La SFEDP a examiné les publications relevantes dans le domaine et les recommandations d'experts. Deux documents nous ont semblé particulièrement relevants : • Une revue publiée récemment par deux experts internationalement reconnus : Frank Rauch et Francis H Glorieux, Osteogenesis imperfecta, The Lancet, 2004, Volume 363Pages 1377-1385. • Un document émanant du groupe T2A-Groupe juste prescription de l'AP-HP et qui classe les indications du pamidronate en 3 catégories : (Groupe I (indications reconnues), Groupe II (pertinentes) et Groupe III (indications non reconnues)) et qui place l'ostéogénèse imparfaite dans le Groupe II, sur la base de l'abondante littérature scientifique supportant son usage. A l'examen de ces documents, la SFEDP considère que les bisphosphonates constituent actuellement le traitement médical de référence de l'ostéogénèse imparfaite et qu'en l'absence d'alternative thérapeutique, il doit pouvoir être proposé à tous les patients ayant une forme sévère, à l'exclusion des formes modérées pour lesquels l'expérience clinique est limitée et le rapport bénéfice/risque inconnu. Nous espérons que votre administration hospitalière et vous pourrez trouver une solution permettant de répondre à la demande médicale légitime de ces patients atteints d'une maladie sévère. En l'absence d'hypercalcémie : les 90 mg (qui correspondent à la dose maximale journalière ou par cure) sont dilués dans 250 ml d'une solution pour perfusion et sont perfusés pendant 2 heures. En cas d'hypercalcémie : il est recommandé d'administrer les 90 mg (qui correspondent à la dose maximale journalière ou par cure) en 4 heures dans 500 ml. Hypercalcémies sévères d'origine maligne : Traitement initial : En association à une hydratation adaptée, la dose totale d'Arédia pour une cure thérapeutique peut être administrée en une perfusion unique ou en plusieurs perfusions réparties sur 2 à 4 jours consécutifs. La dose totale recommandée à utiliser pour une cure thérapeutique est fonction du chiffre de calcémie initiale : Calcémie initiale Dose totale recommandée en mg pour une cure mmol/l mg/l jusqu'à 3 jusqu'à 120 15 à 30 3 à 3,5 120 à 140 30 à 60 3,5 à 4 140 à 160 60 à 90 >4 > 160 90 Répétition des cures : Si l'hypercalcémie récidive, ou si la calcémie ne commence pas à diminuer dans les 2 jours suivant le début du traitement, on peut répéter les perfusions d'Arédia en suivant les mêmes indications posologiques que pour le traitement initial. Dose maximale/cure = 90 mg en 4 heures dans 500 ml. Myélome stade III avec au moins une lésion osseuse : 90 mg toutes les 4 semaines en 4 heures dans 500 ml. Ostéolyses malignes avec ou sans hypercalcémie : La dose recommandée d'Arédia est de 90 mg toutes les 4 semaines. Maladie de Paget : La dose recommandée est de 120 à 180 mg, répartis en 2 à 3 jours consécutifs. En cas de résultats insuffisants et (ou) de reprise de l'évolutivité de la maladie de Paget, il est possible de répéter une nouvelle cure de 120 à 180 mg, six mois au moins après la première cure. La plupart de ces patients recevaient aussi une chimiothérapie et des corticoïdes. La majorité des cas rapportés ont été associés à des interventions dentaires telles que extraction dentaire. (…). Un examen dentaire avec des soins dentaires préventifs appropriés devra être pris en considération avant l’instauration d’un traitement par bisphosphonates chez patients présentant des facteurs de risque associés.
