Lyonandmultiplesclerosis(MS)gobackalongway.Onecouldbetemptedtobelievethatitdatesback to1894,withthefirstdescriptionofacuteneuromyelitisopticabyEugèneDevic,ifthisdiseasehadnotjust beennosologicallyseparatedfromMSasaresultoftheMayoClinicfindings. 1987,i.e.exactly25yearsago,wasanothermilestonewiththeannualECTRIMScongressalreadybeing organisedinLyon:withonly180participantsbutalreadymanyimpassionedandexcitingdebates. Thencamethe1990sandthefirstEuropeanConcertedActiondedicatedtoMS,undertheleadershipof OttoHommes,withthehelpofIanMcDonaldand LuigiAmaducci. Lyon was then put in charge of the development of the European Database for Multiple Sclerosis (EDMUS) – a project specifically for the descriptionofMSinthecontextofacontinuousconcertationbetweenEuropeanexperts.EDMUShasnow reachedversion4.1.1.Itisdistributedin285centresinover40countries.Itisincreasinglyusedasan electronicmedicalrecordforthefollow-upofpatientswithMSandhasalreadyallowedsignificantprogress intheepidemiologyofMS.IthasachievedthesettingupoftheOFSEP(ObservatoireFrançaisdelaSclérose enPlaques),acohortgatheringofover30’000PwMSinFrance. WiththisbackgroundofprotracteddedicationtoMScareandresearch,Lyonisproudtohostthe28thECTRIMS Congress.Youmaybeconfidentthateveryefforthasbeenmadetoselectalistoftopicsandassemblea panelofspeakersthatwillensureaproductiveandexcitingmeeting. Miller (New York, US); D. Bates (Newcastle, UK) Teaching Course 1 In this programme, two experienced MS clinicians, from either side of the Atlantic Ocean will engage the audience in an interactive programme that reviews several clinically important issues regarding the diagnosis and management of MS and related demyelinating disorders. Recently revised diagnostic criteria will be discussed, as well as the use of an increasing number of disease-modifying therapies. Strategies for drug selection that incorporate risk-benefit analysis will be considered. 08.30 – 10.00 Body fluid biomarkers Teaching Course 2 Chairs: C. Teunissen (Amsterdam, NL); J. Kuhle (Basel, CH) Basic introduction on biomarkers in body fluids C. Teunissen (Amsterdam, NL) Biomarkers in MS. State of the art J. Kuhle (Basel, CH) Future use of biomarkers in daily clinical practice F. Sellebjerg (Copenhagen, DK) There is a strong interest in body fluid biomarkers for multiple sclerosis. Biomarkers are objective, dynamic and relatively cheap aids in the diagnosis, prognosis and monitoring of disease progression in MS. This BioMS-eu Teaching Course will provide an introduction in the area of biobanking and biomarker research, including the choice and definition of control groups in CSF biomarker studies. We will give an update on the current state of biomarkers in MS – from DNA to proteins and metabolites. Furthermore, the future use of these biomarkers in daily clinical practice will be discussed. 08.30 – 10.00Disease-modifying therapy in MS – current and emerging treatment Chairs: H. Wiendl (Münster, DE); C. Lubetzki (Paris, FR) Teaching Course 3 First-line therapies (ABCR drugs) H. Wiendl (Münster, DE) Monoclonal antibodies C. Lubetzki (Paris, FR) Oral drugs L. Kappos (Basel, CH) Disease-modifying treatment (DMT) is subject to dynamic change and complexity of the treatment landscape emerges. This course will give the audience an overview of 7 Scientific Programme Wednesday, 10 October 2012 established DMTs including mode of action and recent trials on targeted immunotherapies. While ABCR drugs have the advantage of high general safety under long-term treatment, their problem is their magnitude of effectiveness: partial, non-response or tolerability issues are not uncommon. Immune escalating therapy, especially the group of monoclonal antibodies, is more effective, but carries significant safety risks. For the monoclonal antibody natalizumab the first steps have been taken to allow the individual risk stratification under and before therapy. After successful phase III trials, several agents have filed in for approval, possibly expected already in 2013 (Teriflunomide, BG12, Alemtuzumab). Chances and pitfalls are elaborated here, especially introducing “burden of therapy” in current and future therapy decisions. 08.30 – 10.00Neuro-inflammation, neurodegeneration and Teaching Course 4 tissue repair in RRMS, SPMS, PPMS Chairs: W. Brück (Göttingen, DE); C. Lucchinetti (Rochester,US) The normal-appearing brain in MS N. Schaeren-Wiemers (Basel, CH) Neuro-inflammation in the cortex C. Lucchinetti (Rochester, US) Inflammation and degeneration in MS W. Brück (Göttingen, DE) This course concentrates on the relationship between inflammation, neurodegeneration and repair in MS, in the different MS disease courses and disease stages. N. Schaeren-Wiemers will focus on molecular changes in the normal-appearing white and grey matter of MS brains. Here, early molecular changes occur that may help in identifying possible pathogenetic mechanisms of lesion development. The grey matter is substantially involved in the pathology of MS and reveals extensive demyelination from early disease on and increases with progression. C. Lucchinetti will describe in detail the quantity and quality of inflammation in cortical MS lesions and their role in the development of grey-matter demyelination in the different disease courses. W. Brück will focus on the correlation between inflammation and neurodegeneration, especially axonal damage, in early and chronic MS lesions. He will also describe the extent of spontaneous remyelination in different disease stages and discuss the cause of remyelination failure in MS. 8 Scientific Programme Wednesday, 10 October 2012 08.30 – 10.00Quantification of tissue damage and functional changes in MS Chairs: M. Ciccarelli (London, UK) Resting state fMRI M. Filippi (Milan, IT) Despite the fact that conventional magnetic resonance imaging (MRI) has a high sensitivity in detecting macroscopic tissue abnormalities related to MS, it lacks specificity to the heterogeneous pathological substrates of the disease and, as a consequence, conventional MRI metrics are only weakly correlated to disability. The aim of this Teaching Course is to provide a state-of-the-art knowledge of how quantitative structural and functional MRI techniques are contributing to characterise disease-related abnormalities of the central nervous system at various stages of MS and on adaptive mechanisms with the potential to limit the severity of clinical manifestations associated with irreversible tissue damage. The role of these techniques to monitor longitudinal changes in these patients, especially when treated, will be also discussed. 08.30 – 10.00From dreams to reality: antigen-specific and stem cell therapy in MS? Chairs: K. Selmaj (Lodz, PL); A. Ucelli (Genoa, Italy) Teaching Course 6 Antigen-specific therapy of MS K. Selmaj (Lodz, PL) Autologous mesenchymal stem cells for the treatment of MS A. Ucelli (Genoa, IT) Autologous haematopoetic stem cells for the treatment of MS D. Karussis (Jerusalem, IL) Antigen-specific therapy selectively targets cells specific for a given antigen and disables only a small part of the immune system in contrast to all currently available therapies in MS. Several approaches have been tried with respect to antigen-specific therapy with varied outcomes. We will provide an overview on the currently developing antigen-specific therapies leading to induction of immune tolerance in MS patients. The recent advances in our understanding of stem cell biology have launched stem cell treatments as one of the most exciting and difficult challenges in the MS field. Here we provide an overview about the current status of stem cell research in MS focusing on secured actuality, reasonable hopes and unrealistic myths. 9 Scientific Programme Wednesday, 10 October 2012 08.30 – 10.00Molecular mechanisms of tissue injury Teaching Course 7 in MS lesions Chairs: H. Lassmann (Vienna, AT) We will provide an overview of molecular mechanisms that contribute to MS pathology. R. Liblau will focus on the role of T-cells and discuss the presence of distinct T-cell subsets in MS lesions and how T-cells mediate CNS tissue damage. J. van Horssen will focus on the molecular mechanisms underlying impaired mitochondrial metabolism and free-radical production in MS and discuss the potential of antioxidant protection. H. Lassmann will describe disturbances in iron metabolism in MS brain and how iron can amplify oxidative damage in MS brain. 10.00 – 10.30 Coffee Break 10.30 – 12.00Issues in MS clinical trials: study design and results interpretation Chairs: M. P. Sormani (Genoa, IT); F. Lublin (New York, US) Teaching Course 8 Statistical issues and results interpretation in MS clinical trials M. P. Sormani (Genoa, IT) Outcomes in MS clinical trials F. Lublin (New York, US) Surrogate outcomes for immunomodulation and neuroprotection J. Wolinsky (Houston, US) This course will focus on methodological issues specific of clinical trials in MS. One section will be devoted to design, with a review of the basic principles for sample size calculations and a focus on the case of non-inferiority and equivalence trials. Caveats in results interpretation will be highlighted, signalling the problems of indirect comparisons of treatment effects across trials. A critical review of the classical endpoints for phase II and phase III trials will be offered, together with a discussion about potential surrogate markers valid for immunomodulation and for neuroprotection and repair. 10 Scientific Programme Wednesday, 10 October 2012 10.30 – 12.00 Paediatric MS Teaching Course 9 Chairs: B. Banwell (Toronto, CA) Relapse severity, recovery and progression in paediatric MS E. Waubant (San Francisco, US) Acute disseminated encephalomyelitis in children and MS C. Hemingway (London, UK) This course will discuss the clinical and epidemiological features of MS in children, as well as the clinical course and outcome. In addition, the key features of acute disseminated encephalomyelitis will be presented and contrasted with those of paediatric MS. 10.30 – 12.00MRI in the diagnosis of MS and Teaching Course 10 related disorders Chairs: N. De Stefano (Siena, IT); M. Wattjes (Amsterdam, NL) Lesion characteristics in MS: brain and spinal cord M. Wattjes (Amsterdam, NL) The “radiologically isolated syndrome” N. De Stefano (Siena, IT) Pitfalls in diagnosing “active MS” C. Enzinger (Graz, AT) This course will focus on the use of MRI for the diagnosis of MS. After reviewing the relevance for making the diagnosis of MS of white- and grey-matter lesions in the brain and spinal cord in terms of lesion characteristics (lesion distribution pattern and lesion heterogeneity), peculiar entities such as the radiologically isolated syndrome (RIS) and active/unusual MS will be discussed. Criteria useful for making the diagnosis of these two conditions and the exclusion of differential diagnosis will be provided, together with potential markers of short-term prognosis. 11 Scientific Programme Wednesday, 10 October 2012 10.30 – 12.00 NMO: a practical approach Teaching Course 11 Chairs: D. Wingerchuk (Scottsdale, US) Treating patients with NMO R. Marignier (Lyon, FR) Neuromyelitis optica (NMO) is a relapsing inflammatory demyelinating CNS disease that has several distinguishing features. However, the spectrum of recognised NMO manifestations continues to broaden and may overlap with MS, resulting in diagnostic confusion and therapeutic uncertainty. The aims of this course are: 1) to review current understanding of NMO pathogenesis, including the role and mechanisms of aquaporin-4 autoantibodies; 2) provide a practical guide to NMO diagnosis using existing diagnostic criteria and integrating new serological and neuro-imaging observations; and 3) outline a practical approach to treatment NMO, including management of acute attacks and strategies for attack prevention. 10.30 – 12.00 Gender differences in MS Chairs: H. Harbo (Oslo, NO); M. Tintoré (Barcelona, ES) Teaching Course 12 Genetic aspects and gender differences in disease onset and progression H. Harbo (Oslo, NO) Effects of sex hormones, the menstrual cycle and pregnancy on MS M. Tintoré (Barcelona, ES) Gender differences in the immune system and response to immunotherapy R. Gold (Bochum, DE) This course aims to provide an updated overview of gender differences in MS and their implications. The causes for the significant and increasing prevalence of MS among women compared to men are still not quite clear. Gender appears to play a role in development but also in progression and treatment effects of MS. This course will review current knowledge of gender differences in MS, both from a genetic, immunologic and clinical perspective. The impact of female hormones, pregnancy and breastfeeding on the disease as well as treatment strategies will also be discussed. 12 Scientific Programme Wednesday, 10 October 2012 10.30 – 12.00Controversies in the natural history of multiple sclerosis Chairs: C. Tremlett (Vancouver, CA) Emerging concepts from natural history studies: an overview C. Confavreux (Lyon, FR) Several concepts have emerged from the recent analyses of the natural history of MS in representative cohorts of patients. A. Scalfari will focus on the lack of a clear association between relapses and long-term disability accumulation, the MS onset being exacerbating-remitting or progressive. H. Tremlett will discuss the association between the patients‘ age and the MS clinical course and disability milestones. C. Confavreux will merge these observations with the concept of MS being one disease with different clinical phenotypes, infer about pathophysiology and treatments, and discuss the need for a revised clinical classification of MS. To submit your abstract please go to the congress website www.ectrims.eu/2012 In order to standardise the abstract layout, we kindly ask you to read the guidelines at this link carefully. Guidelines for preparation of abstracts 1. Abstracts may only be submitted online. Abstracts submitted by fax or email will not be accepted. 2. Abstracts must be submitted in English. Please use UK English spelling. 3. The presenting author of an accepted abstract must register and attend the congress. 4. Poster presenters must be present and stand by their posters during their assigned poster sessions. 5.Abstracts should contain only original material not published or presented elsewhere prior to 10 October 2012. 6.Use a short specific title indicating the nature of the investigation. The background and goals of the study should appear clearly to the reader. The methods and results must contain data, and the conclusions should be clearly expressed. 7.All abbreviations must be defined the first time they appear in your text (but, do not define in the title). Example: Multiple Sclerosis Impact Scale (MSIS), before being used as an abbreviation only. 8.Avoid complex mathematical formulae. For the symbols ≤ or ≥, type instead <= or >=. For superscript use caret (^) e.g. 10^6 instead of 106. Do not use Greek letters and symbols. Instead of “IFN-” use for example “IFN-g” or “IFN-gamma”. 9. Use generic drug names. 10.Tables, charts or other graphics may not be included and will be deleted by the editors. 11.The abstract text may not be longer than 2’500 characters including spaces (ca. 500 words). 12. Authors should indicate their presentation preference: • poster presentation only • oral or poster presentation “Poster only” implies that your abstract will be considered for poster presentation only and will not be eligible for the poster prize (see corresponding paragraph). Established by the Roman Emperor Augustus in 1BC as the capital city of the Gauls, it rapidly became one of the major economic, military, religious and political centres in France over the following centuries. Its location at the confluence of Rhone and Saone rivers enabled the town to develop major trading activities.
