of malaria are most often misdiagnosed due to the influenza–like symptoms (17). Heinrich et al showed that malaria was mentioned as suspected diagnosis by the referring doctor in only one third of cases (18). Knowing early diagnosis of malaria and early patient management can prevent severe complications, every symptomatic tourist returning from a tropical country should be checked for malaria on an obligatory basis. ■ 1. Les voyageurs ont besoin de motivations pour suivre un traitement prophylactique adapté. Ils doivent être mieux informés sur les modes de transmission et la gravité du paludisme (11,15). Ils doivent aussi être encouragés à suivre d'autres méthodes de protection, comme le recours aux insecticides, le port de vêtements adéquats et l'utilisation de moustiquaires imprégnés de perméthrine (16). 2. Diagnostic rapide : les premiers stades du paludisme sont souvent mal diagnostiqués en raison des symptômes similaires à ceux de la grippe (17). Heinrich et al ont montré que le diagnostic de paludisme avait été mentionné par le médecin référent chez un tiers seulement des cas de paludisme (18). Un diagnostic et une prise en charge rapide du patient peuvent éviter la survenue de complications sévères.
