We sought to clarify the mechanism for the influx of tissue fluid into the epidermis and the loss of cohesion between keratinocytes in acute eczema that result in spongiosis. We first demonstrated increased intercellular accumulation of hyaluronan (ha) in the spongiotic epidermis by immunochemical staining using hyaluronicacid-binding protein (haBP) and augmented hyaluronan synthase 3 (has3) mrna expression by spongiotic keratinocytes using in situ hybridization. We also showed that the epidermis where the intercellular space was strongly stained with haBP showed weaker expression of membrane e-cadherin. next, we demonstrated—by a sandwich assay using haBP, real-time Pcr, and flow cytometry—that, among various cytokines, only IL-4, IL-13, and IFn-g increased ha production, enhanced has3 mrna expression, and decreased membrane e-cadherin expression by normal human epidermal keratinocytes in both low- and high-ca media. Finally, we demonstrated IL-4, IL-13, their combination, and IFn-g could induce intercellular space widening of the epidermis with increased ha accumulation and decreased ecadherin expression in the organotypic culture. these results suggest that the augmented production of ha and the decreased e-cadherin expression by keratinocytes stimulated with IL-4/IL-13 or IFn-g cause spongiosis in acute eczema. new england Journal of medicine 2009, 360: 2416-25. Background: the cryopyrin-associated periodic syndrome (caPs) is a rare inherited inflammatorydisease associated with overproduction of interleukin-1. canakinumab is a humananti– interleukin-1β monoclonal antibody. Methods: We performed a three-part, 48-week, double-blind, placebo-controlled, randomized withdrawal study of canakinumab in patients with caPs. In part 1, 35 patients received 150 mg of canakinumab subcutaneously. those with a complete response totreatment entered part 2 and were randomly assigned to receive either 150 mg of canakinumab or placebo every 8 weeks for up to 24 weeks. after the completion of part 2 or at the time of relapse, whichever occurred first, patients proceeded to part 3 and received at least two more doses of canakinumab. We evaluated therapeutic responses using disease-activity scores and analysis of levels of c-reactive protein (crP) and serum amyloid a protein (saa). Results: In part 1 of the study, 34 of the 35 patients (97%) had a complete response to canakinumab. of these patients, 31 entered part 2, and all 15 patients receiving canakinumab remained in remission. Disease flares occurred in 13 of the 16 patients (81%) receiving placebo (P<0.001). at the end of part 2, median crP and saa values were normal (<10 mg per liter for both measures) in patients receiving canakinumab but were elevated in those receiving placebo (P<0.001 and P = 0.002, respectively). of the 31 patients, 28 (90%) completed part 3 in remission. In part 2, the incidence of suspected infections was greater in the canakinumab group than in the placebo group (P = 0.03). two serious adverse events occurred during treatment with canakinumab: one case of urosepsis and an episode of vertigo. Conclusions: treatment with subcutaneous canakinumab once every 8 weeks was associated with a rapid remission of symptoms in most patients with caPs. Design: randomized, double-blind, placebocontrolled clinical trial. Setting: a tertiary care referral hospital. Patients: the study included patients of either sex (age range, 18-65 years) with moderate to severe chronic plaque-type psoriasis. Patients were excluded if they were of child-bearing potential or if they had impaired liver or renal function, hyperlipidemia, diabetes mellitus, coronary artery disease, or a body mass index greater than 30 (calculated as weight in kilograms divided by height in meters squared). of the 62 patients screened, 41 were randomly assigned to 2 groups: 22 to an acitretin (25 mg) plus placebo group and 19 to an acitretin (25 mg) plus pioglitazone hydrochloride (15 mg) group. Main Outcome Measure: change in Psoriasis area and severity Index score between the 2 groups from baseline to 12 weeks. results: after 12 weeks of therapy, the percentage of reduction in the Psoriasis area and severity Index score was 64.2% in the acitretin plus pioglitazone group and 51.7% in the acitretin plus placebo group. the majority of the adverse events were mild to moderate except for 1 possibly unrelated episode of acute myocardial infarction in a 49-year-old woman in the acitretin plus placebo group. Conclusions: Pioglitazone has a potential beneficial antipsoriatic effect and may provide a convenient, efficacious, and relatively safe option to combine with acitretin, although further studies are needed. Journal of Investigative Dermatology 2009, 129: 1388–1394 Pseudoxanthoma elasticum (PXe) is an autosomal recessive multisystem disorder characterized by ectopic connective tissue mineralization, with clinical manifestations primarily in the skin, eyes, and cardiovascular system. there is considerable, both intra- and interfamilial, variability in the spectrum of phenotypic presentation. Previous studies have suggested that mineral content of the diet may modify the severity of the clinical phenotype in PXe. In this study, we utilized a targeted mutant mouse (abcc6_/_ ) as a model system for PXe. We examined the effects of changes in dietary phosphate and magnesium on the mineralization process using calcification of the connective tissue capsule surrounding the vibrissae as an early phenotypic biomarker. mice placed on custom-designed diets either high or low in phosphate did not show changes in mineralization, which was similar to that noted in abcc6_/_ mice on control diet. however, mice placed on diet enriched in magnesium (fivefold) showed no evidence of connective tissue mineralization in this mouse model of PXe. the inhibitory capacity of magnesium was confirmed in a cell-based mineralization assay system in vitro. collectively, our observations suggest that assessment of dietary magnesium in patients with PXe may be warranted. Design: a retrospective review of 245 patients with morphea. Setting: university of texas southwestern medical Center–affiliated institutions. Patients: Patients with clinical findings consistent with morphea. Main Outcome Measures: Prevalence of concomitant autoimmune diseases, prevalence of familial autoimmune disease, prevalence of extracutaneous manifestations, and laboratory evidence of autoimmunity (antinuclear antibody positivity). secondary outcome measures included demographic features. Results: in this group, adults and children were affected nearly equally, and african americans were affected less frequently than expected. the prevalence of concomitant autoimmunity in the generalized subtype of morphea was statistically significantly greater than that found in all other subtypes combined (P=.01). Frequency of a family history of autoimmune disease showed a trend in favor of generalized and mixed subgroups. the linear subtype showed a significant association with neurologicmanifestations, while general systemic manifestations were most common in the generalized subtype. antinuclear antibody positivity was most frequent in mixed and generalized subtypes. Conclusions: High prevalences of concomitant and familial autoimmune disease, systemic manifestations, and antinuclear antibody positivity in the generalized and possibly mixed subtypes suggest that these are systemic autoimmune syndromes and not skin-only phenomena. this has implications for the management and treatment of patients with morphea. Objective: to examine the effectiveness of wet dressings for pruritic dermatoses. Methods: this is a retrospective study of adult patients admitted to our inpatient dermatology service between January 1, 2004, and august 31, 2007, treated with wet dressings and topical corticosteroids. improvement was evaluated 1 day after admission and at dismissal. Results: three hundred thirty-one patients with pruritus (54 unique diagnoses) had 391 admissions. improvement was reported for 146 (94%) of 156 admissions at 1 day after admission and for 351 (98%) of 357 admissions at dismissal. Limitations: retrospective nature of study. Conclusions: Wet dressings effectively alleviate recalcitrant pruritic dermatoses in adults. the lack of published reports on this treatment method suggests that wet dressings are underused. creased 2-fold at week 24. actinic keratoses and photoaging were statistically significantly improved. most patients rated photoaging as improved and were willing to undergo the therapy again. Conclusions: topical fluorouracil causes epidermal injury, which stimulates wound healing and dermal remodeling resulting in improved appearance. the mechanism of topical fluorouracil in photoaged skin follows a predictable wound healing pattern of events reminiscent of that seen with laser treatment of photoaging. Design: nonrandomized, open-label 2-week treatment with fluorouracil cream, 5%, followed by clinical and molecular evaluation. Setting: academic referral center. Patients: twenty-one healthy volunteers, 56 to 85 years old, with actinic keratoses and photodamage. interventions: twice-daily application of fluorouracil cream for 2 weeks and biopsies and clinical evaluation at baseline and periodically after treatment. Main Outcome Measures: gene and protein expression of molecular effectors of epidermal injury, inflammation, and extracellular matrix remodeling 24 hours after fluorouracil treatment; clinical improvement measured by evaluators, photography, and patient questionnaires. Results: one day after the final fluorouracil treatment, gene expression of the effectors of epidermal injury (keratin 16), inflammation (interleukin 1_), and extracellular matrix degradation (matrix metalloproteinases 1 and 3) was significantly increased. types i and iii procollagen messenger rna were induced at week 4 (7-fold and 3-fold, respectively). type i procollagen protein levels were in- topical hemotherapy is a method of applying heparinized venous blood directly onto the surface of an ulcer, which is covered with an occlusive hydrocolloidal dressing. it is often effective on chronic ulcers with thick necrosis, because some proteinases and growth factors in plasma are probably involved in the digestion of necrotic tissues and the acceleration of granulation and epithelization. We treated a patient with an intractable ulcer on the toe caused by a peripheral circulatory disturbance due to her systemic sclerosis. as conventional topical hemotherapy cannot be applied to ulcers on the round tip of a toe, we made a device for occlusive dressing of topical hemotherapy which could successfully improve the previously refractory ulcer. Saunders 1992 Fokus Akne Combined oral contraceptive pills for treatment of acne Background: acne is a common skin disorder among women. although no uniform approach to the management of acne exists, combination oral contraceptives (CoCs), which contain an estrogen and a progestin, often are prescribed for women. Objectives: to determine the effectiveness of combined oral contraceptives (CoCs) for the treatment of facial acne compared to placebo or other active therapies. Search strategy: We searched for randomized controlled trials of CoCs and acne in the computerized databases of the Cochrane Central register of Controlled trials (CentraL), medLine, embase, PoPLine, and LiLaCs. We also searched for clinical trials in Clinicaltrials. gov and the international Clinical trials registry Platform (iCtrP). We wrote to authors of identified trials to seek any unpublished or published trials that we might have missed. selection criteria: all randomized controlled trials reported in any language that compared the effectiveness of a CoC containing an estrogen and a progestin to placebo or another active therapy for acne in women were eligible. Data collection and analysis: We extracted data on total and specific (i.e., open or closed comedones, papules, pustules and nodules) facial lesion counts; acne severity grades; global assessments by the clinician or the participant and discontinuation due to adverse events. data were entered and analyzed in revman. Main results: the search yielded 25 trials: 7 placebo-controlled trials made 4 different comparisons, 17 trials made 13 comparisons between 2 different CoC regimens, and 1 additional trial compared a CoC to an antibiotic. CoCs reduced acne lesion counts, severity grades and self-assessed acne compared to placebo. differences in the comparative effectiveness of CoCs containing varying progestin types and dosages, though, were less clear. CoCs that contained chlormadinone acetate or cyproterone acetate improved acne better than levonorgestrel, although this apparent advantage was based on limited data. a CoC with cyproterone acetate might result in better acne outcomes than one with desogestrel; however, the three studies comparing these CoCs produced conflicting results. Likewise, levonorgestrel showed a slight improvement over desogestrel in acne outcomes in one trial, but a second trial found the CoC groups were similar. Author’s conclusion: the four CoCs evaluated in placebo-controlled trials are effective in reducing inflammatory and non-inflammatory facial acne lesions. Few important differences were found between CoC types in their effectiveness for treating acne. How CoCs compare to alternative acne treatments is unknown since limited data were available regarding Focus acné this question. Cohcrane Database of Systematic Reviews 2009 3: CD004425 Comparison of a 1,550 nm Erbium:Glass fractional laser and a chemical reconstruction of skin scars (CROSS) method in the treatment of acne scars: a simultaneous split-face trial Background and objective: acne scarring is a common complication of acne but no effective single treatment modality has been developed. to compare the efficacy of 1,550 nm er:glass fractional laser and chemical reconstruction of skin scar (Cross) method in the treatment of acne scars. Study design / material and methods: a splitface trial was conducted in 20 patients (10 rolling, 10 icepick types) with acne scars. one side was treated with the 1,550 nm er:glass fractional laser three times with a 6-week interval. and the other side was treated with Cross method two times every 12 weeks. Results: significant improvement was observed in both sides of the face. in rolling type, the objective and subjective improvement rates were significantly higher in the sides treated with laser than Cross method. However, in icepick type, there were no statistically significant differences between the two treatment sides. in the laser sides, grades of pain were significantly higher than that of treated with Cross method. However, downtimes and lasting days of erythema were significantly longer in the sides treated with Cross method. Conclusion: a 1,550 nm er:glass fractional laser and Cross method are both well-tolerated and effective treatment options in the acne scars. However, there was a relatively small difference between the two treatment modalities. therefore, dermatologists should consider the acne scar type to select the treatment options. Lasers surg. med. Lasers in Surgery and medicine 2009, Epub ahead of print Dapsone 5% gel: a review of its efficacy and safety in the treatment of acne vulgaris dapsone, a synthetic sulfone that has been available for over 60 years, has been used to treat a myriad of cutaneous disorders. Prior to the general acceptance of isotretinoin, oral dapsone had been reported to be effective in the treatment of nodulocystic acne. However, the potential for systemic toxicity prevented its widespread adoption in the treatment of acne. For many years scientists explored the possibility of developing a topical formulation of dapsone for the treatment of acne in the hope of minimizing the adverse hematologic effects of oral dapsone. such a formulation had been unavailable until recently. dapsone 5% gel (aczone) was recently developed to treat acne vulgaris. this topical formulation was approved in the us based on two randomized, vehiclecontrolled studies. a 12-month, open-label study was also conducted to assess the safety and efficacy of topical dapsone over the long term. Finally, two open-label phase i pharmacokinetic studies were conducted to evaluate the systemic absorption of topical dapsone compared with oral dapsone. this article reports the results of these studies, which show a reduction in acne lesion count comparable to those observed in clinical trials of other approved topical acne therapies. With regard to safety, the studies demonstrated that the concentrations of dapsone and n-acetyl dapsone remain low and do not accumulate over time once steady state is reached. of the total of 50 patients with glucose-6-phosphate dehydrogenase (g6Pd) deficiency in all the studies, only two experienced a drop in hemoglobin levels, and those shifts in values were consistent with fluctuations observed for other study participants. a recent study evaluating the risk of hemolysis in patients with g6Pd deficiency found topical dapsone 5% gel to be safe to use in this patient population. based on the observations noted in the above-mentioned studies, we conclude that topical dapsone 5% gel is safe and effective in the treatment of acne vulgaris. American Journal of Clinical Dermatology 2009, 10 (4) : 221-7 Efficacy of an oral contraceptive containing EE 0.03 mg and CMA 2 mg (Belara) in moderate acne resolution: a randomized, double-blind, placebo-controlled Phase III trial Background: the study was conducted to assess the effects of the monophasic combined oral contraceptive containing ethinyl estradiol (ee) 0.03 mg and chlormadinone acetate (Cma) 2 mg (ee/Cma) on papulopustular acne of the face, décolleté (low neck) and back; on moderate comedonal acne of the face; and on seborrhea, alopecia and hirsutism. Study design: three hundred seventy-seven women were randomized (2:1) to receive ee/ Cma (n=251) or placebo (n=126) for six medication cycles. due to the placebo-controlled, double-blind design of the trial, condoms were supplied for contraception. the primary efficacy end point was defined as a reduction of at 7 Fokus Akne least 50% in the number of papules and/or pustules of the face from admission to medication Cycle 6. Results: in total, 64.1% (161/251) of subjects treated with ee/Cma responded compared with 43.7% (55/126) of those taking placebo (p=.0001). the median reduction in papules/ pustules on the face at Cycle 6 compared with admission was 63.6% (ee/Cma) compared with 45.3% (placebo group). For comedonal lesions of the face, the reduction in lesion numbers was 54.8% (ee/Cma) compared with 32.4% (placebo). moderate papulopustular acne of the décolleté decreased by 92.9% (ee/Cma) vs. 50% (placebo group) and of the back by 86.0% and 58.3%, respectively. For these skin conditions, the p values for the relative difference between groups vs. baseline were <.05 at Cycles 3 and 6, in favor of ee/Cma. as part of a self-assessment rating, at least 70.5% (ee/Cma) vs. 41.3% (placebo) reported an at least satisfactory improvement of their moderate acne. even 39.8% of women taking ee/Cma reported an «excellent improvement» or "complete resolution" of moderate acne compared with 12.7% taking placebo. Conclusion: in addition to its contraceptive efficacy described elsewhere, ee/Cma is an effective treatment for moderate papulopustular acne and other androgen-related skin disorders. Contraception 2009, 80 (1) : 25-33 Hidradenitis suppurativa (acne inversa): early inflammatory events at terminal follicles and at interfollicular epidermis* Hidradenitis suppurativa (acne inversa) is a chronic suppurative and scarring inflammatory disease with predilection in the apocrine gland-bearing areas. Histological investigations in the 1990s showed keratotic occlusion of the terminal follicle structure to be the initial cause. our investigations describe and reproduce the morphology and try to figure out very early lesions of Hs. a total of 262 operative specimens from 60 patients were investigated by routine histology and 11 operative specimens by immunohistochemistry: Hs is dominated by a heterogeneous histological image. 82% of the surgical specimens showed mild or pronounced follicular hyperkeratosis, whereas an isotopic hyperplasia of follicular epithelium was evident in 77%. Pronounced perifolliculitis was seen in 68% and rupture of the follicle structure in 28%. Features which had not so far been described in detail were: epidermal psoriasiform hyperplasia (43%) and subepidermal interfollicular inflammatory infiltrate (78%). in all 11 specimens, immunohistochemical investigations showed a peri8 Focus acné follicular and subepidermal inflammation of Cd-3-, Cd-4-, Cd-68-, Cd-79- and Cd-8-cells, the latter with a striking selective epitheliotropism. to conclude, we could show follicular hyperkeratosis and lymphocytic perifollicular inflammation as early patterns in pathogenesis, whereas rupture of the follicle structure takes place later. Finally, it seems that there are two hot spots of inflammatory events (perifollicular and subepidermal) composed of a comparable inflammatory cell mixture. the Cd-8 cell epitheliotropism (follicular and epidermal) described here and its influence in follicular hyperkeratosis, in hyperplasia of follicular epithelium and in epidermal psoriasiform hyperplasia will be of further interest, for instance, concerning early pharmacological intervention. We provide a summarized update of the current knowledge of the pathobiology of acne vulgaris and new treatment concepts that have emerged in the last 3 years (2005-2008). We have tried to answer questions arising from the exploration of sebaceous gland biology, hormonal factors, hyperkeratinization, role of bacteria, sebum, nutrition, cytokines and toll-like receptors (tLrs). sebaceous glands play an important role as active participants in the innate immunity of the skin. they produce neuropeptides, excrete antimicrobial peptides and exhibit characteristics of stem cells. androgens affect sebocytes and infundibular keratinocytes in a complex manner influencing cellular differentiation, proliferation, lipogenesis and comedogenesis. retention hyperkeratosis in closed comedones and inflammatory papules is attributable to a disorder of terminal keratinocyte differentiation. Propionibacterium acnes, by acting on tLr-2, may stimulate the secretion of cytokines, such as interleukin (iL)-6 and iL-8 by follicular keratinocytes and iL-8 and -12 in macrophages, giving rise to inflammation. Certain P. acnes species may induce an immunological reaction by stimulating the production of sebocyte and keratinocyte antimicrobial peptides, which play an important role in the innate immunity of the follicle. Qualitative changes of sebum lipids induce alteration of keratinocyte differentiation and induce iL-1 secretion, contributing to the development of follicular hyperkeratosis. High glycemic load food and milk may induce increased tissue levels of 5alpha-dihydrotestosterone. these new aspects of acne pathogenesis lead to the con- siderations of possible customized therapeutic regimens. Experimental Dermatology 2009, Epub ahead of print Long-term successful Adalimumab therapy in severe hidradenitis suppurativa Background: several studies report the use of tumor necrosis factor (tnF-) inhibitors in refractory hidradenitis suppurativa (Hs), particularly infliximab and etanercept. However, very limited data have been reported for adalimumab, the newest fully human anti–tnF- monoclonal antibody. We evaluated the long-term efficacy and safety of adalimumab therapy in 6 patients with refractory Hs. in the case of positive culture findings from any draining lesion, antibiotic therapy was administered for at least 2 weeks before initiating adalimumab therapy. adalimumab (in 40-mg subcutaneous injections) was prescribed every other week. if the disease was inadequately controlled, the dosage was increased to 40 mg/wk. if Hs was in persistent clinical remission, adalimumab therapy was gradually decreased to 40 mg every 3 weeks. Quality of life was assessed using the dermatology Life Quality index. Observations: six patients (mean [sd] age, 39.3 [12.9] years) with severe Hs (mean [sd] duration, 22.5 [11.7] years) were treated with adalimumab. significant improvements after 1 month of treatment were seen in the dermatology Life Quality index; in the number of affected regions, nodules, and fistulas; and in the basic laboratory findings. improvements were maintained for a mean (sd) follow-up of 21.5 (7.1) (range, 13-29) months. adalimumab was well tolerated. Conclusion: adalimumab appears to be an effective and safe treatment for refractory Hs. Terminologie Dermatologische Terminologie Terminologie dermatologique Monika HARMS Herpes Herpès Das griechische Ursprungswort ist : Le mot grec d’origine est: ερπειν = kriechen ερπειν = ramper Die Bezeichnung wurde von den englischen Dermatologen Robert Willan (1796 -1808) und seinem Schüler Thomas Bateman (1778 -1821) aufgestellt. Willan, ein Zeitgenosse von Alibert, war der erste, der ein Dermatologie-Lehrbuch mit präzisen klinischen Beschreibungen herausgab, das mit Illustrationen von Dermatosen in verschiedenen Erkrankungsstadien begleitet war. Sein Konzept der Dermatosen war viel moderner als der "Stammbaum der Dermatosen" von Alibert. Herpes ist ein rein deskriptiver Begriff. Er definiert das Vorhandensein gruppenförmig angeordneter kleiner transparenter Bläschen. Die Bläschen in einer Gruppe können das gleiche Alter haben (Herpes zoster) oder verschieden alt sein (Herpes simplex). Weitere Dermatosen können einen herpetiformen Aspekt aufweisen wie die Dermatitis herpetiformis, der Herpes gestationis oder auch der Herpes circinata, eine ältere Bezeichnung der oberflächlichen trichophytischen Mykose der unbehaarten Haut. Il définie des petites lésions vésiculeuses transparentes réunies dans un groupe et entourés d’une auréole rouge. Les vésicules peuvent avoir le même âge dans un groupe (herpès zostère) ou contenir des vésicules d’âges variables (herpès simplex). D’autres dermatoses peuvent prendre un aspect herpétiforme comme la dermatite herpétiforme, herpes gestationis et encore l’herpès circiné, mycose superficielle trichophytique de la peau glabre. / ⊃ / ⊃ APCHPRTIN0809f Information professionnelle abrégée de Protopic® C : Protopic® pommade à 0.03% et Protopic® pommade à 0.1% contiennent respectivement: tacrolimus 0.3 mg/g et 1 mg/g, excipients ad unguentum. I: Protopic® est indiqué pour le traitement d’exacerbations aiguës de la dermatite atopique modérée à sévère, pour le traitement de seconde intention au cas où le traitement conventionnel ne serait pas assez efficace ou s’il survenait des effets secondaires. Pendant toute la durée du traitement, I’exposition excessive de la zone traitée aux rayons UV (UVA ou UVB), par exemple soleil et solarium, doit être évitée. Des méthodes de protection solaire appropriées doivent être recommandées par le médecin traitant. Des préparations émollientes peuvent être utilisées simultanément à Protopic®, en respectant un délai de 2 heures entre les applications des deux produits sur la même zone. Grossesse et allaitement: Protopic® ne doit pas être utilisé pendant la grossesse, sauf en cas de raison impérieuse. Bien que l’absorption systémique de tacrolimus soit limitée après l’application de pommade, l’allaitement est déconseillé pendant le traitement avec Protopic®. Dort arbeiten 5 Ärzte nach dem Managed Care-System, was heisst, dass sie eigentlich fast den ganzen medizinischen Bedarf abdecken. Man muss also nicht zum Spezialisten, wo es mehr kostet, sondern kann hier für alles und erst noch billiger behandelt werden. Kurt Kummer hat sich daher für diese Arztpraxis entschlossen und muss daher weniger Prämie an seine Krankenkasse bezahlen. Seit einigen Tagen ist ihm an der Hüfte ein Hautausschlag aufgefallen. Er zeigt ihn seiner Freundin, weil sie Spitex-Schwester ist, und als diese die Rötung sieht, ahnt sie sofort, was es ist und sagt zu Kurt: "Du hast einen Pilz und musst dich in der neuen Praxis behandeln lassen. Vielleicht ist es ansteckend". Herr Kummer ruft in der Praxis an und fragt, ob er rasch vorbeikommen könne, er habe einen ansteckenden Hautpilz. Die Praxisassistentin Sandra gibt ihm einen Termin auf den nächsten Freitag. In der modern eingerichteten Praxis darf Herr Kummer ins Wartezimmer neben der Cafeteria, wo schon 3 Patienten sitzen. Einer hustet stark, und darum setzt sich Kurt neben die Patientin mit der verbundenen Hand. Schon nach 10 Minuten wird er von der MPA Clio aufgerufen und darf ins hellblaue Sprechzimmer. Der Arzt trägt an seinem Safarihemd ein dekoratives Täfelchen, und Kurt ersieht daraus, dass er bei Dr. M. Carell ist. Sogleich begrüsst ihn dieser und erkundigt sich nach dem Grund seines Kommens. "Ich habe einen Hautausschlag, wohl ein ansteckender Pilz". Der Arzt lässt sich den Befund zeigen, verweilt etwas, fragt, wann der Ausschlag aufgetreten sei, ob er jucke, ob in der Familie Allergien oder Flechten bekannt seien, ob er zu Hause eine Katze habe oder oft ins Schwimmbad gehe. Herr Kummer beantwortet die Fragen korrekt und wartet, bis der Arzt seine Notizen in den Computer getippt hat. Dann erklärt ihm Dr.Carell die Situation: "Der Ausschlag könnte tatsächlich eine Mykose, ein Pilz sein; er ist ja wie üblich ringförmig. Aber man muss natürlich auch an die Borreliose denken, die Wanderröte, und an eine Syphilis". Kurt erschrickt etwas, hat ja nur Sex mit seiner Spitex-Freundin. Der Arzt beruhigt ihn sogleich: "Wir werden eine Pilzkultur machen und Blut zur Untersuchung auf Borreliose und Syphilis einschicken. Mit Ihrem Einverständnis auch für den AIDS-Test." Der Patient ist einverstanden und erhält einen Termin in einer Woche. "Zur Sicherheit gebe ich Ihnen eine Tube Lamisil Pedisan, damit heilt jeder Pilz nach nur eintägiger Behandlung", sagt Dr. Carell und verabschiedet sich. Zu Hause erzählt er seiner Martina begeistert von der gründlichen Untersuchung und dass sich der Doktor wirklich Zeit genommen habe und an alles denke. Dann zieht die Freundin einen Gummihandschuh an und bestreicht den Ausschlag liebevoll mit der Lamisil PedisanCrème. Zwei Tage später juckt die behandelte Stelle und ist vermehrt gerötet. Herr Kummer telefoniert in die Praxis und verlangt Dr. Carell. Die Praxisassistentin Liz verbindet ihn mit Frau Dr. Borezki, welche den Anrufer beruhigt und ihm empfiehlt, etwas Kortisonsalbe anzuwenden, die er am Nachmittag gleich abholen könne. Sie werde eine Tube BurnOUT bereitlegen. Als Kurt am Abend diese Salbe aufträgt, spürt er fast sofort eine erfreuliche Linderung. Am nächsten Morgen stellt er erschreckt fest, dass sich neue Flecken entwickelt haben und er meldet sich notfall18 mässig in der Praxis an. Dr. Carell ist gerade mit einem verstauchten Fuss beschäftigt, aber die Assistentin Gabi führt ihn zum Generalisten Gnoster und bringt die Unterlagen von Herrn Kummer gleich mit. Dr. Gnoster vertieft sich in die Notizen, und unterdessen macht sich der Patient frei. Da erkennt der Arzt, dass die Mykose eskaliert und er verordnet Pilztabletten. Da der Bericht des Pilzlabors noch nicht eingetroffen ist, wählt er Diflucan, welches gegen alle Pilze hilft. Zur Erleichterung des Patienten teilt er ihm mit, dass die Laboruntersuchungen auf Borreliose und Syphilis negativ ausgefallen sind. Als Kurt 3 Tage später duschen will, stellt er fest, dass sich an seinem Oberkörper viele neue rote Flecken gebildet haben und ist verzweifelt. Seine Freundin, welche einen Dermatologen an der Bahnhofstrasse kennt, sagt: "Geh zu Dr. Skinner !" und meldet ihn dort gleich telefonisch an. Schon am Nachmittag kann Kurt in die dermatologische Praxis. Er möchte den Arzt ausführlich über den bisherigen Verlauf und die Befunde informieren und hat dafür speziell Notizen mitgebracht. Dr. Skinner wehrt ab; zuerst will er, dass sich der Patient auszieht und seine Haut zeigt. Skinner schaut sich den Patienten ringsherum an und heisst ihn, sich wieder anzukleiden. Er sagt, dass es sich um eine harmlose und nicht ansteckende Hautkrankheit handle und dass die Pilztabletten nicht nötig sind. Kurt Kummer ist überrascht und verunsichert und bittet den Arzt, den Namen der Krankheit aufzuschreiben, damit er alles vom Internet herunterladen könne. Der Arzt schreibt auf ein Rezeptformular zwei lateinische Worte "Pityriasis rosea"und sagt noch, dass die Krankheit von selbst heile. Wieder daheim, erzählt Kurt seiner Martina vom Arztbesuch. Er ist von Dr. Skinner enttäuscht,weil er nur kurz geschaut hat und die Notizen nicht sehen wollte. Da geht er doch lieber wieder zu Dr. Carell, wo es viel professioneller ist. Dort konnten sie ihm schliesslich auch beweisen, dass er keine Borreliose hat und nicht mit Syphilis angesteckt ist. Drei Wochen später erhält er einen Anruf aus der Praxis am Bahnhofplatz. Das nette Fräulein Mona teilt ihm mit, dass die Pilzuntersuchung negativ, d.h. gut ausgefallen sei. Kurt ist begeistert; nun hat er keinen Pilz mehr, er ist gesund! "Gut, dass ich Managed Care gewählt habe", denkt er, "und zudem sind die Prämien billiger". Aber dass er diesem Skinner, dem Hautarzt, für das Anschauen ganze 53.– Fr. bezahlen soll, reut ihn. Dr. M. Dans ce domaine, le récepteur du facteur de croissance épidermi- folliculaires sur un fond érythémateux (Fig. 2A). Souvent, l’éruption apque (EGFR) constitue une cible de premier ordre. Dans la majorité des cas paraît dès la première semaine de traitement, touche surtout les endroits recevant des inhibiteurs de l’EGFR, on observe, au niveau cutané, des effets séborrhéiques du visage et du torse (Fig. 2B), et est parfois associée à une sensation de brûlures. Stériles à l’origine, les pustules se rompent et se indésirables, caractéristiques de ce type de traitement ciblé. L’EGFR appartient à la classe des récepteurs tyrosine kinases (RTI). A la différence des chimiothérapies classiques, le but principal du traitement par les inhibiteurs d’EGFR est moins la destruction de la tumeur que l’inhibition de sa prolifération. De ce fait, une thérapie par inhibiteurs de l’EGFR (surtout par les inhibiteurs de tyrosine kinase comme l’erlotinib) est surtout indiquée dans les tumeurs avancées et implique d’emblée le concept de traitement prolongé. Dans cette perspective, c’est la question du traitement symptomatique d’une part, soit l’indication d’une diminution ou d’un arrêt de la thérapie de base d’autre part qui importent le plus. Les points suivants sont à considérer dans cette évaluation : En développement clinique • Au premier plan, il y a l’atteinte subjective du patient. Cette dernière est déterminée par la présence de stigmates au niveau du visage, ainsi que par la gêne fonctionnelle des paronychies. Les effets cutanés décrits ne sont pas des réactions d’hypersensibilité, mais des effets biologiques dose-dépendants de l’inhibition de l’EGFR [22]. Même en cas de réactions cutanées sévères, la morbidité objective est faible par rapport à la maladie de base. Certaines études suggèrent une relation directe entre la sévérité des réactions cutanées et la réponse thérapeutique des inhibiteurs de l’EGFR [23-25]. *TKI: inhibiteur de tyrosine kinase °mAb: anticorps monoclonal isolées d’acantholyse intraépithéliale, rappelant un trouble de l’homéostase acrosyringéale évoquant une maladie de Grover, ont été rapportées sous traitement par l’anticorps monoclonal cétuximab [7,11]. • Dermatite xérotique L’évolution ultérieure est marquée par le développement insidieux d’altérations cutanées généralisées comportant une xérose cutanée et un état desquamatif, ainsi qu’une accentuation de la vulnérabilité et de l’irritabilité cutanée qui se manifeste surtout dans les zones exposées à des frictions vestimentaires. Le tableau clinique rappelle un eczéma astéatotique (Fig. 2E,F) ; parfois on observe également des lésions érosives (Fig. 2G). La fréquence rapportée pour cette symptomatologie est de 35% [12-14]. Des rapports isolés suggèrent la présence d’une inflammation à caractère neutrophile également à ce stade [15] (Fig. 2H). L’apparition d’une atrophie épidermique avec amincissement de la couche cornée avait déjà été observée lors des études précliniques avec les inhibiteurs de l’EGFR (revue dans [16]). • • En conclusion, on peut constater que les effets indésirables cutanés sont réversibles et ne présentent pas de caractère dangereux pour le patient. Le but de la prise en charge dermatologique est donc de soulager les symptômes du patient et d’assurer le respect du protocole thérapeutique, si possible sans modification du dosage. Si chez certains patients l’apparition de quelques pustules faciales signifie déjà une forte atteinte de la qualité de vie entraînant une répercussion sociale, d’autres ne se sentent guère gênés par leurs lésions cutanées et ne se focalisent que sur le but thérapeutique escompté. C’est pour ces raisons que l’indication d’un arrêt ou d’une réduction de la posologie de la thérapie échappe à tout schéma décisionnel de principe et doit reposer sur des critères individuels. Paronychie avec hypergranulation Chez environ 10 à 15% des patients, apparaissent, après des semaines voire mois de traitement continu, des paronychies douloureuses des doigts des pieds, parfois également des mains [14,18]. Alors que le symptôme 4. Traitement des effets indésirables cutanés (Tab. 2) initial est une enflure périunguéale douloureuse ou une inflammation abcèdante sans composante infectieuse (Fig 2I), la plupart des patients ne consultent qu’après l’apparition d’une atteinte fonctionnelle. A ce stade, la Comme le mécanisme des effets indésirables décrits reste peu connu, les présentation clinique est le plus souvent marquée par une inflammation ulcérative et luxuriante du pli périunguéal, rappelant un granulome pyogénique (Fig. 2J). Akute EGFR-Folliculitis Xerotische EGFR-Dermatitis Altérations capillaires Chez certains patients apparaît une hypertrichose faciale. La présentation la plus fréquente sur le plan clinique est une trichomégalie ciliaire entraînant une irritation conjonctivale voire une blépharite [3] (Fig. 2K). 3. Classification et évaluation de la sévérité La classification des effets indésirables cutanés sous thérapie par inhibiteurs d’EGFR se base surtout sur la symptomatologie clinique décrite [4,6,12,17]. Certains auteurs utilisent également des critères chronologiques (Fig. 3) [16] ou étio-pathogéniques [17,19]. Les critères du CTCAE2, outil descriptif développé pour les besoins des études de registration, n’ont pas beaucoup d’utilité en pratique clinique. Des propositions de classification clinique spécifiques pour les effets secondaires cutanées des inhibiteurs d’EGFR ont été publiées [18,20 Selon la classification proposée par Pichler, il s’agit de réactions non immunologiques du type [21]. L’atteinte préférentielle des structures folliculaires ainsi que la distribution séborrhéique des lésions reflète probablement la densité augmentée de récepteurs de l’EGF dans ces régions. Les symptômes observés à moyen terme, comme la xérose et les anomalies périunguéales et trichologiques s’expliquent peut-être par l’altération de l’homéostasie de l’épiderme et des annexes régulés par l’EGFR, induisant une réaction de stress dans les kératinocytes [17]. Le point commun des altérations cutanées décrites est leur caractère inflammatoire ; une activité pro-inflammatoire du blocage de l’EGFR pourrait s’expliquer par la perte d’un mécanisme de feedback négatif de l’EGFR sur des chémokines inflammatoires in vitro [26, 27]. Le tableau 2 résume les possibilités thérapeutiques qui font actuellement l’objet d’un consensus [17,20,28]. Ces dernières reposent sur trois piliers : • 22 La prévention passant par des soins cutanés réguliers avec des émollients, par la protection contre l’exposition UV, des soins unguéaux et le port de chaussures appropriées. • • Le traitement topique anti-inflammatoire, avec des crèmes à base de corticostéroïdes combinées avec des antibiotiques ou antiseptiques locaux. Le traitement systémique par tétracyclines si les réactions sont sévères, ou en cas de risque d’abandon du traitement par inhibiteur d’EGFR. Prévention Une information sur la probabilité des effets indésirables cutanés s’impose déjà avant tout traitementpar inhibiteurs de l’EGFR. Les mesures concrètes concernent les soins cutanés réguliers avec des émollients. nous recommandons des crèmes ou des lotions, avec ou sans urée. Il est très important de veiller à une posologie et une application correctes : un tube de 100 grammes de crème devrait être consommé dans l’espace de 10 jours, et l’application devrait se faire immédiatement après la douche ou le bain, mais au moins une fois par jour. La manucure et la pédicure, ainsi que la manière de se chausser revêtent une importance particulière : les ongles et les plis péri-unguéaux devraient être nettoyés et crémés quotidienne- Freies Forum Tribune libre ment. Les callosités et les onychodystrophies pré-existantes suggèrent que les chaussures portées sont probablement inappropriées et doivent être abandonnées. Dans certains cas, des soins spécifiques par des podologues peuvent être utiles, par exemple, en présence d’ongles incarnés préexistants. La correction de ces situations exposant à un risque de paronychie, qui incluent également des infections mycologiques unguéales, s’impose avant le début de la thérapie. Les patients rapportent souvent une tolérance diminuée face à l’exposition solaire, ce qu’il convient de prévenir par des mesures de protection adéquates. Dans la mesure du possible, on évitera l’exposition directe aux UV et on utilisera des filtres topiques avec un facteur de protection élevé. Traitement anti-inflammatoire topique Le traitement spécifique anti-inflammatoire devrait être débuté dès l’apparition des premiers signes inflammatoires. En cas de la folliculite aigüe du visage ou du torse, les corticostéroïdes puissants de classe III constituent le premier choix. Les préparations de classe I et II sont peu efficaces, tandis que les produits de classe IV devraient être évités dans le visage en raison du risque élevé d’effets secondaires. En raison de la tendance à l’impétiginisation secondaire des lésions, l’utilisation de préparations combinées avec antibiotiques ou antiseptiques est utile. L’amélioration clinique est souvent rapide, et après un maximum de 10 jours de traitement quotidien, la fréquence d’application peut être réduite. On évitera un arrêt brusque du traitement, en adoptant p. ex. un schéma de traitement alternant chaque 2e jour pendant les 2 semaines suivantes, ce qui réduit le risque d’une rechute. Si un traitement prolongé s’impose, on peut également utiliser les inhibiteurs topiques de la calcineurine (tacrolimus, pimecrolimus ; évaluation au préalable avec les assureurs car le remboursement peut être refusé dans cette indication !). Au niveau des doigts des mains et des pieds, l’application de corticostéroïdes ultra-puissants (classe IV), même sous occlusion, devrait être débutée dès les premiers signes cliniques. Alternativement, on peut essayer des préparations anti-inflammatoires à base de goudrons (Ichthyol 20% à 50% ou en prescription magistrale). S’il y a déjà présence d’une inflammation à caractère de granulome pyogène, on aura recours à des traitements conservateurs (bains antiseptiques, applications de nitrate d’argent, bandages…). Traitement systémique Etant donné la relation présumée directe entre l’intensité des effets secondaires cutanés et la réponse thérapeutique des inhibiteurs de l’EGFR, on n’échappe pas à la question relative aux conséquences d’un traitement systémique de ces effets cutanés sur la réponse oncologique. Cependant, les expériences de la pratique quotidienne ainsi que la littérature actuelle déposent en faveur de l’indication de tétracyclines systémiques en tant que prophylaxie secondaire, afin d’éviter une réduction du dosage ou un abandon du traitement par les inhibiteurs de l’EGFR [29, 30, 31]. Actuellement nous ne disposons pas encore de données définitives sur la sécurité de traitements systémiques pour les effets indésirables des inhibiteurs de l’EGFR. Des données préliminaires d’une étude clinique sur la prévention des effets secondaires du traitement du cancer du colon métastatique par Panitumumab en combinaison avec Folfiri ou Irinotecan ne montrent aucune diminution de l’efficacité du traitement oncologique par l’utilisation de la doxycycline à raison de 200 mg/jour [32]. Les effets secondaires des cyclines, notamment l’intolérance gastro-intestinale et la photosensitivité, peuvent se confondre avec les symptômes du traitement par inhibiteur de l’EGFR et ainsi passer inaperçus. La doyxcycline est un inhibiteur du CyP3A4 et pourrait donc théoriquement aboutir sur une élévation du taux plasmatique de l’erlotinib. Il n’y a pas de données sur l’interaction entre les cyclines et le cétuximab ou le panitumumab. D’autres propositions thérapeutiques mentionnent l’usage de l’isotrétinoine[29, 34], des stéroïdes systémiques[29] et des vitamines (B2, B6)[31]. Traitement préventif par tétracyclines Des données encourageantes sont actuellement documentées par des communications scientifiques [33] et des rapports préliminaires d’études cliniques [32] : une prophylaxie primaire avec des tétracyclines peut diminuer l’incidence d’effets secondaires sévères des inhibiteurs de l’EGFR, et paraît plus efficace qu’une thérapie réactive. Avant d’émettre des recommandations officielles, il faudra pourtant attendre des données définitives sur la sécurité d’une telle stratégie préventive en termes d’absence d’effets négatives sur l’efficacité du traitement oncologique. Recommandations spécifiques en cas de radiothérapie concomitante Eviter une réduction du dosage de la radiothérapie ou du traitement par inhibiteur de l’EGFR est également l’objectif principal du traitement des symptômes cutanés apparaissant dans les territoires irradiés. La prise en charge de ces réactions cutanées répond aux recommandations pour la prise en charge des radio-épidermites, pour autant qu’ils affichent un degré de sévérité de 2 ou plus. A ce sujet, il existe des recommandations récentes [35]. Tab. 2. Conclusion et perspectives Les effets cutanés indésirables des inhibiteurs de l’EGFR constituent un syndrome dermatologique nouveau et spécifique. Les recommandations thérapeutiques actuelles sont de nature symptomatique et sont utilisés à l’apparition des symptômes cliniques. Des stratégies de prophylaxie primaire avec tétracyclines systémiques sont prometteuses, bien que la sécurité de cette attitude ne soit pas encore clairement établie. L’ importance de mesures prophylactiques conservatrices comme les soins cutanés ciblés et les soins podologiques ne doit pas être négligée. Une recherche approfondie des mécanismes sous-jacents pourrait révéler non seulement de nouvelles stratégies de prise en charge, mais également fournir des notions sur la pathogenèse des dermatoses inflammatoires à caractère neutrophile. Die ganze Glans penis ist von mehreren Knoten eingenommen von denen zwei oberflächlicher gelegen sind. Diese beiden sind von eryhtematöser Farbe mit einem bräunlichen Ton. Bei der Palpation erweisen sie sich hart und sind schmerzlos. Un patient de 81 ans consulte pour des lésions au niveau du gland. Diese Situation ist sehr selten und wenn dann im Rahmen einer HIV- Erkrankung. Die Färbung wäre dann satter rot und die Konsistenz weniger hart. Auch ein Angiom wäre röter oder bläulich und von langer Dauer ( angiomatöse Fehlbildung, Syndrom Bean). Das Granuloma pyogenicum oder Botrimykom ist ein leicht blutender Tumor meist ohne Epidermis. Er wird sehr selten bei älteren Personen angetroffen. Die richtige Diagnose ist : Metastase Bei dem Vorliegen einer Hautmetastase ohne Evidenz des Primitivtumors wird man sich einerseits mit der Topographie orientieren und andererseits eine Histologie anfordern. Bei Lokalisationen wie die Kopfhaut, das Gesicht, der Hals und die Abdominalhaut ist vorzugsweise an Maligome darunterliegender Organe zu denken. Das ist auch der Fall bei Metastasen an der Glans penis. Das Prostatakarzinom und das Blasenkarzinom sind vorerst in Betracht zu ziehen. Diese Situation ist selten in der Literatur festgehalten. Bei dem Verdacht einer Hautmetastase muss der Histopathologe zuerst einen Primitivtumor ausschliessen. Dies kann sich als sehr schwierig erweisen weil bestimmte Metastasen ausserordentlich gut Hauttumoren imitieren können, insbesondere Adnextumoren. Im Allgemeinen aber sind Argumente wie Fehlen einer Verbindung mit der Epidermis, bei der Positionierung des Tumors in der Dermo-hypodermis und besonders bei Vorliegen einer lymphatischen Invasion Argumente, die für eine Metastase sprechen. Die Zueignung der Metastase zu einem bestimmten Tumor ist oft schwierig und allein auf Grund der Morphologie nicht zu lösen. Die Immunhistologie ist ein unerlässliches Hilfsmittel sowohl um eine Metastase von einem Primitivtumor zu differentieren als auch um den Ausgangstumor zu determinieren. Eine Gegenüberstellung der Klinik und der Histologie ist für die Diag- Abb. 1- Fig 1 nosestellung essentiell. Bei dem vorliegenden Fall zeigt das histologische Bild eine atypische epitheliale Proliferation, die zahlreichen glandulären Strukturen formiert, welche für eine Adenokarzinom sprechen (Abb.1). Die Immunohistologie zeigt eine Positivität für PSA (prostate specific antigen, Abb.2) und für PAP (prostatic acid phosphatase) und bestätigt so als Ausgangstumor ein Adenokarzinom der Prostata. Cette situation est assez rare et est plus souvent le cas dans le cadre du Sida. La coloration serait clairement plus rouge et la consistance moins ferme. Un angiome serait également plus érythémateux voir bleuté et de longue durée (malformation angiomateuse, syndrome de Bean). Le granulome pyogénique ou botriomycome se présente comme une tumeur non épidermisée saignant facilement. Il est très rare chez la personne âgée. Le diagnostic : métastase. En présence d’une métastase cutanée sans évidence du cancer primitif l’orientation se porte d’une part sur la topographie et d’autre part sur l’examen histologique. Ainsi, les localisations comme le scalp, la face, le cou et la paroi abdominale sont préférentiellement atteintes par des cancers des organes sous-jacents régionaux. Ceci est également le cas pour les métastases du gland. Le cancer prostatique ou celui de la vessie est à envisager en premier lieu. Cette situation est très rarement rapportée dans la littérature. Devant toute suspicion de métastase cutanée, l’histopathologue doit d’abord écarter une tumeur cutanée primitive. Cela peut s’avérer délicat, car certaines métastases imitent de façon remarquable des tumeurs cutanées, notamment les tumeurs annexielles. En règle générale, l’absence de point de départ épidermique, le siège dermo-hypodermique d’une tumeur ou encore la présence d’une invasion lymphatique sont des indices orientant vers une métastase. La détermination de l’origine d’une métastase est souvent difficile sur la base de la morphologie seule. Les immunomarquages constituent alors un outil complémentaire indispensable à l’histopathologue, tant pour différencier une métastase d’une tumeur cutanée primitive que pour déterminer son origine. Enfin une corrélation anatomo-clinique reste essentielle dans la démarche diagnostique. Dans le cas présent, l’examen histologique a montré une prolifération épithéliale atypique, composée de nombreuses structures glandulaires, correspondant à un adénocarcinome (Fig 1). Les immunomarquages réalisés ont montré une positivité des cellules tumorales pour les marqueurs prostatiques PSA (prostate specific antigen, Fig 2) et PAP (prostatic acid phosphatase), confirmant l’origine prostatique de l’adénocarcinome. Arch Dermatol. 2007; 143: 719-726. 3 IMS 2007 Information médicale abrégée: Enbrel® (Etanerceptum) Indications*: Polyarthrite rhumatoïde active (PR), arthrite psoriasique (APS), et arthrite chronique juvénile (ACJ) lorsqu’un traitement standard antérieur par antirhumatismaux de fond (DMARD) a été insuffisant, PR sévère, active et évolutive de l’adulte non précédemment traité par méthotrexate, spondylarthrite ankylosante (SA) en cas de réponse inadéquate au traitement conventionnel, ainsi que traitement des adultes atteints de psoriasis (PSO) de sévérité moyenne à grave. Posologie: Adultes: 25 mg 2x par semaine par injection sous-cutanée. Alternative: 50 mg 1x par semaine. PSO: 25 mg 2x par semaine par injection sous-cutanée. Alternative 50 mg 1x par semaine (alternative 2x 50 mg pendant les 12 premières semaines). Enfants et adolescents (4–17 ans): 0.4 mg/kg poids (max. 25 mg par injection) 2x par semaine par injection sous-cutanée. Contre-indications: Hypersensibilité à la substance active ou à l’un des excipients. Septicémie ou risque de septicémie. Un traitement par Enbrel® ne devrait pas être initié chez les patients ayant des infections évolutives. Précautions: Infections, insuffisance cardiaque congestive, réactions allergiques, troubles de l’hématopoïèse, anomalies du SNC, de même qu’un risque plus élevé de lymphomes et d’affections malignes. Grossesse/Allaitement: L’utilisation d’Enbrel® chez la femme enceinte et pendant l’allaitement n’est pas recommandée. Effets indésirables: Infections (y compris infections des voies respiratoires & infections graves), tumeurs malignes, autoanticorps. Depuis la mise sur le marché quelques cas de démyélinisation et dyscrasie sanguine ont été rapportés. Réactions au site d’injection. Interactions: Le méthotrexate n’a pas d’effet sur la Wyeth Pharmaceuticals AG pharmacocinétique de l’étanercept. Présentation: 4 seringues pré-remplies à 25 mg* ou flac-amp. à 25 mg* et 2 seringues pré-remplies à 50 mg*. Enbrel® doit être conservé entre 2–8 °C.
