4. Photo Copy / Scanned Passport with min. 6 (six) Month Validity
5. Photo Copy ID Card ( KTP / SIM )
6. 1 (one) Recent Colored Photograph 4 x 6
7. Complete Visa Form

8. Must Hold Return and Award Ticket to Mauritius by Air Mauritius
9. Must Hold Photocopy / Scanned of Accommodation Confirmation in
Mauritius ( Hotel Confirmation Letter) issued by Hotel in Mauritius.
10. Copy of Itinerary Reference
11. Business Trip Visa Application, Please Enclosed
