Spring 2016 French Visa Application Checklist for Citizens of ALL Countries Submission Deadline of ALL materials: November 2-6, 2015 1. ONE ORIGINAL, COMPLETED, SIGNED Visa Application Form The completed visa application form must contain NO mistakes, errors, cross-outs, or white-out. Be sure to use European date format (DD/MM/YYYY, so March 5, 1998 would be 05/03/1998). Leave questions on the visa application blank if they don’t apply to you. Review Johnny Thunderbird’s Sample Visa Application (posted on the “Apply for a Visa Page”) and use it as a guide to complete your visa application. Use the Visa Application Form With Pre-Filled Answers (posted on the “Apply for a Visa Page” o But, if any of the pre-filled answers do not apply to you, e.g., the intended date of entry, you will need to complete the blank visa application, and copy the rest of the pre-filled answers onto your visa application form! Sign and date the visa application at the bottom of page 2 of the visa application form. See page 3 of this Visa Application Checklist for more tips on completing your visa application form! 2. ONE ORIGINAL & SIGNED, Passport + ONE COPY of Bio Page (picture and signature page) Your Passport must (1) be valid until at least November 5, 2016, (2) have been issued in 2006 or later, and (3) include at least 2 blank pages (or more if you plan to travel!) Canadian and Australian Citizens: o In addition to providing your original, signed Passport and bio page copy, you need to provide ONE COPY of Proof of Legal Residence in the United States: Green Card for permanent residents; visa and recently signed Form I-20 for international students. Citizens of all countries other than the U.S., Canada and Australia: o In addition to providing your original, signed Passport and bio page copy, you need to provide ONE COPY of Proof of Legal Residence in the United States: Green Card for permanent residents; visa and recently signed Form I-20 for international students. o *This semester you will be able to participate in the OGS Group Visa Process, but will need to travel with an OGS Administrator to the French Consulate in NYC to submit your visa application. For now, plan to spend the day at the French Consulate in NYC on November 23. OGS will email you with more details. 3. TWO Recent Passport Photos Standard passport photo format (1.5” square inch in size) Taken within the last 6 months You cannot be smiling (and your lips must be closed) The picture must be front facing, in color and on a white background. Please note you cannot have your hair covering your eyes, cannot be wearing glasses, a scarf, a hat or other head covering in these photos. (Please contact the Office of Global Studies if you wear a head covering for religious reasons.) This does not need to be the same photo included in your passport. 4. ONE COPY of Flight Itinerary or Flight Reservation (1) Print out your complete, round-trip flight itinerary or ticket PDF; or (2) Print out your flight reservation showing your date of departure from the U.S. to France; or 1 (Updated: September 25, 2015) (3) Provide a handwritten and legible statement indicating your intended date of departure for France, as well as a formal commitment not to depart before that date, using the following language and format: o First Name Last Name, Passport Number o I, First Name Last Name, will fly from New York (or other relevant U.S. city) to Paris, France (or other relevant city where you will arrive in Europe) arriving on January 12, 2016 (or other relevant date of arrival). I will depart from Europe and return to the U.S. on May 5, 2016 (or other relevant date of departure). o Signature, Date of Signature in European Date format (DD/MM/YYYY) 5. ONE COPY of Proof of Financial Support (1) Student Bank Statement: A recent (within the past 30 days, e.g. October 2015) bank statement verifying at least $3,280 in the student’s personal checking and/or savings account (the student is the only person listed on the bank account); or (2) IF someone else is supporting you financially, you must include: o (A) Guarantor Bank Statement: A recent (within the past 30 days, e.g. October 2015) bank statement verifying at least $3,280 in the guarantor’s personal checking and/or savings account (the guarantor is your family member or friend who will support you financially); AND o (B) A completed Financial Guarantee Form. This form must be notarized. If the guarantor’s bank statement is a joint account held with another individual, the Financial Guarantee Form only needs to be completed by ONE person whose name is listed on the bank account. (3) St. John’s University Office of Student Financial Services Statement that the student has a credit of at least $3,280 that will be dispersed AFTER tuition and fees are covered; or (4) Any combination of (1), (2), or (3) that totals $3,280. Bank Statement Tips! o The bank statement cannot be a screenshot from your bank’s website; o However, if you receive your monthly bank statement electronically in PDF format, a printout of this PDF is acceptable o All financial support documents may be submitted as a copy (e.g., the original Financial Guarantee Form is not needed, so your family member may e-mail you a scanned copy – they do not need to send the original by regular mail). o What does it mean when it says the Financial Guarantee Form must be notarized? When a document is notarized, it means that a Notary Public has watched you sign the document, and puts a stamp on it verifying that your signature is authentic. The Notary cannot be some you’re related to. Most bank branches have a notary, and will provide their services at no charge if you’re a bank customer. 6. ONE COPY of Campus France Confirmation Email Students are strongly encouraged to attend a Campus France Workshop for an OGS Administrator to guide you through the Campus France process o Thursday, October 22, 2015 (2:00-3:15pm) in Sullivan 205, or o Thursday, October 29, 2015 (2:00-3:15pm) in Sullivan 205 (*makeup) 7. ONE ORIGINAL French Immigration (OFII) Form: All students participating in the Discover France: Paris semester program need to complete the OFII Form See the “OFII Instructions for Paris Semester Students Only” on page 5 of this Visa Application Checklist for instructions. 2 (Updated: September 25, 2015) Tips on Filling Out the Visa Application Form! #7 refers to citizenship: if you have a U.S. passport, put “American” #13 refers to your passport number #16 asks for the country your passport is “issued by,” put “USA” if you have a U.S. passport #17: Address you live at while attending school; it must be in New York, New Jersey, Connecticut, or Bermuda. o Remember to use the same address for your Campus France application #25: if you are participating in the home stay option, you will need to list the home stay address here #29: you do not have to disclose the scholarship information #30 only applies if you are relying on family (or someone else) living in France for financial support NOTE: This Visa Application Checklist is only for students applying for a French visa through the St. John’s University Office of Global Studies Group Visa Process. Dans ce cas, vous serez présumé être entré sur le sol français au plus tard cinq jours après cette date. 1. Enregistrement de votre séjour en France : apposition de la vignette OFII dans votre passeport. Votre visa de long séjour ne vaut titre de séjour que s’il a été validé par l’OFII. Pour accomplir cette démarche, vous devez, dès votre arrivée en France: • • • Compléter le formulaire intitulé « Demande d’attestation OFII » qui vous a été remis avec votre visa par les mentions de votre date d’arrivée en France, vos coordonnées en France et le numéro de visa qui vous a été délivré, afin que l’OFII puisse vous convoquer à une visite médicale et/ou à une visite d’accueil et/ou à la validation de votre VLS-TS. Adresser ce formulaire dès votre arrivée en France à la direction territoriale de l’Office français de l’immigration et de l’intégration (OFII) de votre lieu de résidence en France*. Joindre à votre envoi la copie des pages de votre passeport où figurent les informations relatives à votre identité, le cachet apposé par la police aux frontières (ou par les autorités de l’Etat de transit) portant la date de votre passage de la frontière et le visa qui vous a été délivré. Une attestation confirmant la réception de votre dossier (« Attestation de réception du formulaire de demande d’attestation OFII ») vous sera transmise par courrier, ce document provisoire attestera de la régularité de votre séjour en France jusqu’à ce que votre visa soit validé par l’OFII. Vous serez convoqué par l’OFII dans les trois mois après votre entrée en France pour bénéficier de la visite médicale et/ou de la visite d’accueil et/ou faire valider votre VLS-TS. A cette occasion vous devrez vous munir: - de votre passeport, - d’une photo de face tête nue, - d’une attestation de domicile en France (quittance de loyer, facture d’électricité, de gaz ou de téléphone fixe à votre nom, ou à défaut, une attestation d’hébergement accompagnée de la photocopie de la pièce d’identité de l’hébergeant), - éventuellement, du certificat médical si vous avez passé la visite médicale avant votre départ pour la France. Pour plus d’informations, connectez-vous sur www.ofii.fr, - du montant des taxes à acquitter à l’OFII (voir montant de la taxe à payer sur www.ofii.fr ou sur www.timbresofii.fr). Ces formalités seront entérinées par l’apposition sur votre passeport d’une vignette OFII vous autorisant à résider en France jusqu’à la fin de validité du visa de long séjour qui vous a été octroyé. NB: L’accomplissement de ces formalités demandant un certain délai (1 mois environ), vous devez impérativement effectuer ces démarches dès les premiers jours de votre arrivée en France. Le non accomplissement de ces formalités dans le délai de trois mois vous exposerait à une procédure pour séjour irrégulier et au paiement d’une taxe de régularisation. 2. Renouvellement de votre autorisation de séjour Si vous souhaitez obtenir un renouvellement de votre autorisation de séjour, deux mois avant la fin de validité de votre visa, vous devrez vous présenter à la préfecture de votre lieu de résidence afin d’y déposer cette demande. Le personnel d’accueil vous remettra les imprimés nécessaires et vous indiquera les pièces justificatives à fournir, qui pourront varier en fonction du motif de votre séjour. Le non respect de ce délai vous obligera à rentrer dans votre pays d’origine afin de solliciter un nouveau visa. *Si vous êtes étudiant, vous pouvez être dispensé de l’envoi du formulaire “Demande d’attestation OFII” lorsqu’un dispositif d’accueil spécifique est mis en place lors de la rentrée universitaire ou lorsque vous êtes inscrit dans une école partenaire de la préfecture et de l’OFII dans le cadre d’un dépôt groupé. Pour plus d’information vous pouvez consulter le site www.ofii.fr. TRES IMPORTANT : 1) Si vous souhaitez vous rendre à l’étranger dans un pays de la zone Schengen : votre visa de long séjour à entrées multiples vous suffit pour circuler dans l’Espace Schengen. 2) Si vous souhaitez vous rendre à l’étranger hors zone Schengen ou effectuer un séjour dans votre pays d’origine : - Pendant les 3 premiers mois de votre arrivée en France : circulation autorisée sans vignette OFII ou sans attestation de dépôt de dossier OFII avec visa de long séjour à entrées multiples, - Au-delà de 3 mois : obtention obligatoire de la vignette OFII ou à défaut, de l’attestation de dépôt de dossier OFII pour circuler hors de France. Si vous n’avez pas effectué les formalités OFII dans le délai imparti (3mois) une nouvelle demande de visa de long séjour devra être déposée à la représentation diplomatique ou consulaire de votre pays d’origine.
