ENDODONTIE Intérêt des examens complémentaires en endodontie. Relevance of additional examinations in endodontics. PATIENT PRESENTATION. Le patient est âgé de 30 ans le jour de la consultation. The patient is 30-year-old on the day of the consultation. 2. MOTIF DE LA CONSULTATION 2. REASON FOR CONSULTATION Le patient consulte en raison d’une douleur dans le secteur II associée à une tuméfaction. The patient suffers from pains in sector II associated with a swelling. 3. ANAMNÈSE MÉDICALE 3. MEDICAL ANAMNESIS The patient is healthy and non-smoker. He suffers from no systemic disease and has no medical history. ■ The day of the consultation, the patient had taken selfmedication: an antibiotic treatment (amoxicillin) taken orally, associated with nonsteroidal anti-inflammatory drugs (ibuprofen). ■ Le patient, non fumeur, est en bonne santé générale. Il ne souffre d’aucune maladie systémique et ne présente aucun antécédent médical. ■ Le jour de la consultation, le patient s’était autoprescrit un traitement antibiotique per os (amoxycilline) associé à des anti-inflammatoires non stéroïdiens (ibuprofène). ■ 4. ANAMNÈSE DENTAIRE 4. DENTAL ANAMNESIS. ■ Remplacement d’un ancien amalgame par un composite sur la 26 deux ans auparavant. ■ Un an plus tard, apparition de douleurs nécessitant le remplacement du composite par un CVI recouvert d’un composite sur la 26. Une carie en mésial de la 27 a également été traitée et obturée par un composite. ■ Après un silence clinique de quelques semaines, apparition de douleurs violentes, irradiantes diurnes et nocturnes. Un traitement endodontique au niveau de la 26 a été réalisé en urgence. Replacement of a former amalgam with a composite on tooth 26 two years before. ■ One year later, occurrence of pains requiring the replacement of the composite with a GIC covered with a composite on 26. A mesial caries on 27 was also treated and filled with a composite. ■ After a clinical quiet period of several weeks, daytime and nighttime sharp, radiating pains started to occur. EXAMEN CLINIQUE 5. CLINICAL EXAMINATION ■ Tooth 26 shows a voluminous occluso-distal composite. 27 shows a small occluso-mesial restoration. ■ There is a tumefaction at the bottom of the vestibular area around sector II connected to 26 and spread to the tuberosity. This zone is not indurated but it is sensitive to palpation. ■ There is no visible fistula. ■ The mobility is physiological on 25 and scores a 2 on 26 and 27 (visible lateral mobility, inferior to 1 mm). La 26 présente un volumineux composite occluso-distal. La 27 présente une petite reconstitution occluso-mésiale. ■ Il existe une tuméfaction intéressant le fond du vestibule au niveau du secteur II en regard de la 26 et étendue jusqu’à la tubérosité. La zone est non indurée, mais sa palpation est sensible. ■ Il n’y a pas de fistule apparente. ■ La mobilité est physiologique pour la 25 et de l’ordre de 2 pour les 26 et 27 (mobilité latérale visible et inférieure à 1 mm). ■ ■ ■ ■ 6. EXAMEN RADIOGRAPHIQUE 6. RADIOGRAPHIC EXAMINATION La radiographie rétro-alvéolaire est réalisée à l’aide d’un angulateur molaire selon la technique des plans parallèles avec une prise orthocentrée (Machtou 1993 ; Pertot et Simon, 2004) et d’un écran radio luminescent à mémoire (ELRM, Vitascan®). ■ Le radiogramme montre une obturation canalaire satisfaisante en longueur et densité sur la 26, une pulpe légèrement rétractée avec oblitération partielle des lumières canalaires pour la 27. Il n’y a pas d’alvéolyse. (fig. 1). The retro-alveolar X-ray is performed with an angulator using the parallel technique and fitted with an orthocentered grip (Machtou, 1993; Pertot and Simon, 2004) as well as an X-ray luminescent computed screen (ELRM, Vitascan®). ■ The radiograph shows a correct root canal obturation in length and density on 26, a slightly retracted pulp with partial obliteration of the root canal lights on 27. DIAGNOSIS 7.1. Avez-vous suffisamment d’éléments cliniques et radiographiques pour poser un diagnostic ? Non, car plusieurs hypothèses diagostiques sont possibles : • pour la 26 : abcès apical aigu secondaire ou parodontite apicale aiguë ou fêlure ; • pour la 27 : abcès apical aigu ou parodontite apicale aiguë ou fêlure ou nécrose ou pulpite ; • une atteinte simultanée de la 26 et de la 27 n’est pas exclue. Les éléments fournis par l’interrogatoire, les examens clinique et radiologique réalisés ne sont pas suffisants pour poser un diagnostic précis. Des tests cliniques complémentaires comme le sondage parodontal, les tests de percussion et les tests de sensibilité pulpaire sont nécessaires (Lasfargues 2005 ; Machtou 1993 ; Pertot et Simon, 2004). 7.1. Are there enough clinical and radiographic elements to give a diagnosis? No, because several diagnostic hypotheses are possible: • concerning tooth 26: secondary acute apical abscess or acute apical periodontitis or a crack, • concerning tooth 27: acute apical abscess or acute apical periodontitis or a crack, a necrosis or a pulpitis, • a simultaneous affection of both 26 and 27 is not to be excluded. Elements supplied by the interview, the clinical and radiological examinations are not enough to generate a precise diagnosis. Clinical additional tests such as the periodontal probing, percussion tests and pulp sensibility tests are necessary (Lasfargues, 2005; Machtou, 1993; Pertot and Simon, 2004). 7.2. Quel diagnostic le sondage parodontal peut-il valider ? Le sondage parodontal permet de différencier les lésions d’origine endodontique (sondage étroit et profond) des lésions parodontales (sondage large et marginal) ou des lésions combinées endo-parodonales (sondage large et profond). En présence d’une fêlure, il peut exister une perte d’attache ponctuelle le long du trait de fêlure (Lasfargues 2005; Machtou 1993; Pertot et Simon, 2004). 7.2. What kind of diagnosis may the periodontal probing validate? Periodontal probes will differentiate lesions of endodontic origin (narrow and deep probing) from periodontal lesions (wide and marginal probing) or from combined endodontic periodontal lesions (wide and deep probe). In case of a crack, there may be an isolated attachment loss along the crack line (Lasfargues, 2005; Machtou, 1993; Pertot and Simon, 2004). Le sondage réalisé au niveau des 25, 26 et 27 ne montre aucune perte d’attache. Performed on teeth 25, 26 and 27, the probing shows no attachment loss. 7.3. Quelles informations les tests de percussion apportent-ils ? Les tests de percussion permettent d’analyser la santé de la zone apicale. Une sensibilité à la percussion axiale révèle l’inflammation de la zone apicale (parodontite apicale). Une sensibilité à la percussion latérale associée à la palpation vestibulaire apicale (test du choc en retour) permet de mettre en évidence la perte de la corticale (Lasfargues 2005 ; Pertot et Simon, 2004). Les tests de percussion axiale réalisés sur la 26 et la 27 se sont révélés positifs, ainsi que sur 25. L’inflammation de la région étant importante (vestibule soufflé), il est impossible de réaliser le test du choc en retour et il est difficile de s’assurer de la pertinence des résultats des tests de percussion axiale. 7.3. What kind of information may percussion tests deliver? Percussion tests help assessing the apical region condition. A sensibility to axial percussion will show an inflammation of the apical region (apical periodontitis). A sensibility to lateral percussion associated with palpation in the vestibular fold near the apical region (test of “return shock”) highlights a loss of cortical substance (Lasfargues, 2005; Pertot and Simon, 2004). The axial percussion tests conducted on 25, 26 and 27 proved positive. Given the important inflammation of the region (puffed buccal vestibule), it is impossible to perform the “return shock” test and it is difficult to be sure that the results of the axial percussion tests are relevant. 7.4. Comment interpréter les tests de sensibilité pulpaire ? Les tests de sensibilité pulpaire (test au froid, test au chaud, test électrique) ne sont pas fiables à 100 % (Chen et Abbott, 2011 ; Fuss et coll., 1986), surtout pour les dents pluriradiculées. En effet, un canal peut être nécrosé alors que la dent réagit positivement au test. De plus, la réalisation de ces tests dans les secteurs 7.4. How should we interprete the pulp sensibility tests? Pulp sensibility tests (cold and heat testings, electric tests) are not 100% reliable (Chen and Abbott, 2011; Fuss et al., 1986) especially on pluriradicular teeth. Indeed, a canal may be necrotic even though the tooth reacts positively to the test. Furthermore, conducting these tests in the posterior 5 Revue d’Odonto-Stomatologie/Février 2013 ENDODONTIE postérieurs peut s’avérer particulièrement difficile à réaliser de façon fiable, d’autant plus que dans le cas présent, le vestibule est tuméfié. Mais ces tests permettent d’affiner notre démarche diagnostique. Un test au froid et un test électrique ont été réalisés sur la 25, supposée saine, et sur les 26 et 27. Les tests au froid ont été positifs pour la 25 et négatifs pour les 26 et 27. Le test électrique a été positif pour les 25 et 27, avec toutefois une réponse très tardive de la 27 par rapport à la 25, et négatif sur la 26. Le test électrique a été réalisé avec un Pulptester Analytic®, le test au froid avec une boulette de coton et un spray de dichlorodifluorométhane (Superfrost Roeko®). sectors can be particularly difficult, hence a questionable reliability, especially in this case where the buccal vestibule is swollen. Still, these tests will help us precise our diagnostic approach. A cold test and an electric test were performed on the supposedly healthy 25 as well as on 26 and 27. Cold tests proved positive on 25 and negative on 26 and 27. The electric test was positive on 25 and 27, with however a very late answer of 27 compared with 25, and negative on 26. The electric test was performed with Pulptester Analytic®, the cold test with a cotton ball and a spray of dichlorodifluoromethane (Superfrost Roeko®). 7.5. Pourquoi tester la vitalité de la 26 qui est déjà traitée endodontiquement ? Le test de sensibilité pulpaire pratiqué sur cette première molaire maxillaire endodontiquement traitée n’est pas une erreur. En effet, une réponse positive peut révéler la présence d’un canal non traité avec un tissu pulpaire encore vital. 7.5. What is the use of testing the vitality of 26 which is already endodontically treated? The pulp sensibility test conducted on this endodontically treated first maxillary molar is a deliberate act. Indeed, a positive response may show the presence of an untreated canal with a still alive pulp tissue. 7.6. Le diagnostic à la suite de ces tests cliniques peut-il être posé ? Bien que les éléments cliniques nous orientent vers une pathologie pulpaire de la 27, nous ne pouvons pas préciser si la 26 doit être ou non retraitée. 7.6. Can we give a diagnosis after these clinical tests? Although the clinical elements lead us to diagnose a pulp pathology in 27, we still cannot say whether 26 has to be retreated or not. 7.7. Quel examen complémentaire permettra d’établir définitivement le diagnostic ? N’ayant pas réussi à lever le doute sur la participation ou non de la 26 dans le tableau clinique, un examen radiographique complémentaire est nécessaire. L’examen de choix particulièrement adapté à la sphère orale est aujourd’hui la tomographie volumique à faisceau conique ou cone beam computerized tomography (CBCT). Il doit être préféré à un scanner car il est plus précis et moins irradiant (HAS 2009). La lésion inflammatoire péri-radiculaire d’origine endodontique se manifeste radiographiquement par une image radioclaire de destruction osseuse. De nombreuses études comparent les radiographies 2D et 3D, notamment pour la détection et l’évaluation des lésions inflammatoires péri-radiculaires d’origine endodontique ou LIPOE. Toutes montrent la supériorité du CBCT sur les autres examens radiographiques 2D pour la détection des LIPOE (Gao et coll., 2010 ; Tsai et coll., 2012 ; Vandenberghe et coll., 2008 ; Wu et coll., 2009). Le cliché rétro-alvéolaire est systématiquement réalisé pour déterminer le diagnostic et le suivi des LIPOE. Mais la superposition des structures anatomiques empêche souvent de déceler cette image précocement ou du moins d’en apprécier l’étendue, notamment dans le sens vestibulolingual et surtout si la lésion est confinée dans l’épaisseur de l’os basal plutôt que dans l’os cortical (Tsai 7.7. What additional examination will help us establish a definitive diagnosis? Since we have not managed to decide on the potential participation of 26 in the clinical picture, an additional radiographic examination is necessary. The most adequate examination for the oral sphere is the cone beam computed tomography (CBCT). It must be preferred to a scanner because it is more accurate and less irradiant (HAS, 2009). Radiographically, the inflammatory periradicular lesion of endodontic origin is displayed by a radiolucent image of osseous destruction. Many studies have compared the 2D and 3D X-rays, particularly for the detection and the assessment of the inflammatory periradicular lesions of endodontic origin. All of them have showed the superiority of the CBCT on other 2D radiographic examinations in the detection of such lesions (Gao et al., 2010; Tsai et al., 2012; Vandenberghe et al., 2008; Wu et al., 2009). A retro-alveolar radiography is always carried out to determine the diagnosis and the treatment of the inflammatory periradicular lesions of endodontic origin. But the overlapping of anatomical structures often prevents from displaying this image soon enough or at least from assessing its extent, in particular in the vestibulolingual direction and especially if the lesion is confined to the 6 Revue d’Odonto-Stomatologie/Février 2013 ENDODONTIE et coll., Vandenberghe 2008), si l’os cortical est épais, comme par exemple dans la région molaire, et si la lésion est de petite taille (Gao et coll., 2010). D’après une étude d’Estrela et coll. en 2008, il apparaît que le CBCT peut être utilisé comme gold standard pour la détection de ces lésions, avec une spécificité de 100 % (1.0) et une sensibilité de 100 % (1.0) que les auteurs expliquent par la très fine résolution de cet appareil ajoutée à l’absence de bruits anatomiques sur ses images. thickness of the basal bone rather than to the cortical bone (Tsai et al., Vandenberghe, 2008), or if the cortical bone is thick, like for example in the molar region, and if the lesion is small (Gao et al., 2010). According to a study by Estrela et al. in 2008, it seems that the CBCT can be used as “Gold Standard” for the detection of these lesions, with a 100% specificity (1.0) and a 100% sensibility (1.0) which the authors explain with the very high resolution of this device added to the absence of anatomical noises on its images. L’examen CBCT du secteur II du patient met en évidence plusieurs éléments : The patient’s sector II CBCT highlighted several elements: • Pour la 26 : l’absence de traitement du canal MV2 sur la 26 (fig. 2). • On tooth 26: the absence of root canal treatment on MV2 (fig. 2). • Le CBCT montre un léger épaississement de la membrane sinusienne en regard des deux molaires (fig. 2). • The CBCT shows a slight thickening of the sinus membrane connected to both molars (fig. 2). Axial and parasagittal CBCT images of 26, showing the untreated MV2 canal, anatomy of 27, thickening of the sinus mucous membrane (yellow arrow). 2 • On 27: - The presence of a small radiolucent image at the apex of the distovestibular root with loss of vestibular cortex (fig. 3), - the presence of an important radiolucent image at the apex of the palatal root of 27 (fig. 4), showing the presence of an inflammatory periradicular lesion of endodontic origin superior than 2 mm, - the punctual loss of cortical palatine (fig. 3), - the particular anatomy of this palatal root which has two canals sharing a common exit (fig. 4) as well as a onemillimeter diameter hyper-calcified structure in the cervical region looking like a structure defect such as an enamel pearl (fig. 4). • Pour la 27 : – la présence d’une petite image radioclaire à l’apex de la racine disto-vestibulaire avec perte de la corticale vestibulaire (fig. 3) ; – la présence d’une importante image radioclaire à l’apex de la racine palatine de la 27 (fig. 4), révélant la présence d’une LIPOE supérieure à 2 mm ; – la perte ponctuelle de la corticale palatine (fig. 3), – l’anatomie particulière de cette racine palatine ; elle présente deux canaux avec une sortie commune (fig. 4) et une structure hypercalcifiée de 1 mm de diamètre dans la zone cervicale faisant penser à une anomalie de structure type perle d’émail (fig. 4). 7 Revue d’Odonto-Stomatologie/Février 2013 ENDODONTIE Fig. 3 : CBCT coupe coronale, axiale et parasagittale au niveau de la 27, image radioclaire à l’apex de la racine palatine et distovestibulaire et mise en évidence des pertes des corticales vestibulaire et palatine. Le diagnostic qui a été posé est le suivant : • abcès phœnix ou abcès périapical aigu secondaire sur la 27. Le diagnostic étiologique est la nécrose vraisemblablement partielle de la 27. 7.8. What is the final diagnosis? Here was the final diagnosis: • Phoenix abscess or acute secondary periapical abscess on 27. WHAT MAJOR INFORMATION DID THE CONE BEAM GIVE? Le CBCT a permis d’objectiver le défaut de traitement du canal MV2 de la 26. Il est parfois possible d’obtenir cette information en réalisant une seconde radiographie rétroalvéolaire avec une incidence déportée selon la règle de Clark (Machtou 1993 ; Pertot et Simon, 2004). Mais, surtout, le CBCT a permis de visualiser la lésion osseuse sur la 27, contrairement à la radiographie alvéolaire. L’examen 3D permet aussi d’anticiper les difficultés du traitement endodontique sur la 27 (anatomie particulière de la racine palatine). The CBCT allowed to objectify the absence of treatment on the MV2 canal of tooth 26. It is sometimes possible to obtain this information by performing a second retroalveolar X-ray with an oblique incidence according to Clark’s rule (Machtou, 1993; Pertot and Simon, 2004). But above all and unlike the alveolar x-ray, the CBCT could visualize the bone lesion on 27. The 3D examination also allows to anticipate the difficulties of the endodontic treatment on 27 (due to the particular anatomy of the palatal root). Toutefois, le CBCT est un examen qui ne doit pas devenir systématique. However, the CBCT must not be a systematic examination. 9 Revue d’Odonto-Stomatologie/Février 2013 ENDODONTIE D’après le rapport HAS (2009), « l’imagerie CBCT en endodontie peut présenter un intérêt dans certains cas bien sélectionnés, lorsque les informations fournies par la clinique et la radiologie conventionnelle ne sont pas suffisamment contributives au diagnostic et à la gestion technique d’un traitement et qu’une image tridimensionnelle est indispensable ». Dans tous ces cas, il faut respecter : • le principe de justification : le bénéfice de l’examen doit être supérieur au risque potentiel qu’il induit ; • le principe d’optimisation : l’exposition des personnes aux rayonnements ionisants doit être raisonnablement la plus faible possible pour obtenir les résultats escomptés, suivant le principe d’Alara (« As Low As Reasonable Achievable radiation exposure »). According to the report HAS (2009): “CBCT in endodontics can be a useful examination in some carefully selected cases, when clinical information and conventional radiology are not enough to establish a diagnosis and technically manage the treatment, and when a three-dimensional image proves to be necessary.” 10. QUEL EST LE PRONOSTIC À LONG TERME ? 10. WHAT IS THE LONG-TERM PROGNOSIS? N’ayant rencontré aucun incident pendant sa réalisation, le pronostic du traitement endodontique de première intention sur la 27 est favorable, surtout si une reconstitution coronaire étanche est réalisée rapidement (Farzaneh 2004 ; Hommez et coll., 2002 ; Safavi et coll., 1987). Cependant, la présence d’une LIPOE réduit les chances de succès (Ng et coll., 2007 ; Ng et coll., 2008). Le pronostic du retraitement endodontique de la 26 est favorable, mais les chances de succès sont moindres que pour la 27 (retraitement + LIPOE) (Ng et coll., 2007 ; Ng et coll., 2008). As no incident occurred during the treatment, the firstline endodontic treatment prognosis on 27 is favorable, especially if a tight coronal reconstruction is quickly performed (Farzaneh, 2004; Hommez et al., 2002; Safavi et al., 1987). However, the presence of an inflammatory periradicular lesion of endodontic origin reduces the chances of success (Ng et al., 2007; Ng et al., 2008). The prognosis for the endodontic retreatment of tooth 26 is favorable but the chances of success are fewer than on tooth 27 (retreatment + inflammatory periradicular lesion of endodontic origin) (Ng et al., 2007; Ng et al., 2008). In all these cases, it is necessary to respect: • The justification principle: the benefit of the examination must be greater than the potential risk it induces. • The optimization principle: the patient exposure to ionizing radiation has to be reasonably the lowest possible to obtain the expected results, according to the ALARA principle: “As Low As Reasonable Achievable radiation exposure”. CE QU’IL FAUT RETENIR – KEY POINTS Les examens complémentaires en endodontie orientent le diagnostic différentiel quand les signes cliniques ne sont pas pertinents. Le recours au cone beam est indiqué « lorsque les informations fournies par la clinique et la radiologie conventionnelle ne sont pas suffisamment contributives au diagnostic et à la gestion technique d'un traitement ». Additional examinations in endodontics lead to a differential diagnosis when the clinical signs are not relevant. The use of CBCT is indicated “when clinical information and conventional radiology are not enough to establish a diagnosis and technically manage the treatment”.
