Fall 2016 French Visa Application Checklist for Citizens of ALL Countries Submission Deadline of ALL materials: April 18-22, 2016 1. ONE ORIGINAL, COMPLETED, SIGNED Visa Application Form The completed visa application form must contain no mistakes, errors, cross-outs, or white-out. Be sure to use European date format (DD/MM/YYYY, so March 5, 1998 would be 05/03/1998). Leave questions on the visa application blank if they don’t apply to you. Review Johnny Thunderbird’s Sample Visa Application (posted on the “Apply for a Visa Page”) and use it as a guide to complete your visa application. Use the Visa Application Form With Pre-Filled Answers (posted on the “Apply for a Visa Page” o But, if any of the pre-filled answers do not apply to you, e.g., the intended date of entry, you will need to complete the blank visa application, and copy the rest of the pre-filled answers onto your visa application form! Sign and date the visa application at the bottom of page 2 of the visa application form. See page 3 of this Visa Application Checklist for more tips on completing your visa application form! 2. ONE ORIGINAL & SIGNED, Passport + ONE COPY of Bio Page (picture and signature page) Your Passport must (1) be valid until at least June 16, 2017, (2) have been issued in 2007 or later, and (3) include at least 2 blank pages (or more if you plan to travel!) Canadian and Australian Citizens: o In addition to providing your original, signed Passport and bio page copy, you need to provide ONE COPY of your Proof of Legal Residence in the United States: for permanent residents, a Green Card (valid until at least June 16, 2017); for international students, a U.S. visa (valid until at least August 2017) and a recently signed Form I-20 (valid through date of SJU graduation). Citizens of all countries other than the U.S., Canada and Australia: o In addition to providing your original, signed Passport and bio page copy, you need to provide ONE COPY of your Proof of Legal Residence in the United States: for permanent residents, a Green Card (valid until at least June 16, 2017); for international students, a U.S. visa (valid until at least August 2017) and a recently signed Form I-20 (valid through date of SJU graduation). o *This semester you will be able to participate in the OGS Group Visa Process, but will need to travel with an OGS Administrator to the French Consulate in NYC to submit your visa application. For now, plan to spend a day at the French Consulate in NYC in early June 2016 (OGS will email you with more details). 3. TWO Recent Passport Photos Standard passport photo format (1.5” square inch in size) Taken within the last 6 months You cannot be smiling (and your lips must be closed) The picture must be front facing, in color, on a white background, and on photo-quality paper. Please note you cannot have your hair covering your eyes, and you cannot be wearing glasses, a scarf, a hat or other head covering in these photos. (Please contact the Office of Global Studies if you wear a head covering for religious reasons.) This does not need to be the same photo included in your passport. 1 (Updated: February 16, 2016) 4. ONE COPY of Flight Itinerary or Flight Reservation (1) Print out your complete, round-trip flight itinerary or ticket PDF; or (2) Print out your flight reservation showing your date of departure from the U.S. to France; or (3) Provide a handwritten and legible statement indicating your intended date of departure for France, as well as a formal commitment not to depart before that date, using the following language and format: o First Name Last Name, Passport Number o I, First Name Last Name, will fly from New York (or other relevant U.S. city) to Paris, France (or other relevant city where you will arrive in Europe) arriving on 23 August 2016 (or other relevant date of arrival). I will depart from Europe and return to the U.S. on 16 December 2016 (or other relevant date of departure). o Signature, Date of Signature in European Date format (DD/MM/YYYY) 5. ONE COPY of Proof of Financial Support (1) Student Bank Statement: A recent (within the past 30 days, e.g. March2016) bank statement verifying at least $3,280 in the student’s personal checking and/or savings account (the student is the only person listed on the bank account); or (2) IF someone else is supporting you financially, you must include: o (A) Guarantor Bank Statement: A recent (within the past 30 days, e.g. March 2016) bank statement verifying at least $3,280 in the guarantor’s personal checking and/or savings account (the guarantor is your family member or friend who will support you financially); AND o (B) A completed Financial Guarantee Form. This form must be notarized. If the guarantor’s bank statement is a joint account held with another individual, the Financial Guarantee Form only needs to be completed by one person whose name is listed on the bank account. (3) St. John’s University Office of Student Financial Services Statement that the student has a credit of at least $3,280 that will be dispersed after tuition and fees are covered; or (4) Any combination of (1), (2), or (3) that totals $3,280. Bank Statement Tips! o The bank statement cannot be a screenshot from your bank’s website; o However, if you receive your monthly bank statement electronically in PDF format, a printout of this PDF is acceptable. The print-out from the Internet will not be accepted by the French Consulate unless the name of the bank and the name of the account holder are clearly stated. o All financial support documents may be submitted as a copy (e.g., the original Financial Guarantee Form is not needed, so your family member may e-mail you a scanned copy – they do not need to send the original by regular mail). o What does it mean when it says the Financial Guarantee Form must be notarized? When a document is notarized, it means that a Notary Public has watched you sign the document, and puts a stamp on it verifying that your signature is authentic. The Notary cannot be someone you’re related to. Most bank branches have a notary, and will provide their services at no charge if you’re a bank customer. 2 (Updated: February 16, 2016) 6. ONE COPY of Campus France Confirmation Email Students are strongly encouraged to attend a Campus France Workshop for an OGS Administrator to guide you through the Campus France process o Monday, April 4, 2016 (2:00pm-3:15pm) in DAC 206, or o Thursday, April 14, 2016 (2:00pm-3:15pm) in Sullivan 207 (*makeup) Note: you will not receive the Campus France Confirmation Email prior to the visa deadline, OGS will email you with a deadline to submit the Campus France Confirmation Email 7. ONE ORIGINAL French Immigration (OFII) Form: All students participating in the Discover France: Paris and Discover France: Biology and Chemistry semester programs need to complete the OFII Form See the “OFII Instructions for Paris Semester Students Only” on page 5 of this Visa Application Checklist for instructions. Tips on Filling Out the Visa Application Form! #7 refers to citizenship: if you have a U.S. passport, put “USA” #13 refers to your passport number #16 asks for the country your passport is “issued by,” put “USA” if you have a U.S. passport #17: Address you live at while attending school; it must be in New York, New Jersey, Connecticut, or Bermuda. o Remember to use the same address for your Campus France application #25: if you are participating in the home stay option, you will need to list the home stay address here #29: Box 1: should say “Cash and Credit Card.” Box 2: you do not have to disclose the details of scholarship information. #30 only applies if you are relying on family (or someone else) living in France for financial support NOTE: This Visa Application Checklist is only for students applying for a French visa through the St. John’s University Office of Global Studies Group Visa Process. If you are applying for a visa on your own, you (1) should have already scheduled your visa appointment with the French Consulate, (2) must contact the Office of Global Studies to request the required verification letters, (3) must include a copy of your StormCard with your visa application, and (4) should review the Independent Visa Application Guide (posted on the “Apply for a Visa Page”). If you will need your passport to travel outside the U.S. between April 22, 2016 and the beginning of August 2016, you will need to apply for your visa independently.
