Summary Toxoplasmosis is an infectious disease affecting both animals and humans, which is caused by the protozoan parasite Toxoplasma gondii (1). Acquired toxoplasmosis is usually asymptomatic and benign (2). In pregnant women, however, the infection acquires a special significance as the parasite may enter the fetal circulation through the placenta and cause congenital toxoplasmosis. Different studies have shown that the risk and severity of congenital toxoplasmosis is greatest when acquired during the first trimester of pregnancy. The consequences of congenital toxoplasmosis range from spontaneous abortion and prematurity to generalized and neurological symptoms, which often involve ocular complications. Infants with congenital toxoplasmosis may also remain asymptomatic for several weeks after birth (3-6). Toxoplasmosis can be life-threatening in immmunocompromised patients (AIDS, organ transplantation). In AIDS patients, acute toxoplasmosis usually occurs due to re-activation of a latent previously acquired infection, although primary infections are also diagnosed (6,7). Principle The toxocell latex reagent is a suspension of polystyrene latex particles of uniform size coated with soluble Toxoplasma gondii antigen. Latex particles allow visual observation of the antigenantibody reaction. If the reaction takes place, due to the presence of toxoplasma antibodies in the serum, the latex suspension changes its uniform appearance and a clear agglutination becomes evident. When the latex reagent is mixed with the serum, if the serum contains approximately more than 10 IU/ml of toxoplasma antibodies, a clear agglutination will appear. Results are expressed in IU/ml of total anti-toxoplasma antibodies based on the WHO 3rd International Standard. Components REF 3000-4525. - Latex reagent: 1 x 4 ml. PROCEDURE Quality control: Before performing a set of determinations it is advisable to test the reagent with each of the controls included in the kit, following the steps outlined in the section QUALITATIVE TEST. The reaction between the positive control and the reagent should show a clear agglutination different from the uniform appearance of the negative control. QUALITATIVE TEST - Allow the reagents to reach room temperature. Place 50 µl of the serum (or one drop of control) onto one section of the slide. Shake the reagent vial and add one drop of reagent next to the drop of sample. Mix both drops with a stirrer covering the whole surface of the slide section. Rotate the slide for 5 minutes manually or on a rotary shaker set at 80-100 rpm. Observe for presence or absence of agglutination. POSITIVE REACTIONS: Presence of agglutination. 3+ Large clumping with clear background. 2+ Moderate clumping with fluid slightly opaque in background. 1+ Small clumping with opaque fluid in background. NEGATIVE REACTIONS: Absence of agglutination, uniform suspension. Interpretation of the results A qualitative test performed on a single serum sample can be used to estimate the immunologic response of the individual. A negative reaction should be interpreted as the absence of toxoplasma antibodies or titers lower than 10 IU/ml. If the serum tested corresponds to a pregnant woman, it is advisable to repeat the test during the pregnancy period (once every month) to detect a possible seroconversion. A positive reaction should be interpreted as presence of toxoplasma antibodies which may reflect either a past infection or an acute toxoplasma infection. In this case it is recommended to perform the determination of the antibody titer by usual quantitative tests and investigate the presence of specific IgM toxoplasma antibodies. SEMIQUANTITATIVE TEST - Place 50 µl of normal saline onto slide sections 2 through 6. Using an automatic pipette, place 50 µl of the sample onto slide sections 1 and 2. Using the same pipette take in and release the sample and the saline solution on section 2 several times until they are well mixed. Take 50 µl of the mixture made on section 2 and transfer it to section 3. Repeat the aforementioned operations to obtain a thorough mixing of reagents, through section 6, thereafter discarding 50 µl. Um resultado positivo apenas indica uma exposição prévia. O título de anticorpos não pode ser utilizado para determinar uma infecção ativa ou o estado da doença. Para confirmar uma infecção aguda é necessário realizar testes IgM específicos. The approximate titer will correspond to the highest serum dilution that still presents a clearly visible agglutination. A positive result only indicates previous exposure; level of antibody may not be used to determine active infection or disease stage. To confirm acute infection specific IgM tests should be done. Limitações do procedimento Em certas ocasiões, soros fortemente positivos podem apresentar fenômenos de pró-zona. Por isso, quando se suspeita da existência de toxoplasmose e o resultado do exame é negativo, recomenda-se repetir o teste com uma diluição 1:5 do soro. Publicou-se que os testes de aglutinação de látex produzem falsos resultados positivos. Estes resultados estão associados com uma imunoglobulina M não específica. Valores previstos Diferentes estudos realizados demonstram que a incidência de anticorpos anti-toxoplasma depende da população estudada. Em um estudo realizado em 1962, os resultados relativos à presença de anticorpos variaram desde 3% em soldados dos Estados das Montanhas Rochosas (EUA) à quase 100% da população feminina adulta de Paris (9). Em um recente estudo multicêntrico realizado em Barcelona (Espanha) com mais de 16000 amostras de soro de mulheres grávidas, 28,6% das mulheres tinham anticorpos anti-toxoplasma. A presença de anticorpos IgM específicos se detectou em 2,22% das que tinham anticorpos antitoxoplasma (10). Características funcionais Se avaliou a sensibilidade do toxocell latex por comparação com um Imunoensaio Enzimático de Micropartículas (MEIA) comercializado para a detecção de IgG e IgM, usando um painel de 72 amostras positivas para anticorpos IgG, 52 amostras positivas para anticorpos IgG e IgM e 76 mostras negativas. A sensibilidade relativa foi de 100%, sendo que 124 amostras foram positivas com o reativo toxocell latex. A especificidade relativa foi de 84,2% (64/76). O valor prognóstico positivo para o reativo toxocell latex foi de 91,2% e o valor prognóstico negativo de 100%. Se realizou outro estudo com 215 amostras de soro de mulheres grávidas, conhecidas como positivas de IgG anti-toxoplasma pelo referido ensaio MEIA comercial, o qual revelou uma sensibilidade relativa de 100%, sendo que 41 amostras positivas foram positivas com o reativo látex. A especificidade relativa foi de 95,4% (166/174). O valor prognóstico dos testes positivos foi de 83,7% e o valor prognóstico negativo foi de 100%. A funcionalidade do toxocell latex foi comparada com a do Dye test como técnica de referência. A sensibilidade do reativo foi de 98,7%, a especificidade de 95,8%, o valor prognóstico positivo de 99,4% e o valor prognóstico negativo de 92,0% (11). Em um estudo realizado com amostras de soro de indivíduos transplantados, o reativo toxocell latex foi capaz de detectar uma elevação do título de anticorpos anti-toxoplasma em transplantados de coração que tinham experimentado uma infecção primária ou uma reativação de uma infecção já passada (12). Estudou-se um painel de 10 amostras de soro durante 3 dias consecutivos seguindo o teste semiquantitativo. Os resultados deste estudo indicam que a reproduzibilidade do toxocell latex é de 100%. Limitations of the procedure Occasionally a prozone effect may be observed with strong positive sera. Therefore, in those cases where a suspected case of toxoplasmosis gives a negative result, the test should be repeated using a 1:5 serum dilution. There has been reported that the latex agglutination tests produce some false positive reactions. These reactions are associated with an unspecified immunoglobulin M. Expected values Different studies show that the incidence of toxoplasma antibodies depends on the population studied. In a study carried out in 1962, the results of presence of antibodies raised from 3% on soldiers from Rocky Mountain States (USA) to nearly 100% of adult female population in Paris (9). In a recent multicenter study performed on more than 16,000 serum samples from pregnant women in Barcelona (Spain), 28.6% of the women had toxoplasma antibodies. The presence of specific IgM anti-toxoplasma was detected in 2.22% of those with toxoplasma antibodies (10). Performance characteristics The toxocell latex sensitivity was studied in comparison to commercially available IgG and IgM Microparticle Enzyme Immunoassays (MEIA) using a serum panel of 72 positive samples for IgG antibody, 52 positive samples for IgG and IgM anti-toxoplasma antibodies, and 76 negative samples. The relative sensitivity was 100%, all 124 samples were positive with the reagent. The relative specificity was 84.2% (64/76). The positive predictive value for toxocell latex was 91.2% and the negative predictive value was 100%. In a study performed with serum samples from transplant recipients, the toxocell latex was able to detect a rise in toxoplasma antibody titers in heart transplant recipients experiencing primary infection or reactivation of a past infection (12). A panel of 10 serum samples was tested on 3 consecutive days using the semiquantitative test. The results of the study indicate that toxocell latex reproducibility was 100%. La toxoplasmosis en su forma adquirida es una infección frecuente, generalmente asintomática y benigna (2). Sin embargo, adquiere especial importancia en caso de ocurrir en la mujer embarazada, pudiendo entonces el parásito alcanzar la circulación fetal a través de la placenta y causar toxoplasmosis congénita. Diferentes estudios realizados muestran que el riesgo y severidad de la toxoplasmosis congénita son mayores cuando la infección aguda sucede durante el primer trimestre del embarazo. Las consecuencias de la toxoplasmosis congénita van desde el aborto espontáneo y prematuridad hasta lesiones viscerales y neurológicas, siendo especialmente frecuentes las complicaciones oculares. Si debido a la presencia de anticuerpos antitoxoplasma en el suero tiene lugar dicha reacción, la suspensión de látex pierde su aspecto uniforme haciéndose evidente una clara aglutinación. Cuando se mezcla el reactivo látex con el suero, si el suero tiene aproximadamente más de 10 UI/ml de anticuerpos antixoplasma se produce una clara aglutinación. As amostras podem ser conservadas por 2 dias entre 2-8°C. Para guardar por um período de tempo mais longo as amostras devem ser congeladas (-20°C). PROCEDIMENTO Controle de qualidade: Antes de efetuar uma série de determinações é aconselhável testar o reativo latex com cada um dos controles incluídos no kit. Os controles devem ser testados utilizando-se o TESTE QUALITATIVO. A reação entre o controle positivo e o reativo deve dar uma clara aglutinação, diferente da aparência uniforme do controle negativo. Se estes resultados não são obtidos, não utilize o kit. Para assegurar uma correta dosagem do reativo o conta-gotas deve ser colocado verticalmente e deixar cair apenas uma gota. TESTE QUALITATIVO - Deixar que os reativos alcancem a temperatura ambiente. Depositar 50 µl de soro (ou uma gota de controle) em uma das seções da lâmina. Agitar o frasco do reativo e colocar uma gota de reativo junto à gota de amostra. Misturar ambas gotas com um palito, cobrindo-se toda a superfície da seção da lâmina. Agitar a lâmina manualmente ou com um agitador rotatório (80-100 rpm) durante 5 minutos. Observar a presença ou ausência de aglutinação. REAÇÕES POSITIVAS: Presença de aglutinação. 3+ Agregados grandes sobre fundo transparente. 2+ Agregados médios sobre fundo levemente opaco. 1+ Agregados finos sobre fundo opaco. Todo el material de origen humano utilizado en la preparación de este producto ha sido encontrado negativo a la presencia de anticuerpos de los virus HIV-1/HIV-2 y HCV, así como a la del antígeno de superficie de la hepatitis B, utilizando un método aprobado por la Food and Drug Administration (USA). ATENCIÓN: MATERIAL DE RIESGO BIOLÓGICO. Ya que ningún método puede ofrecer la total seguridad de la ausencia de agentes infecciosos, este producto debe manejarse con precaución (8). Interpretação dos resultados Um teste qualitativo realizado com uma única amostra de soro pode ser utilizado para avaliar o estado imunológico do indivíduo. Uma reação negativa indica a ausência de anticorpos anti-toxoplasma ou níveis inferiores a 10 UI/ml. Se o soro é de uma mulher grávida, recomenda-se repetir o exame periodicamente durante o período de gravidez (uma vez por mês) a fim de detectar uma possível soroconversão. Uma reação positiva indica a existência de anticorpos anti-toxoplasma, os quais podem denotar tanto uma infecção passada como uma toxoplasmose aguda. Neste caso, convém determinar o título de anticorpos com as técnicas quantitativas habituais, além de investigar a presença de IgM específica anti-toxoplasma. TESTE SEMI-QUANTITATIVO - Colocar 50 µl de solução salina em cada uma das seções de 2 a 6 da lâmina. Com uma pipeta automática, depositar 50 µl de soro nas seções 1 e 2 da lâmina. Com a mesma pipeta aspirar e expulsar várias vezes o soro e a solução salina contidos na seção 2 até conseguir misturá-los bem. Tomar 50 µl da mistura obtida na seção 2 e transferir para a seção 3. Realizar as mesmas operações descritas anteriormente para conseguir a mistura correta dos reativos sucessivamente até a seção 6 e então descartar 50 µl. A toxoplasmose em sua forma adquirida é uma infecção freqüente, geralmente benigna e assintomática (2). No entanto, esta doença tem mais relevância quando atinge mulheres grávidas, dado que o parasita pode alcançar a circulação fetal através da placenta e causar toxoplasmose congênita. Os estudos realizados demonstram que a presença de infecção aguda durante o primeiro trimestre da gravidez aumenta a probabilidade de que se originem lesões fetais, cujo risco e gravidade diminuem à medida que a gravidez avança. Bebês com toxoplasmose congênita podem permanecer assintomáticos por várias semanas após o nascimento (3-6). A toxoplasmose pode ser uma seria ameaça em pacientes imunodeprimidos (AIDS, transplante de órgãos). Nos pacientes com AIDS, a toxoplasmose aguda costuma ocorrer devido a uma reativação de uma infecção latente previamente adquirida, embora também se tenham diagnosticado infecções primárias (6,7). Todos os materiais de origem humana utilizados na preparação deste produto foram testados e apresentaram resultados negativos em relação a presença de anticorpos contra os vírus HIV1/HIV-2 e HCV, assim como para o antígeno de superfície da hepatite B, utilizando um método aprovado pela Food and Drug Administration (USA). ATENÇÃO: MATERIAL DE RISCO BIOLÓGICO. Dado que nenhum método pode oferecer a total segurança da ausência de agentes infecciosos, este produto deve ser manipulado com precaução (8). Agitar el vial del reactivo y añadir una gota del reactivo junto a la gota de muestra. Mezclar ambas gotas con un palillo cubriendo toda la superficie de la sección del portaobjetos. Agitar el portaobjetos manualmente o en un agitador rotatorio (80-100 rpm) durante 5 minutos. Observar la presencia o ausencia de aglutinación. Interpretación de los resultados El test cualitativo realizado con una única muestra de suero puede utilizarse para la valoración del estado inmunológico del individuo. Una reacción negativa indica la ausencia de anticuerpos antitoxoplasma o niveles inferiores a 10 UI/ml. Si el suero es de una mujer embarazada, se aconseja repetir el análisis periódicamente durante el embarazo (una vez por mes) para detectar una posible seroconversión. Una reacción positiva indica la presencia de anticuerpos antitoxoplasma que pueden reflejar tanto una infección antigua como como una toxoplasmosis aguda. En este caso se recomienda proceder a la determinación del título de anticuerpos antitoxoplasma con las técnicas cuantitativas habituales, así como investigar la presencia de IgM específicas antitoxoplasma. TEST SEMICUANTITATIVO - Dosificar 50 µl de solución salina en cada una de las secciones 2 a 6 del portaobjetos. Dosificar con una pipeta automática 50 µl de suero en las secciones 1 y 2 del portaobjetos. Con la misma pipeta aspirar y expulsar varias veces la muestra y la solución salina contenidas en la sección 2 hasta lograr una buena mezcla. Tomar 50 µl de la mezcla realizada en la sección 2 y transferirlos a la sección 3. Realizar las mismas operaciones descritas anteriormente para lograr la mezcla correcta de los reactivos hasta la sección 6 y desechar entonces 50 µl. Un resultado positivo sólo indica exposición previa; el título de anticuerpos no puede utilizarse para determinar una infección activa o el estado de la enfermedad. Para confirmar una infección aguda deben realizarse tests IgM específicos. In ihrer erworbenen Form ist die Toxoplasmose eine häufig vorkommende Krankheit, die in der Regel symptomfrei und gutartig verläuft (2). Bei schwangeren Frauen kann sie jedoch schwerwiegende Folgen haben, da der Parasit über die Plazenta den fetalen Blutkreislauf erreichen und eine konnatale Toxoplasmose hervorrufen kann. Verschiedene Studien belegen, dass das Risiko und die Schwere der konnatalen Toxoplasmose größer ist, wenn die Infektion in den ersten drei Monaten der Schwangerschaft stattfindet. Die Folgen der konnatalen Toxoplasmose können Fehl- und Frühgeburten bis hin zu viszeralen und neurologischen Läsionen sein, und besonders häufig treten okulare Komplikationen auf. Bei Kindern mit konnataler Toxoplasmose kann es auch vorkommen, dass die ersten Wochen nach der Geburt (3-6) symptomfrei verlaufen. Die Toxoplasmose kann eine ernsthafte Bedrohung für immundepressive Patienten (AIDS, Organtransplantationen) darstellen. Bei den AIDS-Patienten ist die Reaktivierung einer latenten, zurückliegenden Infektion häufig die Ursache einer akuten Toxoplasmose, wobei aber auch primäre Infektionen diagnostiziert worden sind (6,7). Gli studi svolti mostrano che l’infezione acuta nel primo trimestre di gestazione causa il maggior pericolo di lesioni fetali. Le conseguenze della toxoplasmosi congenita vanno dall’aborto spontaneo e dalla prematurità alle lesioni viscerali e neurologiche gravi nel feto, particolarmente frequenti quelle oculari. I neonati con toxoplasmosi congenita possono rimanere asintomatici nelle prime settimane dopo il parto (3-6). La toxoplasmosi può rappresentare un grave pericolo per pazienti immunodepressi (AIDS, trapianti di organi). Nel pazienti affetti da AIDS la toxoplasmosi acuta suole manifestarsi per effetto della riattivazione di una infezione latente previamente acquisita, ma ci sono anche casi di infezioni primarie (6,7). Un résultat positif indique seulement une exposition préalable ; le titre d’anticorps ne peut servir à déterminer une infection active ni le stade de la maladie. Pour confirmer une infection aiguë, il faut faire des tests IgM spécifiques. Limites de la procédure Certains sérums fortement positifs peuvent présenter des phénomènes de zone. Pour cette raison, si l’on soupçonne une toxoplasmose et si le résultat a été négatif, refaire le test avec une dilution 1:5 du sérum. D’après certains travaux publiés, les tests d’agglutination de latex produisent des résultats faussement positifs. Ces résultats sont associés à une immunoglobuline M non spécifique. Valeurs attendues Plusieurs études indiquent que l’incidence des anticorps antitoxoplasme dépend de la population étudiée. Dans une étude réalisée en 1962, les résultats de la présence d’anticorps ont oscillé de 3% chez des soldats des États des Montagnes rocheuses (USA) à près de 100% sur la population féminine adulte de Paris (9). Dans une étude multicentrale réalisée récemment à Barcelone (Espagne) sur plus de 16 000 échantillons de sérum de femmes enceintes, 28,6% présentaient des anticorps antitoxoplasme et sur 2,22% de ces femmes, la présence d’anticorps IgM spécifiques a été détectée (10). Caractéristiques fonctionnelles La sensibilité du toxocell latex a été comparée à celle d’un enzymo-immunotest de microparticules (MEIA) commercialisé pour la détection des IgG et des IgM, en utilisant un panel de 72 échantillons positifs aux anticorps IgG, 52 échantillons positifs aux anticorps IgG et IgM et 76 échantillons négatifs. La sensibilité relative a été de 100%, les 124 échantillons ont donné positif avec le réactif toxocell latex. La spécificité relative a été de 84,2% (64/76). La valeur prédictive positive pour le réactif toxocell latex a été de 91,2% et la valeur prédictive négative de 100%. Une autre étude a porté sur 215 échantillons de sérum de femmes enceintes, dont la positivité aux IgG antitoxoplasme avait été déterminée par le même test MEIA commercial. La sensibilité relative a été de 100%, les 41 échantillons positifs ont donné positif avec le réactif latex. La spécificité relative a été de 95,4% (166/174). La valeur prédictive des résultats positifs a été de 83,7% et la valeur prédictive négative de 100%. La fonctionnalité du toxocell latex a été comparée à celle du Dye test qui a servi de technique de référence. La sensibilité du réactif a été de 98,7%, la spécificité de 95,8%, la valeur prédictive positive de 99,4% et la valeur prédictive négative de 92,0% (11). Dans une étude portant sur des échantillons de sérum de sujets greffés, le réactif toxocell latex a été capable de détecter une augmentation du titre d’anticorps antitoxoplasme chez des patients qui, après une greffe du cœur, avaient subi une infection primaire ou la réactivation d’une infection ancienne (12). Un panel de 10 échantillons de sérum a été analysé pendant 3 jours consécutifs en appliquant le test semi-quantitatif. Les résultats de cette étude indiquent une reproductibilité de 100% pour le toxocell latex. Ein positives Ergebnis ist lediglich ein Hinweis auf eine zurückliegende Infektion. Der AntikörperTiter kann nicht verwendet werden, um eine aktive Infektion oder den Krankheitszustand zu bestimmen. Zur Bestätigung einer akuten Infektion sind spezifische IgM-Tests durchzuführen. Begrenzung der Methode In einigen Fällen kann bei stark positiven Seren ein Prozoneneffekt auftreten. Deshalb sollte der Test, wenn der Verdacht auf Toxoplasmose besteht und das Ergebnis negativ ausgefallen ist, mit einer 1:5 Verdünnung des Serums wiederholt werden. Es sind Studien veröffentlicht worden, die von falschen positiven Ergebnissen bei LatexAgglutinationstests berichten. Diese Ergebnisse sind auf einen unspezifischen Immunoblobulin M zurückzuführen. Erwartete Werte Verschiedene Studien haben gezeigt, dass die Inzidenz von Anti-Toxoplasma-Antikörpern populationsabhängig ist. In einer 1962 durchgeführten Studie schwankten die Ergebnisse bezüglich der Anwesenheit von Antikörpern zwischen 3% bei Soldaten der Rocky MountainStaaten (USA) und annähernd 100% in der erwachsenen femininen Bevölkerung von Paris (9). In einer kürzlich in Barcelona (Spanien) durchgeführten multizentrischen Studie an über 16.000 Serumproben schwangerer Frauen, hatten 28,6% Anti-Toxoplasma-Antikörper. Sommaire Matériel nécessaire non fourni La toxoplasmose est une maladie infectieuse causée par le parasite protozoaire Toxoplasma gondii (1) affectant les animaux et les humains. La toxoplasmose acquise est une infection fréquente, souvent asymptomatique et bénigne (2). Chez les femmes enceintes, cependant, le parasite peut passer dans la circulation du foetus via le placenta et causer une toxoplasmose congénitale. Différentes études réalisées ont montré que le risque et la sévérité de la toxoplasmose congénitale sont plus importants quand l’infection a lieu dans le premier trimestre de la grossesse. Les conséquences d’une toxoplasmose congénitale vont de l’avortement spontané ou de la prématurité aux lésions viscérales et aux symptômes neurologiques, qui impliquent souvent des complications oculaires. Les nouveau-nés atteints par la toxoplasmose congénitale peuvent rester asymptomatiques pendant plusieurs semaines après la naissance (3-6). La toxoplasmose peut constituer un risque sérieux pour les patients immunodéprimés (SIDA, greffe d’organes). Chez les personnes atteintes du SIDA, la toxoplasmose aiguë est généralement la réactivation d’une infection latente acquise auparavant, bien que des infections primaires aient également été diagnostiquées (6,7). Solution saline (0,9% NaCl uniquement pour le test semiquantitatif), pipettes automatiques, agitateur rotatif, bâtonnets et chronomètre. Principe Le réactif toxocell latex est une suspension de particules de latex de polystyrène de dimension uniforme sensibilisées avec l’antigène solubilisé du Toxoplasma gondii. Les particules de latex permettent une observation visuelle de la réaction antigène-anticorps. Si la réaction a lieu, à cause de la présence des anticorps toxoplasmiques, la suspension de latex change son apparence uniforme et une agglutination claire devient évidente. Quand on mélange le réactif latex avec le sérum, si celui-ci contient plus de 10 UI/ml d’anticorps toxoplasmiques, une agglutination nette apparaît. Les résultats sont exprimés en UI/ml d’anticorps toxoplasmiques totaux par rapport au 3e Standard International de l’OMS. REF 3000-4525. - Utiliser du sérum frais. Les échantillons peuvent être conservés à 2-8°C pendant 2 jours. Pour des périodes plus longues, les échantillons doivent être congelés (-20°C). PROCÉDURE Contrôle de qualité: Avant de réaliser une série de déterminations, tester le réactif latex avec chaque contrôle fourni dans le coffret. Les deux contrôles seront utilisés selon les étapes décrites dans le TEST QUALITATIF. La réaction entre le contrôle positif et le réactif doit donner une agglutination nette différente de l’apparence uniforme du contrôle négatif. Si les résultats attendus ne sont pas obtenus, ne pas utiliser le coffret. Pour assurer une bonne distribution, tenir le compte-gouttes de réactif en position verticale et verser une seule goutte. TEST QUALITATIF - Composants - Prélèvement de l’échantillon Réactif latex: 1 x 4 ml. Suspension de particules de latex de polystyrène sensibilisées avec l’antigène toxoplasmique dans un tampon avec albumine sérique bovine. Contient < 0,1% d’azide sodique. Contrôle positif: 1 x 1 ml. Sérum humain dilué avec IgG de lapin antitoxoplasme. Contient < 0,1% d’azide sodique. Contrôle négatif: 1 x 1 ml. Sérum humain négatif dilué. Contient < 0,1% d’azide sodique. Lames jetables: 18 unités (x6). Précautions toxocell latex est destiné à un usage diagnostique IN VITRO. L’azide sodique peut réagir au contact de tuyauteries et de bouches d’écoulement des eaux en plomb ou en cuivre. Cette réaction donne lieu à des azides métalliques hautement explosives. Lorsque vous jetez les restes de réactifs, veuillez laisser couler l’eau abondamment. Tout le matériel d’origine humaine utilisé dans le cadre de la préparation de ce produit a donné des résultats négatifs quant à la présence d’anticorps aux virus HIV-1/HIV-2 et HCV, de même qu’à celle de l’antigène de surface de l’hépatite B. La méthode utilisée est agréée par la Food and Drug Administration (USA). ATTENTION: MATÉRIEL À RISQUE BIOLOGIQUE. Compte tenu du fait qu’aucune méthode n’est capable d’offrir une garantie absolue de l’absence d’agents infectieux, ce produit doit être manipulé avec précaution (8). Déposer 50 µl de l´échantillon (ou une goutte de contrôle) sur une des sections de la lame. Agiter le flacon du réactif et ajouter une goutte de réactif à côté de la goutte d’échantillon. Mélanger les deux gouttes avec un bâtonnet en recouvrant toute la surface de la section de la lame. Agiter la lame avec un léger mouvement de rotation, manuellement ou à l’aide d’un agitateur rotatif (80-100 tpm) pendant 5 minutes. Vérifier la présence ou l’absence d’agglutination. RÉACTIONS POSITIVES: Présence d’agglutination. 3+ Grands agrégats sur fond transparent. 2+ Agrégats modérés sur fond légèrement opaque. 1+ Agrégats fins sur fond opaque. RÉACTIONS NÉGATIVES: Absence d’agglutination, suspension uniforme. Interprétation des résultats Le test qualitatif réalisé sur un seul échantillon de sérum peut servir à évaluer l’état immunologique du patient. Une réaction négative indique l’absence d’anticorps antitoxoplasme ou des niveaux inférieurs à 10 UI/ml. Dans le cas d’une femme enceinte, il est conseillé de refaire l’analyse sérologique périodiquement (une fois par mois) pour détecter une éventuelle séroconversion. Une réaction positive indique la présence d’anticorps antitoxoplasme, soit par une infection ancienne, soit par une toxoplasmose aiguë. Dans ce cas-là il est recommandé de procéder à une détermination du titre d’anticorps antitoxoplasme à l’aide des techniques quantitatives habituelles, et de rechercher la présence d’IgM spécifiques antitoxoplasme. TEST SEMIQUANTITATIF Placer 50 µl de solution saline sur les sections 2 à 6 de la lame. Déposer, à l’aide d’une pipette automatique, 50 µl de sérum sur les sections 1 et 2 de la lame. Avec la même pipette mélanger l’échantillon et la solution saline de la section 2 plusieurs fois. Prélever 50 µl de la dilution obtenue dans la section 2 et les transférer dans la section 3. Répéter l’étape antérieure et suivre de suite jusqu’à la section 6, prélever 50 µl et les jeter. Analyser chaque dilution selon il est décrit dans la section TEST QUALITATIF. 11
