479 — INFORM ATIONS ÉPIDÉM IOLOGIQUES EPIDEMIOLOGICAL NOTES VARIOLE SMALLPOX Europe Europe Royaume-Uni: Londres. — Un cas importé de variole à Londres a été notifié par téléphone le 17 octobre chez un enfant pakistanais de 3 ans, arrivé à Londres le 1er octobre par Boeing 707, vol PK 715, de la Pakistan International Airlines. L ’avion avait fait escale au Caire, à Rome et à Paris. L ’enfant tomba malade le 9 octobre; le 10, une éruption apparut qui devint vésiculaire le 14 octobre. Le diagnostic de variole a été confirmé par les examens de labora toire. Il est rappelé qu'aux termes du Règlement sanitaire international, les administrations sanitaires n'ont pas le droit de considérer Londres comme une circonscription infectée de variole en raison de ce cas importé. United Kingdom: London. — By telephone on 17 October, one imported case of smallpox was notified in London. The patient, a Pakistani child aged 3, arrived in London on 1 October by Boeing 707 of the Pakistan International Airlines, flight PK 715, which stopped in Cairo, Rome and Paris. The child fell sick on 9 October; a rash appeared on 10 October and became vesicular on 14 October. The diagnosis of smallpox was confirmed by laboratory tests. It is recalled that, under the provisions o f the International Sanitary Regulations, health administrations have not the right to consider London as a smallpox infected local area on account o f this imported case. ÉPIDÉMIES PARMI LE PERSONNEL DE LABORATOIRE TRAVAILLANT SUR DES SINGES CERCOPITHECUS EN PROVENANCE D’AFRIQUE ORIENTALE) OUTBREAKS IN LABORATORY PERSONNEL WORKING WITH CERCOPITHECUS MONKEYS FROM EAST AFRICA1 Europe Europe République fédérale d'Allemagne et Yougoslavie. — Au cours du mois d ’août, une épidémie a fait son apparition parmi le personnel de deux laboratoires de la République fédérale d’Allemagne manipu lant des tissus ou du sang de Cercopithecus aethiops en provenance d’Afrique orientale. Au total, trente cas cliniquement confirmés ou douteux, dont sept mortels, ont été déclarés. Huit de ces cas et deux des décès sont survenus chez des employés de l’Institut Paul Ehrlich et de l’Hôpital universitaire de Francfort; 22 cas, dont cinq mortels, ont été enregistrés parmi les employés des Behringwerke et de l’Hôpital universitaire de Marbourg. En République fédérale d’Allemagne, le premier cas a été diagnostiqué le 7 août et le der nier le 5 septembre. Un vétérinaire de l’Institut de Santé publique de Serbie, à Bel grade, qui avait autopsié des singes Cercopithecus de plusieurs car gaisons arrivant d’Afrique orientale a été atteint au début du mois de septembre. Cliniquement, sa maladie a été reconnue analogue à celle des employés des laboratoires allemands. Un cas secondaire a été également observé à Belgrade chez une personne qui avait soigné le vétérinaire et lui avait fait des prises de sang. Ces deux malades sont maintenant guéris. Federal Republic o f Germany and Yugoslavia. —- An outbreak of disease among laboratory personnel handling tissues or having contact with blood from Cercopithecus aethiops from East Africa occurred in two laboratories in the Federal Republic of Germany during August. A total of 30 clinically confirmed or questionable cases, seven of them fatal, have been reported. Eight of the cases and two deaths occurred among employees of the Paul Ehrlich Institute, and the University Hospital, Frankfurt, and 22 cases and five deaths among employees of the Behringwerke and the University Hospital, Marburg. The first case in the Federal Republic of Germany was recognized on 7 August and the last on 5 September. At the Public Health Institute for Serbia, Belgrade, a veterinarian who had made an autopsy on Cercopithecus monkeys from various shipments from East Africa became ill at the beginning of September. Clinically the disease was recognized as similar to the one affecting the laboratory workers in Germany. A secondary case was also reported from Belgrade in a person who took care of the patient and took blood from him. Both patients have by now recovered. Tableau clinique Clinical picture Le tableau clinique a été décrit comme suit: Début brusque avec fièvre, céphalées, conjonctivite, myalgias et prostration. L ’hyperthermie peut régresser pendant un ou deux jours, puis remonter, accompagnée d’un exanthème scarlatiniforme et d’un énanthème hémorragique. On observe en outre une augmen tation des taux de transaminases, une oligurie ou une anurie et, dans certains cas, une myocardite. The clinical picture has been reported as the following: Sudden onset with fever, headache, conjunctivitis, myalgia and prostration. The pyrexia may decrease for one or two days and then rise accompanied by a scarlatiniform rash and a haemorrhagic enanthema. There is increase in transaminase levels and oliguria or anuria and in some cases myocarditis. Anatomie pathologique Pathology Le foie présente de nombreuses lésions irrégulièrement dissé minées dans la totalité de l’organe, non limitées à la zone intermé diaire des lobules. Il n ’y a pas réaction inflammatoire, ni ictère, mais parfois dégénérescence graisseuse grave. Dans la rate, on observe de nombreuses lésions inflammatoires des follicules et une réaction inflammatoire de la pulpe. Une myo cardite séreuse avec une certaine infiltration du tissu interstitiel a été observée dans quelques cas. L’examen du SNC n ’est pas encore terminé. Numerous lesions have been found in the liver. They are scattered irregularly all over the organ and not limited to the inter mediate zone of the lobuli. There is no inflammatory reaction and no icterus, but sometimes severe fatty degeneration. In the spleen numerous inflammatory lesions of the folliculi and inflammatory reaction of the pulp are observed. A serous myocar ditis with some interstitial cell infiltration has been observed in some cases. Europe (suite) Europe (continued) Origine des singes Origin of the monkeys Tous les singes incriminés sont des Cercopithecus aethiops arrivant d’Afrique orientale après un séjour dans une animalerie de l’aéroport de Londres. Aucun cas d’infection humaine n ’a été signalé parmi le personnel qui s’est occupé de ces singes à leur lieu d’origine ou pendant le transport, ni parmi celui d ’autres laboratoires ayant accueilli des singes de même provenance. All the monkeys incriminated in the outbreaks are Cercopithecus aethiops coming from East Africa and passing through an animal hostel at London Airport. No infections of man have been reported in personnel handling the monkeys at the place of origin or during transport, nor from other laboratories having received monkeys from the same origin. Epreuves sérologiques et essais d’isolement Serology and isolation attempts Le Centre international de référence pour les Arbovirus a indi qué que les tests IH (inhibition de l ’hémagglutination) et FC (fixation du complément) pour plus de 60 arbovirus, effectués sur des séries de sérums provenant d’un certain nombre de malades, ont été négatifs. Le Microbiological Research Establishment, de Porton, a lui aussi signalé l’obtention de résultats négatifs de tests IH pour un choix d ’arbovirus effectués sur des sérums prélevés précocement chez des malades. Le South African Institute for Medical Research, de Johannesburg, a signalé la présence de titres faibles d’anticorps IH pour certains arbovirus du groupe B dans le sérum de quelques malades. Dans différents laboratoires, du matériel d’autopsie et des échan tillons de sang provenant de malades à la phase aiguë ont été inoculés dans des œufs embryonnés, à des souriceaux à la mamelle et à des souris adultes, à des cobayes et dans plusieurs cultures cellulaires. Les résultats ont été négatifs sauf dans le cas de l’ino culation au cobaye. Des chercheurs de Marbourg, de Francfort et de Porton ont signalé l’apparition constante chez les cobayes ino culés d’un état fébrile, quatre à six jours après l’inoculation. A Porton, on a fait six passages consécutifs. La période d ’incubation est tombée à trois jours et la plupart des cobayes sont morts, de 13 à 15 jours après l’inoculation. Des sérums de convalescent prélevés sur des cobayes fébriles et sur des patients ont été éprouvés au moyen de tests FC, par rapport aux antigènes de la rickettsiose vésiculeuse, du typhus et de la fièvre pourprée des Montagnes Rocheuses. Tous les résultats ont été négatifs à la dilution de 1: 5. Des sérums de convalescent prélevés sur des cobayes fébriles, éprou vés par des tests IH pour une série d’arbovirus, ont donné des résultats négatifs. Etudié au moyen de tests FC par rapport à des sérums de cobayes convalescents, un antigène préparé à partir de la rate de cobayes infectés a donné des résultats positifs. Par rap port à ce même antigène, des sérums couplés de malades présen taient une augmentation significative du titre d’anticorps. Des singes Cercopithecus seront inoculés avec du matériel infec tieux prélevé sur des cobayes. The International Reference Centre for Arboviruses has reported negative results with the serial sera from a number of patients in HI (hemagglutination inhibition) and CF (complement fixation) tests against more than sixty arboviruses. The Microbiological Research Establishment, Porton, has also reported negative results on early sera from patients by HI tests against representative arboviruses. The South African Institute for Medical Research, Johannesburg, has reported low titres of H I antibodies for some Group B arboviruses in the sera of some patients. Post mortem material and acute blood samples from patients have been inoculated into embryonated eggs, suckling and adult mice, guinea pigs and several cell cultures in different laboratories. All have been negative with the exception of inoculation into guinea pigs. Workers at Marburg, Frankfurt and Porton have reported that inoculated guinea pigs consistently develop fever four to six days after inoculation. At Porton, six consecutive passages have been made. The incubation period has shortened to three days and most guinea pigs die 13 to 15 days after inoculation. Convalescent sera from febrile guinea pigs and patients have been tested against rickettsial pox, typhus and Rocky Mountain Spotted Fever antigens in CF tests. All were negative at dilution 1/5. Convalescent sera from febrile guinea pigs were tested in HI tests against a series of arbo viruses and were negative. An antigen prepared from infected guinea pig spleens was tested against sera from convalescent guinea pigs by CF tests with positive results. Paired sera from patients have shown significant increase in antibodies against this same antigen. Cercopithecus monkeys will be inoculated with the infective I material from guinea pigs.
