P409 A rare collateral damage of antiretroviral therapy S. Käppeli, J. Gubler, U. Karrer Cantonal Hospital Winterthur (Winterthur) A 37 year old, HIV-infected woman presented to our outpatient clinic with rapidly progressive hemiparesis 11 weeks after starting combination antiretroviral therapy (cART). She was HIV-infected by IVDU since 1988, but had only been treated with Zidovudin for 6 months during pregnancy in 1999. In August 2010, she was profoundly immunosuppressed with a CD4-count of 11 cells/ul and a viral load of 82000 copies/ml. cART with Atazanavir, Ritonavir, Emtricitabin and Tenofovir was started. After one month, viral load was already ‘undetectable’ (<50 copies/ml) but CD4-cells remained low. Two months later, the patient developed progressive paresis of the right arm, numbness of the right arm and leg, slight amnesic aphasia and a gait disturbance. In the MRI of the brain, T2 and FLAIR sequences demonstrated multiple hyperintense lesions in the white matter with substantial surrounding contrast enhancement. CSF-analysis was normal concerning cellularity, protein and glucose. A diagnosis of progressive multifocal leukoencephalopathy (PML) in conjunction with immune reconstitution inflammatory syndrome (IRIS) was confirmed by detection of JC-virus DNA in the CSF. High dose corticosteroid therapy was initiated to curtail CNS-inflammation and cART was adapted for optimal CNS-penetration (Abacavir, Lamividin, Tenofovir, Lopinavir/Ritonavir). After a follow up MRI demonstrated regression of contrast enhancement (i.e. inflammation), corticosteroids were tapered and finally stopped. Neurologic symptoms gradually improved with the help of intensive physical therapy. 17 weeks after the beginning of cART and 6 weeks after symptoms started, only partial paresis of the right arm and slight numbness of the right extremity persist. Discussion: PML is caused by the JC polyomavirus (JCV) that destroys oligodentrocytes and their myelin processes. Significant reactivation of JCV in the CNS only occurs in situations of profound cellular immunodeficiency. Before the era of cART, PML was inevitably fatal. IRIS is defined as the onset of inflammatory processes after cART induced improvement of immune function. It is well recognized in the context of mycobacterial, viral (CMV) and fungal infections (Cryptococcus, Pneumocystis) and has been described in PML before. As in our case, IRIS may unmask clinically silent opportunistic infections. Since IRIS is a form of immune pathology, transient immune modulation with corticosteroids is often the treatment of choice. P410 Leberabszess mit Klebsiellae pneumoniae als Ursache einer Sepsis bei Asiaten C. Horvath, S.A. Shaw, U. Peter, R. Imoberdorf, U. Karrer Kantonsspital Winterthur (Winterthur) Fall: Eine 63 jährige gebürtige Philippinin wurde mit Schüttelfrost und unklarem Infekt zugewiesen. Anamnestisch seit einem Tag Fieber, Schüttelfrost, Kopfschmerzen, Übelkeit und Erbrechen. Seit 1980 in der Schweiz lebend, letzter Asienaufenthalt vor 7 Monaten. Im Status Temperatur 38.1°C, Blutdruck 85/60, Puls 88/min; Abdomen weich, indolent; Klopfdolenz der linken Nierenloge. Bei mässiger Leukozyturie und partiell hyperechogener Niere links wurde die Symptomatik beurteilt als Pyelonephritis/Urosepsis und behandelt mit Ceftriaxon. Blut- und Urinkultur im Verlauf positiv mit Klebsiella pneumoniae. Wegen akuter respiratorischer Verschlechterung am Folgetag Durchführung eines CT Thorax/Oberbauch: Als Zufallsbefund Nachweis eines solitären, 4.8cm grossen Leberabszesses im Segment VIII mit erweiterten Gallenwegen aber ohne Konkremente. Auf eine Abszess-Punktion oder Drainage wurde wegen der ungünstigen Lage und des kleinen liquiden Anteils verzichtet. Antibiotische Therapie über 2 Wochen mit Ceftriaxon i.v., danach für 4 Wochen Ciprofloxacin p.o., jeweils in Kombination mit Metronidazol p.o. Darunter rasche und anhaltende klinische Verbesserung, jedoch nur langsame Grössenabnahme des Abszesses. Diskussion: Leberabszesse mit Klebsiella pneumoniae stellen bei asiatischen Patienten eine eigene Entität dar und kommen auch dann deutlich häufiger vor, wenn die Betroffenen schon seit Jahren nicht mehr im asiatischen Raum leben. Ursache dieser geographischen Verteilung sind spezielle Virulenzfaktoren (K1 Serotyp, hypermukovisköser Phänotyp, magA Gen). Prädisponierend ist der Diabetes mellitus. Entsprechend sollte bei Asiaten mit Nachweis von Klebsiellen in der Blutkultur immer eine Bildgebung der Leber erfolgen. Therapeutisch sollten Abszesse wenn immer möglich früh punktiert und drainiert werden, da dies die Prognose verbessert und die Therapiedauer verkürzt. Weil neben Klebsiellen oft auch Anaerobier vorkommen, ist eine entsprechende antibiotische Abdeckung sinnvoll. Die Dauer der Therapie richtet sich nach Klinik und Grössenverlauf der Abszesse.
