Introduction
The incidence of invasive fungal disease (IFD) is increasing in all parts of the
world. The etiology for this ongoing epidemiological development is not
completely understood. However, major contributing factors are the increasing
number of transplantation procedures undertaken around the world (estimated
at 500,000 per year), a widening of the indications for intensive chemotherapy,
and the growing number of other clinical conditions requiring
immunosuppressive treatment.
Therapeutic standards have been developed for the most frequent IFD, i.e.
candidiasis, aspergillosis and cryptococcosis. However, the so called “emerging
fungi” are also a reason for the increased number of IFDs. Thus, clinicians are
now facing infections due to a variety of different fungi without any reliable
treatment recommendations and therapeutic decision making is not evidencebased. Prevalence of “emerging fungi” is virtually unknown and there is an
additional knowledge gap in pathogen biology, since isolates have not been
collected systematically.
These shortcomings have been described in detail in the recent Clinical Practice
Guidelines of the European Fungal Infection Study Group (ESCMID-EFISG) and
the European Confederation of Medical Mycology (ECMM).
In order to help alleviate the lack of knowledge on epidemiology, clinical
course, biology and pathomechanisms, and finally to aid in facilitating an
evidence based diagnostic-therapeutic integrated approach of IFD caused by
“emerging fungi”, Fungiscope™ - A Global Emerging Fungal Infection Registry
has been created in 2003.
FungiScope™ uses a web-based electronic case form accessible via
www.fungiscope.net.
For inclusion into the registry cultural, histopathological, serological or
molecular evidence of IFD and associated clinical symptoms and signs are
required. The clinical data on underlying disorder and IFD are entered through
a web-based interface. Ideally, the pathogen isolated and samples of infected
tissue are collected, examined and stored in a bio-bank called FungiThek. The
clinical and biological data are subsequently evaluated and analysed. Figure 1a
and b present the data / sample flow and overall project structure,
respectively. The clinical database and the bio-bank are connected, and
anonymized data are easily accessable via a web-based search engine FungiQuest (Figure 2).
In the following report we introduce the most current results and
achievements. We also discuss future goals and ongoing developments of the
project.

Current Results
Until now, 444 cases have been documented and considered valid. The most
frequently registered pathogens were mucoromycotina (n=201; 45.3%), Fusarium
spp. (n=65; 14.6%), yeasts (n=56; 12.6%), and Dematiaceae (n=50; 11.3%)
followed by Scedosporium spp. (n=21; 4.7 %) and Penicillium spp./Paecilomyces
spp. (n=20; 4.5%) (Figure 3). The contribution of cases by country is shown in
Figure 4. A list of major contributors is given in Figure 5, whereby the majority of
cases has been documented by central laboratories that have an intermediary
function between contributors from their country and FungiScope. The majority of
documented patients were Caucasian (71%) followed by Asian (23%) and
North/South American Indian origins (2%) (Figure 6).
Sites of infection differ among pathogens (Figure 7). In patients with invasive
mucormycosis, the lungs were the most commonly involved organs with an
incidence of 70.1%. Other involved sites were the paranasal sinuses (21.4%), deep
soft tissues (14.9%) and the central nervous system (13.4%). Fusarium spp.
infections most frequently presented as blood stream infections (46.2%) or skin
infections (46.2%). Other commonly affected sites were lungs (36.9%) and deep
soft tissues (21.5%). Infections with Dematiaceae mostly involved the paranasal
sinuses, eyes, deep soft tissues, lungs, and skin (range from 12.2% to 28.6%).
Yeasts were most often diagnosed from blood cultures (67.9%) followed by
pulmonary infections (30.4%).
The evaluation of risk factors for the development of IFD revealed differences
between individual pathogens shown in Figure 8. The most common risk factors for
developing an infection with Mucorales and Fusarium spp. were chemotherapy
(55.9% and 56.9%, respectively) or haematopoetic stem cell transplantation
(HSCT) (31.6% and 29.2%, respectively) followed by Intensive Care Unit (ICU) stay
(26.4% and 20%, respectively) and diabetes mellitus (16% and 17%, respectively).
In patients presenting with yeast infections chemotherapy was also the most
common risk factor (53.6%). In addition, 37.5% of patients were treated in
intensive care units. Other risk factors were haematopoetic stem cell transplantation
(21.4%), diabetes mellitus (14.3%), chronic pulmonary disease (12.5%), and solid
organ transplantation (8.9%).
By contrast, patients with a Dematiaceae infection presented most frequently with
diabetes mellitus (42%) and/or chronic renal disease (12%).
All-cause-mortality, mortality due to fungal infection and response to treatment
differed among individual pathogens and are shown in Figure 9. The highest allcause-mortality was observed in patients with mucoromycosis (56.5%), followed by
patients with yeast (55.4%) and Fusarium spp. infections (49.2%). When looking
at the mortality due to the fungal infection it was highest in infections caused by
Mucorales (43.8%) followed by Fusarium spp. (36.9%) and yeast (38.2%). By
contrast, the lowest all-cause-mortality was revealed in patients with Dematiaceae,
equaling only 12.2%. Just 4% of these deaths was due to the fungal infection. The
highest rates of complete and partial remissions were observed in patients with IFD

caused by Dematiaceae and Fusarium spp (74% and 50.8%, respectively). Patients
with a Mucorales infection displayed the lowest rate of response to treatment
(38.8%) followed by yeast (48.2%).
In conclusion, the clinical relevance of invasive fungal infections by emerging fungi
is increasing. Over the past 10 years a large number of cases from several countries
world-wide could be documented and showed a broad spectrum of pathogens.
Further investigators and coordinators are encouraged to contribute to the success
of FungiScope™.

Public Relation Activities
We were pleased to present FungiScope at ICAAC 2013 in Denver at a booth
in the exhibition hall. There we offered information about the project in
pamphlets as well as reminders of the activity (Fungiscope pens and post-it
pads). Over the 3 days of the
convention, an estimated 100
conversations, either brief or
more in-depth, were held about
the project with interested booth
visitors. The majority of visitors,
who were mainly clinicians or
microbiologists, had never heard
of
Fungiscope,
and
many
commented that there is a
definite need for this registry and
were happy to realize that it
exists and could be a receptacle
for cases from their institution. Approximately 250 flyers were taken, along
with 500 pens and 250 post-it pads.
Karen Pankraz, TA from our team, was at the booth fulltime to welcome
inquiries and distribute information. Additionally, Dr. Wahlers, Dr. Vehreschild
and Prof. Cornely were often present to answer specific and in-depth questions
about the project. Where appropriate, contact information was collected for
future collaboration.
Overall, this was a positive experience for our team—many inquiries, much
positive feedback.
