Social Health Insurance
Background
Delivered annually by the International Training Centre
of the ILO, this two week course has evolved constantly
throughout the last ten years. Knowledge transfer on
changes, reforms and strategies is constantly adapted
to reflect the latest issues in the management of social
insurance schemes in order to provide timely access to
quality health services aiming towards universal
coverage of the population in each country.
At national level, social health insurance contributes to
the improvement of the national health status, wider
access to health care and sound financing of health
services. In many countries, under-funding and bad
management of health services lead to dissatisfaction
with the quality and quantity of the health services
provided by public systems. In other countries, user
fees are a major impediment for accessing health care.
Social health insurance schemes are good responses to
such issues.
On an international level, the UN Social Protection
Floor Initiative promoted since 2009 at country level,
aims at providing minimum access to essential services
and transfers to the population in need of such
protection. Universal access to health services being a
key component of the social protection initiative, this
course also contributes to the capacity building
component of the social protection floors being
developed at national levels.
The course will review the importance of social health
insurance in crisis and post crisis situations, issues
related to the feasibility of introducing a scheme and
planning for implementation; designing the appropriate
contribution levels and benefits, monitoring the
provision of services and ensuring the sound
governance and financing of the scheme.
ITC courses are well documented and participants can
share with their institutions what they have learned.

The course is aimed at executives, managers, planners,
financial officers and other responsible professionals of
health care and health insurance schemes, both in
government and non-governmental sectors. The course
primarily addresses the needs of practitioners &
policy-makers; however, persons in research functions
within health systems also benefit from participation in
this training activity.
Structure and contents
As part of the course, participants will engage in a
comparative analysis of different national situations and
in discussions on the options for health system reform.
This analysis and discussion will be based on case
studies of recent experiences in selected countries that
have undertaken such reforms. Special attention will
be devoted to financial aspects such as financial
viability, controlling finances, provider payments,
contributions and their impact, as well as experiences
in formal and informal economies.

Participation in the course costs 3,250 Euros (course
fees 1,570 Euros and participant subsistence 1,680
Euros), excluding international air travel. This is
payable in advance by the participant's sponsoring
organization. This covers: tuition fees; the use of
training facilities and support services; training
materials and books; accommodation and full board at
the Centre's campus; a lump sum for incidental
expenses; emergency medical care and insurance.

Contexte
Le Centre international de formation de l’OIT propose
ce cours de deux semaines depuis plus de dix ans.
Cette année, nous sommes heureux d’offrir le cours
pour la première fois en français et en anglais. Le
transfert de connaissances sur les changements, les
réformes et les nouvelles stratégies est revu chaque
année afin d’y incorporer les dernières questions sur la
gestion des régimes d’assurance maladie sociale, et ce
afin d’aider les diverses institutions à offrir un meilleur
accès à des soins de santé de qualité et une couverture
universelle des populations dans chaque pays.
Au niveau national, l’assurance maladie sociale
contribue à un meilleur statut en matière de santé, à
un plus grand accès aux soins de santé et à un
financement adéquat des services de santé. Dans
plusieurs pays, le manque de financement et la
mauvaise gestion ont suscité l’insatisfaction quant à la
qualité et à la quantité des services publics de santé.
Ailleurs, les frais facturés aux utilisateurs sont un
obstacle à l’accès aux soins. Les régimes d’assurance
maladie sociale apportent des réponses à ces
questions.
Au niveau international, l’initiative des Nations Unies
sur le socle de protection sociale vise à fournir un accès
minimum aux services et transferts sociaux aux
populations requérant une telle protection. L’accès
universel aux soins de santé étant une composante
importante de l’initiative sur le socle de protection
sociale, ce cours contribue au renforcement des
capacités au sein de cette composante pour les socles
de protection sociale développés au niveau national.
Ce cours se penchera sur l’importance de l’assurance
maladie sociale dans les situations de crise et
d’après-crise, les questions reliées à la faisabilité d’un
nouveau régime, la conception des contributions et des
prestations adéquates, le contrôle de l’offre de services,
ainsi que la bonne gouvernance et le financement des
régimes.
Les cours du CIF-OIT sont bien documentés et les
participants reçoivent la documentation nécessaire pour
partager les nouvelles connaissances acquises avec
leurs institutions.
Objectifs
Le cours a trois objectifs:

planification et autres professionnels de la santé et de
l’assurance santé dans les secteurs publics et non
gouvernementaux. Le cours aborde en premier lieu les
besoins des praticiens et des personnes qui élaborent
des politiques sur ces questions; cependant, les
professionnels qui font de la recherche dans le domaine
des systèmes de santé seront eux aussi intéressés par
cette activité de formation.

Pendant le cours, les participants seront
exposés à une analyse comparative des différentes
situations au niveau national et des options disponibles
pour une réforme du système de santé. Cette analyse et
cette discussion se feront au moyen d’études de cas sur
des expériences récentes dans certains pays qui ont
entrepris des réformes. Une attention particulière sera
accordée aux aspects financiers tels que la viabilité
financière, le contrôle financier, les paiements aux
fournisseurs de services, les contributions, l’impact,
ainsi que les expériences dans les économies formelle
et informelle.

Méthodologie
Le CIF adopte une approche en trois étapes:
