Methods The French version of the MDADI questionnaire was translated and culturally adapted following established guidelines (EORTC quality of life group translation procedure and the Principles of Good Practice for the Translation and Cultural Adaptation Process for Patient-Reported Outcomes). The translation and cultural adaptation consisted of the following steps: November 2015 A. Preparatory: The translation coordinator did a literature search to ascertain that the MDADI did not exist in the French language. This was followed by requesting the MDADI developer’s permission to translate the original questionnaire. B. Forward translation: Two French (one from France and the other from the French-speaking part of Switzerland) native professional translators translated the original English MDADI into French. These French translations were compared; the coordinator discussed discrepancies with both translators and came to a resolution. C. The coordinator reconciled the two French translations to provide a single provisional forward translation. Both translators approved this reconciled French translation. December 2015 D. Backward translation: The reconciled French translation was translated back into English by a translation agency (www.artenao.com) working with native English translators. January 2016 E. Cognitive interview: To test alternative wording included in the provisional forward translation and to ensure that the translation is culturally relevant, five patients with dysphagia were administered the questionnaire and were subsequently interviewed about the clarity and comprehension of the questions. Following this step slight modifications were made to the provisional forward translation. The final product was the finished forward translation which was pilot tested. February to March 2016 F. Pilot testing: To identify any remaining issues and ensure that the questionnaire is suitable for use, the finished French translation was pilot tested on 17 patients with head and neck cancer. 1 Recherche appliquée et développement Angewandte Forschung und Entwicklung 2. Forward translation into French The independent forward translations of the MDADI were done by Christine Lamarque-Choy (from France) and Nonny Bamba (from Switzerland) who were both professional translators (English –French) and native French speakers. The discussion on the discrepancies between the forward translations and the process of cultural adaptation were overseen by the translation process coordinator (Christina Moses Passini, Haute Ecole de Santé, Fribourg). The following criteria were used for resolving discrepancies: • If both versions were identical no changes were made • If there was a difference the most appropriate translation was chosen: o The sentence as close as possible to the original meaning, but also fitting into the Swiss French cultural setting o When sentence in the two versions had the same meaning we chose the one patients were more likely to understand and use o The shortest sentences Below we briefly describe the discrepancies between the forward translations and their resolution. After a discussion with both translators, a combination of both forward translations was preferred: «J’ai la sensation d’avaler une très grande quantité de nourriture». The other questionnaire items not presented here were similarly translated by both translators. 3. Backward translation into English The independent backward translations of the 20-item MDADI French translation was conducted by a translation agency Artenao.com, that works with native English translators. The agency was not provided with the original MDADI in English language. The back translation was adequate (see Appendix 2) and we thus moved on to administer the French forward translation and conduct a cognitive interview with five dysphagia patients to gain more insight into their views about the questionnaire. 4 Recherche appliquée et développement Angewandte Forschung und Entwicklung 4. Cognitive interview The cognitive interview (initial pilot test) was conducted from in January 2016 with five hospitalized patients (4 males 1 female) hospitalized for head and neck cancer. There were no comprehension problems related to question items E4, P6, E5, P7, P3, E3, P8, F3, F2, P5, P1, E6, P4, F4. These items were considered easy to understand, not conveyed with difficult words, not confusing, and not upsetting. However, these patients who were hospitalized at the time of the cognitive interview considered question items E3, F3, F2, F4 inappropriate for in-patient settings. Question 1 (global question) Two patients had issues with this question: one patient found the word “déglutir” to be a specific word used in medicine and not in everyday language. He suggested that this word should be replaced by “avaler” or put in quotes. Another patient did not understand the term “au jour le jour” but he did not suggest any replacement for this phrase. As discussion with the translators during the forward translation process had highlighted that “déglutir” could likely pose a problem we decided to replace this verb with a proposition made by one of the translators which addresses the concerns made by the two patients. • Ma capacité de déglutition est un frein à mes activités quotidiennes. Question E2 One patient reported she found this question confusing: she did not see the link between “embarrassment” and her “eating habits” though she understood the question. This may be because the patient is hospitalized and have not been exposed to a situation where she had experienced this. We did not modify this question. Question F1 Three patients found this question either difficult to understand (2 patients) or upsetting (1 patient). They found “les gens” too vague. They suggested this phrase should be replaced by listing the individuals being referred to such as family, and friends. We modified this question by choosing the initial translation of one of the two forward translators which was closer to the suggestion of the patients. • Mon entourage a des difficultés à me préparer à manger. Question P2 Two patients found this question difficult to understand because it was too restrictive in terms of the time period referred to. They suggested employing an open question. We decided not to follow this suggestion, as the issue with the question was not one of comprehension but of scope. 5 Recherche appliquée et développement Angewandte Forschung und Entwicklung Question E7 Three patients found this question difficult to understand. They found the expression “mal à l’aise” too general. One of these patients considered the question as a double negative. After discussion with the developer of the questionnaire, we decided this concern was unfounded. As the other issue raised relates to the scope of the question, we did not modify this question. Question F5 Four of the patients saw this question as irrelevant. They argued that they had medical insurance that covered their sick leave and their stay in the hospital. Our view is that these patients have not yet been faced with the situation being described in this question. This question will likely be relevant for patients who have been experiencing dysphagia over a longer period of time. 6 Recherche appliquée et développement Angewandte Forschung und Entwicklung 5. Pilot testing Background details The 20-item reconciled French translation of the MDADI item was pilot-tested on 17 patients (13 males, 4 females) with age ranging from 41 to 81 years (M = 66, SD = 11). There were 13 Swiss, an English national and an Italian, all fluently French speaking. Their clinical characteristics were as follow. They all had diagnosis of head and neck cancer: oral cavity (n = 5), oropharynx (n = 9), and larynx (n = 3). Concerning treatment modalities, five patients had received only surgery, four patients received chemoand radiotherapy, a patient had received radiotherapy and surgery, and four patients had received all three treatment modalities while two patients had received none. The questionnaire was completed in the hospital by six of the patients who were hospitalized and by nine others in an outpatient clinic. Four of the patients had percutaneous endoscopic gastronomy (PEG). Descriptive statistics The descriptive statistics for the data collected from the pilot-tested patients are as follows (N = 17). Satisfying reliability was achieved for all scores; however, this implied removing one item of for the emotional and functional subscale as indicated in the table above. This may reflect the fact that about one-third (n = 6) of the patients in this pilot sample were still hospitalized when they completed the questionnaire, and that some of the questions were irrelevant in this early treatment phase. Thus, we recommend to reconfirm the reliability of the subscales when the full MDADI French version is applied in patients with head and neck cancer during treatment phase. 7 Recherche appliquée et développement Angewandte Forschung und Entwicklung 6.
