I undersigned, sanitary veterinarian authorized by the veterinary services, certify that the following declaration are complied with:
1. Het/de beslag(en) waarvan de hierboven beschreven verzending afkomstig is werden geïnspecteerd. Na een grondig fysisch onderzoek
werden de beslagen vrij bevonden van de symptomen van de volgende besmettelijke infectieuze ziekten: ISDH (infectieus syndroom van het
dikke hoofd), infectieuze encefalomyelitis, infectieuze bronchitis, de ziekte van Newcastle, vogelpokken, leukemie lymphoïde, EDS (egg drop
syndroom), aspergillose, ornithose-psittacose, vogelcholera, infectieuze laryngotracheïtis, de ziekte van Gumboro, aviaire coryza, de ziekte
van Marek, infectieuze hepatitis, aviaire influenza, salmonellosis (pullorum, gallinarum, enteritidis, typhimurium), mycoplasmosis gallisepticum /
Le ou les troupeaux d’où provient l’expédition décrite ci-dessus ont été inspectés. A la suite d’un examen physique minutieux, les
troupeaux ont été trouvés exempts de tout symptôme des maladies infectieuses et contagieuses suivantes: SIGT (Syndrôme
Infectieux de la Grosse Tête), encéphalomyélite infectieuse, bronchite infectieuse, maladie de Newcastle, variole aviaire, leucose
lymphoïde, EDS (syndrôme de chute de ponte), aspergillose, ornithose-psittacose, choléra aviaire, laryngotrachéite infectieuse,
maladie de Gumboro, coryza aviaire, maladie de Marek, hépatite infectieuse, peste aviaire, salmonellose (pullorum, gallinarum,
enteritidis, typhimurium), mycoplasmose gallisepticum / The herd(s) the consignment is originating from have been inspected. Following a
thorough physical examination, the herd(s) was (were) found free from any symptoms of the following contagious infectious diseases: IBHS
(Infectious Big Head Syndrome), infectious encephalomyelitis, infectious bronchitis, Newcastle disease, fowl pox, leukemia lymphoid, EDS
(egg drop syndrome), aspergillosis, ornithosis-psittacosis, avian cholera, infectiouslaryngotracheitis, infectious bursal disease, avian coryza,
Marek’s disease, infectious hepatitis, avian influenza, salmonellosis (pullorum, gallinarum, enteritidis, typhimurium), mycoplasmosis
gallisepticum.
2. De beslagen van oorsprong werden gevaccineerd tegen de ziekte van Gumboro, de ziekte van Newcastle, aviaire encefalomyelitis en
infectieuze bronchitis / Les troupeaux d’origine ont été vaccinés contre la maladie de Gumboro, la maladie de Newcastle,
l’encéphalomyélite aviaire et la bronchite infectieuse / The herds of origin have been vaccinated against the Gumboro disease, the
Newcastle disease, avian encephalomyelitis and infectious bronchitis.
3. De beslagen van oorsprong werden niet tegen mycoplasmosis gevaccineerd / Les troupeaux d’origine n’ont pas été vaccinés contre la
mycoplasmose / The herds of origin have not been vaccinated against mycoplasmosis.
4. De beslagen van oorsprong werden gevaccineerd tegen Salmonella enteritidis / Les troupeaux d’origine ont été vaccinés contre

5. De verzending vindt plaats in nieuwe, propere en voor eenmalig gebruik bestemde dozen / L’expédition est faite dans des boîtes

6. Het vervoermiddel werd op voorhand gereinigd en gedisinfecteerd / Les moyens de transport ont été préalablement nettoyés et

1. De bedrijven waarvan de broedeieren afkomstig zijn, zijn erkend door het voogdijministerie van het land van oorsprong en worden
regelmatige gecontroleerd door de officiële veterinaire diensten van België / Les élevages d’où sont issus les œufs à couver sont agréés
par le Ministère de tutelle du pays d’origine et soumis régulièrement à l’inspection sanitaire des Services vétérinaires officiels
belges / The farms the hatching eggs are originating from are approved bij the supervising Ministry of the country of origin and are regularly
controlled by the official Belgian veterinary services.
2. De beslagen van oorsprong van de broedeieren zijn gelegen in een zone met een straal van 60 km, waar geen enkele
quarantinemaatregel werd toegepast omwille van een uitbraak van infectieuze aviaire ziekten van de OIE A-lijst sinds minstens een jaar / Les
troupeaux d’origine des œufs à couver sont situés dans une zone de 60 km de rayon, où aucune mesure de quarantaine pour cause
d’épidemie de maladies aviaires infectieuses de la liste A de l‘OIE n’a été déclarée depuis au moins un an / The herds of origin of the
hatching eggs are located in a 60 km zone where no quarantine measures have been implemented related to outbreaks of infectious diseases
of the OIE A-list since at least one year.
3. De dieren of producten beschreven in het certificaat zijn niet afkomstig van bedrijven of van dieren of van eieren afkomstig van bedrijven
die onderworpen zijn aan beschermende maatregelen (beslaglegging) door de veterinaire diensten, omwille van een verdenking van gebruik
van voeder dat mogelijks besmet is met dioxine, zoals voorzien door communautaire waarschuwing. De producten zijn dus niet besmet door
deze vervuiling, en overeenkomstig de monitoringsplannen overschrijdt het dioxinegehalte de aanvaarde internationale normen niet / Les
animaux ou produits décrits dans le certificat ne proviennent pas d’exploitations ou d’animaux ou d’œufs issus d’exploitations qui
font l’objet de mesures conservatoires (mise sous séquestre) par les services vétérinaires, du fait d’une suspicion d’utilisation
d’aliments susceptibles d’être contaminés par la dioxine, tel que prévu par alerte communautaire. Les produits ne sont donc pas
contaminés par cette pollution, et conformément aux plans de surveillance, le taux de dioxine ne dépasse pas les normes
internationales admises / The animals or products described in this certificate do not originate from farms or from animals or eggs originating
from farms which are subjected to conservatory measures (seizure) by the veterinary services, in the frame of a suspicion of use of feed
possibly contaminated by dioxins, as prescribed by communaurtary alert. The products are not contaminated by this pollution and, according to
the monitoring plans, the dioxin level does not exceed the international accepted standards.

Het origineel van het certificaat vergezelt de zending tot op de eindbestemming / L’original du certificat devra accompagner l’envoi jusqu’au lieu de destination final / The original of the certificate must accompany the
consignment to its final destination.
