IMPORTANT BENEFITS CONTINUATION INFORMATION
You may be a participant in some of the benefits below. It is important for you to know which
benefits may be continued after you leave Emory Healthcare. For any benefit continuation, you are
responsible for timely election and payment of premiums. It is very important you read and
understand the following benefit information and work directly with these vendors.
HEALTH , DENTAL and VISION COVERAGE (includes prescription drug plan) will cease on
the last day of the month in which your separation occurs (or the last day of the month in
which payment was received if payments are not current). You may elect to continue coverage
through Continuation of Coverage (COBRA). You will receive information regarding benefit
continuation along with a Certificate of Coverage at your home address. Your prompt
attention to this information is important, as the election to continue coverage must be
completed within a specified period of time.
BASIC Life, SUPPLEMENTAL Life ends at date of separation. Basic and supplemental
coverage may be continued and/or converted within 31 days of separation. To continue your
coverage, you must contact The Standard directly at 1-866-756-8118.
Accidental Death & Dismemberment is discontinued with no conversion or portability
option.
403(B) RETIREMENT PLAN. If you are no longer employed by any Emory entity, you have
options regarding current loans and distribution of your vested funds. Please contact your
retirement vendor(s) immediately to discuss your options: Vanguard- 800-523-1188; Fidelity 800-343-0860 and/or TIAA-CREF - 800-842-2888.
EMORY HEALTHCARE PENSION PLAN. Contact the Retirement Plan Service Center at 678-5691022 to discuss your possible distribution options.
457(b) RETIREMENT PLAN. If you were participating in the 457(b) plan, please contact your
retirement vendor(s): Vanguard - 800-523-1188; Fidelity - 800-343-0860 and/or TIAA-CREF
800-842-2888. You have 90 days from separation to make a one time irrevocable decision on
when to take a distribution. If you wait more than 90 days, the money will be distributed to
you in a lump sum and will be subject to taxes.
DISABILITY INSURANCE ends at the date of separation. You may convert the long-term
disability and supplemental long-term disability plan to an individual disability plan within
31 days of separation. Conversion packages are available from the Benefits Department.
Short-term disability plans are not eligible to be converted.
LONG-TERM CARE. Your coverage will continue until the end of the month in which you
paid your last premium. You may elect to continue the long-term care coverage you had while
employed on a direct bill basis. The election for continued coverage must be made within 31
days of separation. Contact UnumProvident at 1-800-227-4165 to arrange for direct billing.
HYATT GROUP LEGAL may be continued through the end of the year in which you separate
by contacting Hyatt Legal at 1-800-821-6400.

HEALTH CARE SPENDING ACCOUNT cancels at separation, but may be continued to the
end of the plan year on an after-tax basis through COBRA. Further details will be mailed to
you. If you choose not to continue your health care spending account, claims incurred prior to
separation must be submitted for reimbursement by May 15 of the following year. The Benny
card is no longer available after separation.
DEPENDENT CARE SPENDING ACCOUNT ends at date of separation. Claims incurred prior
to separation date must be submitted to UMR for reimbursement by March 30 of the following
year. The Benny card is no longer available after separation.
METLIFE AUTO AND HOME payroll deductions cease with your last paycheck. Metropolitan
Property and Casualty Insurance Company will bill you directly for future premium
payments. You will no longer be eligible for the Emory discount. Contact Met at
1-800-GET-MET8 to change to a direct bill program.
AFLAC ends at date of separation. You may contact AFLAC at 1-800-992-3522 to arrange for
direct billing.
EMPLOYMENT VERIFICATION. For employment only, call The Work Number at 1-800-3675690. For employment and income verification, call 1-800-367-2884.
COMPREHENSIVE LEAVE (PTO) BALANCE is automatically paid out to employees who
were employed for six months or more. This payout occurs with a separate paycheck after all
hours worked have been paid.
Please notify Human Resources in writing of any address change to ensure your W-2 forms are
properly mailed to you at year end. Send only change of address information to:
