Comparison of long-term functional outcomes following colorectal resection or conservative rectal surgery for deep endometriosis infiltrating the rectum Abstract ID : 1107 Soumis par : Basma Darwish Le 2016-01-31 21:33:36 Nom de la catégorie : SEUD CONGRESS Typologie : Communication orale / Oral communication Statut : validé Autorisation de diffusion : Yes/Oui ------------------------------------Introduction and objective: To compare long-term digestive outcomes in women managed by either conservative rectal surgery or colorectal resection for deep endometriosis infiltrating the rectum Methods: A retrospective comparative study including 74 patients managed by surgery for deep endometriosis infiltrating the rectum whose follow up ranged from 5-10 years. 49 patients received conservative rectal surgery (shaving in 46 and disc excision in 3) and 25 had colorectal resection.. Evaluation included standardized GI symptoms: the GIQLI, KESS , the bristol stool score and the wexner score for anal continence. Results: Patients managed by conservative rectal surgery had better overall KESS score, Wexner score and GIQLI values. In regards to KESS score, significant differences were revealed in 9 out of 11 items; and better values were invariably asscociated with conservative rectal surgery and indicated a lower rate for postoperative constipation. Statistically significant differences were not revealed for either pelvic pain or fertility outcomes. Rectal recurrences occured in 8% managed by conservative surgery of which 4% received colorectal resection and 4% shaving. Patients managed by conservative surgery were significantly more satisfied by their postoperative general health, and were more likely to recommend surgery to a friend suffering from the disease. Discussion and Conclusion: Our data suggests that performing conservative rectal surgery leads to better functional digestive otcomes in women presenting with rectal endometriosis. ------------------------------------Mots clefs : rectal endometriosis; colorectal resection; shaving; disc excision; colorectal endometriosis Auteurs : Références : , , , Auteurs Basma DARWISH 1, Horace Roman 1, Mathilde Milles 1, JJ Tuech 2, Emmanuel Huet 2, 1.
