Patients and methods: This study is based on casts taken at appliance removal (T1) and one year after removal (T2) in two groups of 15 privately-treated adolescent patients. One group received anterior fixed retention (G2), and the other did not (G1). We applied the seven criteria of the American Board of Orthodontics Objective Grading System: alignment (anterior/ posterior), vertical positioning of the posterior marginal ridges, interproximal contacts, buccolingual inclination of the posterior teeth, posterior occlusal contacts, sagittal occlusal relationships, and overjet (anterior/posterior). They may be removable (Hawley retainers, thermoformed slip-covers, positioners, etc.) or fixed (bonded lingual wires, cast-metal splints, fiber-reinforced composite splints, etc.) and potentially combined with surgical techniques (circumferential supracrestal fibrotomy [1], gingivectomy, frenectomy). The current literature offers insufficient proof that any one approach is superior to another [2]. Reitan has shown that, for an average of 232 days after orthodontic tooth movement, the stretched gingival and periodontal fibers have a tendency to pull the teeth back towards their initial pretreatment position [3]. However, even if retained in the short or medium term, teeth will wind up moving in the long-term. This manifests itself most notably by the appearance or reappearance of mandibular incisal crowding [4] due to a combination of post-orthodontic relapse and normal dental arch maturation [5–7]. Hence certain clinicians’ advocacy, at least as far as the anterior teeth are concerned, of long-term or “permanent” retention [7,8]. All patients included in the study had been treated with an edgewise technique using fixed multibracket vestibular appliances (.02200 .02800 slots). Their orthodontic records were complete and contained 3 casts taken prior to treatment (T0), at appliance removal (T1), and 12-14 months after removal (T2), respectively. Patients with missing teeth (except premolars extracted for orthodontic purposes) or prosthodontic restorations were excluded from the study. After a preselection of 90 cases meeting these criteria, only those patients treated during adolescence were retained (optimal cast quality). Final sample size was 30 patients. Fifteen patients (G1) received removable maxillary and mandibular retaining devices (Hawley retainers for 6 months’ continuous wear followed by 6 months nighttime only). For each case, a score of 0, 1, or 2 points was attributed to each tooth for each criterion. More precisely, we noted an increase in anterior overjet, as have Lyotard et al. [10], Ormiston et al. [12], and Park et al. [13](fig. 3).This is no doubt linked to the fact that 43% of our initial population presented with Class II division 1 malocclusion (often severe). Moreover, a large proportion of patients had been treated using a technique similar to Tweed-Merrifield, in which the active phase ends with occlusion overcorrected almost to an incisal edge-to-edge position. These changes in posterior occlusion are most likely due to the effects of masticatory forces, muscle pressure, and the intrinsic eruption potential of the teeth (especially the second molars). Removable retainers apparently do not stand in the way of these changes. This is either because they are designed to leave the posterior teeth some freedom to move, or because they are actually worn little. The Tweed-Merrifield technique, on which G1’s treatment was based, anticipates this type of tooth movement during “denture recovery” [19]. It allows for appliance removal upon achieving what can be called “Tweed occlusion” [19], quite distinctively characterized by a nearly edge-to-edge position of the incisors, plus infraclusion and distal inclination of the second upper molars. “Settling of the occlusion” then takes on the utmost importance and should be seen to refer to a normal, beneficial process. Indeed, settling contributes to occlusal stability and therefore to treatment stability as well (fig. 5). The space left by the bands might explain the improvement in interproximal contacts. There actually remained some minor diastemas at debanding which closed spontaneously (with help from the transseptal fibers?).This phenomenon was all the more pronounced in G1 because even the anterior teeth were fitted with bands (unlike G2 which received brackets). It is very likely that the occlusal changes reported would not happen as quickly or as completely in an adult population. Observation lasted 1 year, offering relatively limited clinical hindsight. On the other hand, Lyotard et al. [10] have reported numerous post-orthodontic changes at 4 weeks. The end of the initial month(s) thus seems critical. Retention is not as important on the posterior teeth, except perhaps in initial cases of pronounced rotation [7]. Retention should apply to both arches since they are equally subject to change. Efforts could be made to raise awareness about the importance of such retention amongst general practitioners who follow these patients before and after treatment.
