Free vaccines are made available to the members of high-risk groups each fall. Check the ads in your local newspaper for more information, or go to www.csss.sudlanaudiere.ca. RENEWING YOUR HEALTH INSURANCE CARD Should your health insurance card not be automatically renewed when you renew your driver’s license, you can have it authenticated at your CLSC. Simply go to the reception area at one of the following CLSCs, remembering to bring along all of the required documents: • 193, boul. Important: Under the Civil Code of Québec, youth aged 14 years or older can obtain health services without the authorization of their parents and this with the utmost confidentiality. PRENATAL COURSES Offered by community organizations, prenatal courses provide parents-to-be with support and advice on how to best cope with pregnancy and the birth of their child, as well as their newborn's first few days. Fees apply. Visits are also possible for those not enrolled in prenatal courses. For an appointment, call 450 654-7525, extension 12156 PREGNANCY LOG During the mother-to-be's first appointment with the physician who will help her give birth, she will receive a pregnancy log and a questionnaire designed to better identify the family’s needs and expectations. Remember to bring this log to all of your appointments! OLO PROGRAM The OLO program (eggs, milk and oranges) seeks to foster the birth of healthy babies by helping pregnant women on a tight budget eat well-balanced meals. For more information on this program, contact your CLSC. CLSC Lamater: 450 471-2881, extension 4064 CLSC Meilleur: 450 654-7229, extension 61210 6 POSTNATAL VISITS All new parents in the territory are visited by a nurse within 24 to 72 hours of the mother’s release from the hospital. These professionals offer support, particularly with regard to breast feeding, answer any questions and ensure that everything is going well. VACCINATIONS The vaccines listed in your child's vaccination schedule can be received at any of the local community service centres (CLSCs) in the territory. An appointment is required for children between the ages of 0 and 4: CLSC Lamater: 450 471-2883 CLSC Meilleur: 450 654-3498 Children in primary or secondary school are vaccinated in CLSCs or by nursing teams visiting area schools as part of a provincial program. SERVICES FOR NURSING OR YOUNG INFANTS If you are the parents of a newborn or young infant, your CLSC can help. The services offered include: • Personalized follow-up for parents in challenging situations. • Consulting services in the areas of psychology, nutrition, speech therapy, psychoeducation, social work, nursing and specialized education. CHILD DEVELOPMENT • Are you concerned with your child’s development (motor skills, communication, socialization)? Your CLSC can provide you with speech therapy, ergotherapy and psychoeducational services. • CRÉDEL, the regional child development evaluation agency for the Lanaudière region, offers diagnostic evaluations of children between the ages of 0 and 5. For more information, speak with your physician or local CLSC. 7 CHILDREN, YOUTH AND FAMILIES PERINATAL GRIEF SERVICES OFFERED IN SCHOOLS Are you having a hard time recovering from a miscarriage or other pregnancy loss and resuming your regular activities? Services tailored to your specific needs are available. CLSC Lamater: 450 471-2881, extension 4249 CLSC Meilleur: 450 654-9012 Local community service centres (CLSCs) cooperate with school authorities to offer a wide variety of services. Primary school: psychosocial services; vaccination of 4th grade students; group activities on healthy lifestyle habits, sexuality and safety; preschool vaccines (at CLSCs); dental hygiene; screening of youth at greater risk for cavities; and, addition of teeth sealant. Secondary school: vaccination of students in the 9th grade (secondary 3); screening for blood-borne and sexually transmitted infections; emergency contraceptive pills; pregnancy tests; contraceptive devices; smoking cessation support and prevention; psychosocial services. PSYCHOSOCIAL AND MENTAL HEALTH SERVICES Your CLSC psychological services unit can provide you with support and help in dealing with family-related problems such as parent-child or parent-adolescent conflicts, behavioural problems or child social adaptation difficulties. Should the issue in question be associated to a mental health problem that is detrimental to your child's daily activities, a multidisciplinary team will provide you with advice and support or refer you to the appropriate resources. Accès-Liaison-Jeunesse / 450 471-2951 PHYSICAL DISABILITIES, INTELLECTUAL DISABILITIES AND PERVASIVE DEVELOPMENT DISORDERS Our CLSCs offer the following services to you and your loved ones: global evaluation of needs, psychosocial support, help in identifying respite or residential and long-term care resources, and any necessary referrals. INTEGRATED PERINATAL AND EARLY CHILDHOOD SERVICES (SIPPE) These services were developed for families and soon-to-beparents, where : • the young pregnant woman or mother is under 20 years of age and has not obtained a high-school diploma OR is in a low-income group; • the young pregnant woman or mother is over 20 years of age and has not obtained a high-school diploma AND is in a low-income group. Follow-up is offered to parents and children according to expressed needs. The team is comprised of various health professionals who cooperate with community organizations and daycare services. CLSC Lamater: 450 471-2881, extension 4064 CLSC Meilleur: 450 654-7229, extension 61210 8 CLINICS FOR YOUTH BETWEEN THE AGES OF 12 AND 21 Youth clinics offer free and confidential nursing, medical and psychosocial services, including contraceptive devices, pregnancy tests, emergency contraceptive pills, information and referrals for the voluntary termination of pregnancy, screening for blood-borne and sexually transmitted infections (treatment and follow-up), vaccination, and varied services for youth with problems of a personal nature (listening, follow-up and referrals). Consultations with a nurse or psychosocial professional are available with or without an appointment. Appointments are, however, required for all medical consultations. Iberville, Repentigny, 450 654-1023 SERVICES FOR VICTIMS OF SEXUAL ABUSE The CSSS du Sud de Lanaudière is the regional centre mandated to extend medical follow-up and psychosocial support services to sexual assault victims (aged 0 to 17 years) and their families. PIERRE-LE GARDEUR HOSPITAL The Pierre-Le Gardeur hospital offers emergency medical services, physical health care for adults and children, a wide variety of diagnostic examinations, general and specialized surgical interventions, psychiatric services and rehabilitation services. Victims are subsequently referred to the appropriate resources. Do you find yourself having suicidal thoughts? If so, call your CLSC to speak to a counsellor who will, depending on your situation, provide you with information or refer you to the appropriate resources. Professional support is also available if you or a loved one are affected by a mental illness. HOME SUPPORT Home support is offered to persons who continue to live in their natural environment in spite of having temporary or permanent disabilities. Home support services include: nursing and medical care, rehabilitation services, nutritional support, respiratory therapy, psychosocial consultations, fall prevention measures, personal hygiene assistance, help preparing meals, loaning of equipment, etc. Requests for home support services must be submitted through the home support point of contact by the hospital, your family doctor or yourself. RESPITE FOR CAREGIVERS Respite services for caregivers are available on weekdays (during the day or evening) as well as on Saturdays. They are specifically designed for those loved ones who care for persons of all ages with temporary or permanent disabilities. Respite can be of several types: temporary accommodations for the person with diminished capacities, granting of a respite-care services subsidy, home support services or activities in a day centre. HOUSEKEEPING HELP 450 654-CLSC (2572) 1 855 692-CLSC (2572) Social economy organizations in our region offer housekeeping help that includes light or heavy household cleaning (for example, spring cleaning), laundry, meal preparation and the running of errands. You may, depending on your specific situation, be eligible for financial support to help you benefit from such services at a reduced cost. DAY CENTRES Information: Home support and other services point of contact – see page 18. Home support and other services point of contact: Day centres offers rehabilitation and respite services, as well as support destined to help maintain acquired skills. These services may consist of individual sessions (physical therapy and ergotherapy) or group activities such as memory enhancement workshops or physical or relaxation exercises. Support is also available for persons with specific illnesses such as Parkinson's, arthritis or osteoarthritis, diabetes, etc. Requests for an evaluation to determine your eligibility to day centre services must be submitted to the home support and other services point of contact. RESIDENTIAL AND LONG-TERM CARE CENTRES The region's various residential and long-term care centres provide a comfortable living environment for adults or seniors experiencing a loss of autonomy who are no longer able to live in their home, even with the help of their loved ones. Depending on a person's needs, these centres can offer: • A living environment and services for persons with a severe loss of autonomy. • Help from a family-type or intermediary resource for persons with a slight or average loss of autonomy. Requests for an evaluation to determine your eligibility for residential and long-term care must be submitted to the home support and other services point of contact. The CSSS du Sud de Lanaudière Foundation’s mission consists of collecting donations to purchase medical equipment, implement projects that will make it possible to enhance the services offered while improving the health condition of the residents served by the CSSS du Sud de Lanaudière.
