This study analyses the immediate effects of a manipulation and Ischemic compression technique on pressure pain threshold in latent MTP’s in the upper trapezius muscle. Methods: 81 subjects were divided randomly into three groups: « manipulation», « ischemic compression» and control. The main criteria of inclusions were to establish both the presence of MTP in the trapezius muscle and the presence of somatic dysfunction at the C3/C4 level. The outcome measure was the pressure pain threshold on the MTP in the trapezius muscle detected using an algometer. This measurement was taken prior to, and then 1.5, and 15 minutes after treatment. ANOVA test, Bonferroni test and Kruskal-Wallis test were used to compare groups. Alpha risk was set at 0.05. Results: Variance Analysis showed a significant effect over the time period for the control (p=0.003), for the manipulation (p=0.0006) and for the ischemic compression (p=0.0353). Comparison between the groups revealed no significant difference. Conclusion: Manual techniques are thought to modify the pressure pain threshold of the MTP. Our results do not allow us to conclude that the benefits of ischemic compression and manipulation technique are any greater than those of the sham technique. Plusieurs techniques présentant un rapport risque/bénéfice différent sont utilisées. Notre étude a comparé les effets à court terme d’une manipulation et d’une compression ischémique sur le seuil de douleur à la pression du PG latent du faisceau supérieur du muscle trapèze. Méthodes : 81 sujets ont participé à cette étude, répartis aléatoirement en trois groupes : « manipulation », « compression ischémique » et «placebo». Les principaux critères d’inclusion étaient un PG du trapèze associé à une dysfonction cinétique C3/C4 homolatérale. Nous avons utilisé un algomètre pour mesurer le seuil de douleur à la pression du PG du trapèze avant, 1,5 et 15 minutes après technique. ANOVA, test de Bonferroni et test de Kruskal-Wallis ont été utilisés pour comparer les valeurs au risque alpha 0,05. Résultats : Nos résultats montrent un effet significatif dans le temps pour la manipulation (p=0,0006), la compression ischémique (p=0,0353) et le placebo (p=0,003). Il n’y a pas de différence significative entre chaque groupe sur le seuil de douleur à la pression. Conclusion : L’utilisation de techniques manuelles pourrait modifier favorablement le seuil de douleur à la pression des PG à court terme. Nos résultats ne permettent cependant pas de conclure à une différence versus placebo.
