Introduction Le gène humain ETV6 (ETS Variant gene 6, appelé également TEL) est composé de 8 exons mesurant environ 240 kb sur le bras p13 du chromosome 12 (1,2). Le gène ETV6 code pour un facteur de transcription nécessaire spécifiquement à l'hématopoïèse dans la moelle osseuse (3). La protéine ETV6 contient deux domaines critiques: le domaine de dimérisation hélice-boucle-hélice (HLH) 5', codé par les exons 3 et 4, et le domaine 3' de liaison à l'ADN spécifique à la transformation d'E26, codé par les exons 6 à 8 (2). La translocation d'ETV6 a été identifiée avec différents partenaires chromosomiques (4). Les deux sondes ADN utilisées pour la détection par hybridation in situ en fluorescence à signal divisé des translocations impliquant le gène ETV6 sont conçues pour s'hybrider en amont et en aval de la région du segment de rupture commun. La colocalisation des sondes provoque un signal jaune, alors que les évènements de translocation dans la région vont diviser le signal en deux signaux séparés, l'un vert (fluorescéine) et l'autre rouge (rouge Texas). Cible (rouge) La sonde ADN marquée au rouge Texas (ETV6-TR) se lie à un segment de 264 kb télomère à la région du segment de rupture (breakpoint) d'ETV6 sur le chromosome 12p13. Cible (verte) La sonde marquée à la fluorescéine (ETV6-Flu) se lie à un segment de 483 kb centromère à la région du segment de rupture d'ETV6 sur le chromosome 12p13. Ecart-type (entre les sondes) L’écart entre ETV6-TR et ETV6-Flu est de 44 kb. Réactif fourni ETV6 FISH DNA Probe, Split Signal, est un mélange de deux sondes ADN marquées par un fluorochrome et de deux sondes PNA de blocage non marquées. Les sondes ADN du mélange consistent en une sonde marquée à la fluorescéine (ETV6-Flu) et une sonde marquée au rouge Texas (ETV6-TR) et elles doivent être utilisées avec la trousse Dako Cytology FISH Accessory Kit, code K5499 sur les échantillons cytologiques. Le réactif est fourni sous forme liquide dans une solution d'hybridation contenant du formamide à 45 %, du sulfate de dextran à 10 %, du NaCl à 300 mmol/L, du phosphate à 5 mmol/L et un agent de blocage. Chaque flacon contient 200 µL, correspondant à 20 tests (10 µL par test). Spécificité 99,4 %. Définie comme la moyenne de six numérations indépendantes de 1 050 cellules de sang périphérique normal. Introduction The human ETV6 gene (ETS Variant gene 6, also known as TEL) consists of 8 exons spanning ~240 kb on chromosome 12 band p13 (1, 2). The ETV6 gene encodes a transcription factor required specifically for hematopoiesis within the bone marrow (3). The ETV6 translocation has been identified with different chromosome partners (4). The two DNA probes used for split-signal FISH detection of translocations involving the ETV6 gene are designed to hybridize upstream and downstream of the common breakpoint cluster region. Co-localization of the probes results in a yellow signal, whereas translocation events in the region will split the signal in separate green (fluorescein) and red (Texas Red) signals. Target (red) The Texas Red-labeled DNA probe (ETV6-TR) binds to a 264 kb segment telomeric to the ETV6 breakpoint cluster region on chromosome 12p13. Target (green) The fluorescein-labeled DNA probe (ETV6-Flu) binds to a 483 kb segment centromeric to the ETV6 breakpoint cluster region on chromosome 12p13. Probe gap The gap between ETV6-TR and ETV6-Flu is 44 kb. Reagent provided ETV6 FISH DNA Probe, Split Signal, is a mixture of two fluorochrome-labeled DNA probes and unlabeled PNA blocking probes. The DNA probes in the mixture consist of a fluorescein-labeled probe (ETV6-Flu) and a Texas Red-labeled probe (ETV6-TR), and should be used in combination with Dako Cytology FISH Accessory Kit, code K5499, on cytology specimens. The reagent is provided in liquid form in hybridization solution containing 45% formamide, 10% dextran sulphate, 300 mmol/L NaCl, 5 mmol/L phosphate, and blocking agent. Each vial contains 200 µL corresponding to 20 tests (10 µL per test). Specificity 99.4%. Defined as the mean of 6 independent countings of 1050 cells from normal peripheral blood. Sensitivity 96.7%. Defined as the mean of 6 independent countings of 1050 cells in a translocation-positive cell line REH (5). Cut-off value When counting 50 cells, specimens with more than 4 split signal events should be considered ETV6 translocation positive. Based on the specificity and sensitivity of the probe, this will ensure that 99% of all specimens containing less than 2% cells with split signal events will be characterized as translocation negative, while 99% of all specimens containing more than 12% cells with split signal events will be characterized as translocation positive. Das ETV6 Gen kodiert einen Transkriptionsfaktor, der spezifisch für die Hämatopoese im Knochenmark benötigt wird (3). Das ETV6 Protein enthält zwei kritische Domänen: die 5´ Helix-Schleife-Helix (HLH)Dimerisierungsdomäne, die durch Exone 3 und 4 kodiert werden, sowie die 3´ E26 Transformations-spezifische DNA bindende Domäne, die durch die Exone 6-8 kodiert wird (2). Die zwei DNASonden, die zur Split Signal Fish-Detektion von Translokationen unter Beteiligung des ETV6 Gens verwendet werden, sind so ausgelegt, dass sie sowohl upstream wie auch downstream von der gemeinsamen „breakpoint cluster region“ hybridisieren.
