PROTECTED A when completed 758 ARGUS SQUADRON RC(AIR)CS PERMISSION FORM 758 Squadron will be conduction an Aircrew Survival Field Training Exercise activity as part of our mandatory training at Albion Hills Conservation Area, 16500 Hwy 50, Palgrave ON, L0N 1P0, Tel 905880-4855 on the weekend of 27 to 29 May 2016. Cadets will report to Local Headquarters (LHQ), 55 Avondale Boulevard, Brampton, Ontario at 1800 hrs (6:00pm) on Friday 27 May 2016 with Health Card and personal kit per Personal Kit List (attached). Cadets will return and shall be picked up at the LHQ at 1500 hrs (3:00pm) on Sunday 29 May 2016. In case of a family emergency during the activity, the emergency contact number to reach the squadron is 905-793-7992. Alternate is 905-880-4855. Parent / Guardian are asked to complete this form as well as the Inspection Consent Form and submit to Lt Sharma by no later than Monday, 09 May 2016 @ 21:30 hrs (9:30pm). Please keep a copy of this form for yourself. NOTE: Health Card check will be done by staff before the cadet boards the bus to depart. Cadet will not be able to participate in the activity if they do not have a health card, have inappropriate or insufficient kit, or do not have a signed Permission, and Inspection Consent Form. Dress is field Training Uniform or civilian attire as listed on the Personal Kit List (attached). Rations are provided for this activity, and accommodations are by tent. Cadets with epi-pens or inhalers must bring them with them or they will not be allowed to participate. Corrective measures could be taken against a cadet for failing any criteria or search or for refusing to submit to an inspection, in accordance with CATO 15-22 Conduct and Discipline – Cadets, up to and including being expelled from the training or activity. Please pack according to the kit list and clearly label all your items and your kit bag. 2. All prescription medication(s) must be in its original container and clearly labeled 3. DO NOT BRING ANY OF THE FOLLOWING or it will be confiscated for the duration of the exercise: a. Any form of Electronic Devices (including cell phones and cameras), Knives, Pyrotechnics, Food/Drink (food will be confiscated as it attracts animals into the camp site), etc. 4. Denim (Jeans) and inappropriate / oversized clothing is NOT allowed. 5. HEALTH CARD IS REQUIRED – You will not be allowed to board the bus without it! 6. Note that clothing and kit will get wet and dirty. Bring all clothing as detailed below. 7. Cadets are not authorized to wear CADPAT (Canadian Disruptive Pattern Field Uniform) but may wear the OG-107 CF Combat Field Uniform in accordance with SSOs. If you wish to purchase Combat Field Uniform and don’t know where, please speak with your Flight Sergeant or check the for sale board outside the training office as some personnel may have kit to sell. PARENTS: Please ensure that this kit list is followed. If your Cadet does not come properly dressed or prepared for the exercise, you will be notified to make arrangements for immediate pick up of the Cadet to return home. This will be strictly enforced for the safety of the Cadets.
