PROOF OF RESIDENCY
PROOF OF RESIDENCY MUST BE SUBMITTED WITH THIS APPLICATION. To verify your address and your Community School
District (CSD), you must provide one of the following documents (copies of these are acceptable):
• Gas, electric or water bill in resident’s day (must be dated within 60 days of application submission)
• Document from a federal, state, or city agency that includes resident’s home address (must be dated within 60 days of application
submission)
• Lease agreement or deed/mortgage agreement for a house
• Official payroll documentation from an employer such as aW-4 form or payroll stub (must cover the time period during which the
application was submitted).
• Document from NY City Housing Authority or the Human Resources Administration that includes the resident’s home address
(must be dated within 60 days of application submission)
Please note that we reserve the right to ask for additional documentation to establish your address and your community school district. If
you move prior to the lottery, please notify the school with your new address.
Families giving false proof of residency in Community School District 3 will automatically be placed at the end of the waiting list.
AGREEMENT
I affirm that the information I have submitted above is true to the best of my knowledge. I understand that submitting this application does
not guarantee admission to the New York French-American Charter School, but serves as entry into a lottery that will be held to select
students for the 2015-2016 school year.
I agree that school records of the student for whom I am submitting this application may be used for studies of this charter school. In these
studies, only aggregate outcomes, not individual student’s outcomes, will be reported.

LOTTERY DATE
The lottery will be held on Friday, April 10, 2015 at the school, which is located at 311 W. 120th Street in Manhattan.
Families need NOT be present for the lottery. The lottery will be conducted by an independent third party who will certify the
fairness of the lottery.

- Une fiche de paye provenant de votre employeur ou fiche W4.
- Document de NYC Housing Authority avec votre adresse daté de moins de trois moins au jour de l’inscription
Veuillez noter que nous nous réservons le droit de vous demander de plus amples documents afin de déterminer votre
adresse et votre zone scolaire. Si vous changez d’adresse avant le tirage au sort, merci de nous en tenir informés.
Toute famille fournissant une fausse preuve de résidence dans le Disctrict 3 sera automatiquement placée en fin de la liste
d’attente.

TIRAGE AU SORT
Le tirage au sort aura lieu le vendredi 10 avril 2015 à l’école qui se trouve au 311 West 120th à Manhattan. Les familles n’ont
PAS besoin d’être présentes. Le tirage au sort est conduit par une tierce partie indépendante qui garantit l’équité du procédé.
