758 Squadron will be conduction an Aircrew Survival Field Training Exercise activity as part of our
mandatory training at Albion Hills Conservation Area, 16500 Hwy 50, Palgrave ON, L0N 1P0, Tel 905880-4855 on the weekend of 16 to 18 October 2015.
Cadets will report to Local Headquarters (LHQ), 55 Avondale Boulevard, Brampton, Ontario at 1800 hrs (6:00pm) on
Friday 16 October 2015 with Health Card and personal kit per Personal Kit List (attached). Cadets will return and shall
be picked up at the LHQ at 1500 hrs (3:00pm) on Sunday 18 October 2015.
In case of a family emergency during the activity, the emergency contact number to reach the squadron is 905-793-7992.
Alternate is 905-880-4855.
Parent / Guardian are asked to complete this form as well as the Inspection Consent Form and submit to Lt Sharma by no
later than Monday, 05 October 2015 @ 21:30 hrs (9:30pm). Please keep a copy of this form for yourself.
NOTE: Health Card check will be done by staff before the cadet boards the bus to depart. Cadet will not be able
to participate in the activity if they do not have a health card, inappropriate or insufficient kit, and signed
Permission, and Inspection Consent Form.
Dress is field Training Uniform or civilian attire as listed on the Personal Kit List (attached). Rations are provided for this
activity, and accommodations are by tent. Cadets with epi-pens or inhalers must bring them with them or they will not be
allowed to participate. Medications brought on this activity must be in the original container which shall be clearly
marked with Cadets name as well as usage instructions.

INSPECTION CONSENT

INSPECTIONS AND SEARCHES
INSPECTIONS ET FOUILLES
At different moments during the training or activity, the cadet
may be subjected to an inspection of his/her sleeping
accommodations, luggage, kit, equipment, locker(s) or storage
area(s). These will be conducted or supervised by a Canadian
Armed Forces member, and will serve to verify that:

the cadet’ sleeping accommodations, locker(s) and storage
area(s) are clean and orderly;
•
la chambrée, le(les) casier(s) et espaces de rangement
sont propres et ordonnés;
•
the cadet is carrying the proper equipment and it is in good
condition and properly maintained;
•
le cadet a en sa possession le matériel approprié et que
celui-ci est en bon état et entretenu adéquatement;
•
the cadet’s health and safety is not at risk; and
•
la santé et la sécurité du cadet n’est pas à risque; et
•
the cadet does not have prohibited, restricted or
unauthorized items in his/her possession, as listed in the
joining instructions or other information document provided
under separate correspondence.

If found, prohibited and restricted items will be handed to the
applicable police agency (military or civilian) with some
exceptions, while unauthorized items shall either be
confiscated for the duration of the training or activity or sent
back to the cadet’s home at the parents expense, whichever is
more practical and economical with some exceptions.

Corrective measures could be taken against a cadet for failing
any criteria or search or for refusing to submit to an inspection,
in accordance with CATO 15-22 Conduct and Discipline –
Cadets, up to and including being expelled from the training or
activity.

échoue tout critère d’une inspection ou d’une fouille, ou refuse

If needed, searches of a cadet’s person, property, lockers,
luggage, kit or sleeping area for the purposes of discovering
contraband, illicit or stolen property, or some evidence of guilt
to be used in the prosecution of an offence, will be conducted
only by the Military Police or a civilian police agency.

PERSONAL KIT LIST – SPRING / FALL WEEKEND FTX
1. Pack according to the kit list and clearly label all your items and your kit bag.
2. All prescription medication(s) must be in its original container and clearly labeled
3. DO NOT BRING ANY OF THE FOLLOWING or it will be confiscated for the duration of the
exercise: Any form of Electronic Devices (including cell phones and cameras), Knives, Pyrotechnics,
Food/Drink (food will be confiscated as it attracts animals into the camp site) Denim (Jeans) and
inappropriate / oversized clothing is NOT allowed. Officers may confiscate other items if deemed
inappropriate.
4. HEALTH CARD IS REQUIRED – You will not be allowed to board the bus without it!
5. Cadets are not authorized to wear CADPAT (Canadian Disruptive Pattern Field Uniform) but may wear
the OG-107 CF Combat Field Training Uniform in accordance with SSOs. If you wish to purchase
Combat Field Uniform and don’t know where, please speak with your Flight Sergeant or check the for
sale board outside the training office as some personnel may have kit to sell.
6. Note that clothing and kit will get wet and dirty. Bring all clothing as detailed below.
PARENTS: Please ensure that this kit list is followed. If your Cadet does not come properly dressed or
prepared for the exercise, you will be notified to make arrangements for immediate pick up of the Cadet to
return home. This will be strictly enforced for the safety of the Cadet.
