Les trois tomes de VictimologieVictimology constituent le triptyque des recherches sur les pathologies traumatiques menées depuis plus de dix ans. Ils constituent une synthèse de contributions à la communauté internationale en sciences humaines cliniques. Ce travail conséquent n’aurait pu aboutir dans sa publication sans le soutien des Presses universitaires de Grenoble, la collaboration précieuse de madame Myriam Linnell pour les traductions en langue anglaise et l’ensemble des partenaires de la pratique clinique, thérapeutique et universitaire en France (Université Paris VII. L’ensemble de ces contributions s’étaye sur une praxis quotidienne de la victimologie clinique et des états posttraumatiques. Cette crédibilité de la praxis clinique et thérapeutique tend à argumenter les propositions théorisantes, méthodologiques et évaluatives du corpus pratique qui en constitue les fondements. Les mutations sociétales de la postmodernité engagent la recherche fondamentale et appliquée vers une internationalisation des travaux et de leurs échanges notamment en clinique interculturelle et en clinique humanitaire. Cette décennie de travail en victimologie générale, comparée et clinique s’enrichit des questionnements des étudiants de l’université Grenoble II et I, Montpellier I et III et des ONG (MDM, MSF, Handicap International, Afesip International) avec lesquelles nous avons collaboré. Les mutations de la mondialisation et la revendication scientifique des méthodologies quantitatives de type DSM nous apparaissent insuffisantes face aux complexités culturelles Victimologie 2.indd 9 23/07/08 10:55:43 10 Victimologie-Victimology et individuelles du psychisme humain non réductible à des variables secondaires ou parasites. La réduction des pathologies posttraumatiques au Post Traumatic Stress Disorder ou à l’Acute Stress Disorder aussi indispensable soit l’évaluation des signes cliniques, ne nous paraît pas répondre aux complexités des atteintes identitaires à l’œuvre dans le trauma individuel ou collectif, d’origine naturelle, accidentelle ou provoquée. La confusion entre stress et trauma est l’illustration de cette réduction sémiologique comme celle d’accorder à la rumination mentale posttraumatique une appréhension cognitive. L’affect ne peut se rabattre pas à l’émotion tout en produisant des effets d’affectation et non d’affection. Le délire n’est pas un trouble de la perception mais une distorsion du réel (néoréalité) par construction projective des imagos forclos. La rumination mentale véhicule des pensées mais bute sur leur organisation de cohérence sémantique et syntaxique non par troubles cognitifs mais par saturation des affects et défaillance de l’ordre symbolique. Le fading mental en est un autre exemple. La fuite panique, comme à l’opposé la sidération, ne relèvent pas de troubles de comportement mais de la sidération confuso-stuporeuse ou de la phobie paranoïde. Si la sémiologie permet les études nécessaires à la communicabilité des recherches et leurs études comparatives, la notion de preuve en sciences humaines cliniques ne peut se départir du qualificatif psychique si on émarge aux efficacités thérapeutiques auxquelles les psychopathologies victimaires nous convoquent. Les oppositions entre clinique objective et clinique subjective sont dépassées face aux evidentia des articulations entre production symptomatique et structure dynamique et économique du sujet. Les modélisations scientifiques répondent aux modélisations éthiques en produisant les débats stériles des endogamies ou des baronnies locales universitaires. Éthique et scientifique sont éminemment complémentaires en clinique et en psychopathologie. Les opposer dans une revendication arrogante d’exclusion relève des autoproclamations idéologiques dignes des régimes totalitaires. Notre ouverture vers les formes pluriculturelles du traumatisme permet de relativiser toutes modélisations aussi pertinentes et respectables soient-elles. La clinique de la torture, du terrorisme, du viol collectif, de l’éradication ethnique, des actes de barbarie, de la terreur, Victimologie 2.indd 10 23/07/08 10:55:43 Avant-propos 11 du trafic d’enfants ou d’organes, de la prise d’otages, etc., tendent à renouveler le débat clivé ou dépassé des localisations cérébrales ou de l’eugénisme. Cela n’enlève en rien la validité scientifique de nos collègues chercheurs ayant comme objet de recherche ce champ de travail. La recherche en clinique est autant quantitative que qualitative et non idéologique. Chacun sait, d’un point de vue sociohistorique, que l’intégration du PTSD dans le DSM est la résultante du lobbying des GI rescapés de l’horreur du Viêt-nam. Enfin, Les confusions entretenues entre organique et fonctionnel, cognitif, psychique et mental, thérapeutique et pédagogique inhibent les heuristiques nécessaires aux avancées de la connaissance. Les trois tomes de Victimologie-Victimology tentent avec humilité de contribuer aux questionnements nécessaires face aux pathologies traumatiques avec leurs parts d’inconnue et d’inachèvement. Cette décennie de travail fait émerger les thématiques suivantes non exhaustives. 1. La notion de psychose posttraumatique et les productions sémiologiques et syndromiques psychotiques sur des structures éminemment névrotiques. Cette proposition oblige à repenser les modèles psychodynamiques quant aux incursions de psychoses dans les structures névrotiques. Cela suppose aussi d’accorder aux systèmes pré et protoreprésentatif, notamment l’inscription pictographique et les conditions nécessaires de représentabilité de l’objet à la psyché, une attention plus soutenue. Il en va de nos efficacités thérapeutiques à l’égard des victimes. 2. Les pathologies traumatiques sexuelles, notamment celles issues de viol, comme non sexuelles mais identitaires. Elles concernent directement le démantèlement des enveloppements psychiques primaires et la constitution de l’unité somatopsychique. Ce démantèlement permet de penser le rapport entre trauma et contenant psychique ; trauma et névrose d’organe. Les études menées sur les inscriptions corporelles par l’école américaine de psychiatrie (embodyment memory) sollicitent le renouveau des apports princeps des recherches en épistémosomatique de P. Marty et de l’Institut de psychosomatique de Paris. Victimologie 2.indd 11 23/07/08 10:55:44 12 Victimologie-Victimology 3. La victimologie clinique, notamment les pathologies traumatiques extrêmes et sévères, souligne les impacts sur la déstructuration temporelle et spatiale avant toute préoccupation de réparation sexuelle. L’intérêt de ces impacts accorde à la temporalité une valeur essentielle de trame et de support à la représentation. La reviviscence traumatique illustre cet aspect essentiel. De même, la construction des espaces proximaux et distaux révèle toute son importance notamment dans la constante pathologique des espaces agglutinés et adhésifs du trauma. 4. Les interactions psychisme/culture se stigmatisent face aux évolutions des interculturalités de la postmodernité tant dans ces dimensions intranationales qu’internationales. Les cultures, considérées en logique de processus de lien social, interagissent avec le lien psychique et groupal. Les traumatismes liés aux guerres, aux enfants soldats, aux génocides, aux terrorismes et trafics d’êtres humains, etc., souvent en contexte pluriculturel ou en clinique humanitaire mettent à jour les rapports interactifs entre psychisme et culture. Mieux, ils soulignent combien structurellement, le psychique est conçu de culture et joue un rôle déterminant tant dans la construction du sujet que sa résilience. 5. Enfin, ces dix ans de recherche confirment la nécessité d’associer les études en victimologie avec celles en criminologie au même titre que la nécessité d’ouvertures interdisciplinaires face aux interactions du lien psychique et du lien social. La question des expertises médico-psychologiques et le double échec des experts psychologues et psychiatres dans l’affaire d’Outreau par exemple en France, montre l’urgence de penser les synergies entre épistémologie et discipline complémentaire. L’heuristique de la recherche est dans cet enjeu d’une pensée de la victimologie et de la victime en regard de la criminologie et du criminel ; l’un et l’autre prenant sens dans une victimogenèse et une criminogenèse. These three volumes represent the triptych of research on traumatic pathologies for over ten years. They are a synthesis of contributions by the international community in clinical social sciences. This book could not have been publicized without the support of the auto-conservation (Editors), the precious collaboration of Mrs. Myriam Linnell for the English translation, and of all the clinicians, therapists, and university lecturers in France (University Paris VII, Prof. M. Wolf-Fédida; University Montpellier III, Prof. J.P. Martineau; University Nice II, Prof. C. Miollan; University Toulouse III, Prof. O. Lescaret) and abroad (Cambodia Royal University, Prof. H. Nhong, Prof. S. Nhong; Buenos Aires University, Prof. D. Maldavsky, Dr. L. Alvarez; University of Liège, Prof. C. Mormont). The whole of this contribution is from a daily praxis on clinical victimology and post traumatic states. The credibility of a clinical and therapeutic praxis tends to argue the case for propositions of theories, methods and assessments of the practical corpus which forms its basis. Society mutations, due to post-modernity lead fundamental and applied research towards an internationalization of the works and their exchange, notably cross-cultural and humanitarian clinic. The decade of work on general, comparative, and clinical victimology has been enriched by the questioning of students from the University of Grenoble II and I, Montpellier I and III and the NGOs (MDM, MSF, Handicap International, Afesip International) with whom we have collaborated. The mutations of the internationalization and the scientific claim of quantitative methods such as the DSM appears inadequate to us considering the cultural and individual complexity of the human psyche that cannot be reduced to secondary variables or interference. No matter how relevant the assessment of clinical signs appears to be, post-traumatic pathologies being resumed by Post Traumatic Disorder Victimologie 2.indd 13 23/07/08 10:55:44 14 Victimologie-Victimology (PTSD) or Acute Stress Disorder are inadequate when considering the complexity of identity attacks in the individual or collective trauma, of a natural, accidental, or induced cause. The confusion between stress and trauma is a good illustration of this semiological restriction such as the granting of a cognitive apprehension to post-traumatic mental reflection. The affect cannot be linked to the emotions and produce effects of affectation and non affectation. Delusion is not a perception disorder but a distortion of reality (neo-reality) through the projective construction of foreclosed images. Mental reflection conveys thoughts but fails to organize semantic and syntax coherence not because of cognitive disorders but because of the saturation of affects and failure of the symbolic. Mental fading is another example. To flee in a panic, as well as being disturbed, is not the consequence of behavioral disorders but of confusion-stuporous sideration or paranoid phobia. Oppositions between objective clinic and subjective clinic are overwhelming in front of the evidence of articulations between symptomatic production and the dynamic and economic structure of the subject. Scientific modeling responds to ethical modeling by producing the sterile debates of endogamies or local universities. Ethics and science are, to a great extent, complementary in clinic and psychopathology. To oppose them by excluding them is arrogant and is equivalent to ideological self-proclamations encountered in totalitarian systems. Our opening on multi-cultural forms of the trauma allows relativizing any modeling, however pertinent and respectable it might be. The clinic of the torture, terrorism, collective rape, ethnic cleansing, barbaric acts, terror, children or organ trafficking, hostage taking, etc., tends to renew the cloven or overrun debate on cerebral localizations or eugenics. This does not diminish the scientific validity of our research colleagues who work in this field of research. The research in clinic is quantitative as well as qualitative and not ideological. It is common knowledge, from a social-historical point of view, that the integration of the PTSD in the DSM is due to the lobbying of GI’s rescued from the horror of Viêt-nam. Finally, the confusions maintained between organic and functional, cognitive, psychic and mental, therapeutic and pedagogic inhibits the necessary heuristics for greater knowledge. Victimologie 2.indd 14 23/07/08 10:55:44 Foward 15 The three volumes of Victimology attempt, with humility, to contribute towards the necessary questioning about incomplete and sometimes vague traumatic pathologies. Following non-exhaustive themes emerge from this decade of work: 1. The notion of post-traumatic psychosis and the psychotic semiological and syndromic productions on neurotic structures. This proposition compels us to rethink the psychodynamic models with the incursion of psychoses on neurotic structures. It also implies that we need to pay more attention to pre– and pro-representative systems, notably the pictographic inscription and the necessary conditions of representability of the object for the psyche. The efficiency of our therapy depends on it. 2. Sexual traumatic pathologies, notably those born from rape, not considered as sexual but considered as identity pathologies. Sexual traumatic pathologies directly concern the dismantling of primary psychic sheaths and the building of the somatic-psychic unity; trauma and organ neurosis. Results of studies on embodyment memory conducted by the American School of Psychiatry demonstrates that P. Marty’s (and the Institute of Psychosomatic in Paris, France) research on epistemosomatics is outdated. 3. Clinical victimology, particularly in the context of extreme and severe traumatic pathologies, takes into account the impacts on temporal and spatial destructuring before any sexuality repair. These impacts grant the temporality with an essential value of framework and support of the representation. Traumatic reviviscence illustrates this essential aspect. Also, the construction of proximal and distal takes all its importance in the pathological constant of the trauma agglutinated and adhesive spaces. 4. The interactions psyche/culture are stigmatized in the presence of the cross-cultural evolutions of post-modernity, as much in its national dimensions as international dimensions. Cultures interact with the psychical and groupal bonds. Traumatisms related to wars, children soldiers, genocides, terrorism, and humantrafficking, etc., often in a cross-cultural context or humanitarian clinic, disclose the interactive relationships between the psyche and culture. Victimologie 2.indd 15 23/07/08 10:55:44 16 Victimologie-Victimology Even better, they underline how, structurally, the psyche is made up of culture and plays an essential part as much in the construction of the subject as in his or her resilience. 5. Finally, ten years of research confirm the necessity to associate victimology and criminology studies, as well as cross-science openings in the presence of interactions between psychical bond and social bond. The question of medico-psychological expertise and the double failure of the psychologist and psychiatrist experts in the Outreau’s trial in France, demonstrates the urgency to re-think the synergies between epistemology and complementary sciences. The heuristic of the research is in this challenge of thinking of victimology and the victim with regard to criminology and the criminal; both making sense in a victimogenesis and a criminogenesis.
