Discussion
(Cubertafond 1989) qui requière souvent une bonne paroi sous-jacente. Suivant d'anciennes études (Bourgeon 1972) et la notre, nous avons
choisit l'approche intra péritonéale. Cette technique ne nécessite pas la dissection des différents plans de la paroi, ce qui diminue l'incidence
des abcès de paroi post-opératoires. Certains auteurs insistent sur le risque d'occlusion intestinale post-opératoire et de fistule de l'intestin
grêle en positionnant la prothèse en intra péritonéale (Daufman 1981 ; De Guzman 1995). Ces adhérences peuvent être évitées, en interposant
du grand épiploon lorsque cela est possible. Une récente étude clinique et expérimentale a suggéré l'utilisation de treillis composites,
pouvant réduire les adhérences intra péritonéales. Ces résultats doivent être confirmés par des études prospectives randomisées.
