PRECAUTIONS Information for the use of the REZA BAND® UES Assist Device, the REZA BAND External Manometer and the REZA BAND Pressure Sensor are provided in this labeling and the Patient Information Handbook. Before using the REZA BAND UES Assist Device, the REZA BAND External Manometer or the REZA BAND Pressure Sensor please read the information thoroughly. Direct any questions to Somna Therapeutics, LLC, Germantown, Wisconsin, 53022, USA (262-345-5553). • The REZA BAND, the External Manometer or the Pressure Sensor should only be used by health care providers trained to prescribe and apply the device. • The safety of the REZA BAND for use during pregnancy, in breastfeeding females or in patients under 18 years of age has not been established. • The safety of the REZA BAND in patients currently receiving treatment with continuous positive airway pressure (CPAP) has not been established. DESCRIPTION The REZA BAND UES Assist Device is a non-invasive, non-sterile device worn by the patient when sleeping. It is intended to reduce the symptoms of laryngopharyngeal reflux (LPR) disease. The REZA BAND works by providing pressure at the cricoid cartilage region, which increases the luminal pressure within the upper esophageal sphincter (UES) and stops reflux from rising above the UES. • Do not use if the package of the REZA BAND is opened or damaged. • If the patient feels unusual pain, skin irritation or the worsening of symptoms, they should discontinue the use of the REZA BAND and contact their health care provider. • If the REZA BAND does not stay in place, the patient should stop using the REZA BAND and contact their health care provider. The patient may need to be re-fitted for the REZA BAND. The REZA BAND consists of a Cushion, a Comfort Band, a Frame, Comfort Dial and Clasp. The Comfort Dial of the REZA BAND allows the patient to slightly adjust the REZA BAND for comfort. • The External Manometer is not designed for liquid immersion. Only use wipe cleaners and/or disinfectants. The External Manometer should be cleaned with a damp cloth using warm water or pre-saturated isopropyl alcohol wipes. INTENDED USE / INDICATIONS The REZA BAND UES Assist Device is indicated for patients 18 years and older to reduce the symptoms of laryngopharyngeal reflux (LPR) disease by reducing the regurgitation of stomach contents from passing through the upper esophageal sphincter. The device is worn by the patient when sleeping. • The skin is to be intact, clean and dry before wearing the REZA BAND. • During the clinical study with patients that used the REZA BAND, there were some reports of skin reaction, discomfort, and feeling acid in the throat. These were mostly mild and short in duration. WARNINGS • The safety and effectiveness has not been demonstrated for the following conditions: » Patient with implants or implant parts that reside in the area where the REZA BAND® is applied. Summary Of Clinical Studies » Patient with an implanted pacemaker, implanted cardioverter defibrillator (ICD), vagus nerve stimulator, or other such similar devices implanted in the neck. Multi-Center, Non-Randomized, Prospective Study of the REZA BAND Upper Esophageal Sphincter (UES) Assist Device for the Treatment of Esophagopharyngeal Reflux » Patient diagnosed with glaucoma. A non-randomized, prospective, open label study of 95 patients treated at 5 investigational sites was conducted. Subjects that had been clinically diagnosed with laryngopharyngeal reflux (LPR) (i.e., chronic cough, choking, aspiration, chronic post nasal drip, globus, sore throat, throat clearing) and who met the inclusion and exclusion criteria were enrolled into the study and fit by the physician with the REZA BAND. There were 69.5% female, 30.5% male, with an average age of 48.8 years, an average Body Mass Index (BMI) of 25.5, 81.1% Caucasian, 8.4% Hispanic, 4.2% African American, 1.1% Asian and 1.1% reported as “Other”. » Patient had a malignancy of the neck, including neck surgery. » Patients that may have an altered mental status including due to the use of sedative drugs or narcotics. » Patients with carotid artery disease, thyroid disease, a history of cerebrovascular disease, or any disorder of connective tissues (e.g., Marfan’s Syndrome or EhlhersDanlos Syndrome). • The REZA BAND is packaged and designed for single patient use only. • The Pressure Sensor is packaged and designed for a one-time use only. The objective of the study was to assess the safety and 1 effectiveness of the REZA BAND when worn by subjects that had been clinically diagnosed with LPR (i.e., chronic cough, choking, aspiration, chronic post nasal drip, globus, sore throat, throat clearing). The subjects wore the REZA BAND at bedtime and took it off upon waking. to the Reflux Symptom Index, the SF-36® Health Survey, the Functional Outcomes of Sleep Questionnaire and patient diaries. Safety Results The adverse events that were reported during the study were generally mild, short in duration and the majority of those events were not related to the device. Those events that were related to the device were also generally mild and short in duration. Device related adverse events did not result in reduced outcomes in relation to the change of the RSI score from baseline to Visit 3 (Week 4), as they were consistent with the overall population. There were no deaths in the study, there were no unexpected adverse events, and none of the subjects withdrew from the study due to an adverse event. The primary effectiveness endpoint was the percent reduction in the Reflux Symptom Index (RSI) from Baseline to Visit 3 (Week 4). The RSI is a validated nine-item patient-administered outcome questionnaire designed to document the symptoms and severity of LPR. Patients were instructed to rate how the nine symptoms affected them on a scale of 0 (no problem) to 5 (severe problem), with a maximum total score of 45. The change in the RSI from baseline to each post treatment visit, as well as any post-treatment visit, were found to be statistically improved for both Visit 2 and Visit 3 (p<0.0001). The Primary Effectiveness endpoint was defined as the percent change in the RSI from Baseline (Visit 1) to End of Study. Table 2 provides the percent change in the RSI from the baseline for Visit 2 (2 weeks) and Visit 3 (4 weeks) for the 89 of 95 (93.7%) subjects that wore the REZA BAND for at least 2 weeks and provided one post-baseline RSI assessment. The 6 subjects that were excluded did not return for Visit 2 (2 week) or Visit 3 (4 week). 2 Table 2 Reflux Symptom Index (RSI) Percent Change in RSI From Visit 1 N = 89 All study subjects had an RSI score of greater than 13 at baseline. In the literature, an RSI score of greater than 13 suggests the presence of LPR1. Seventy-five of the 89 subjects (84.3%) that completed the study reported taking acid reducing medications at the time of enrollment. Of these 75 subjects, 61 (81.3%) continued taking acid reducing medication during the study and 14 (18.7%) discontinued taking acid reducing medications during the study. The study was not intended to show nor did the analysis of the data show that the REZA BAND® provides a significant reduction in use of acid reducing medication. Safety of an Intentionally Displaced REZA BAND Upper Esophageal Sphincter (UES) Assist Device Objective The safety of the REZA BAND was evaluated when worn as intended over the cricoid, as well as when intentionally displaced laterally over each side of the neck, based on changes in heart rate, blood pressure, cardiac rhythm and intraocular pressure (IOP), as compared to when the REZA BAND was not being worn. (1) p-Value based on paired t-test to test hypothesis of no mean percent change. (2) p-Value based on one-sided one sample t-test to test hypothesis of mean change >25%. 1 2 Week Methods Twenty (20) subjects who met the inclusion and exclusion criteria were enrolled in the study, and were representative of the intended REZA BAND patient population. The study population consisted of 60% males and 40% females, 95% Caucasian and 5% Belafsky PC, Postma GN, Koufman JA. Validity and reliability of the reflux symptom index (RSI). J Voice. 2002 Jun;16(2):274-7. 3 African-American subjects, with a mean age of 46.3 years, and a mean Body Mass Index (BMI) of 28.7. when compared to the baseline values. Additionally, electrocardiograms (ECGs) were evaluated to determine whether there was any impact on cardiac rhythm, when the REZA BAND was placed as intended, or intentionally displaced, over the jugular and carotid vasculature. It was determined that there was no change in the ECGs for any of the subjects at any time during the study, when comparing the baseline ECG, to any other ECG. Measurements of heart rate, blood pressure, cardiac rhythm and IOP were taken immediately after the REZA BAND was placed on the neck in each of the specified scenarios (cricoid, right displacement and left displacement), 5 minutes after placement (except for IOP) and again 15 minutes after placement. The REZA BAND was removed and 3 minutes later, IOP was again measured in each placement scenario. When the REZA BAND is set to apply its maximal pressure with the Comfort Dial fully actuated, and then displaced laterally, the pressure being applied by the REZA BAND is significantly reduced (mean of 59.2%, when displaced laterally to the left and a mean of 55.2%, when displaced laterally to the right), as compared to the applied pressure over the cricoid. This further reduces the risk of increased pressure to the sensitive structures of the neck. After the fitting of the REZA BAND, the Comfort Dial was fully actuated, to achieve the maximum pressure. The maximum pressure with the Comfort Dial fully actuated was also measured with the REZA BAND displaced laterally over the vascular structures on both sides of the neck. Results Conclusion The results showed that there was no effect on heart rate, blood pressure, cardiac rhythm or IOP, when the REZA BAND was worn as intended as well as when the REZA BAND was intentionally displaced laterally, as compared to the baseline. No adverse events were reported during this study. In the event the REZA BAND is displaced laterally, the likelihood of impacting sensitive structures of the neck is low. There was no effect on heart rate, blood pressure, cardiac rhythm, or intraocular pressure, either when the REZA BAND is placed as intended over the cricoid cartilage, or when it is laterally displaced, over the jugular and carotid vasculature. It was concluded that based on the results of this study, the REZA BAND is safe, when worn as intended and if it were to be were to be displaced laterally. Analyses of Variance (One Way) were conducted on heart rate, blood pressure and intraocular pressure to determine whether there was any effect of the REZA BAND being placed as intended or laterally displaced, when compared to not wearing the REZA BAND. These analyses showed that there was no significant difference for any of the measurements at any point in time, Fitting The REZA BAND To The Patient • Position the REZA BAND around patient with the Cushion positioned over the cricoid cartilage area. Connect the REZA BAND Frame to the Comfort Band with the Clasp. Ensure that the RB logo is on the patient’s right side. The REZA BAND should be loose around the patient. • Have the patient sit still and look forward. Ensure that the patient does not bend their neck or tilt their head as the REZA BAND is being fit. • Press the “on” button of the External Manometer. Adjust the Comfort Band until desired pressure of between 20-30 mmHg is seen on the External Manometer display. • Remove the REZA BAND and the External Manometer. Reapply the REZA BAND and confirm the patient’s comfort. • Connect the Pressure Sensor to the External Manometer. • Have the patient apply the REZA BAND on their own, multiple times, so that it is clearly understood what is the correct position, when wearing the REZA BAND. Ensure that it is positioned correctly. • Place the Pressure Sensor between the Cushion of the REZA BAND and the patient. • No further adjustments are needed, except for patient comfort, using the Comfort Dial. • Please make sure the patient understands that the Comfort Dial can be used to apply slight additional pressure that is a fine adjustment for their comfort. 4 PATIENT INSTRUCTIONS Handling and storage of this product, as well as factors relating to the patient, diagnosis, treatment and other matters beyond Somna Therapeutics, LLC’s control directly affect the product and the results obtained from its use. MÉTODOS Veinte (20) sujetos que cumplían los criterios de inclusión y exclusión fueron incluidos en el estudio, y eran representativos de la población de pacientes REZA BAND prevista. La población de estudio estuvo constituida por sujetos 60% hombres y 40% mujeres, 95% caucásicos y 5% afroamericanos, con una edad media de 46.3 años y un índice de masa corporal (IMC) de 28.7. Le dispositif est porté par le patient durant son sommeil. • Si la REZA BAND ne reste pas en place, le patient doit immédiatement arrêter l’utilisation de la REZA BAND et contacter son prestataire de soins de santé. Le patient peut nécessiter la réadaptation de la REZA BAND. • Le manomètre externe n’est pas conçu pour être plongé dans un liquide. N’utilisez que des lingettes et/ou des désinfectants. Le manomètre externe doit être nettoyé avec un tissu humide en utilisant de l’eau tiède ou des lingettes présaturées d’alcool isopropylique. Le paramètre principal d’efficacité était le pourcentage de réduction de l’indice de symptôme de reflux (ISR) depuis le début de l’étude jusqu’à la visite 3 (Semaine 4). L’ISR est un questionnaire validé administré par le patient sur neuf éléments de résultat conçu pour documenter les symptômes et la gravité du RLP. Il a été demandé aux patients de classer comment les neuf symptômes les affectaient sur une échelle de 0 (pas de problème) à 5 (problème grave) avec un score total maximum de 45. MÉTHODES Vingt (20) sujets satisfaisant aux critères d’inclusion et d’exclusion ont été recrutés dans l’étude et étaient représentatifs de la population voulue portant une REZA BAND. La population étudiée consistait en 60% d’hommes et 40% de femmes, 95% de sujets de type caucasien et 5% de sujets de type afro-américain avec un âge moyen de 46,3 ans et un indice moyen de masse corporelle (IMC) de 28,7. La pression maximum avec le cadran de réglage de confort tourné à fond a aussi été mesurée avec la REZ BAND déplacée latéralement sur les structures vasculaires des deux côtés du cou. De plus les électrocardiogrammes (ECG) ont été évalués pour déterminer s’il y avait un impact sur le rythme cardiaque quand le REZA BAND était placée comme prévu ou intentionnellement déplacée sur les veines jugulaires et carotides. Il a été déterminé qu’il n’y avait pas de changement dans les ECG à tout moment pour tous les sujets durant l’étude en comparaison avec les ECG de début d’étude ou avec tout autre ECG. RÉSULTATS Les résultats ont indiqué qu’il n’y avait pas d’effet sur la fréquence cardiaque, sur la pression artérielle, sur le rythme cardiaque ou sur la PIO quand la REZA BAND était portée comme prévu ainsi que quand la REZA BAND était intentionnellement déplacée latéralement en comparaison des résultats de début d’étude. Aucun évènement indésirable n’a été rapporté durant cette étude. Quand la REZA BAND est réglée pour appliquer la pression maximum avec le cadran de réglage de confort tourné à fond, puis déplacée latéralement, la pression appliquée par la REZA BAND est notablement diminuée (moyenne de 59,2% quand déplacée latéralement vers la gauche et moyenne de 55,2 quand déplacée latéralement vers la droite) lorsque comparée à la pression appliquée lorsque placée sur le cricoïde. Ceci réduit davantage le risque de pression augmentée sur les structures sensibles du cou. Des analyses de variance (un facteur) ont été effectuées sur la fréquence cardiaque, la pression artérielle et la pression intraoculaire pour déterminer si il avait un effet quand la REZA BAND était placée comme prévu ou déplacée latéralement comparé à quand elle n’était pas portée. VERWENDUNGSZWECK / INDIKATIONEN REZA BAND™ OÖS Hilfsgerät eignet sich ab 18 Jahre und älter zum Reduzieren der Symptome von laryngopharyngealem Reflux (LPR) durch Verringern des Rückflusses des Mageninhalts, der durch den oberen Ösophagussphinkter gelangt. Das Gerät wird vom Patienten während des Schlafens getragen. • Der Externe Manometer wurde nicht für Eintauchen in Flüssigkeiten konzipiert. Nur Reinigungsmittel zum Abwischen bzw. Desinfektionsmittel verwenden. Der externe Manometer sollte mit einem feuchten Tuch mit warmem Wasser oder mit in Isopropylalkohol vorgetränkten Tüchern gereinigt werden. WARNHINWEISE • Das REZA BAND nicht verwenden, wenn Sie ein Implantat oder Implantatteile im Bereich haben, an welchem das REZA BAND getragen wird. • Wenn das REZA BAND nicht an Ort und Stelle bleibt, sollte der Patient aufhören das REZA BAND zu verwenden und seinen Arzt kontaktieren. Das Ziel der Studie war es, die Sicherheit und Wirksamkeit des REZA BANDs zu bewerten, wenn es von den Probanden getragen wird, die klinisch mit LPR (d.h. chronischer Husten, Würgen, Aspiration, chronische Postnasal-Drip, Globus, Halsschmerzen, Räuspern) diagnostiziert wurden. Die Probanden legten das REZA BAND vor dem Schlafengehen an und nahmen es nach dem Aufwachen ab. VERFAHREN Zwanzig (20) Probanden, die den Ein- und Ausschlusskriterien entsprachen, wurden in die Studie aufgenommen und waren repräsentativ für die beabsichtigte REZA BAND Patientenpopulation. Die Studienpopulation bestand zu 60% aus Männern und 40 % Frauen, 95 % waren europäischer Abstammung und 5 % afro-amerikanischer Abstammung. DESCRIZIONE Il Dispositivo di Assistenza REZA BAND UES è un dispositivo non invasivo, non sterile indossato dal paziente durante il sonno. Esso è destinato a ridurre i sintomi del disturbo da reflusso laringofaringeo (LPR). USO PREVISTO/INDICAZIONI Il dispositivo di assistenza REZA BAND UES è indicato per persone di almeno 18 anni di età per la riduzione dei sintomi del reflusso laringofaringeo (LPR) negli adulti riducendo il passaggio del rigurgito del contenuto dello stomaco attraverso lo sfintere esofageo superiore. Sintesi degli Studi Clinici Uno studio prospettivo multi-centro, non randomizzato del Dispositivo di Assistenza Sfintere Esofageo Superiore (UES) REZA BAND per il trattamento del reflusso Esofagofaringeo • Non usare REZA BAND se il paziente ha avuto un tumore maligno o ha subito un’operazione chirurgica sul collo. • Non usare REZA BAND nei pazienti che possono avere uno stato mentale alterato a causa dell’uso di farmaci sedativi o sostanze stupefacenti. È stato condotto uno studio prospettivo aperto nonrandomizzato di 95 pazienti trattati in 5 siti di sperimentazione. Metodi Venti (20) i soggetti che hanno incontrato i criteri di inclusione e di esclusione sono stati registrati nello studio, ed erano rappresentativi della popolazione di pazienti REZA BAND prevista. Il REZA BAND dovrebbe stare allentato intorno al paziente. • Fate stare fermo il paziente e fatelo guardare avanti. Assicurarsi che il paziente non pieghi il collo o inclini la testa non appena REZA BAND sia indossato. • Premere il pulsante “on” del Manometro Esterno. Regolare la Banda di Comfort finché la pressione desiderata tra 20-30 mmHg viene mostrata sul display del Manometro Esterno. • Collegare il Sensore di Pressione al Manometro Esterno. • Rimuovere REZA BAND e il Manometro Esterno.
