Veuillez choisir un maximum de trois activités, mais assurez-vous qu’il n’y a pas de conflit
d’horaire.

Une personne qui participe à la même activité plus d’une fois ne pourra remporter qu’une
seule médaille.

Un maximum de 100 personnes pourront s’inscrire au tournoi de golf.

Si la participation dans n’importe quelle discipline n’est pas adéquate, il est possible que
l’activité soit annulée. Si une de vos activités est annulée, vous serez avisé, soit par courriel ou
par téléphone afin de faire un autre choix d’activité.

La marche chronométrée n’est pas considérée comme une activité.
Games selection form

Select a maximum of three activities but make sure that there are no schedule conflicts.

No more than one medal will be granted to a person who participates in the same activity more
than once.

A maximum of 100 people will be accepted for the golf tournament.

If the participation to an activity is deemed to be too low, the activity may be cancelled. If one
of your activities is cancelled, you will be notified, either by e-mail or by phone, and given the
option to choose another activity.

The timed walk is not considered an activity.
