EXCELLENCE IN NURSING AWARDS
CAREER ACHIEVEMENT AWARD
Description
The recipient of this award is recognised by her colleagues as an exceptional nurse who has
contributed to the profession for over 25 years. Her initiatives have been evident within the
hospital, the community and the country. In addition to pursuing excellence in her own domain of
practice she has been a visible and active role model and leader for the profession at large. The
impact of her contribution is lasting and will encourage and influence other nurse professionals to
expand their scope and influence in a similar manner, recognizing the immense and positive
influence this can have on patient care and outcomes.
Eligibility criteria
The Career Achievement Award is awarded to a Nurse who has:
●
Shown a lifelong commitment and contribution to the profession of nursing within the
Hospital, the community and the country.
●
Exhibited a personal pursuit of excellence in his/her own area of practice
●
Been a visible and active role model to the Nurses and interdisciplinary team members
with whom she works
●
Provided excellent mentorship over the course of his/her career
●
Provided substantial leadership in the development of nursing, nursing practice and
interdisciplinary care at the Jewish General Hospital
Application Process
The person who nominates the candidate must:
 Complete a Nomination Form (available in Training and Development, room
B-102, and on the JGH Nursing website: www.jgh.ca/en/nursing);
 Write a letter of support clearly describing how the candidate meets all of the eligibility
criteria;
 Obtain a letter of support from another nurse supporting the candidate’s nomination and
clearly describing how the candidate meets all of the eligibility criteria.
N.B.
The letters of support should include specific information regarding the candidate’s
contributions as they relate to the award criteria.
All documents are to be submitted to Training and Development (B-102), to the attention of
Ms. Valerie Frunchak, Associate Nursing Director by Monday, April 8th, 2013.

RÉALISATION DE LA CARRIÈRE
Description
Ce prix sera remis à une infirmière reconnue par ses collègues comme étant une infirmière
exceptionnelle qui a contribué à la profession pour plus de 25 ans. Ses initiatives ont été
manifestes au sein de l'hôpital, la communauté et le pays. En plus de poursuivre l'excellence dans
son propre domaine de la pratique, elle a été un modèle de rôle visible et actif et un leader pour la
profession dans son ensemble. L'impact de sa contribution est durable et encourage les autres
professionnelles des soins infirmiers d'élargir d'une manière similaire leur portée et leur influence,
tout en reconnaissant l’influence immense et positive que cela peut avoir sur les soins aux patients
et les résultats.
Critères d’éligibilité
Le Prix de réalisation de la carrière est décerné à une infirmière qui a:
●
fait preuve d'un engagement continu et d’une contribution à la profession des
soins infirmiers au sein de l'hôpital, la communauté et le pays;
●
démontré une poursuite personnelle de l'excellence dans son propre domaine de la
pratique;
●
été un modèle visible et active aux infirmières et aux membres de l'équipe
interdisciplinaire avec qui elle travaille;
●
fourni un mentorat excellent au cours de sa carrière;
●
assuré un leadership important dans le développement des soins infirmiers, la pratique
infirmière et les soins interdisciplinaires à l'Hôpital général juif.
Mises en candidature
La personne déposant la candidature doit :
 Remplir le formulaire de mise en nomination (disponible au Bureau de la formation et du
développement (B-102) et sur le site Internet des soins infirmiers de l’HGJ :
www.jgh.ca/fr/soinsinfirmiers);
 Composer et remettre une lettre de mise en nomination décrivant clairement comment la
candidate répond à ces critères.
 Remettre une seconde lettre, écrite par une infirmière autre que celle déposant la
candidature, appuyant la nomination de la candidate et décrivant clairement comment elle
répond à ces critères.
N.B.
Les lettres doivent inclure des informations spécifiques concernant les contributions de la
candidate en ce qui concerne les critères d’éligibilité. Veuillez faire parvenir les mises en
candidature au Bureau de la formation et du développement (B-102) à l’attention de Mme
Valerie Frunchak, Directrice adjointe en soins infirmiers, d’ici lundi le 8 avril 2013.
